Background: Patients with hand masses present for consultation either for pain, loss of function, or cosmetic embarrassment caused by the mass. The majority of hand masses are benign soft tissue tumors. The aim was to review the histology results of hand masses operated on at the Chris Hani Baragwanath Academic Hospital Hand Unit in Johannesburg, South Africa, to explore the relationship of the types of masses according to age, sex, side, and compare the findings with what is in the current literature. Methods: Patients operated on in the hand unit, for hand masses between April 2016 and April 2019 with histology results were included in the study for statistical  analysis. Results: There were 64 males and 105 females with a mean age of 41.03 ± 18.81 years. The most frequent masses were ganglion cysts.  Females appeared to be more affected than males by the different hand masses, but there were no statistically significant differences. Of the 21 giant cell tumors, 15 occurred on the right hand (p-value = 0.021). Conclusion: The profile of hand masses at a high-volume hand unit in Johannesburg, were comparable to the reported literature. There  were no significant differences between sex and diagnosis, however, there was a relationship between diagnosis and side for giant cell tumors of tendon sheaths, requiring further exploration.

Subependymal giant cell tumors (SGCTs) are observed in 5–20% of patients with tuberous sclerosis complex (TSC) but account for ~ 25% of neurological morbidity. The authors report the case of a 7-year-old girl with TSC and multiple cortical tubers who presented with worsening seizures in the context of the rapid growth of a cystic, calcified, extraventricular SGCT in the right frontal lobe, initially thought to represent a cortical tuber. The tumor and surrounding tubers were excised, and clinical seizures resolved. This is the first report of an extraventricular SGCT in a child with TSC outside the neonatal period.

Background: Benign soft-tissue tumors of the hand are more common than both their benign bone and malignant soft-tissue counterparts. This study evaluates the characteristics and treatment of benign soft tissue tumors in light of 1 institution’s experience. Methods: Histologically confirmed benign soft-tissue tumors of the hand were retrospectively identified using International Classification of Disease codes from 1992 to 2015. A medical chart review was conducted to collect patient demographics, tumor epidemiology, and treatment. Results: A total of 199 soft-tissue tumors were identified. The median patient age at time of treatment was 47.4 ± 14.7 years in age. The majority of tumors were located in the digits (n = 168, 84%) and treated by excision (n = 191, 96%). Localized type tenosynovial giant cell tumors (n = 71, 36%) were the most common and had the highest rates of recurrence (8.5%) in this series. Other frequent histologies included hemangioma, schwannoma, and glomus tumors. Conclusion: Awareness and understanding of tumor characteristics may help physicians with diagnosis and treatment. There is an extensive variety of tumors, but the principles of clinical and imaging diagnosis are common to all of them. Marginal excision for the treatment pain, improvement of function, and cosmetic correction applies to all these tumors independent of the histology.

Smoking is well known to be correlated with cardiovascular abnormalities, in particular atherosclerosis and heart diseases. This article investigates the effect and relationship of smoking tobacco on the thickness of the intima–media (IMT) belonging to the common carotid arteries (CCAs), and also blood concentration of the lipid profile (LP), mainly the total cholesterol, High Density Lipoprotein (HDL), Low Density Lipoprotein (LDL) and also triglycerides. Nineteen male tobacco smokers and thirty-five healthy male non-smoking Saudi participants were involved in this study after obtaining their informed consent. An ultrasound and a spectrophotometer were used to determine the IMTs and lipid parameters, respectively. The thicknesses of the smokers’ right (RCA) and left carotid (LCA) arteries (0.72 and 0.7 mm, respectively) were significantly greater than the thicknesses of the arteries of the non-smokers (0.58 and 0.62 mm, respectively) (p-value = 0.005 and 0.04). Insignificant differences between the means of the other parameters in the two groups were studied. Smoking is a risk factor for stroke, because it significantly increases the IMTs of both the right and left carotid arteries.

AbstractGiant cell tumors of soft tissue (GCT-ST) are rare low-grade neoplasms that were at one time thought to represent the soft tissue counterparts of GCT of bone (GCT-B) but are now known to lack the H3F3 mutations characteristic of osseous GCT. We present six distinctive giant cell-rich soft tissue neoplasms that expressed keratins and carried a recurrent HMGA2-NCOR2 gene fusion. Patients were five females and one male aged 14–60 years (median, 29). All presented with superficial (subcutaneous) masses that were removed by conservative marginal (3) or wide (2) local excision. The tumors originated in the upper extremity (2), lower extremity (2), head/neck (1), and trunk (1). Five patients with follow-up (median, 21 months; range, 14–168) remained disease-free. Grossly, all tumors were well-demarcated but not encapsulated with variable lobulation. Histologically, they were composed of bland plump epithelioid or ovoid to spindled mononuclear cells admixed with evenly distributed multinucleated osteoclast-type giant cells. Foci of stromal hemorrhage and hemosiderin were seen in all cases. The mitotic activity ranged from 2 to 14/10 high power fields (median: 10). Foci of necrosis and vascular invasion were seen in one case each. The mononuclear cells were immunoreactive with the AE1/AE3 keratin cocktail and less frequently/less diffusely for K7 and K19 but lacked expression of other lineage-associated markers. RNA-based next-generation sequencing revealed an HMGA2-NCOR2 fusion in all tumors. None of the keratin-negative conventional GCT-ST showed the HMGA2-NCOR2 fusion (0/7). Metaplastic bone (4/9) and SATB2 expression (3/4) were frequent in keratin-negative conventional GCT-ST but were lacking in keratin-positive HMGA2-NCOR2 fusion-positive tumors. The distinctive immunophenotype and genotype of these tumors strongly suggest that they represent a discrete entity, differing from conventional GCT-ST and other osteoclast-rich morphologic mimics. Their natural history appears favorable, although a study of additional cases and longer follow-up are warranted.

