Background: Aim: To compare the response of human auditory brain stem evoked by clicks stimuli and chirps. Patients and Methods: A study of cross-sectional design was chosen to evaluate the objective of the study. Children between 1-10 years were enrolled from the attendants of the Dept. of Surgery and Audiology, Al-Jamhoori Teaching Hospital, Ministry of Health /Nineveh health Directorate, and the outpatient clinics in al-alwiyah teaching hospital for children and done in the privet clinics of the researchers. The data collection extended over the period from 2019 January to 2020 August. A total number of 70 children involved in the study according to the parents’ complaints, full history taken and the clinical examination by otoscopy. The probable conductive problem excluded by using the Tympanometry. Moreover, free field test was done before chirp and click. Paired t-test was used for the statistical analysis. Results: The males represent (44.3%) and the females (55.7%). The mean age of children included was 49 months ± 27.7 SD. Wave V of the chirp shows lower latency means in all intensities in comparison with that of click stimuli. While waves I and III, show longer latency as the intensity go down, but the differences between chirp and click are insignificant. Wave V amplitudes getting lower values as the intensity decreasing in both chirp and click and become significantly higher than that of click. Wave I and III show decreasing amplitudes with the decreasing intensity in both chirp and click, with higher amplitudes in click in comparing with that of chirp, which are insignificant statistically down to below 70 dBnHL. Conclusions: The chirp stimuli are highly significant and more efficient from the click in the detection of hearing loss among the children regarding both latency and amplitude particularly at wave V.

Abstrak Penyebab dismenore belum semuanya diketahui, ada dugaan peningkatan proses peroksida lipid yang akan mengaktivasi mediator inflamasi pada endometrium yang menimbulkan rasa nyeri haid (dismenore). Tujuan penelitian ini adalah menentukan perbedaan kadar malondialdehide dan tromboksan B 2  pada dismenore dan tanpa dismenore. Studi observasional ini menggunakan desain potong lintang komparatif. Subjek penelitian terdiri dari dua kelompok yaitu 23 remaja dismenore dan 23 remaja tanpa dismenore dengan waktu penelitian dari Juni sampai Juli 2014. Analisis sampel dilakukan di Laboratorium Biomedik dan Biokimia Universitas Andalas Padang. Pemeriksaan kadartromboksan B 2  menggunakan metode ELISA dan kadar malondialdehide menggunakan metode Asam Thiobarbiturat (TBA). Hasil penelitian diperoleh bahwa rerata kadar malondialdehid pada remaja dengan dismenore yaitu 2,60±0,63 µmol/ml, rerata remaja tanpa dismenore 1,98±0,12 µmol/ml dengan probabilitas p<0,05 (0,000), sedangkan reratakadar Tromboksan B 2  pada remaja dengan dismenore 20,043±9,56 ng/ml, rerata remaja tanpa dismenore 19,222±10,79 ng/ml, dengan probabilitas p>0,05 (0,786). Kesimpulan penelitian ini adalah terdapat perbedaan yang signifikan rerata kadar malondialdehid pada remaja dengan dismenore dan tanpa dismenore dan tidak terdapatperbedaan signifikan pada kadar tromboksan B 2 pada remaja dengan dismenore dan tanpa dismenore.Kata kunci: remaja, dismenore, malondialdehide, tromboksan B2 Abstract The precise cause of dysmenorrhea is still unclear, there may be increased lipid peroxidation process will activate the inflammatory mediators at endometrium that cause menstrual cramps (dysmenorrhea). The objective of this study was to determine the difference of malondialdehyde levels and thromboxane B 2  levels in dysmenorrhea and without dysmenorrhea. It was an observational study with comparative cross-sectional design. The subjects consisted of two groups, they are 23 adolescent with dysmenorrhea and 23 adolescents without dysmenorrhea, done in Juny -July 2014. Sample analysis was conducted in Laboratory of Biochemistry and Biomedical Laboratory of Andalas University Padang. The examination of Thromboxane B 2  levels used ELISA and the examination of malondialdehyde levels used a Thiobarbituric acid method. The results showed the mean of malondialdehyde levels in adolescents withdysmenorrhea was 2.60±0.63 µmol/ml, the mean level in adolescent without dysmenorrhea was 1.98±0.12 µmol/ml with probability p<0.05 (0.000), while the mean levels of thromboxane B 2  in adolescents with dysmenorrhea was 20.043±9.56 ng/ml, the mean level in adolescent without dysmenorrhea was 19.222±10.79 ng/ml, with probabilityp>0.05 (0.786). It can be concluded that there is a significant difference in the mean of malondialdehyde levels between adolescents with dysmenorrhea and without dysmenorrhea and there is no significant differences in thromboxane B 2 level between adolescents with dysmenorrhea and without dysmenorrheaKeywords: adolescent, dysmenorrhea, malondialdehyde, thromboxane B2

