The aim: To study and analyze the dynamics of women’s reproductive health in Ukraine as a factor in population reproduction and to study the features of menstrual disorders in Ukraine in 2010-2019. In women of reproductive age with hyperproliferative pathology of the endometrium without atypia. Materials and methods: A descriptive epidemiological study was conducted using data from the departmental statistical reporting of the Ministry of Health of Ukraine on the number of cases of gynecological pathology for 2010–2019. The peculiarities of menstrual function in patients of reproductive age with atypical hyperproliferative pathology of the endometrium were studied. We examined 84 patients of the gynecological department of the Sumy Regional Clinical Perinatal Center with a diagnosis of hyperproliferative pathology of the endometrium without atypia, who sought medical help during 2017-2020 for hyperplastic processes of the endometrium in reproductive age (18 to 49 years). All patients were divided into three groups, up to group I women with endometrial hyperplasia without atypia (HGE) (n = 30); Group II – patients diagnosed with endometrial polyps (n = 30); group III included patients with combined hyperproliferative pathology (uterine body polyps and endometrial hyperplasia without atypia) (n = 24). Results: As a result of the conducted systematic analysis it was established: the higher the prevalence of PMC, the higher the frequency of diseases of the genitourinary system and some gynecological diseases. This is confirmed by the calculated coefficients of correlation of the prevalence of PMC with the frequency of diseases of the genitourinary system (r = 0.75, p<0.001), salpingitis (r = 0.63, p<0.001) and endometriosis (r = 0.42, p<0, 05). The assessment of the relative risk of gynecological diseases has shown that the greatest attention needs to be paid to improving the diagnosis of infertility and endometriosis, as well as the prevention and treatment of salpingitis and uterine cancer. When women with menstrual disorders go to the gynecologist, it is necessary to pay attention to the presence of hyperproliferative pathology, and if women have other chronic diseases, offer rehabilitation of these foci, which can further have a positive effect on reproductive health. Conclusions: The highest rates of menstrual irregularities were found in women with endometrial hyperplasia without atypia and a combination diffuse hyperplasia with endometrial polyps in contrast to patients with only endometrial polyps. Menstrual irregularities can be considered as a marker and indicator of dysfunction of the genitourinary system and the presence of a hyperproliferative process at the prehospital stage.

The peculiarities of benign proliferative pathology of endometrium including their combination in women of reproductive age are reviewed in the article.Materials and methods. The results of pathohistological research of benign proliferative pathology of endometrium (without atypia) were analyzed. Statistical data processing was performed by means of MedStat software package.Results. The obtained results revealed that benign proliferative pathology of endometrium is one of the most frequent gynaecological malignancies among female patients of reproductive age accounting for 52.2 % cases. Endometrial polyps were found to be accompanied by morphological peculiarities indicating chronic inflammatory process in endometrium in 56.5% cases (р<0.05) in comparison with endometrial hyperplasia in 38.2% cases, proving the presence of long-term inflammation in endometrial tissue and its trigger role in the development of the proliferative processes. Among patients with chronic salpingo-oophoritis, infertility was revealed in almost half of cases (44.5% of patients with endometrial polyps, 40.5% of patients with endometrial hyperplasia and 48.3% of women with combined proliferative pathology of endometrium) clinically confirming the data of morphological research. Peculiar signs of proliferative processes in genitals were determined, namely coexistence of uterine and endometrial pathology: endometrial hyperplasia was found in 40.4% of patients with uterine leiomyoma and 30.3% of patients with adenomyosis. The same combinations were peculiar for patients with endometrial polyps: endometrial hyperplasia was found in 30.1% of patients with uterine leiomyoma and 36.3% of patients with adenomyosis. Menstrual disorders were revealed in every third woman with endometrial hyperplasia (30.3%) and co-existent polyposis (30.2%).

