Acupuncture to Boost Breast Milk in Postpartum Hypogalactia: Randomized Controlled Trial

Background: Breast milk is appropriate and useful but the exclusive breastfeeding rate in Thailand was quite low at only 23.1%. Hypogalactia is one of the important barriers of breastfeeding. Studies in Asia and the west have shown that acupuncture could boost breast milk. However, some articles had controversial result. There has been no study to assess efficiency of acupuncture in early postpartum hypogalactia. Objective: To compare the efficacy of acupuncture and conventional treatment for stimulating breast milk in early postpartum hypogalactia at 48 hours. Materials and Methods: A randomized, prospective, clinical trial was used for the present study. Sixty postpartum hypogalactic women of term gestation in postpartum ward in Charoenkrung Pracharak Hospital between March 1, 2020 and February 28, 2021 were enrolled. They were randomized into acupuncture and control groups. In the study group, mothers received once a day acupuncture at 13 acupoints without electrode for three consecutive days by qualified Traditional Chinese Medicine practitioner other than conventional treatment. Breast milk quantity were measured at day 1, day 7, and day 14 after the intervention. Results: Mean age of all participants was 29.72±5.96 year, mostly primigravida, delivered by vaginal route. Mean milk volume in the treatment group were significantly higher than the control group (p<0.05) with day 1 at 19.00±22.25 versus 5.97±6.08 mL, day 7 at 78.10±60.38 versus 40.17±28.82 mL, and day 14 at 128.33±64.86 versus 75.00±57.96 mL. The amount of breast milk in the treatment group were 3-fold, 2-fold, and 1.7-fold times more than the control group at day 1, day 7, and day 14, in respective order (p<0.05). Conclusion: Accurate acupuncture intervention could boost breast milk production in early postpartum hypogalactia at 48 hours. Albuminuria is characteristic of early-stage diabetic nephropathy (DN). The conventional treatments with angiotensin receptor blockers (ARB) are unable to prevent the development of albuminuria in normotensive individuals with type 2 diabetes mellitus (T2DM).Purpose. The present study aimed to evaluate the effect of ARB combined with a Chinese formula Qidan Dihuang grain (QDDHG) in improving albuminuria and Traditional Chinese Medicine Symptom (TCMS) scores in normotensive individuals with T2DM.Methods. Eligible patients were randomized to the treatment group and the control group.Results. Compared with baseline (week 0), both treatment and control groups markedly improved the 24-hour albuminuria, total proteinuria (TPU), and urinary albumin to creatinine ratio (A/C) at 4, 8, and 12 weeks. Between treatment and the control group, the levels of albuminuria in the treatment group were significantly lower than in the control group at 8 and 12 weeks (p<0.05). In addition, treatment group markedly decreased the scores of TCMS after treatment.Conclusion. This trial suggests that QDDHG combined with ARB administration decreases the levels of albuminuria and the scores for TCMS in normotensive individuals with T2DM.

Objective: To estimate the efficacy of traditional Chinese medicine (Chining decoction, CHIN) for radiation-induced oral mucositis in patients with head and neck cancer. Methods: From May 2014 to December 2015, 70 consecutive patients were randomly assigned to receive CHIN (treatment group) or recombinant human epidermal growth factor (rhEGF) spray (control group) at a 1:1 ratio. CHIN was administered to treatment group from the first day of radiotherapy until the completion of radiotherapy. Simultaneously, the rhEGF spray was administered to control group on the oral mucosa of irradiated area. The clinical benefit was determined by gradation of mucositis (Common Terminology Criteria for Adverse Events v4.0), oral pain, and xerostomia (visual analysis scale) for each week during radiotherapy. Body mass index was evaluated before and after radiotherapy. Results: Patients in the treatment group had prominent remission of oral pain and grade of mucositis on each observing point compared with those in control group ( P < .01). Xerostomia was decreased notably in treatment group compared with control group ( P < .01). Body mass index in the treatment group exhibited advantage over control group after radiotherapy, but there was no statistical significance (19.8 ± 3.26 vs 18.8 ± 2.5 kg/m2, P = .153, >.05). Conclusions: CHIN presented an obvious advantage in preventing radiation-induced oral mucositis compared with rhEGF spray.

