Introduction: Latent TB is still a problem in Indonesia, the absence of symptoms in patients with latent TB makespatients feel they don't need treatment. Aim of study: The purpose of this study was to analyze the relationship oflatent TB incidence with TB contact patients. Method: This study uses research data on the role of gammainterferon as a marker of the effectiveness of the BCG vaccine, the sample in this study has a complete examinationrecord, especially IGRA examination, then the data are analyzed by the Chi squre method. The total sample was 421respondents. Results and Discussion: Based on the results of the study of female respondents was 265 (62.9%), withan age range of 21-40 were 186 (44.2%) respondents, Positive IGRA 123 (29.2%) respodents, patient contact withTB patients was 105 people, and no contact history with TB patients was 316 people. The prevalence of latent TBwas 29 percent, while respondents who have a history of contact with tuberculosis patients about 33.3 percent andwithout a contact history of 27.8 percent. Conclusion: From the results of the analysis, there was no significantrelationship between latent tuberculosis events and contact history with tuberculosis patients (P value 0.284).Therefore, further research needs to be done to include variables that have not been included in the beginning suchas economic factors, respondent nutrition, and conduct a re-examination to determine the current health status ofrespondents.

Premature birth is one of the most important factors in neonatal morbidity and mortality. Based on the survey shows an increase in the incidence of premature birth at dr. H. Chasan Boesoirie hospital in 2015-2017. The purpose of this study was to determine the relationship between the history of maternal complication and the incidence of premature birth at dr. H. Chasan Boesoirie hospital used an analytic correlation research design with a cross-sectional approach. The population is all multiparity and grand multiparity at dr. H. Chasan Boesoirie hospital on January-December 2018, the sampling technique used simple random sampling, with the total sample are 196 respondents. The results show that 66,3% do not have the history of maternal complication and 85,7% do not have an incidence of premature birth. Then, that were analyzed using Spearman-rank. The results show that p-value is 0,001 (p-value<α). The conclusion that there is a relationship between the history of maternal complication and the incidence of premature birth at dr. H. Chasan Boesoirie hospital. So, it is necessary to provide information and education to pregnant women in order to plan their pregnancy well and build a referral system with primary care facilities.

BACKGROUND: Malnutrition in chronic kidney disease (CKD) patients undergoing hemodialysis (HD) interferes with the natural and adaptive immune response, consequently, increasing the latent tuberculosis (TB) reactivation. AIM: This study therefore aims to determine the relationship between nutritional status and latent TB in routine HD, using interferon gamma release assays (IGRA), to screen for latent TB. METHODS AND STUDY DESIGN: This study has an analytical observation cross-sectional design, and was conducted on 120 CKD-HD patients aged 18 years and above, and has been undergoing HD twice weekly for over 3 months, without malignancy, human immunodeficiency virus/acquired immunodeficiency syndrome, history of TB, or radiological evidence at the HD Unit of the Dr. Hasan Sadikin Hospital, Bandung, Indonesia, between March and May 2020, and not currently receiving immunosuppressant or TB therapy. In addition, the age, gender, history of Bacille Calmette-Guerin vaccine, CKD etiology, length of HD, HD adequacy, TB contact history, number of family members, smoking status, body mass index, albumin, malnutrition inflammation score, triceps skinfold thickness (TST), biceps skinfold thickness, suprailiac skinfold thickness (SIST), mid-upper arm circumference (MAC), and normalized protein catabolic rate between positive and negative IGRA groups, of each patient, were determined. RESULTS: In this study, all the patients met the inclusion and exclusion criteria. Based on the IGRA test, 47 patients (39.17%) tested positive, and 68 (56.67%) tested negative, while the results for the remaining 5 (4.16%) were indeterminate. The malnutrition inflammation score (MIS) score with positive IGRA 23.3 (20.0–26.7) was discovered to differ insignificantly (p value of 0.252) from the negative counterpart 20.0 (16.7–28.4). Meanwhile, in the HD adequacy assessment based on urea reduction rate, a statistically significant difference (p = 0.042) occurred between the positive 70.45 (65.70–76.61) and negative 74.15 (70.71–77.33) IGRA groups. In the smoking status, the positive and negative IGRA were discovered to differ significantly (30 (63.8% vs. 28 (41.2% p = 0.017) OR 2.521 (1.172–5.425). However, in the history of contact with TB patients, the positive and negative IGRA did not differ significantly (4.3% vs. 11.8% p = 0.160). Furthermore, there was a significant difference in TST and MAC, between MIS >5 and MIS ≤5 (p < 0.05). CONCLUSION: The assessment of nutritional status level, TST, MAC, smoking status, and adequate HD is crucial for CKD patients with routine HD, as these factors present risks of latent TB.

