Abstract: Diabetes mellitus is characterized with hyperglycemia that occurs due to abnormal secretion of insulin, abnormal function of insulin, or both. It is a metabolic disorder that genetic and clinic heterogeneous with manifestation including loss of carbohydrate tolerance. Diabetes mellitus is characterized by fasting hyperglycemia, postprandial hyperglycemia, atherosclerosis, and neuropathy. Diabetic foot is one of the chronic complications related to macroangiopathy, microangiopathy, and neuropathy. In diabetes mellitus suspected platelets disfunction occurs in patients with diabetes mellitus. This research is observational analytic with design of case control. Subjects of the study is a patients with diabetes mellitus type 2 and complications diabetic foot and without the complications diabetic foot is treated at poly endocrine, poly foot at hospital BLU/RSUP. Prof. Dr. R. D. Kandou Manado. Statistical evaluation was performed by SPSS, normality data test using Kolmogorov-Smirnov, analyzed with Mann Whitney U-test and the correlation analysis using Spearman Correlation Test. The result normality test for the case group Kolmogorov-Smirnov test value p=0.715, as for the control group p=0.455 distribution is normal. And the result analyzed Mann Whitney U-test value p=0.017. The calculation of the Spearman correlation test value p= 0.015 and coefficient= -0.381. In this study conclude that platelets count in the normal range but there are differences in average of number of platelets both group, the group with diabetic foot were higher compared to the group non diabetic ulcer. The average number of platelets in group with diabetic foot was higher which indicated that could be either the cause or the due effect of the vascular complications. Platelets may play a role and can be used as a simple parameter to assess the vascular in diabetes.Keywords: trombosit platelets, diabetes mellitus, diabetic footAbstrak: Diabetes melitus merupakan penyakit metabolik dengan karakteristik hiperglikemia yang terjadi karena kelainan sekresi insulin, kerja insulin atau kedua-duanya. Gangguan metabolisme terjadi secara genetik dan klinis termasuk heterogen dengan manifestasi hilangnya toleransi karbohidrat. Penyakit ini ditandai dengan hiperglikemi puasa dan potstprandial, aterosklerosis, dan neuropati. Kaki diabetik merupakan penyakit vaskular mikroangiopati dan salah satu komplikasi kronik utama diabetes melitus yang terkait makroangiopati, mikroangiopati, neuropati. Pada diabetes melitus diduga terjadi disfungsi trombosit. enis penelitian ini ialah analitik observational dengan disain case control. Subyek penelitian ialah penderita diabetes mellitus tipe 2 dengan komplikasi kaki diabetik dan yang tidak dengan komplikasi kaki diabetik yang berobat di poli endokrin penyakit dalam dan poli kaki BLU/RSUP. Prof. Dr. R. D. Kandou Manado. Pengolaan evaluasi data ini menggunakan SPSS dengan uji normalitas data menggunakan Kolmogorov-Smirnov yang dianalisis dengan Mann Whitney U-test dan untuk korelasinya dianalisis menggunakan Spearman Correlation test. Hasil uji normalitas kelompok kasus nilai p=0,715, untuk kelompok kontrol p=0,455 yang berarti berdistribusi normal. Hasil statistik Mann Whitney U-test nilai p=0,017. statistik dengan Spearman Correlation test menunjukan hasil p=0,015 dan koefisien -0,381. Dalam penelitian ini jumlah trombosit dalam batas normal tetapi terdapat perbedaan rerata angka trombosit kedua kelompok dimana pada kelompok dengan kaki diabetik lebih tinggi dari pada kelompok diabetes melitus tanpa ulkus diabetik. Hal ini menunjukkan bahwa meningkatnya rerata angka trombosit bisa menjadi salah satu penyebab atau efek dari komplikasi vaskular. Oleh karena itu trombosit mungkin berperan dan dapat digunakan sebagai parameter sederhana untuk menilai pembuluh darah pada diabetes.Kata kunci: angka trombosit, diabetes melitus, kaki diabetik