This study aimed to determine the efficacy of Integrated Rehabilitation Techniques of Traditional Chinese Medicine (IRT-TCM) on patients with ischemic stroke as an alternative therapy to conventional rehabilitation techniques. Sixty-nine patients with ischemic stroke were randomly assigned to receive either IRT-TCM (intervention group, n = 46) or conventional rehabilitation techniques (control group, n = 23). The IRT-TCM consisted of a sequential combination of acupuncture and massage techniques. The Fugl-Meyer Assessment (FMA), National Institutes of Health Stroke Scale (NIHSS), Barthel index (BI) and modified Rankin Scale (mRS) were measured on day 0 (baseline, before treatment), day 21, and day 90. We observed that the scores in FMA and BI were increased, and NIHSS were decreased in both groups on day 21 and 90, compared with the baseline (day 0). Furthermore, significantly better scores in FMA of lower limbs and NIHSS were found in patients treated with IRT-TCM on day 21 and 90. For mRS, the percentage of patients ranking 0 and 1 in the intervention group presented a striking contrast to the control group on day 90 but with no significant difference. The results indicated that, as a feasible alternative therapy, IRT-TCM is beneficial for patients with ischemic stroke. Further research with larger sample size, long-term observation, and strict blinding are still in need to confirm the efficacy of IRT-TCM.

Abstrak: Produksi ASI yang kurang dan lambat keluar dapat menyebabkan ibu tidak memberikan ASI pada bayinya dengan cukup. Selain hormon prolaktin, proses laktasi juga bergantung pada hormon oksitosin, yang dilepas dari hipofise posterior sebagai reaksi terhadap penghisapan putting. Rolling massage salah satu terapi relaksasi yang bertujuan menstimulasi saraf pusat pada hipofisis posterior dan anterior sehingga dapat meningkatkan produksi ASI khususnya pada ibu post partum dan memberikan kenyamanan dan rileksasi setelah persalinan. Adapun tujuan penelitian adalah mengetahui efektifitas produksi ASI pada ibu post partum dengan massage rolling (punggung) di BPM Sri Sukeni Sleman. Metode penelitian ini adalah Quasi Experimental Design dengan rancangan penelitian two group post test design. Kelompok kontrol adalah ibu postpartum tanpa dipijat Massage roliing dan kelompok perlakuan adalah kelompok ibu postpartum yang dipijat masssage Roliing. Analisis data menggunakan uji independent t test dan paired sample t test. Hasil penelitian uji t sampel menunjukkan bahwa ada perbedaan yang bermakna (p=0.023<) produksi ASI (berat badan bayi) antara kelompok kontrol ( 2687,07 ± 160,155) dengan kelompok perlakuan (2846,13± 198,968). Perbedaan ini terlihat pada rerata berat badan bayi pada kelompok perlakuan lebih besar dibandingkan dengan kelompok. Massage rolling memengaruhi produksi ASI di BPM Sri Sukeni Sleman Tahun 2017. Abstract: The inadequate, slow production of breast milk makes mothers unable to provide enough breast milk to their babies. Besides the prolactin hormone, the lactation process also depends on oxytocin hormone which is secreted from the posterior hypophysis as a reaction on nipple suckling. Rolling massage is one of the relaxation therapy which aims to stimulate the central nerve on posterior and anterior hypophysis, so the breast milk production can be increased, particularly in the postpartum mothers, and comfort and relaxation after childbirth can be provided. This research aims to investigate the effectiveness of breast milk production in the postpartum mothers with massage rolling (back) in Sri Sukeni Community Empowerment Agency, Sleman. The methodology in this research is Quasi-Experimental Design with two-group posttest design. The control group is postpartum mothers without massage rolling and the treatment group is postpartum mother treated with the massage rolling. Data analysis used the independent t-test and the paired sample t-test. The research shows the result that in samples of t-test, there is a meaningful difference (p = 0.023<) in breast milk production (weight of baby) between control group (2,687.07 ± 160.155) and treatment group (2,846.13 ± 198.968). This difference is shown in the average weight of baby in treatment group which is higher compared with the control group. Massage rolling affects breast milk production in Sri Sukeni Community Empowerment Agency, Sleman in 2017.