The article presents the comparative analysis of the state of the cervix in women with endometrial polyps and micropolyps. Patients and methods. The study involved 130 patients aged 18-35 years: 70 patients with endometrial polyps (group I), 30 patients with micropolyps (group II) and 30 patients of the control group (group III). Results. According to the anamnesis of women in the I group were significantly more frequent diseases of the cervix, which corrected physical surgery methods, in particular cryodestruction. In group II, the representatives of these indicators were similar to healthy. Normal colposcopic picture met significantly less frequently in patients and I, and II group. The differences in the incidence of HPV high oncogenic risk in all groups were not statistically significant. Conclusion. Destructive methods used in the detection of any changes in the cervix are often overly aggressive, form scars and contributing to inflamaciones process. In the chain of events leading to the formation of PE, cervical pathology and its correction can take the basic place. Abnormal uterine bleeding (AUB) is one of the most common indications for hysteroscopy. Most of the AUB cases occur due to endometrial or myometrium pathology. Among it, endometrial polyps (EP) and chronic endometritis (CE) prevalent in reproductive age, while endometrial hyperplasia (EH) and EP dominate in perimenopause. It was determined that EP and CE are characterized with menorrhagia and metrorrhagia approximately equally, whereas EH reveals AUB with oligomenorrhoea. Verification of exact endometrial pathology by ultrasound examination is hindered, that results in deviations of ultrasound and histological diagnosis. The usage of ultrasound data and AUB’s characteristics may improve the diagnostic accuracy on preadmission period.

Objective. To analyze pathogenetic mechanisms underlying the development of endometrial hyperplasia in women of reproductive age. Patients and methods. We have examined 143 women of reproductive age with endometrial hyperplasia (EH). Study participants were divided into three groups: Group I included EH patients without atypia; Group II included patients with atypical hyperplasia of the endometrium; Group III (control group) comprised 56 women with abnormal uterine bleeding, in whom we excluded adenomyosis, uterine fibroids, endometrial hyperplasia, endometrial cancer, and iatrogenic causes of uterine bleeding. Genomic DNA was isolated using phenol-chloroform extraction. Real-time polymerase chain reaction (RT-PCR) was used to detect microRNA-210, -18a, -221, and -222. The detection of tumor pyruvate kinase M2 was performed using the ScheBo® Tumor M2-PK kit designed for quantitative assessment of this metabolic cancer marker in plasma and endometrial tissue samples. Results. Significant risk factors triggering the pathogenetic mechanism of EH development in reproductive age include extragenital disorders (obesity, thyroid diseases, diseases of the urinary system, hypertension) and gynecological diseases (pelvic inflammatory diseases, adenomyosis, benign breast dysplasia, uterine fibroids). Alterations affecting estrogen receptors lead to changes in microRNA messengers, which, in turn, affect target genes and cause changes in the adaptive abilities of the cell. Expression of pyruvate kinase M2 in this chain confirms proapoptotic changes in the cell and the risk of its atypia. Conclusion. The pathogenesis of EH is based on the following factors: polymorphism of the ERS1 and PRG genes, increased expression of miRNA-210, -18a, and -222, decreased expression of miRNA-221, and overexpression of pyruvate kinase M2. Background: Thyroid dysfunction is 10 times more common in women than in men. Hypothyroidism among women of reproductive age group is linked to menstrual irregularities, polycystic ovarian syndrome (PCOS), miscarriage and infertility. Several studies are available in the literature reporting regarding impact of hypothyroidism on one or the other isolated problems of reproductive health problems.Methods: This cross-sectional study is conducted including 290 participants having various reproductive health problems attending the gynaecology OPD of a rural based tertiary care hospital of Tamil Nadu during the year 2017-2018.These participants were investigated for thyroid dysfunction and its correlation of hypothyroidism with reproductive health problems viz. menstrual irregularities, Polycystic ovarian diseases (PCOS), infertility and pregnancy loss was studied.Results: It was observed that 80.6% of the 290 participants were euthyroid and 16.97% were hypothyroid, who presented with menstrual irregularities like menorrhagia (28.6%), oligomenorrhea (20.8%), amenorrhoea (21.3%), PCOS (28.6%), infertility (21.6%) and 23.8% hypothyroid women had pregnancy losses of varying number.Conclusions: In present study the occurrence of hypothyroidism was found to be 16.89%. Since hypothyroidism has close association with problems like menstrual irregularities, PCOS, miscarriages and infertility, thyroid function test should be routinely recommended for these women. Thyroid dysfunction can be corrected with simple, cost-effective treatment.  This will help in improvement in pregnancy outcome and also avoid subjecting women for major surgeries for menorrhagia.

