ANALYSIS OF PREVALENCE AND RISK FACTORS OF SOCIAL ANXIETY AND EATING DISORDERS IN MEDICAL STUDENTS

В современной литературе широко обсуждается проблема коморбидности тревожных расстройств и расстройств пищевого поведения. Однако, на сегодняшний день тематика коморбидности социальной тревоги и расстройств пищевого поведения среди студентов, в частности, студентов медицинских ВУЗов, остается малоосвещенной. Целью данного исследования послужил анализ данных литературы последних лет (2011 год и позднее), посвященных проблематике коморбидности пищевых расстройств и социальной тревоги среди обучающихся высшей медицинской школы. Особую значимость представляют работы по выявления предикторов развития и персистирования социальной фобии и пищевых нарушений среди студентов медицинских ВУЗов, поскольку специфика и сложность образовательной программы способствует формированию тревожных расстройств и пищевых нарушений, более того, социальные страхи и фобии приводят к ухудшению академической успеваемости обучающиеся. Помимо этого, студенты медицинского направления сами в будущем должны будут оказывать помощь пациентам, обеспечивая здоровье нации, следовательно, студентам высшей медицинской школы важно иметь как соматическое, так и психическое здоровье The problem of comorbidity of anxiety disorders and eating disorders is widely discussed in the modern literature. However, to date, the topic of comorbidity of social anxiety and eating disorders among medical students remains poorly uncovered. The purpose of this study is to analyze the literature data of recent years (2011 and later) on the problems of comorbidity of eating disorders and social anxiety among students of higher medical school. Of particular importance is the work on identifying predictors of the development and persistence of social phobia and eating disorders among medical students, since the specificity and complexity of the educational program contributes to the formation of anxiety disorders and eating disorders. Moreover, social fears and phobias lead to a deterioration in academic performance of students. In addition, medical students themselves in the future will have to provide assistance to patients, ensuring the health of the nation, therefore, it is important for students of higher medical school to have both somatic and mental health

Introduction: As per research studies have shown that social anxiety disorder has a worldwide prevalence of 5 to 10% and a lifetime prevalence rate of 8.4% to 15% among high school students.1 Rural Indian children are facing this problem of lethal evil more as compared to Urban high school students in India. Objective: To assess the prevalence of social anxiety disorder [sad] and its determinants among high school students of selected high school of Bagalakote. Methods: Descriptive survey approach was used for the study with cross sectional survey design. 120 high school students between 14 years to 16 years of age were selected Disproportionate stratified random sampling technique method from high school students studying in selected high school of Bagalkot. The data regarding determinants was collected by structured interview schedule and self-report method. The Social Phobia Inventory (abbreviated as SPIN) is a five point scale was used to categories the high school students according to their social anxiety disorders. The association was explored by Chi square test, Fisher’s exact probability test and linear regression. Results: Total 120 high school students were responded for Social Phobia Inventory (SPIN) scale, in that level of social anxiety disorders had reveals that majority 83.3% had non phobic, 15.8% had mild and 0.8% had moderate, there is no extremely phobic. A significant association was found between the social anxiety disorders of adolescents and their selected socio-demographic variables like monthly income (χ2= 21.199; P<0.05). Conclusion: The findings revealed that there was a positive correlation found between the psychological wellbeing and academic performance among adolescents, recommended health education, early recommended understanding the social anxiety disorder and its determinants by their students especially among the fresher`s. Effectiveness of teaching programmes regarding impact of its determinants on level of social anxiety disorders among adolescents can be done had positive effect on adolescents health, which were also statistically significant. ABSTRACT Objective Social anxiety disorder (SAD) and body image dissatisfaction (BID) are common problems among college students, but few studies focused on medical students. We aimed to estimate the prevalence, severity and correlates of SAD symptoms and BID among medical students of a Brazilian public university. Methods A cross-sectional study with 479 students, using structured instruments: Social Phobia Inventory (SPIN), Body Shape Questionnaire (BSQ), and Beck Depression Inventory (BDI). Bivariate analyses were followed by logistic regression models to obtain independent predictors of SAD symptoms, BID and both outcomes combined. Results Most students were single (99%) and female (58.7%). The prevalence rates of SAD symptoms (SPIN ≥ 19) and BID (BSQ ≥ 81) were 36.3% and 34.7%, respectively. Depressive symptoms (BDI ≥ 19) occurred in 8.8% of the sample. SAD symptoms were independently associated with: BID, thoughts of abandoning the course, difficulty making friends, depressive symptoms, and mental health treatment prior to university. Besides SAD symptoms, BID was associated with female sex, difficulty making friends, depressive symptoms, and body mass index (BMI). Seventy-eight students (16.3%) presented SAD symptoms and BID, which was associated with female sex, difficulty making friends, dissatisfaction with the course, depressive symptoms and BMI. Conclusion SAD symptoms and BID are common and related problems that should be screened for among medical students. The identification of specific correlates could contribute to the elaboration of preventive measures, minimizing the distress and negative impact of these mental health problems on relationships and academic performance.

