Teratogenic is an abnormal development on embryo and is the cause of congenital defect or birth defect. This study aims to determine the effect of the addition of Dioscorea alata L. ethanol extracts to the embryo development on pregnant mice whose given orally to 24 mice which divided to 4 treatment groups, they are the normal group (NG) with NaCMC 0.5%; 28 mg/KgBB treatment group; 35 mg/KgBB; 42 mg/Kg BB. The addition of Dioscorea alata L ethanol extracts was done on the sixth day until the 15th day of pregnancy. On the 18th day of pregnancy, Laparaktomi was done to the pregnant mice and the embryo was taken out of the uterus. The observation was done to the fetus numbers, weight weighing of the fetus's body, dan length measurement of the fetus's body. Another observation is the observation of the external organ defect of the embryo. The study results that the addition of Dioscorea alata L ethanol extracts with various doses have no significant effect (P>0.5) to the mice external fetus development. On the examination of the fetus, we can conclude that Dioscorea alata L ethanol extracts don’t give any effect that may cause the defect of the fetus’ external organ.

In vitro culture systems are suboptimal as compared to in vivo. Previous next-generation sequencing analysis of in vivo fertilized and in vitro cultured (IVC) or in vivo cultured (IVV) porcine blastocyst stage embryos identified an arginine transporter (SLC7A1) expressed 63 fold higher in IVC compared to IVV blastocysts (Bauer et al. 2010 Biol. Reprod. Epub ahead of print). Arginine catabolism may play important roles in placental and conceptus growth and development as it is a substrate for synthesis of nitric oxide synthase and polyamines. The objective of this study was to determine the effects arginine had on both embryo development and mRNA expression in in vitro fertilized embryos. Cumulus–oocyte complexes were matured for 44 h in M199 supplemented with EGF, FSH, and LH. Oocytes with a visible polar body (metaphase II) were selected and fertilized in modified Tris Buffered Medium for 5 h and then placed into one of 5 treatment groups (Porcine Zygote Medium 3 (PZM3) with 0 mM, 0.12 mM (current concentration of arginine in PZM3), 0.36 mM, 0.72 mM, or 1.69 mM arginine). Twenty-eight hours post-fertilization, cleaved embryos were selected and moved into 25 μL drops of respective culture media and cultured to day 6 in 5% CO2, 5% O2, 90% N2 at 38.5°C. To determine the effect arginine had on development the percent of embryos that made it to the blastocyst stage for each treatment group were analysed using PROC GLM in SAS (SAS Institute, Cary, NC). A least significant difference post test comparison was completed to determine if significant differences existed between treatment groups (a,b,cP < 0.05). The percentage of cleaved embryos on Day 6 that developed to blastocyst was 57.2%b,c, 50.2%c, 67.3%a,b, 67.3%a,b, 70.4%a (N = 147, 163, 150, 120, and 134) in 0 mM, 0.12 mM, 0.36 mM, 0.72 mM, and 1.69 mM arginine, respectively. Real-time PCR was then completed to assess the affect arginine supplementation had on SLC7A1 mRNA expression. Three biological replicates, each containing 10 blastocyst pools to ensure enough starting material, were collected for each treatment group. RNA was isolated from each sample and 5 μL was linearly amplified (NuGEN Ovation Pico WTA System) so multiple genes could be compared and then purified using Bio-Rad MicroSpin Columns. Expression levels were calculated relative to the reference sample and the housekeeping gene, YWHAG. The ΔΔCT values were log-transformed and analysed using PROC GLM in SAS. The expression of SLC7A1 mRNA was decreased (P = 0.0006) compared to PZM3 in the 1.69 mM arginine group. These results illustrate the positive effects that additional arginine may be having on porcine embryo development during culture from the 2-cell to the blastocyst stage. Supplementing arginine to a final concentration of 1.69 mM during culture increases development of porcine embryos to blastocyst compared to PZM3 and also decreases the expression of SLC7A1. Evaluation of the transcriptional profile appears to be a good method of letting the embryo tell us what it needs for development, and in this case arginine. Funded by F21C.