The decrease of breast milk production of postpartum mother in the early breast feeding process that become a problem for mother to provide breast milk  early on her baby. Based on a preliminary study of the lowest exclusive breastfeeding rate coverage in the Kediri city is at Sukorame Public Health Center. This is because many postpartum mothers who felt less that give their milk formula or food other than breast milk in early lactation. The breast milk production could be stimulated by the use of intervention  marmet technique. The purpose of this study was to investigate the influence of giving marmet technique to the breast milk production of the postpartum mother. This study is a quasi-experimental research design with post-test only control group design. The population in this study were all post partum mothers who met the inclusion criteria for the study in BPM working area of Sukorame Public Health Center Kediri City. The sampling technique is consecutive sampling with a sample of 26 respondents. Data collection was done by dividing into 2 groups: the experimental group and the control group and to assess the breast milk production using the observation sheet of breast milk production. Data analysis technique used is the Fisher Exact Test Test. The results obtained p value count (0.0074) < = (0.05) then H0 is rejected, meaning that there is the effect of giving marmet technique to the breast milk production. The conclusion of this study was the Marmet technique affects breast milk production of post partum mother. Giving Marmet technique can be applied to help the adequacy of breast milk production. Suggestions researchers is that all medical person can continue to support the improvement of service quality by taking into account maternal postpartum milk production that can impact the breastfeeding process. Keywords: marmet technique, breast milk production, post partum

Abstract Background: Stroke is a severe and life-threatening disease, owns high rates of disability and mortality.[1] Stroke and ischemic heart disease, chronic obstructive pulmonary disease are the world’s three main killers.Ischemic strokes account for the vast majority of strokes.[2] Modern medicine has some advantages in treating ischemic stroke, but there are also limitations. Traditional Chinese medicine has thousands of years of experience in treating stroke, but there are few high quality clinical Randomized controlled trial.Methods: This is a multicenter, randomized, double-blind, placebo-controlled trial. 286 patients were randomly divided into test group and Control Group. Both groups received General Western medicine treatment, the test group combined with Chinese medicine treatment, the control group combined with placebo treatment. The duration of treatment was 30 days and the follow-up was 90 days. evaluation indicators include: Modified Rankin Scale, NIH stroke Scale, Glasgow Coma Scale, Barthel Index, Case fatality rate.Laboratory specifications and safety assessments will also be taken into account.Discussion: The aim of this study was to evaluate the safety and efficacy of ZFXNY in the treatment of acute cerebral infarction. Our research will provide a reliable evidence-based medicine basis for the treatment of acute cerebral infarction with traditional Chinese medicine, and provide another option for the treatment of acute cerebral infarction.Trial registration: ChiCTR2100043796, Registered February 28th, 2021.

The pathological basis of non-ST-segment elevation acute coronary syndrome (NSTE-ACS) is severe coronary stenosis, unstable plaque erosion, and rupture, resulting in coronary blood flow reduction and myocardial ischemia, leading to acute thrombosis cardiovascular disease events. This subject intends to study the treatment of NSTE-ACS patients with blood stasis and toxin syndrome by Qingre Jiedu Huoxue Huayu Decoction, observe its clinical efficacy, and explore the effects of serum lipoprotein phospholipase A2 (Lp-PLA2) and tumor necrosis factor- α (TNF- α), the effect of placental growth factor (PIGF) expression. In this study, 100 patients with blood stasis and toxin syndrome of NSTE-ACS treated in the cardiovascular department of Enshi National Hospital from August 2020 to August 2021 were selected as the research object. They were randomly divided into traditional Chinese medicine comprehensive treatment groups and conventional western medicine control groups, with 50 cases. The conventional western medicine control group was treated with hydroclopidogrel tablets orally, and the comprehensive treatment group of traditional Chinese medicine combined with Qingre Jiedu Huoxue Huayu formula orally. The patients in both groups were treated for four weeks. The results showed that after treatment, the practical clinical rate of the comprehensive treatment group was significantly higher than that of the conventional western medicine control group. After treatment, the TCM syndrome score, angina pectoris attack duration, and angina pectoris attack frequency, myocardial zymogram index level, serum Lp-PLA2 and TNF of the two groups were measured- α. The levels of PIGF were significantly lower than those before treatment. The decline of the above indexes in the comprehensive treatment group of traditional Chinese medicine was significantly better than that in the control group of conventional Western Medicine (P<0.05). The incidence of MACE events in the TCM Comprehensive treatment group was significantly lower than that in the conventional western medicine control group (P <0.05).