The author avoids using the term master/Master, instead referring to the concept of a Teacher when describing the people who played a significant role in her life and who are the main protagonists of this article. These people are the Professors Anna Kutrzeba-Pojnarowa and Witold Dynowski, ethnologists, excellent lecturers and researchers; and also people full of passion, respect for the achievements of other scholars, fascinated with the history of the discipline they represent and the interrelation of “macro” and “micro” history. First and foremost, they are great PEOPLE. Reminiscing about encounters with them, the author presents the model relationship between a teacher and a pupil, close to her, which is not limited to passing knowledge. The relationship is based on kindness, openness to others, and motivating them to overcome their limitations, fears and difficulties. The skills of noticing the potential of others, encouraging them to develop, acceptance, keeping one’s spirits up, support and co-existence should also be mentioned. Another person who also played the role of Teacher, although she has never been designated as such by the author, is her Grandmother Janina Orzechowska. She was the one who shaped the author’s attitudes to life, her way of looking at other people, her passions and ways of being. She was the person whose selfless love expressed by her behavior, gestures and looks accompanied the author from an early age. The motif of the relationship is very explicit in the text. The author emphasizes that relationships are not always joyful and constructive. Sometimes they clip one’s wings, they hurt and take away the light. Relationships which are beautiful in the beginning can also change under the influence of an illness, of one person affecting the others. The author describes changes like that via the example of the relationship with someone who appears in the text as the anonymous Teacher. It was a person from whom the author received a bitter lesson in life, far from fairy tales with a happy end.

Introduction: In couples, one of whom is diagnosed with HIV, generally the partner is emphasized not to have unprotected sex. This includes the selection of contraceptive methods for women with HIV/AIDS to achieve their reproductive goals. Objective: to analyze the relationship between age, education level, and history of contraceptive use with the choice of contraceptive method in women with HIV/AIDS. Methods: This study uses a descriptive correlative design using Fisher test analysis with a total sample of 20 people who meet the criteria. Results: the relationship between age and current contraceptive use with a p value of 0.241, a history of contraceptive use and current contraceptive use with a p value of 1.00, and the level of knowledge with current contraceptive use with a p value of 0.122. Conclusion: there is no relationship between age, history of contraceptive use, level of knowledge, and current use of contraceptives with p value > 0.005.

Abstract This article explores relations between Muscovy and the so-called Later Golden Horde successor states that existed during the fifteenth and sixteenth centuries on the territory of Desht-i Qipchaq (the Qipchaq Steppe, a part of the East European steppe bounded roughly by the Oskol and Tobol rivers, the steppe-forest line, and the Caspian and Aral Seas). As a part of, and later a successor to, the Juchid ulus (also known as the Golden Horde), Muscovy adopted a number of its political and social institutions. The most crucial events in the almost six-century-long history of relations between Muscovy and the Tatars (13–18th centuries) were the Mongol invasion of the Northern, Eastern and parts of the Southern Rus’ principalities between 1237 and 1241, and the Muscovite annexation of the Kazan and Astrakhan khanates between 1552 and 1556. According to the model proposed here, the Tatars began as the dominant partner in these mutual relations; however, from the beginning of the seventeenth century this role was gradually inverted. Indicators of a change in the relationship between the Muscovite grand principality and the Golden Horde can be found in the diplomatic contacts between Muscovy and the Tatar khanates. The main goal of the article is to reveal the changing position of Muscovy within the system of the Later Golden Horde successor states. An additional goal is to revisit the role of the Tatar khanates in the political history of Central Eurasia in the fifteenth and sixteenth centuries.