Perfectionism is the refusal to accept any standard short of perfect. Perfectionist students might strive for ‘faultless performance, meticulous attention to detail and high levels of competency' (Humphris & Kaney, 1998). In a field as complex and advanced as medicine, faultless performance is unrealistically expected. This chapter will discuss how the rigours of medical school promote perfectionistic behaviours in students. What is perfectionism? How does perfectionism in medical school affect students? How can educators and administrators identify perfectionism, provide psychoeducation, or address maladaptive perfectionism? How can students address perfectionism? It is essential to recognise perfectionism in medical students as research has shown that maladaptive perfectionism has been correlated to physician suicide (Scutti, 2014). The chapter concludes with recommendations on how students and medical educators can employ strategies to promote sound mental health and wellbeing during medical school.

ABSTRACT The inclusion, adherence and success of low-income undergraduates are important goals in Brazilian higher education, as well as in other parts of the world, especially in busy and full-time courses such as medicine. This paper analyzes the performance of undergraduate medical students by comparing two groups: those who applied for and received a scholarship during the academic years (scholarship holders) and the others (without scholarship). We analyzed data from 417 medical students who graduated between 2010 and 2013, corresponding to four years of a retrospective cohort at a free public university in Brazil. Statistical analysis was performed using Fisher’s exact test, Student’s t test, Mann-Whitney test, and linear regression to compare the scores of these groups in the sixth and twelfth semesters (middle and final semesters) and in the admission exam for medical residency programs, consisting of: total score, multiple choice test for knowledge assessment, simulated structured clinical assessment, interview and written questions. The independent variable was to receive a scholarship, while the control variables were age, socioeconomic strata, extra gratuities for high school in public institution and self-declaration of race, score in the vestibular entry exam (general and in each area assessed) and parents’ level of education. A total of 243 students (58.2%) received a scholarship, most of them as a scientific initiation grant (217 or 89.3%), while 10.7% received social assistance, the average income per capita was about 16% lower among students who received a scholarship (p = 0.01) compared to those who did not. Scholarship recipients achieved better academic performance in the sixth (p<0.01) and in the twelfth (p<0.01) semester, but not in admission to medical residency programs. Good performance was independent of age, race, receipt of bonuses for admission to medical school, and educational background of their parents. Therefore, we conclude that receiving a scholarship at the undergraduate level was associated tobetter student performance during the undergraduate medical course. It is important to emphasize the importance of reinforcing similar programs, especially to help support students who are most vulnerable socioeconomically.

Abstract Aims Compared to the general population, adoptees are more often referred to specialist psychiatric treatment, exhibit increased risk of suicide and display more symptoms of attention-deficit/hyperactivity-disorder. However, little is known about the impact of being an adoptee on the risk of developing an eating disorder. The aim of the present study was to assess whether international adoptees have a higher risk for eating disorders than native Swedes. Methods In the present retrospective cohort study, data from the Swedish total population registers on individuals born between 1979 and 2005 were used to assess whether international adoptees residing in Sweden (n = 25 287) have a higher risk for anorexia nervosa (AN) and other eating disorders (OED) than non-adoptees with Swedish-born parents from the general population (n = 2 046 835). The patterns of these results were compared to those for major depressive disorder (MDD), obsessive-compulsive disorder (OCD), and anxiety disorders to determine whether any observed effects were unique to eating disorders or reflected a more general impact on mental health outcomes. Results A survival analysis adjusting for relevant demographic covariates revealed an elevated risk of all examined psychiatric disorders in international adoptees: hazard ratios (95% confidence intervals) are 1.21 (1.04–1.41) for AN, 1.60 (1.44–1.79) for OED, 1.90 (1.81–2.00) for MDD, 1.25 (1.09–1.44) for OCD, and 1.69 (1.60–1.78) for anxiety disorders. Conclusions Elevated risk of eating disorders as well as of MDD, OCD, and anxiety disorders was found in international adoptees. A parallel pattern between AN and OCD was observed, which both display less elevated rates than the other diagnoses. A considerable number of biological, environmental, and societal factors have been suggested to explain the observed differences in mental health between adoptees and non-adoptees, but they remain primarily theoretical.