ABSTRAKLatar Belakang : Quality of Life (QoL) atau Kualitas hidup pasien dengan ulkus kakik diabetes melitus lebih buruk, daripada pasien dengan Diabetes Melitus (DM) tanpa ulkus kaki dalam populasi umum. Tujuan dari penelitian ini adalah untuk mengetahui gambaran Quality of Life (Kualitas hidup) pada pasien dengan ulkus kaki diabetes melitus (Diabetic Foot Ulcer / DFU) di Bali.Metode : Desain deskriptif dengan pendekatan cross sectional. Teknik sampel direkrut menggunakan convenience sampling yang melibatkan pasien dengan ulkus kaki diabetes melitus (Diabetic Foot Ulcer / DFU) dengan total sampel yang melibatkan 201 orang responden. yang dilakukan di ruangan department rawat jalan bedah Rumah Sakit Wangaya serta klinik perawatan luka di Bali Indonesia. Data dikumpulkan menggunakan kuesioner Diabetic Foot Ulcer Scale-Short Form (DFS-SF) dan Independent t-test digunakan untuk memahami gambaran kualitas hidup antara perempuan dan laki-laki dengan DFU. Penelitian ini sudah mendapatkan ijin etik oleh komite etik Universitas Udayana.Hasil : Karakteristik responden perempuan (n= 103; 51.2%), dan laki-laki (n= 98; 48.8%). Kualitas hidup sebagian besar memiliki kualitas hidup rendah yang buruk yaitu (<50)(n=133; 66.2%), dan kualitas hidup yang lebih baik yaitu (> 50)[n=68; 33.8%]. Skor rata-rata kualitas hidup pada pasien dengan DFU 42.4±15.5.Kesimpulan: Laki-laki dan perempuan pada pasien dengan ulkus kaki diabetes melitus menunjukkan kualitas hidup perempuan lebih rendah daripada laki-laki dengan diabetes melitus (DFU). Keluarga diharapkan tetap memotivasi dan mendukung pasien agar dapat berpartisipasi dalam aktivitas keagamaan atau kegiatan positif lainnya. Berdasarkan hasil penelitian yang telah didapat, Pasien dengan ulkus kaki diabetes melitus, baik laki-laki maupun perempuan merupakan suatu hal yang perlu kita perhatikan.  Kata Kunci : Ulkus Kakik Diabetes Mellitus, Jenis kelamin, dan Kualitas Hidup  ABSTRACTBackground: Quality of Life (QoL) or Quality of Life of patients with diabetic ulcer diabetes is worse, than patients with Diabetes Mellitus (DM) without foot ulcers in the general population. Conclusion: Male and female patients with diabetes mellitus foot ulcers show a lower quality of life for women than men with diabetes mellitus (DFU). ABSTRAK Penderita Diabetes Mellitus (DM) berisiko mengalami penurunan sensitivitas pada kaki. Kebiasaan maupun perilaku penderita seperti kurang menjaga kebersihan kaki dan tidak menggunakan alas kaki saat beraktivitas akan beresiko terjadi perlukaan pada daerah kaki. Keadaan kaki diabetik lanjut yang tidak ditangani secara tepat dapat memicu dilakukannya tindakan amputasi kaki.  Tujuan penelitian untuk menganalisis Pengaruh Senam Kaki Diabetes terhadap Leg Sensitivity Monofilament Test pada penderita Diabetes Melitus di Wilayah Posyandu Lansia Endrosono Surabaya. Penelitian ini menggunakan metode pre-experiment designs dengan rancangan yang digunakan yaitu one group pretest-postest dengan sample 54 responden lansia di Posyandu Lansia Endrosono Kelurahan Wonokusumo Kecamatan Semampir Kota Surabaya diambil menggunakan tehnik sampling Simple Random Sampling. Pengukuran Leg Sensitivity dipeoleh dari observasi menggunakan Monofilament Test. Data dianalisis menggunakan Uji Mc. Nemar didapatkan p value = 0,008 pada kaki kanan dan p value = 0,003 pada kaki kiri dengan α=0,05 (p< α). Hal ini berarti tidak terdapat pengaruh antara senam kaki diabetes dengan leg sensitivity monofilament test (H0 diterima, H1 ditolak). Implikasi hasil penelitian bahwa senam kaki diabetes tidak dapat mempengaruhi leg sensitivity monofilament test. sehingga perlu lebih ditingkatkan aktivitasnya sehari-harinya dan melakukan senam kaki diabetes 5-7x perhari. Kata Kunci : Senam Kaki Diabetes, Leg Sensitivity, Monofilament Test   ABSTRACT Patients with Diabetes Mellitus (DM) are risked with Leg Sensitivity decreasing. Patient’s habits or behavior such as less maintaining of their cleanliness and not wearing footwear or slippers will make the risk of them infect their feet increase. If the conditions of further diabetics are not handled properly, it can increase the act of foot amputations.  