Present study to evaluate and detect thyroid dysfunction and its correlation in patients with a menstrual disorder in the reproductive age group. Research objectives are to study the prevalence of thyroid disorders in relation to menstrual disorders and study the correlation between menstrual irregularities and thyroid dysfunction. Precise assessments of thyroid hormones in serum become achievable easily and accurately with the introduction of new hormographical research techniques. Hypothyroidism diagnosis is advantageous because it typically relieves the patient of all symptoms. Therefore, the assessment of thyroid activity forms an essential component for the evaluation of a female with menorrhagia or otherwise menstrual irregularities. The objective: to study the role of immunological mechanisms in the development of adenomyosis with endometrial hyperplasia in women of reproductive age and the phased introduction of an integrated hormonal and immunostimulating therapy. Patients and methods. The authors investigated the effect of hormone therapy on the immune system of women with adenomyosis and endometrial hyperplasia: We evaluated the influence of hormone therapy 135 women of reproductive age with adenomyosis in combination with endometrial hyperplasia: group i - 45 women using a progestogen (6 months); II – 45 women who took agonist of gonadotropin-relesing hormone (a-GnRH) (6 months) III – 45 women who received complex therapy involving a-GnRH during the first 6 months and immunomodulator intramuscularly every other day No. 20, with subsequent use of tablets of 0.15 g of 1 times a week (course – 6 months), then in the next 6 months was used progestogen in the second phase of the menstrual cycle from 16 to 25день. Evaluation of indicators of health status were performed after 3, 6 and 12 months of therapy. NK (natural killer) activity was estimated by the method of Hamaoka modification N.T. Rekowа. Results.We studied the role of immune system in the pathogenesis of adenomyosis, endometrial hyperplasia and endometrial and combined pathology of endo- and myometrium in women of reproductive age by assessing the cytotoxic activity of NK-cells, since this component determines its oversight role in the dynamics. Identified new pathogenic mechanisms in the development of benign pathology of the uterus due to the dysfunction of the immune system, especially expressed in terms of reduction of cytotoxic index of NK-cells. Conclusion. Developed a comprehensive phased hormone therapy for women of reproductive age with combined benign uterus pathology, including immunomodulator and the estimation of influence on parameters of immune status in the dynamics of observation. The article presents an analysis of literature data and the results of our own retrospective studies of the characteristics of the menstrual cycle in female combatants who have undergone concussion. Clinical and anamnestic factors have been identified that negatively affect the menstrual function of women of reproductive age, who received contusions during hostilities. The relationship between the state of mental health of women and menstrual irregularities has been established. Aim: to identify and evaluate changes in the menstrual function of women - veterans of reproductive age. Also, to establish the relationship between menstrual irregularities in women - veterans of reproductive age who have undergone contusion, and the state of their somatic and mental health.Materials and method: The research was conducted at the Department of Obstetrics, Gynecology and Reproductology of the National University of Health of Ukraine named after P.L. Shupika. To achieve this goal, a comprehensive clinical and laboratory examination was performed in 567 women of reproductive age (main group and comparison group) who participated in hostilities and suffered contusions, the average age of the subjects was 27.08 ± 4.23 years.The period of stay in the combat zone is 29.34 ± 9.21 months, the time from the moment of receiving a mild traumatic brain injury (contusion) is 18.8 ± 9.2 months. Group I - 399 fertile women who suffered contusions during the fighting with PKS. Group II - 168 women of childbearing age who suffered contusions during hostilities without PKS (mean age 32.21 ± 7.32 years).Results: The results of the studies revealed statistically significant deviations in the characteristics of the menstrual cycle of women who participated in hostilities who were injured with PKS, compared with women without PKS. It has been proven that women-viskovosluzhvits more often develop menstrual irregularities, namely a decrease or increase in the number, lengthening or shortening of the menstrual cycle by 7 days. Taking into account the revealed menstrual irregularities in the women under study, it can be argued that the detected changes are due to the influence of both craniocerebral trauma and stress factors associated with military service. This justifies the need to develop a program for monitoring and supporting the reproductive health of women-viyskovoservices, as well as medical and psychological rehabilitation of veterans, aimed at their full adaptation to a peaceful life.