Smoking is one of the habits that can affect human health. One type of cigarette that is currently the choice is electric cigarettes. The e-cigarette content includes nicotine, propylene glycol, flavorings and glycerin. The purpose of this study was to determine the effect of exposure to e-cigarette smoke on mice strain balb / c with the parameters of the number of spermatozoa. This study used 15 male mice which were 8-10 weeks old and weighed 25-30 grams of male mice which were divided into 3 treatment groups namely control group, treatment group one and treatment group two. In the control group no exposure to cigarette smoke was carried out while in the treatment group one was exposed to cigarette smoke in mice with a dose of 0 mg nicotine and the second treatment group was exposed to e-cigarette smoke at a dose of 18 mg nicotine. Electric cigarette smoke exposure is carried out for 30 days using a suction device from a syringe. The exposure of cigarette smoke is carried out every day with successive doses of 20 times as much as 3 times with a time lag of 10 minutes (5 minutes the chamber is closed and 5 minutes of the chamber is opened). The results showed that the average number of spermatozoa in the control group was 16.816 million / ml. The average number of spermatozoa in treatment one was 10.432 million / ml and the average number of spermatozoa in treatment two was 5.234 million / ml. This shows that exposure to cigarette smoke can reduce the number of spermatozoa from the control group to the first treatment by 42.56% and the second treatment shows a decrease from the first group by 34.65%.

Background: Distal radius fracture is a common type of bone fracture in patients of all ages. Most of the complications of this type of fracture are related to intra-articular fractures. Aim: This study aimed to compare the treatment results of distal radius fracture with two closed pinning and plate plastering methods in young patients admitted to Imam Khomeini Hospital in Jiroft in 2019. Method: The sample size was 176 patients with distal radius fractures admitted to Imam Khomeini Hospital in Jiroft in 2019. The patients were randomly divided into two treatment groups: (1) closed reduction and percutaneous pinning, and (2) plastering with plates. Finally, patients were compared in terms of the mentioned variables immediately after surgery and three months after that. The collected data were coded, entered, and analyzed by using SPSS. Results: The mean age of the selected patients in the closed pinning treatment group was 27.32 years and in the plate plastering group was 27.72 years. The minimum age of patients was four years, and the maximum was 59 years. Patients in the plate plastering treatment group seemed to have a higher mean age than those in the closed pinning treatment group, but this difference is not statistically significant ( [Formula: see text]). There was no statistically significant difference between men’s and women’s ratio in terms of being in the two treatment groups ( [Formula: see text]). There was no statistically significant difference between the right hand and left dominant hand ratio in the two treatment groups ( [Formula: see text]). There was a statistically significant difference between the ability and disability ratio to perform the activity in the two treatment groups ( [Formula: see text]). There was no significant difference between the subjective ratio of no pain, minor pain, and occasional pain in the two treatment groups ( [Formula: see text]). There was a significant difference between the ratio of movement limitation less than 20%, limitation 20–50%, and limitation more than 50% in the two treatment groups ( [Formula: see text]). There was a significant difference between the ratio of normal wrist strength, strength 30–60, and strength 60–90 in the two treatment groups ( [Formula: see text]). There was no statistically significant difference between the uncomplicated ratio and partial crepitation in terms of placement in the two treatment groups ( [Formula: see text]). Finally, there was no statistically significant difference between the total score ratio, closed pinning, and plate plastering treatment groups ( [Formula: see text]). Conclusion: According to this study results, there is no difference between the two treatment methods of closed pinning and plastering with a plate. However, the pinning treatment group had the highest frequency of excellent scores than the plate plastering method.