This research objective is to analyze the effect of Diabetics Foot Exercise against Leg Sensitivity Monofilament Test to patients with Diabetes Mellitus in Posyandu Lansia Endorsono Surabaya. This research is using pre-experiment designs methods and the design used is one group pretest-posttest with 54 elderly respondents from Posyandu Lansia Endorsono Wonokusumo Village, Semampir sub-district, Surabaya as sample and using Simple Random Sampling as the sampling technique. Leg sensitivity measuring is collected from Monofilament Test observation. The Data are analyzed by SPSS 16.0 program and using Mc. Nemar obtained p value = 0,008 for the right foot and p value = 0,003 for the left foot α=0,05 (p< α). It means that there is no effect between Diabetic Foot Exercise with the Leg Sensitivity Monofilament test (H0 is approved, H1 is disapproved). The implication of the research is that Diabetic Foot Exercise cannot be affecting Leg Sensitivity Monofilament Sensitivity Test. As the result patients with diabetes mellitus need to increase their daily activity and do the Diabetic Foot Exercise 5-7 times per-day. ABSTRACT: DIABETIC FOOT SELF CARE ON DIABETES PATIENTS Background: Diabetes Mellitus (DM) can cause complications in various body systems. One of the complications of DM is complications in the feet which can cause diabetic foot ulcers and lead to leg amputation. Diabetic foot complications can be prevented by performing routine foot care or diabetic foot care. Objective: The purpose of this study was to determine the description of diabetic foot care in patients with diabetes mellitus consisting of personal self-care, podiatric care, and footwear and socks. Method: This type of research is descriptive quantitative research. Data was collected using a diabetic foot care questionnaire. The number of samples in this study was 51 people. The sampling method is a non-probability technique using consecutive sampling. The study was conducted in the Kuranji Health Center Working Area in July – November 2021. Result: The results showed that most respondents (64.70%) had poor personal self-care in foot care, 82.3% of respondents had poor podiatric care habits. and 52.94% of respondents have good habits in choosing footwear. Conclusion: From the results of the study, it was concluded that people with diabetes have bad habits in performing foot care so that this is one of the risk factors for complications in the feet. Pengumpulan data dilakukan menggunakan kuesioner diabetic foot care. Jumlah sampel dalam penelitian ini adalah 51 orang. Metode pengambilan sampel adalah dengan Teknik non probability dengan menggunakan consecutive sampling. Penelitian dilakukan di Wilayah Kerja Puskesmas Kuranji pada Bulan Juli – November 2021.Hasil : Hasil penelitian menunjukkan bahwa sebagian besar responden (64,70%) memiliki personal self care yang kurang baik dalam perawatan kaki, 82,3% responden memiliki kebiasan podiatric care yang kurang baik dan 52,94% responden memiliki kebiasaan baik dalam pemilihan alas kaki.Kesimpulan : Diabetisi memiliki kebiasaan yang kurang baik dalam melakukan perawatan kaki sehingga ini menjadi salah satu faktor resiko terjadinya komplikasi pada kaki. Diabetic ulcers are the most feared complications of Diabetes mellitus patients because of reduced blood supply to the tissues, causing tissue death and compounded with bacterial infections that can cause amputations and even have wide impact because they can cause death, morbidity, disability, and decreased quality of life. The purpose of this study was to determine the relationship of diabetic ulcers in patients with diabetes mellitus with body image. The research design used was analytic survey with cross sectional design. The sample in this study proves 40 samples taken Probability of Samples with the type of sampling in a row that was a diabetic ulcer sufferer who performs examinations in the clinic of Jember. This research was conducted by observing the degree of injury for diabetic ulcers and questionnaire sheets for body image. The results of this study were analyzed using the statistical test Spearman rank test. The results of the analysis showed that first-degree injuries were 22 respondents (55%), second-degree injuries were 17 respondents (42.5%), and third-degree injuries were respondent (2.5%). While the body image results in diabetics with 13 respondents (32.5%) had a negative body image, while 27 respondents (67.5%) had a positive body image. The results of the analysis of the two variables obtained by the value