Introduction: Selection of anterior teeth for the fabrication of artificial denture during Prosthodontic rehabilitation is a complex process. Different authors have suggested various methods, however no single method can be used reliably to select artificial teeth. This study aims to assess dimensional relationship of maxillary anterior teeth with inter hamular distance which may assist in the artificial teeth selection. Materials and methods: This clinical study was carried out on 95dentate subjects meeting the inclusion criteria. Prior to impression making, hamular notch was located precisely with T-burnisher and was marked with indelible pencil. Impression of maxillary arch was made using irreversible hydrocolloid impression material and casts were poured using Type III dental stone. Measurements were made directly on the cast using digital vernier caliper. The contact point widths of maxillary right and left maxillary central incisors, lateral incisors and canines were measured. The distance between the right and left hamular notch was measured. The sum of contact point widths of all maxillary anterior teeth was calculated. All the data were recorded and statistically analyzed. Result: There is a positive correlation between combined width of maxillary anterior teeth and inter-hamular notch distance with Pearson’s correlation coefficient (r) 0.217 and p value 0.035 which is statistically significant. Conclusions: There is significant correlation between combined width of maxillary anterior teeth and distance between right and left hamular notch. This correlation is valuable and practicable in estimating the six maxillary anterior teeth width.

Introduction The importance of handedness lies in its relationship with the lateralization of the brain function, especially of language. The aim of the present study was to provide an authentic database on right and left second to fourth digit (2D:4D) ratios in 2 different ethnic groups of a particular age and gender, and to study its correlation with handedness and sexual dimorphism. Materials and Methods The present study was conducted among 400 Tibeto-Nepalese (TN) subjects (200 of each gender) and 400 Indo-Nepalese (IN) subjects (200 of each gender) aged ≥ 18 years old. The values for the 2D:4D ratio were calculated for both hands. Hand preference was established according to the Edinburgh Handedness Inventory, and five hand preference determination groups were constituted after the calculation of the laterality score. The results were tabulated and subjected to statistical analysis. A p-value < 0.05 was considered as statistically significant. Results The mean value of right sided 2D:4D ratio of TN female was higher as compared to IN female; and for left sided 2D:4D ratio, the value was lower in TN female as compared to IN female. When the values of the 2D:4D ratios were assessed by gender, the values were found to be statistically significant (p < 0.001), whereas when they were assessed by ethnicity, only the right hand 2D:4D ratio was found to be statistically significant (p < 0.001). Conclusion When the relationships between the laterality score and the 2D:4D ratios were examined, the values were found to be statistically significant in both ethnic groups only on the left side. The 2D:4D ratio shows significant ethnic and population differences in the present study.

Giant cell tumor of tendon sheath is one of the most common soft tissue tumors of the hand. These tumors typically occur in the third or fourth decade of life and present as solitary nodules on a single digit. Currently, the greatest reported number of lesions found within a single digit is five. Although uncommon, giant cell tumor of tendon sheath does occur in the pediatric population. Herein we present a report of a rare case of GCTTS in a child in which seven lesions were identified within a single digit—the greatest number of lesions within a single digit reported to date.

Compliance in the use of partographs  is one of the most important thing done by midwives to make clinical decisions, monitoring, evaluation, handling labor, early problem detection and complicated labor so that they can plan actions to overcome the problem or to give refferal the mother in the right conditions. The purpose of this study was to find out the relationship between knowledge with the compliance of midwives in the use of partographs. This study used an analytical observational design with a cross-sectional approach. The subjects in the study were all midwives who worked in the Maternity Room of Undata Palu Hospital in March-June 2018 as many as 26 respondents. Bivariate analysis used the Sperman Rank test. The results of this study were good knowledgeable midwives as many as 18 respondents (69.3%), obedient midwives as many as 16 respondents (61.5%), and the results of bivariate analysis showed that the relationship of knowledge with midwife compliance in using partograph  was rs 0.529 and p-value 0.005. Conclusion: There was a relationship between knowledge and compliance of midwives in using Partographs with medium correlation strength. Suggestion: This result can be an evaluation material for respondents about the use of partograph