Abstract Background: Currently, coronavirus disease-2019 (COVID-19) is continuously and rapidly circulating, resulting in serious and extensive impact on human health. Due to the absence of antiviral medicine for COVID-19 thus far, it is desperately need to develop the effective medicine. Traditional Chinese medicine (TCM) has been widely applied in the treatment of epidemic diseases in China, hoping to produce clinical efficacy and decrease the use of antibiotics and glucocorticoid. The aim of this study is to evaluate the efficacy and safety of Baidu Jieduan granule in curing COVID-19. Methods/design: This multicenter, open-label randomized controlled trial is conducted 300 cases with COVID-19. The patients will be randomly (1:1) divided into treatment group or control group. All cases will receive standard therapy at the same time. The experiment group will receive Baidu Jieduan granule treatment twice a day for 14 days. The outcomes are assessed at baseline and at 3, 5, 7, 14 days after treatment initiation. The primary outcome is the rate of symptom (fever, fatigue, and coughing) recovery. Adverse events (AEs) will be monitored throughout the trial.Discussion: The study will provide a high-quality clinical evidence to support the efficacy and safety of Baidu Jieduan granule in treatment of moderate COVID-19, and also enrich the theory and practice of TCM in treating COVID-19. Abstract Background:Prediabetes is a growing health concern,a large percentage of whom will develop full 2 type diabetes. The prominent symptom of patients with diabetes and prediabete is fatigue.The syndrome of spleen-deficiency phlegm-dampbess is the main syndrome of prediabetes.Effective intercentions on prediabetes will delay or prevent the occurrence or development of diabetes. Methods:Randomized controlled trial is implemented in this study.The study term is 3 months.Participants are recruited from Binhai hospital of Tianjin of Traditional Chinese Medicine,Jinghai hospital of Tianjin of Traditional Chinese Medicine and affiliated Wuqing hospital of Tianjin university of Traditional Chinese Medicine.160 participants are randomized to treatment group(Liu Jun Zi granules) and control group(Liu Jun Zi granules emulsifier):80 participants each.Participants being included in this study must have been diagnosed as prediabetes via western medicine criteria and traditional Chinese Medicine criteria.The primary outcomes is the score of fatigue-scale 14 and the rate of blood glucose return to the normal.The secondary outcomes inludes fasting plasma glucose,2-hour post-meal blood glucose and blood lipid test.The indexes of safety include general medical examination;electrocardiogram(ECG),liver function(ALT) and renel function(BUN,Creatinine) test and record of adverse event. Discussion:The aim of this study is to evaluate the effectiveness and safety of Liu Jun Zi granules for the treatment of patients with prediabetes. ABSTRACT Background: Goat milk has some potential nutrition than cow milk. Studies suggested that goat milk is better digested and absorbed than cow milk. It also has less allergen protein and lower lactose. Previous studies examined the benefit of goat milk as infant feeding to increase infant growth compared with cow milk-based formula. However, the studies that analyze the effect of goat milk in increasing breast milk production are scarce. This study aimed to examine the effectiveness of goat milk to increase breast milk volume in lactating mothers. Subjects and Method: This was a randomized controlled trial (RCT). The study was conducted in Madiun, East Java, in October 2019. A sample of 100 lactating mothers with children aged 1 to 3 months was selected purposively. The dependent variable was volume of breast milk. The independent variable was the effectiveness of goat milk. The data were collected by food recall 24 hours and observational sheet. The data were analyzed by t test. Results: Breast milk production after giving goat milk in the intervention group (Mean= 121.5; SD= 12.81) was higher than control group (Mean= 97.06; SD= 13.59), and it was statistically significant (p<0.001).