Background: Acne vulgaris is a disease of the pilosebaceous unit that often occurs in adolescents and young adults. The pathogenesis of acne vulgaris involves several factors, one of which is the acidity (pH) of the skin.  Objective: This study aimed to determine the difference in the degree of acidity of facial skin, chest and back of patients with acne vulgaris.  Methods: This was a comparative analytic study with cross sectional design. The sample collection technique was consecutive sampling. Data were analyzed using ANOVA-test.  Results: The majority of patients with acne vulgaris who came to Murni Teguh Memorial Hospital Medan and the private clinic of dr. Rudyn Reymond Panjaitan, M.Ked (KK), Sp.KK was a 17-25 year old. The mean acidity of facial skin with acne vulgaris was 5.66, the mean acidity of chest skin was 5,84 and the mean acidity of back skin was 6.09. There was a significant difference in the mean value of acidity in facial, chest and back skin of patients with acne vulgaris (p = 0,000). Conclusion: There is a significant difference in average degree of acidity on the facial, chest and back skin in patients with acne vulgaris.

The aim of this study was to see the difference in the mean ratio of sodium levels between pregnancy with severe preeclampsia and eclampsia. The method in this research is an analytical study using a cross sectional design. The research was conducted in the delivery room Dr. M. Djamil Padang from 15 January 2016 to 31 December 2017. There were 60 patients as research subjects, the sample was divided into 2 groups, namely severe preeclampsia (PEB) and eclampsia. Anamnesis and physical examination were then carried out to obtain data and clinical diagnosis. The data were recorded in a research form that had been provided, then the blood electrolytes were examined for sodium and potassium. Statistical analysis to assess meaning using the T-test. The results showed that in the PEB and Eclampsia groups, it was found that multiparity parity had the highest respondents. This is in accordance with the literature where the incidence of preeclampsia is more often found at gestational age near term. The conclusion of this study there was no significant difference in the mean sodium ratio between preeclampsia, PEB and eclampsia. Background: Click-evoked auditory brainstem responses (ABRs) are a valuable tool for probing auditory system function and development. Although it has long been thought that the human auditory brainstem is fully mature by age 2 yr, recent evidence indicates a prolonged developmental trajectory. Purpose: The purpose of this study was to determine the time course of ABR maturation in a preschool population and fill a gap in the knowledge of development. Research Design: Using a cross-sectional design, we investigated the effect of age on absolute latencies, interwave latencies, and amplitudes (waves I, III, V) of the click-evoked ABR. Study Sample: A total of 71 preschoolers (ages 3.12–4.99 yr) participated in the study. All had normal peripheral auditory function and IQ. Data Collection and Analysis: ABRs to a rarefaction click stimulus presented at 31/sec and 80 dB SPL (73 dB nHL) were recorded monaurally using clinically-standard recording and filtering procedures while the participant sat watching a movie. Absolute latencies, interwave latencies, and amplitudes were then correlated to age. Results: Developmental changes were restricted to absolute latencies. Wave V latency decreased significantly with age, whereas wave I and III latencies remained stable, even in this restricted age range. Conclusions: The ABR does not remain static after age 2 yr, as seen by a systematic decrease in wave V latency between ages 3 and 5 yr. This finding suggests that the human brainstem has a continued developmental time course during the preschool years. Latency changes in the age 3–5 yr range should be considered when using ABRs as a metric of hearing health.