of p value Sig. (two tailed) of 0.023 α α (0.05) which shows that there was a relationship between diabetic ulcers in patients with diabetes mellitus with body image. Clients who suffer from diabetes mellitus will affect the appearance of the body, especially will have an impact on body image so it is very important for clients diabetes mellitus to improve adjustment in dealing with the disease process and the consequences of the disease. Keywords: diabetic ulcer; body image ABSTRAK Ulkus diabetikum merupakan komplikasi yang paling ditakuti pasien Diabetes Melitus karena berkurangnya suplay darah ke jaringan tersebut menyebabkan kematian jaringan dan diperparah dengan infeksi bakteri yang dapat menyebabkan amputasi bahkan berdampak luas karena dapat menyebabkan kematian, morbiditas, peningkatan biaya perawatan, dan penurunan kualitas hidup. Tujuan dari penelitian ini adalah untuk mengetahui hubungan ulkus diabetik pada penderita diabetes melitus dengan citra tubuh di Klinik Jember. Desain penelitian yang digunakan yaitu survey analitik dengan rancangan coss sectional. Sampel dalam penelitian ini diambil secara Probability Sample dengan jenis Consecutive Sampling yaitu penderita ulkus diabetik yang melakukan pemeriksaan di klinik jember sebanyak 40 responden Penelitian ini dilakukan dengan cara melihat/observasi derajat luka untuk ulkus diabetik dan lembar kuesioner untuk citra tubuh. Hasil penelitian ini dianalisa menggunakan uji statistic spearman rank test. Hasil analisanya didapatkan bahwa luka derajat 1 sebanyak 22 responden (55%), luka derajat 2 sebanyak 17 responden (42,5%), dan luka derajat 3 sebanyak 1 responden (2,5%). Sedangkan hasil citra tubuh pada penderita diabetes melitus sebanyak 13 responden (32,5%) memiliki citra tubuh negatif , sedangkan 27 responden (67,5%) memiliki citra tubuh positif. Hasil analisa dari kedua variabel didapatkan nilai p value Sig (two tailed) sebesar 0,023 ≤ α (0,05) yang menunjukkan bahwa ada hubungan ulkus diabetik pada penderita diabetes melitus dengan citra tubuh. Klien yang menderita diabetes mellitus akan mempengaruhi penampilan tubuh, terutama akan berdampak pada citra tubuh sehingga sangat penting bagi klien diabetes mellitus untuk meningkatkan penyesuaian dalam menangani proses penyakit dan konsekuensi penyakit. Background<br />The number of people with diabetes mellitus (DM) is increasing due to population growth, aging, and increasing prevalence of obesity. Diabetic retinopathy and diabetic nephropathy are two main complications of DM. Some studies suggest a correlation between diabetic nephropathy and diabetic retinopathy. However, other studies found that renal cortex echogenicity is associated with chronicity of kidney disease and renal histopathology. The aim of this study was to determine whether there is a correlation between renal cortex echogenicity as determined by renal ultrasonography and degree of retinopathy as determined by funduscopy in subjects with DM.<br /><br />Methods<br />A cross sectional study was conducted on 41 DM subjects from September to November 2014. Data obtained by anamnesis, physical examination, and examination of ureum, creatinine, urinalysis, glycated hemoglobin (HbA1c), renal and urinary tract ultrasonography and funduscopy, were collected from all subjects. Blood samples were taken from the median cubital vein for biochemical measurements using COBAS automated analyzers. Normality of data distribution was tested using the Shapiro-Wilk test. To determine the relationship between variables the Spearman correlation test was used. <br /><br />Results<br />Using the Spearman correlation test, a strongly significant correlation was found between degree of renal cortex echogenicity and degree of retinopathy (r=0.773; p=0.0001). A significant relationship was also found for the degree of retinopathy with age (r=0.317; p=0.044), duration of diabetes mellitus (r=0.639; p=0.0001) and HbA1c (r=0.681; p=0.001).<br /><br />Conclusion<br />This study found that renal cortex echogenicity increased the degree of diabetic retinopathy in diabetic subjects. Renal ultrasonography for patients with type 2 DM has a great role in diagnosing and grading diabetic retinopathy.