ABSTRAKLatar Belakang : Keberadaan kader di posyandu sebagai salah satu sistem penyelenggarakan pelayanan sangat dibutuhkan. Mereka adalah ujung tombak  pelayanan kesehatan yang merupakan kepanjangtanganan puskesmas Jawa Tengah tahun 2011 jumlah gizi kurang 5,35% dan gizi buruk 0,10%. Untuk Kabupaten Semarang dari 23.562 balita yang ditimbang pada tahun 2011 gizi lebih 1,13%, gizi baik 93,51%, gizi kurang 4,86% dan gizi buruk 0,49% (DepKes Prov Jateng, 2011). Tujuan Penelitian : Penelitian ini bertujuan untuk mengetahui hubungan peran kader posyandu dengan status gizi balita. Metode Penelitian : Desain penelitian ini adalah survey analitik dengan menggunakan pendekatan cross sectional. Populasi dalam penelitian ini adalah seluruh balita di Posyandu Mawar di Desa Gedangan sejumlah 40 responden, dengan teknik total sampling dan analisa data chi square. Hasil Penelitian : Hasil perhitungan chi square di peroleh X² hitung 10.644 pada df=4, P.value 0.031 dimana probabilitas lebih kecil dari level of significant 5 % (0,001 < 0,05) berarti Ha diterima dan Ho ditolak. The article advances a hypothesis about the composition of Michel de Montaigne’s Essays. Specialists in the intellectual history of the Renaissance have long considered the relationship among Montaigne’s thematically heterogeneous thoughts, which unfold unpredictably and often seen to contradict each other. The waywardness of those reflections over the years was a way for Montaigne to construct a self-portrait. Spontaneity of thought is the essence of the person depicted and an experimental literary technique that was unprecedented in its time and has still not been surpassed. Montaigne often writes about freedom of reflection and regards it as an extremely important topic. There have been many attempts to interpret the haphazardness of the Essays as the guiding principle in their composition. According to one such interpretation, the spontaneous digressions and readiness to take up very different philosophical notions is a form of of varietas and distinguo, which Montaigne understood in the context of Renaissance philosophy. Another interpretation argues that the Essays employ the rhetorical techniques of Renaissance legal commentary. A third opinion regards the Essays as an example of sprezzatura, a calculated negligence that calls attention to the aesthetic character of Montaigne’s writing. The author of the article argues for a different interpretation that is based on the concept of idleness to which Montaigne assigned great significance. He had a keen appreciation of the role of otium in the culture of ancient Rome and regarded leisure as an inner spiritual quest for self-knowledge. According to Montaigne, idleness permits self-directedness, and it is an ideal form in which to practice the freedom of thought that brings about consistency in writing, living and reality, in all of which Montaigne finds one general property - complete inconstancy. Socratic self-knowledge, a skepticism derived from Pyrrho of Elis and Sextus Empiricus, and a rejection of the conventions of traditional rhetoric that was similar to Seneca’s critique of it were all brought to bear on the concept of idleness and made Montaigne’s intellectual and literary experimentation in the Essays possible.

Abstract Background Prisons are considered as major reservoirs for tuberculosis. Preventive therapy for latent TB infection (LTBI) is an adjunctive strategy to control TB. However, LTBI data in Thai prisoners is limited. This study assessed the prevalence of LTBI and feasibility of isoniazid preventive therapy (IPT). Methods A cross-sectional study was conducted among prisoners in Klong Prem Central Prison, Bangkok. Participants were screened for active TB by questionnaire and chest X-ray. LTBI was evaluated by Tuberculin skin test (TST) and QuantiFERON-TB Gold Plus (QFTP) among subgroup. Participants with positive TST or QFTP were considered to have LTBI. Participants with LTBI were offered IPT. Results From August 2018–November 2019, 1002 participants were analyzed. All participants were male with a median age of 38 (IQR 32–50) years. LTBI identified by either TST/QFTP was present in 466 (46.5%) participants. TST was positive in 359 (36%) participants. In the subgroup of 294 participants who had both TST and QFTP results, 181/294 (61.6%) tested positive by QFTP. Agreement between TST and QFTP was 55.1% (Kappa = 0.17). The risk factors associated with LTBI were previous incarceration (aOR 1.53, 95%CI, 1.16–2.01, p = 0.002), history of prior active TB (aOR 3.02, 95%CI, 1.74–5.24, p < 0.001) and duration of incarceration ≥10 years (aOR 1.86, 95%CI, 1.24–2.79, p = 0.003). Majority of LTBI participants (82%) agreed to take IPT. Three hundred and 56 (93%) participants completed treatment whereas 27 (7%) participants discontinued IPT due to the side effects of INH. Conclusion This is the first study to evaluate the prevalence of LTBI and feasibility of IPT among Thai prisoners. LTBI prevalence in male prisoners in Thailand is high. LTBI screening and treatment should be implemented together with other preventive components.

This article deals with the history of Russian philosophers ‘acquaintance with the ideas of O. Spengler, set forth in his work “The Decline of the West”. The authors point out that the initial orientation of Russian thought towards Historiosophy, problems of history and ontology became the key factor of Spengler’s popularity in Russia. The article considers and analyzes critical and methodological approaches to the theory of cultural and historical types by O. Spengler and N. Ya. Danilevsky within the framework of Russian philosophical thought. The authors pay attention to the ideological influence of the United States as the country which adheres to the ideas of the Enlightenment, as well as to German thinkers, who visited this country in the early twentieth century. It is concluded that the global scenario of the human civilization development, that used to be the mainstream of its formation before the events of the beginning of this year, is unsuitable and untenable. The authors insist on the important role of the theory of cultural and historical types supported and developed by Russian emigration representatives, and focus on the importance of the religious factor in the process of cultural revival.