AbstrakPreeklampsia-eklampsia sampai saat ini masih merupakan disease of theory. Kelainan yang terjadi pada penyakit ini adalah disfungsi endotel yang dapat disebabkan oleh perubahan kadar profil lipid. Tujuan penelitian ini adalah mengetahui perbedaan rerata kadar profil lipid pada preeklampsia dengan kehamilan normal pada etnik Minangkabau. Penelitian observasional dengan rancangan cross sectional ini dilakukan di RSUP dr. M. Djamil Padang, RS Reksodiwiryo Padang dan UPTD Laboratorium Kesehatan Daerah Sumatera Barat. Subjek terdiri dari 2 kelompok yang diambil secara consecutive sampling dengan jumlah masing-masing 27 orang. Pemeriksaan profil lipid dilakukan dengan metode kalorimetri. Analisis data dilakukan secara uji statistik independent t-test. Hasil penelitian diperoleh rerata kadar total kolesterol pada kelompok preeklampsia dan kehamilan normal adalah 270,19+68,955 mg/dL dan 247,56+44,415 mg/dL dengan nilai p=0,158, rerata kadar trigliserida pada kelompok preeklampsia dan kehamilan normal adalah 296,07±157,993 mg/dL dan 272,89±84,608 mg/dL dengan nilai p=0,504, rerata kadar HDL pada kelompok preeklampsia dan kehamilan normal adalah 51,93±19,882 mg/dL dan 63,33±11,222 mg/dL dengan nilai p=0,012, rerata kadar LDL pada kelompok preeklampsia dan kehamilan normal adalah 159±52,038 mg/dL dan 129,67±38,692 mg/dL dengan nilai p=0,023, rasio perbandingan LDL/HDL pada kelompok preeklampsia dan kehamilan normal adalah 3,86 dan 2,04. Dari hasil penelitian dapat disimpulkan pada kedua kelompok tidak ada perbedaan kadar kolesterol total dan trigliserida, tetapi ada perbedaan kadar HDL dan LDL , rasio perbandingan LDL/HDL lebih tinggi pada preeklampsia daripada kehamilan normal.Kata kunci: preeklampsia, total kolesterol, trigliserida, HDL, LDL, etnik Minangkabau AbstractPreeclampsia-eklampsia still being a disease of theory. The mean levels of triglyceride in preeclampsia and normal pregnancy were 296.07±157.993 and 272.89±44.415 mg/dL with p-value 0.504. The mean level of HDL in preeclampsia and normal pregnancy were 51.93±19.882 mg/dL and 63.33 mg/dL p-value 0.012. The mean level of LDL in two groups were 159±52.038 mg/dL and 129.67±38.692 mg/dL with p-value 0.023 and the mean levels of LDL/HDL ratio were 3.86±3,09 mg/dLin  preeclampsia and 2,08±0,64 in normal pregnancy. It can be concluded that there is no significant difference in total cholesterol and triglyseride between preeclampsia and normal in HDL and LDL and there is significant difference in LDL/HDL ratio between preeclampsia and normal pregnancy at Minangkabau Ethnic.Keywords: preeclampsia, total cholesterol, triglyceride, HDL, LDL, Minangkabau Ethnic

This study's purpose was to determine if a correlation exists between cochlear antioxidant activity changes and auditory function after induction of aminoglycoside (AG) ototoxicity. Two groups of five 250-350 g albino guinea pigs served as subjects. For 28 days, albino guinea pigs were administered either 200 mg/kg/day amikacin, or saline subcutaneously. Auditory brainstem response testing was performed prior to the first injection and again before sacrifice, 28 days later. Cochleae were harvested and superoxide dismutase, catalase, glutathione peroxidase, glutathione-S-transferase, glutathione reductase activities and malondialdehyde levels were measured. All antioxidant enzymes had significantly lower activity in the amikacin group (p ≤ 0.05) than in the control group. The difference in cochlear antioxidant enzyme activity between groups inversely correlated significantly with the change in ABR thresholds. The greatest correlation was for the high frequencies, which are most affected by aminoglycosides. This study demonstrates that antioxidant enzyme activity and amikacin-induced hearing loss significantly covary.