Medical students experience higher rates of mental illness than the general population. With competition rising for success in medical school, and residency, increasing incidence of distress are leading this population to experience higher rates of thoughts of dropping out of school, and even suicide. Since many stigmas deter medical students from receiving mental health counseling, such as the perceived inability to handle the stresses of medical school, and the potential lack of competitiveness for residencies if reported, prevention of mental illness may be a better course to take in reducing prevalence in this population. Regular exercise has demonstrated a positive effect on not only promoting physical health, but also mental health. Exercise encourages a healthy mood, positive self esteem, and better cognition, while decreasing the chances of depression, anxiety, and burnout. Implementing exercise time into medical school curriculums, just like the basic sciences, albeit for less time in the day, could provide a feasible way to ensure that all students are taking time to partake in this important activity for their well being. Though medical schools are rigid with attempts to make changes in their curriculum, thirty minutes a day, three to five times a week of exercise of the students’ choice not only is more cost effective than counseling, but it also reduces the chances that they will experience burnout, which if left untreated could transcend into a compromised training experience.

Background: Social anxiety disorder (social phobia) is a type of anxiety disorder which is characterized by significant anxiety and discomfort about being embarrassed, humiliated, rejected, or looked down on in social interactions. Although it affects about 30% of adults worldwide at some point in their lives, lifetime social anxiety disorder affects only about 4% of the world population. People with this disorder experience extreme fear of social interactions (e.g., public speaking and meeting new people). This anxiety affects daily functions and lasts at least six months. They may also experience strong physical symptoms like rapid heart rate, nausea, vomiting, and full-blown attacks. Social phobia can be treated by a combination of psychotherapy and medical treatment (e.g., anti-anxiety, antidepressants, and beta-blockers). Methods: This study was conducted using the Arabic SPIN and a group of questions to assess the associated factors, complications, and sociodemographic determinate of social anxiety disorder and included a total of 375 medical students from different universities and educational years. Results: The overall prevalence of social anxiety disorder among our participants was 61.3%, of which 19.2% had mild, 21.6% moderate, 10.9% severe, and 9.6% had very severe SAD. There was a significant difference regarding self-esteem, academic achievement, and drug addiction between students with social phobia and students with no social phobia. Conclusion: Social phobia is quite prevalent among Sudanese medical students, particularly the severe form of the disorder with no significant gender differences. It seems to affect self-esteem and academic achievement and can be associated with drug addiction.

Background Epidemiological studies indicate that social anxiety disorder is one of the most common mental health disorders. However, despite the prevalence of social anxiety disorder, a large amount of information, the possibilities of psychotherapy and medical treatment, many patients for various reasons do not receive or do not seek help. Aim Generalization of actual knowledge and research on the aetiology and pathogenetic mechanisms of social phobias and coverage of the actual issues of low referral of people suffering from social phobia Methods For review, the following databases, such as ScienceDirect, ResearchGate, PubMed and Google Scholar, were used. The search was performed using the keywords: social anxiety disorder, sociophobia, social anxiety, cognitive-behavioral model, neurobiology, mental health. Results The general information about social anxiety disorder, its prevalence and its consequences were covered. The main etiological mechanisms, modern views on the neurobiological and psychological basis of the disorder are considered. In addition, the peculiarities of the clinical picture and its influence on the social functioning of the individual, including the referral of help, were analyzed. The aspects that are useful to consider during the development of recommendations for specialists in general medical practice and centers of public mental health were suggested. Conclusion A social anxiety disorder should be considered as a complex mental health disorder. Recognition of signs of social anxiety disorder in their component often leads to a false interpretation of clinical signs as manifestations of depression or other neurotic disorders among primary care professionals. Informing general practitioners and specialists of public mental health centers about the traits of the clinical picture and the social functioning of patients with this disorder can help to overcome the stigma and improve the referral of qualified assistance.