Purpose of the study: to examine the morphological and immunohistochemical features of the endometrium in postmenopausal women with uterine bleeding, depending on body weight and metabolic syndrome.Materials and methods. The study included 114 women with postmenopausal uterine bleeding. Women were divided into 3 groups. Group I included 41 patients with overweight and obesity class I–II, group II – 43 women with metabolic syndrome, group III (control) consisted of 30 patients with normal body weight. Hysteroscopy with morphological analysis of biopsied material was performed for all patients of groups I and III and 41 patients of group II. Pathohistological and immunohistochemical analysis were used to diagnose endometrial pathology. The level of cell proliferation in the endometrium was assessed by immunohistochemical analysis using monoclonal antibodies for Ki-67 expression; antiapoptotic activity was determined by the Bcl-2 and COX-2 expression.Results. Endometrial pathology was diagnosed in 94.74% of women. Endometrial polyps in the structure of the identified pathologyprevailed in group I and control group III (82.93% and 83.33%, respectively), while endometrial polyps were detected in 25.59% of women in the group II. Endometrial hyperplasia was diagnosed in the vast majority (69.76%) in group II, hyperplasia without atypia was detected in 51.16% and endometrial hyperplasia with atypia in 18.60%.Conclusions. Endometrial hyperplasia value was higher in postmenopausal women with metabolic syndrome compared to patients with normal weight, overweight and obesity. There was an increase of Ki-67, Bcl-2 and COX-2 expression in the endometrial glands in women with metabolic syndrome in comparison to patients with normal weight and obesity, for which only increased Bcl-2 expression in the stroma was observed. This confirms the existing data on the predominance of the apoptosis inhibition over proliferation in the formation of endometrial diseases in the postmenopausal period and confirms the combination of hyperproliferative processes and apoptosis inhibition if there is metabolic syndrome.

The objective: to find out the level of verification of endometrial hyperplastic processes (GPE) in women with uterine infertility based on the comparison of ultrasound and pathomorphological studies. Materials and methods. 64 patients of reproductive age was performed ultrasonographic study and diagnostic endometrial biopsy. Results. Three groups of GPE have been identified: polyps – 33 cases, hyperplasia –15, combination of glandular hyperplasia with endometrial polyp – 16. The endometrial polyps included: glandular – seven cases, glandular-fibrous with an advantage of the glandular component – 13, and glandular-fibrous with the advantage of the stromal component – 13. The accuracy of the verification of glandular polyps was 82%. Glandular-fibrotic polyps with an advantage of the glandular component were diagnosed in 82%. Glandular-fibrous endometrial polyps with the advantage of the stromal component were the most difficult for ultrasonic verification. The accuracy of their diagnosis was 50%. Such characteristics as nodular form, intramural position, increased echogenicity and absence of inclusions were the basis for erroneous diagnosis of uterine fibromyomas. Endometrial hyperplasia was verified at 97%. The remaining 3% were histologically diagnosed with endometrial polyposis, which were not established by ultrasound, and were considered as hyperplasia of the endometrium. Ultrasound diagnosis of the combination of endometrial hyperplasia and polyps reached 77%. The polyps on the background of endometrial hyperplasia appeared as fibromyomatous nodes. The determining role in correct diagnosis was played by hysteroscopy. Conclusion. The comparison of ultrasound data and morphological evidence suggests high accuracy of GPE detection and morphological verification. However, in some cases GPE ultrasound does not precisely determine the nature of pathological changes. Therefore, the study of the pathomorphological features of remodeling of the endometrium in the GPE remains open and requires new promising approaches. One of them, based on certain pathomorphological changes in GPE, is the use of blood flow research, in particular transvaginal color doppler, which may allow to improve the accuracy of the diagnosis of GPE.