Auditory brain stem response, auditory sensitivity, speech discrimination function, acoustic reflex threshold, and reflex decay were studied in seven children with adrenoleukodystrophy (ALD), nine adults with adrenomyeloneuropathy (AMN), one child with neonatal ALD, and two carriers of ALD. Significant auditory dysfunctions were found in only two patients; a retrocochlear type auditory disorder in one child with ALD, and a profound deafness in a child with neonatal ALD. None of the patients showed significant abnormality in acoustic reflex threshold and reflex decay except for one patient with ALD who showed a slightly elevated reflex threshold. All but two ALD patients showed significantly prolonged wave latencies and interwave intervals. The latency values revealed the highest incidence of abnormality in the I to III interval; this was followed by the III to V interval, then by wave I. Significantly, one child with asymptomatic ALD and both heterozygous female relatives showed abnormal ABRs, demonstrating the high sensitivity of ABR in detection of the existence of pathophysiological condition in subclinical or presymptomatic ALD.

Abstract: Haemoglobin is the main component of red blood cells that serves as a transporter of oxygen and carbon dioxide in the blood. The normal range of haemoglobin values can be used to determine the degree of anemia according to age and gender. Geographical condition such as altitude influences the haemoglobin value. This study aimed to obtain the difference of haemoglobin levels between teenagers with good nutrition status who live at the highland and those at the seaside. This was an observational analytical study with a cross sectional design. Samples were students with good nutrition status of SMPN 3 Tomohon (living at the highland) and those of SMP Kristen Nazaret Tuminting (living at the seaside). There were 60 students who met the inclusion criteria as follows: adolescent, good nutrition status, healthy, aged 13-15 years, lived at the highland or at the seaside ≥ 6 month, and willing to be performed blood examinations, and had been approved by their parents. The Mann-Whitney test for the difference between haemoglobin levels of the two groups showed a p value < 0,001. Conclusion: There was a very significant difference between haemoglobin levels of good nutrition teenagers who lived at the highland and at the seaside. Batas normal nilai hemoglobin dapat digunakan untuk menetapkan derajat anemia, dengan distribusi usia dan jenis kelamin spesifik didasarkan pada sampel referensi sehat. Kondisi geografis, seperti ketinggian tempat dari permukaan laut menjadi faktor pertimbangan dalam distribusi nilai normal hemoglobin. Penelitian ini bertujuan untuk mendapatkan perbedaan kadar hemoglobin antara remaja gizi baik yang tinggal di pegunungan dengan yang tinggal di tepi pantai. Jenis penelitian ini analitik-observasional dengan desain potong lintang. Sampel penelitian yaitu remaja gizi baik yang tinggal di pegunungan yaitu siswa SMPN 3 Tomohon dan yang di tepi pantai yaitu siswa SMP Kristen Nazaret Tuminting. Subjek penelitian berjumlah 60 remaja yang memenuhi kriteria inklusi yaitu gizi baik, sehat, berumur 13-15 tahun, berdomisili di pegunungan atau tepi pantai ≥6 bulan, bersedia dilakukan pemeriksaan darah, dan telah disetujui oleh orang tua. Data yang diperoleh dianalisis menggunakan uji Mann-Whitney dengan bantuan program SPSS. Hasil uji Mann-Whitney mengenai perbedaan kadar hemoglobin antara kedua kelompok menunjukkan nila p <0,001. Simpulan: Terdapat perbedaan yang sangat bermakna kadar hemoglobin remaja gizi baik yang tinggal di pegunungan dengan di tepi pantai.Kata kunci: kadar hemoglobin, remaja gizi baik, pegunungan, tepi pantai