BACKGROUND Currently, there are no specific effective treatments for SARS-CoV-2 infection; however, various COVID-19 treatment options are under investigation. It is vital to continue investigating the landscape of SARS-CoV-2–induced pneumonia and therapeutic interventions. OBJECTIVE This paper presents the protocol for a randomized controlled trial that aims to compare the pneumonia exacerbation rate between ciclesonide (ALVESCO; Teijin Pharma Limited) administration and symptomatic treatment in patients with COVID-19 and to determine the efficacy of ciclesonide. The secondary objectives are to investigate the safety of ciclesonide administration, changes in clinical and laboratory findings, and the number of viral genome copies of SARS-CoV-2 over time between the 2 groups. METHODS In this investigator-initiated, exploratory, prospective, multicenter, parallel-group, open-label, randomized controlled trial, a total of 90 patients diagnosed with COVID-19 will be recruited from 21 hospitals in Japan based on specific inclusion and exclusion criteria. Participants will be randomized either to the ciclesonide group, which will receive a 400-µg dose of ciclesonide 3 times per day over a 7-day period, or to the symptomatic treatment group. Both groups will receive antitussives and antipyretics as required. Data collection for various parameters will be conducted on days 1, 2, 4, 8, 22, and 29 to record baseline assessments and the findings over an extended period. Computed tomography images taken prior to drug administration and 1 week following treatment will be compared, and efficacy will be confirmed by checking for pneumonia exacerbation. Primary endpoint analysis will be performed using the Fisher exact test to determine statistically significant differences in the pneumonia exacerbation rate between the ciclesonide and symptomatic treatment groups. RESULTS The first trial participant was enrolled on April 3, 2020. Recruitment is expected to be completed on September 30, 2020, while follow-up assessments of all participants are expected to be completed by October 31, 2020. The study results will be published in a peer-reviewed scientific journal. CONCLUSIONS The RACCO (Randomized Ciclesonid COVID-19) study will provide definitive comparative effectiveness data and important clinical outcomes data between the ciclesonide and symptomatic treatment groups. If the hypotheses that pneumonia exacerbation rate reduction is more significant in the ciclesonide treatment group than in the symptomatic treatment group and that ciclesonide is safe for use are valid, ciclesonide will serve as an important therapeutic option for patients with COVID-19. The main scope of this study was to evaluate the likelihood of a helper effect of agar-embedded cleaved embryos on a low number of free embryos at a similar stage of development within the same culture droplet. Such an improved system could be beneficial within ovum pickup/in vitro embryo production (OPU/IVEP) combined protocols whenever a low number of OPU-derived cleaved embryos are produced per donor. Oocytes were recovered from abattoir ovaries, and after in vitro maturation (IVM) and in vitro fertilization (IVF), presumptive zygotes were deprived of cumulus investment and allocated into culture droplets for 24 h. At 48 h from IVF, 4- to 8-cell cleaved embryos were randomly allocated into a control and a treatment group. Control groups consisted of 1, 3, 5, and 10 embryos, respectively, in 50-μL droplets. Treatment groups consisted of 1, 3, and 5 free embryos with the addition of 9, 7, and 5 embryos, respectively, at a similar stage of development embedded in agar chips, so as to reach a total number of 10 cleaved embryos in each culture droplet. Culture was performed for both the control and treatment groups in SOF medium droplets covered with mineral oil, with the supplementation of essential and nonessential amino acids in a controlled gas atmosphere consisting of 5% CO2, 7% O2, and 88% N2 at 39°C. Final embryo output was checked at Day 7 from IVF. When considering only free embryos, the difference in progression to blastocyst development was highly significant between the control and treatment groups: 1) group 1 v. 1 + 9: 6.6 v. 84.3% (P = 0.00000); 2) group 3 v. 3 + 7: 11.1 v. 41.3% (P = 0.00001); 3) group 5 v. 5 + 5: 24.4 v. 42.2% (P = 0.00001). Rate of blastocyst development in the control group containing 10 cleaved embryos was not significantly different from free cleaved embryos in the 3 + 7 (39.2 v. 41.3%, P = 0.71) and 5 + 5 treatment groups (39.2 v. 42.2%, P = 0.54), but was significantly lower when compared with the 1 + 9 treatment group (39.2 v. 84.3%, P = 0.000). For 1, 3, 5, and 10 control group embryos, the numbers of replicates and total cleaved embryos used (n) were 30 (n = 30), 27, (n = 81), 27 (n = 135), and 39 (n = 390), respectively. For the 1 + 9, 3 + 7, and 5 + 5 treatment group embryos, the numbers of replicates and total cleaved embryos used were 32 (n = 32), 29 (n = 87), and 27 (n = 135), respectively. In conclusion, a beneficial effect of agar-embedded embryos on the development of free embryos within the same culture droplet was shown. A striking improvement in late-stage embryo development was particularly evident when considering the 1 v. 1 + 9 control and treatment groups. These results may foster a different strategic approach in in vitro culture to enhance embryo development from highly valuable donors.