Abstract: Diabetes mellitus (DM) is a group of metabolic diseases with characteristic hyperglycemia that occurs due to insulin secretion, insulin action or both. This disease affects many societies and continuously growing in Indonesia. Periodontal disease is an oral health problem which has a relatively high prevalence in the community where periodontal disease in all age groups in Indonesia.The purpose of this study was to determine the periodontal disease in patients with diabetesmellitus in RSUP Prof. dr. R. D. Kandou Manado. This descriptive study has 68 sample taken with total sampling technique. The sample is examined using evaluation criteria gingival index and CPITN index. The result show that subjects with periodontitis with a score of 4 is the highest as many as 18 people (44%) and subject with a score of 2 is that at least as many as 8 people (19.5%). And subjects with bad gingivitis is the highest as many as 10 people (52.6%) and subject with the good gingivitis is the at least as many as 5 people (26.3%). Based on the result of this study, periodontal disease in patients with diabetes mellitus in RSUP Prof. Dr. R. D. Kandou most that periodontitis with the number of 41 people (68.3%) compared to gingivitis which is just as many as 19 people (31.7%)Keywords: diabetes mellitus, periodontitis, gingivitis, periodontalAbstrak:Diabetes Melitus (DM) merupakan suatu kelompok penyakit metabolik dengan karakteristik hiperglikemia yang terjadi karena sekresi insulin, kerja insulin atau kedua-duanya. Penyakit ini merupakan penyakit yang banyak diderita kalangan masyarakat dan terus berkembang di Indonesia. Penyakit periodontal merupakan masalah kesehatan gigi dan mulut yang memiliki prevalensi cukup tinggi di masyarakat dimana penyakit periodontal pada semua kelompok umur di Indonesia.Tujuan penelitian ini adalah untuk mengetahui penyakit periodontal pada penderita diabetes mellitus di RSUP Prof. dr. R. D. Kandou Manado. Jenis penelitian ini adalah penelitian deskriptif dengan jumlah sampel 60 orang yang diambil dengan teknik total sampling. Sampel diperiksa dengan menggunakan kriteria penilaian indeks gingiva dan indeks CPITN. Hasil menunjukkan bahwa yang mengalami periodontitis dengan skor 4 adalah yang paling tinggi yaitu sebanyak 18 orang (44%), dan subjek yang mengalami skor 2 adalah yang yang paling sedikit yaitu sebanyak 8 orang (19.5%). Sedangkan yang mengalami gingivitis yang paling tinggi yaitu gingivitis buruk sebanyak 10 orang (52.6%) dan yang paling sedikit adalah yang mengalami gingivitis ringan yaitu sebanyak 5 orang (26.3%). Berdasarkan hasil penelitian ini dapat disimpulkan bahwa penyakit periodontal yang paling banyak ditemui pada penderita diabetes melitus di RSUP Prof. Dr. R. D. Kandou Manado adalah penyakit periodontitis yaitu sebanyak 41 orang (68.3%) dan yang paling sedikit adalah gingivitis yaitu sebanyak 19 orang (31.7%)Kata kunci: diabetes melitus, periodontitis, gingivitis, periodontal

Background: Diabetic peripheral neuropathy (DPN) is known to predict foot ulceration, lower-extremity amputation and mortality. Patients with diabetes mellitus have a predisposition toward developing chronic inflammatory demyelinating polyneuropathy, and this may also facilitate the formation of diabetic foot and cutaneous impairment, which are considered one of the most serious impairments of diabetes mellitus, with a prevalence of 4–10% in this population. Biomarkers research provides opportunities for the early diagnosis of these complications for specific treatments useful to prevent amputation and, therefore, physical inability and mental disturbance. The recent literature has suggested that glycemic levels may be a novel factor in the pathogenesis of diabetic foot complications and is an important mediator of axonal dysfunction. The aim of this systematic literary review is to determine whether hemoglobin A1c (HbA1c) is