Objectif : Décrire les aspects épidémiologique, clinique et pronostique associés aux étiologies des hémorragies digestives hautes au CHU Campus de Lomé Patients et méthode: Etude transversale à collecte rétrospective, à visée descriptive et analytique menée du 1er Janvier 2014 au 31 Décembre 2019. Le seuil de significativité était retenu pour p<0,05. Résultats: Deux cent cinquante et un patients avaient été inclus. L’hémorragie était d’origine hypertensive portale chez 69 patients (27,71%) ; ulcéreuse gastro-duodénale chez 100 patients (39,84%). Chez 25 patients (9,96%), la fibroscopie oesogastroduodénale était normale. Une rupture de varices oesophagiennes était retrouvée chez 98,55% des patients présentant une hypertension portale. Les ulcères gastroduodénaux représentaient 54,94% des hémorragies digestives hautes d’origine non hypertensive portale. La valeur moyenne du score de Rockall était de 4(±1) chez les patients présentant une hémorragie d’origine hypertensive et de 3(±1) chez les patients avec hémorragie non hypertensive portale (p<0,001). La valeur moyenne du score de Glasgow-Blatchford était de 10(±3) chez les patients présentant une hémorragie d’origine hypertensive et 9(±3) chez les patients avec hémorragie digestive haute d’origine non hypertensive (p<0,001). La récidive hémorragique et le décès étaient survenus chez les patients présentant un saignement d’origine hypertensive portale dans respectivement 54,84% (p<0,001) et 71,42% (p<0,001). Conclusion: Les lésions inflammatoires aiguës et chroniques représentent la première étiologie des hémorragies digestives hautes dans le service d’Hépato-gastroentérologie du CHU Campus. Ces hémorragies sont associées à la prise de médicaments gastrotoxiques et à un moindre risque de récidive hémorragique et de décès. Objective: To describe epidemiological et prognostic outcomes associated with etiologies upper gastrointestinal bleeding in Campus Teaching Hospital of Lome Patients and method: Cross-sectional study with retrospective collection, descriptive and analytical aim carried out from January 1, 2014 to December 31, 2019. Results: Two hundred and one patients were included. The hemorrhage was of portal hypertensive origin in 69 patients (27.71%); peptic ulcer in 100 patients (39.84%). In 25 patients (9.96%), the oesogastroduodenal fibroscopy was normal. Ruptured esophageal varices were found in 98.55% of patients with portal hypertension. Peptic ulcers accounted for 54.94% of upper GI bleeding of non-portal hypertensive origin. The mean value of the Rockall score was 4(±1) in patients with hemorrhage of hypertensive origin and 3(±1) in patients with non-portal hypertensive hemorrhage (p<0.001). The mean Glasgow-Blatchford score was 10(±3) in patients with hemorrhage of hypertensive origin and 9(±3) in patients with upper GI hemorrhage of nonhypertensive origin (p<0.001). Hemorrhagic recurrence and death occurred in patients with bleeding of hypertensive origin in 54.84% (p<0.001) and 71.42% (p<0.001) respectively. Conclusion: Acute and chronic inflammatory lesions represent the first etiology of upper GI bleeding in the Gastroenterology Department of the Campus Teaching Hospital of Lome. These hemorrhages are associated with the use of gastrotoxic drugs and with a lower risk of recurrence of hemorrhage and death.

Objectives: To compare the hearing thresholds obtained with auditory brainstem response (ABR) and auditory steady state response (ASSR) audiometry in children with hearing loss. Methods: Hearing thresholds were obtained by ABR and ASSR in children who presented with suspicion of deafness at Ear, nose & throat department of Al-Nafees Medical College Hospital Islamabad, between January to August 2018. The mean hearing thresholds obtained by two tests were compared within each category of severity of deafness. Time taken by both tests was also compared. Results: A total of 57 patients (114 ears) were included in the study. Among them 27 (47.4%) were male and 30 (52.6%) were female. The mean age of patients at presentation was 42 months (±30.9) with age range from one to 12 years. Mean hearing thresholds obtained by click ABR, chirp ABR, ASSR (1, 2, 4 kHz) & ASSR (0.5, 1, 2, 4 kHz) was 56.25 (±27.61), 58.88 (±27.44), 58.03 (±21.26) & 56.35 (±22.86) respectively. Mean thresholds were comparable between click ABR & ASSR (1, 2, 4 kHz) and between chirp ABR & ASSR (0.5, 1, 2, 4 kHz) in all degrees of hearing loss categories except in those patients with normal hearing thresholds. The mean time taken by clicks ABR, chirp ABR and ASSR were four minutes seven seconds, three minutes 15 seconds and 16 minutes and 7 seconds respectively. Conclusions: Hearing thresholds obtained by ABR and ASSR are comparable in all categories of severity of hearing loss. The time taken by ABR is less as compared to ASSR.