Abstract Abstract 4753 Introduction For many patients with sickle cell anemia (SCA), jaundice, yellow discoloration of the skin and mucous membranes, may be the most visible manifestation of their disease. The degree to which jaundice impacts the daily lives of patients with SCA has not been previously described. This study aims to assess the effect of jaundice on the health-related quality of life (HRQOL) of adults living with SCA. Methods This study was a cross-sectional survey of patients with SCA. A convenience sample of patients age > 17 years with SCA were approached during routine clinic visits to Parkland Memorial Hospital or UT Southwestern University Hospitals. Consenting patients completed a 15 question survey instrument. The survey was divided into three subscales (personal, relational, and behavioral) where individual items were presented in a 5-point Likert scale. This instrument was reviewed by non-study hematologists, nurses and patients for face validity. No other assessment of reliability or validity was performed. Additional data collected included serum total bilirubin (TB), hemoglobin, reticulocyte count (%) and patient self-report of current treatment for SCA (hydroxyurea [HU], chronic blood transfusions, or other). We tested for associations between survey subscales and TB, reticulocyte count, or hemoglobin using Spearman correlation coefficients. We compared the Spearman correlations of the survey subscales and TB between treatment and non-treatment groups using the Fisher's z transformation. Gender and age adjustments were made using linear regression. TB was transformed by natural log for regression analysis. Results We surveyed 100 subjects (58% female) with SCA. The median age was 25 years (range: 18–63), and the median TB was 2.5 mg/dL (range: 0.4–13.8). Nearly half of patients reported receiving treatment with 28% on HU and 14% on chronic transfusions. The majority of patients reported a history of jaundice, with 79% listing responses of ‘sometimes’, ‘often’, or ‘always’. Summary results for the remainder of the survey are listed in the table. TB was positively correlated with the 3 subscales (personal, r=0.32, p=0.002; relational, r=0.38, p=0.0002; behavioral, r=0.31, p=0.003). We compared these correlations between ‘treatment’ and ‘no treatment’ groups and observed strong correlations in the ‘no treatment’ group (personal, r=0.55, p<0.0001; relational, r=0.63, p<0.0001; behavioral, r=0.52, p=0.0002). TB was not correlated with the subscales in the ‘treatment’ group. After adjusting for age, gender, and disease-modifying therapy, the three subscales were associated with TB (personal, β = 0.066, 95% Confidence Interval (CI) [0.027, 0.104], p=0.001; relational, β = 0.061, 95% CI [0.031, 0.092], p=0.0001; behavioral, β = 0.104, 95% CI [0.045, 0.164], p=0.0009). Conclusion Jaundice negatively impacts the lives of many adults with SCA. The personal and relational subscales suggest more impact on the self-image of respondents, but only a few reported behavioral changes. The impact of jaundice appears to be mitigated by disease-modifying therapy for SCA (HU or chronic transfusions); however, this study was not created to determine the effectiveness of these reported treatments. Despite the fact this instrument was not validated, our study results suggest jaundice should be well-represented in HRQOL assessment tools in adults with SCA. Moreover, prospective studies are needed to clarify potential benefits of disease-modifying therapies, particularly HU, on the burden of jaundice in SCA. Tanaman Suruhan (Peperomia pellucida (L.,) Kunth.) merupakan tanaman obat yang memiliki senyawa metabolit sekunder seperti alkaloid, flavonoid, saponin, tanin, dan steroid. Berbagai senyawa kimia tersebut yang berpotensi sebagai obat tetapi perlu diperhatikan kemungkinan adanya efek samping terhadap organisme khususnya pada masa kehamilan. Penelitian ini bertujuan menguji efek teratogenik ekstrak tanaman suruhan terhadap fetus mencit (Mus musculus L.), meliputi mortalitas dan resorpsi. Penelitian ini menggunakan metode Rancangan Acak Lengkap (RAL) yang terdiri dari 4 perlakuan dengan  20 ekor mencit betina dibagi ke dalam 4 kelompok yaitu: K(+) (Aquabides), P1 diberi ekstrak tanaman suruhan dengan dosis (1,68 mg/g bb), P2 (3,36 mg/g bb), dan P3 (6,72 mg/g bb). Hasil penelitian terhadap persentase fetus yang mengalami mortalitas dan resorpsi fetus antara kontrol K(+) dan perlakuan dengan ekstrak etanol suruhan (P1, P2, dan P3) tidak menunjukkan perbedaan yang bermakna secara statistik berdasarkan uji ANOVA satu faktor (p value 0,418). Hasil penelitian yang didapat menunjukkan bahwa ekstrak etanol tanaman suruhan tidak menyebabkan mortalitas pada fetus mencit, namun menyebabkan resorpsi pada fetus mencit pada pemberian dosis sebesar 1,62 mg/g BB, 3,36 mg/g BB, dan 6,72 mg/g BB.   Pepper elder (Peperomia pellucida (L.,) Kunth.) is a medicinal plant that has secondary metabolites such as alkaloids, flavonoids, saponins, tannins, and steroids. The side effects for organisms of those chemical compounds, which are potentially beneficial for their medicinal use, still need to be considered especially in pregnancy. This study aims to determine the teratogenic effects of pepper elder extract on mortality and resorption of mice (Mus musculus L.) fetus. This study uses total random sampling design with 4 treatments consisting of 20 female mice divided into 4 groups: the control group (K(+)) which is given Aqua distillation and the treatment groups which are all given the pepper elder extract with different doses, where the first treatment group (P1) is given 1.68 mg/g body weight of the extract, the second treatment group (P2) is given 3.36 mg/g body weight of the extract, and the third treatment group (P3) is given 6.72 mg/g body weight of the extract. The results obtained of mortality and resorption percentage of mice fetus between the control group (K(+)) and the treatment groups (P1, P2, and P3) showed that there is no statistically meaningful difference based on one-way ANOVA test (p value 0,418). This study showed that ethanol extract of pepper elder causes no mortality in mice fetus, yet it causes resorption on mice fetus at given doses of 1,62 mg/g body weight, 3,36 mg/g body weight, dan 6,72 mg/g body weight.