a positive predictor for diabetic foot peripheral neuropathy and its complications, such as foot cutaneous impairments. There is a lack of consensus regarding the effect of glycemic variability on diabetic foot peripheral neuropathy, unlike other complications such as retinopathy, nephropathy or micro/macrovascular pathology. Methods: Relevant articles were searched in the Medline database using PubMed and Scopus and relevant keywords. The primary search terms used were “glycated hemoglobin” OR “HbA1c” AND “diabetic neuropathies” AND “Foot”. Results: A number of articles (336) were initially identified while searching the scientific literature regarding this topic, and 32 articles were selected and included in this review. Conclusions: This review highlights the role of HbA1c in diabetic foot peripheral neuropathy. Biomarkers play an important role in the decision-making process, and HbA1c levels are extensively used for diabetic foot clinical outcomes and settings, but biomarker research in diabetic foot peripheral neuropathy is in its infancy and will require careful attention to a number of factors and associations, since the consequences of DPN also include neurological alterations. HbA1c is an accurate and easy-to-administer test and can be an effective biomarker in establishing the diagnosis of diabetes, but future research should focus on standardizing the HbA1c level and selecting which DPN value and its correlated complications, such as foot cutaneous impairments, are the most informative.

IntroductionThe use of non-invasive vascular and perfusion diagnostics are an important part of assessing lower extremity ulceration and amputation risk in patients with diabetes mellitus. Methods for detecting impaired microvascular vasodilatory function in patients with diabetes may have the potential to identify sites at risk of ulceration prior to clinically identifiable signs. Spatial frequency domain imaging (SFDI) uses patterned near-infrared and visible light spectroscopy to determine tissue oxygen saturation and hemoglobin distribution within the superficial and deep dermis, showing distinct microcirculatory and oxygenation changes that occur prior to neuropathic and neuroischemic ulceration.Research designs and methods35 patients with diabetes mellitus and a history of diabetic foot ulceration were recruited for monthly imaging with SFDI. Two patients who ulcerated during the year-long longitudinal study were selected for presentation of their clinical course alongside the dermal microcirculation biomarkers from SFDI.ResultsPatient 1 developed a neuropathic ulcer portended by a focal increase in tissue oxygen saturation and decrease in superficial papillary hemoglobin concentration 3 months prior. Patient 2 developed bilateral neuroischemic ulcers showing decreased tissue oxygen saturation and increased superficial papillary and deep dermal reticular hemoglobin concentrations.ConclusionsWounds of different etiology show unique dermal microcirculatory changes prior to gross ulceration. Before predictive models can be developed from SFDI, biomarker data must be correlated with the clinical course of patients who ulcerate while being followed longitudinally.Trial registration numberNCT03341559.

Surgical intervention for chronic deformities and ulcerations has become an important component in the management of patients with diabetes mellitus. Such patients are no longer relegated to wearing cumbersome braces or footwear for deformities that might otherwise be easily corrected. Although surgical intervention in these often high-risk individuals is not without risk, the outcomes are fairly predictable when patients are properly selected and evaluated. In this brief review, we discuss the rationale and indications for diabetic foot surgery, focusing on the surgical decompression of deformities that frequently lead to foot ulcers.