Background: Commercial broiler is a rapidly growing sector in Bangladesh. The broilers are supplied with the balanced ration prepared from a number of ingredients which are not available uniformly throughout the years. On the other hands, manufactured feeds are costly and inconvenient for the rural farmers. This study is undertaken to use unconventional feed ingredients aiming to decrease the broiler production cost. Method: A total of ninety-nine day- old broiler chicks (Hubbard) of either sex were used in this study in 3 treatment groups to assess processed unconventional feed item (shoti, blood meal, poultry dropping and molasses) on growth and profitability upon rearing for 60 days. Broilers were reared in dip litter system and were fed ad libitum on conventional and unconventional diet dividing into 3 treatment diets namely, treatment diet 1 (T1: Conventional as control), treatment diet 2 (T2: unconventional with blood meal and shoti), and treatment diet 3 (T3: unconventional with blood meal, shoti, poultry droppings and molasses) throughout the trial period. Treatment group 1 (T1) was used as control. Results: Broilers fed on control diet (conventional feed) achieved higher (P<0.01) body weight, while the broilers offered diets with the highest amount of unconventional feed had the lowest body weight. Increased (P<0.01) feed intake was observed during 21days and 49 days of age when broilers fed diets with the supplementation, but no significant differences were found among the groups in terms of feed consumption during 33 day of age. FCR differed significantly (P<.01) throughout the trial period with the broilers fed diets without supplementation of unconventional feed item had the superior FCR than the others. Mortality rate was 6.06%,9.09%, and 9.09% in treatment group I, treatment group II, and treatment group III respectively while live weights, feed conversion and feed consumption was unaffected by all the dietary groups with/without incorporation of unconventional feed item up to day 60 days of age. Statistically significant (P<0.01) decreased live weight was observed among the treatment groups in 60-day-old birds. Productivity and cost-benefit analysis were performed. Conclusion: Use of unconventional feed ingredients in broiler production greatly reduces the feed cost with little hampering the growth rate and hence, it is profitable for the farmers.

The current study explored the differences in the effectiveness of first and second generation long-acting injections and orally administered antipsychotics in reducing the rehospitalization rate among patients with schizophrenia receiving home care services in a medical center in Southern Taiwan. Longitudinal data between 1 January 2006, and 31 December 2015, were collected retrospectively. Patients were classified into three treatment groups: First generation antipsychotic (FGA) long-acting injection (LAI), second generation antipsychotic long-acting injection (SGA) (LAI), and oral antipsychotics. The primary outcomes were the rehospitalization rate and the follow-up time (duration of receiving home care services) until psychiatric rehospitalization. A total of 78 patients with schizophrenia were recruited. The average observation time was about 40 months. The oral treatment group tended to be older with a higher number of female patients and a lower level of education. The FGA treatment group tended to have a higher frequency and duration of hospitalization before receiving home care services. We found no significant differences in the follow-up time or psychiatric rehospitalization rate after receiving home care services among the three treatment groups. We propose that oral and LAI antipsychotics were equally effective when patients received home care services. Our results can serve as a reference for the choice of treatment for patients with schizophrenia in a home care program.
