Aim: To evaluate the role of newly created transcanal endoscopic antrostomy hole as a passage in restoring the aeration of epitympanum and mastoid antrum.Materials & Methods: Study design: Retrospective case series study. Study duration: From January 2013 to January 2014. Study place: Private tertiary care hospital. Patients: 27  ears of 23 adult patients (age ranging from 15years to 54years) underwent transcanal endoscopic tympanoplasty for their chronic middle ear diseases in presence of inflamed middle ear mucosa along with radiological shadows of stagnant fluid in their epitympanum and mastoid antrum. Structured three months follow-up was ensured in every case. Intervention: A hole was created at a selected site of posterior meatal wall purely through transcanal endoscopic approach that established direct communication between mastoid antrum and external auditory canal. Thereafter this newly created passage was used for three purposes- assessment of the condition of mastoid antrum, performing the water test for checking epitympanic patency and in few cases placement of temporary tube for postoperative ventilation and drainage of middle ear. Main out come measures: The feasibility, performance and management of transcanal endoscopic antrostomy hole as passage for reestablishing the aeration of epitympanum and mastoid antrum. Results: The chosen site for antrostomy hole was found effective and safe in providing convenient entrance into the mastoid antrum in every case in this study with out facing technical complexity and failure. Postoperative healing of skin over antrostomy hole was found complete in all ears without any inward growth of skin in to mastoid antrum. Available post operative CT scan imaging of temporal bones showed improved aeration in their epitympanum and mastoid antrum. Conclusions: The role of transcanal antrostomy hole has been proved worthy in restoring ventilation pathway to epitympanum and mastoid antrum during tympanoplasty. This antrostomy hole has the potentiality to be considered in future for placement of long term mastoid ventilation tube in order to treat persistent atelectatic middle ear.

One interesting aspect of the Revolution of 1911 in China was the role of the new-style army. The new-style troops, as one category of revolutionary activists distinct from the civilian radical intellectuals, determined the opening phase of the revolution, initiating the Wuchang uprising and bringing pressure to bear on most of the provincial leaders. Their contribution was the physical strength which the revolutionary intellectuals, who provided the ideology, lacked. The army played its vital role, not in the beginning of the revolutionary movement, but at a later stage when the prevailing order had been discredited and the imperial government had lost the allegiance of the people. Indeed, the success of the revolution reflected the interaction between revolutionary ideas and military power.

Treatment of the predisposing factors that are identified in the nose and throat in several ENT diseases is mandatory prior to the definitive management of the latter. When surgical management is indicated it has been traditional to use staged procedures. This study was undertaken to assess the role of concurrent surgical procedures in ENT and evaluate their cost-effectiveness compared to similar staged procedures. This was a retrospective case series of 100 consecutive patients undergoing concurrent and similar staged ENT surgical procedures. On analysis, it was noted that the average duration of surgery, anaesthesia and hospital stay was significantly less in the concurrent procedures group than in the staged procedures (2.35/3.1.hours; 3.05/3.30.hours and 2.5/6.5 days, respectively). The average hospital bill for the concurrent procedures was also lower than for the staged procedures. Hence, in patients requiring multiple ENT surgical procedures for definitive treatment, concurrent procedures are more cost-effective than staged procedures and should be considered the treatment of choice in a tertiary care centre.

Anyone who has visited a large old garden and seen the twisted remnant shrubs and big old trees in it may wonder about their past and worry about their future. The lumps and bumps on the ground and half-buried lines of stone may reveal clues to previous garden beds and structures. Old photographs may show a house in a luxuriant setting where we see only remnant trees and shrubs, and evidence of many phases of rise and decline. The study of a mature garden is inexorably linked to the study of the plants grown within it, as much as it is to an understanding of the people who made the garden and those who altered it and recorded it in successive generations.

AbstractDespite the ubiquitous presence and vital role of invertebrates in all known ecological systems, insects and arachnids are largely viewed as repugnant by people. Consequently, until nature intervenes in the form of infestations, swarms or plagues, we largely prefer to ignore them, lest our attention invite unwelcome interaction. In contrast, the people of ancient Egypt did not distance themselves from invertebrates but instead celebrated their myriad forms. Egyptian appreciation of insects and arachnids is reflected in a range of art, artefacts, and texts dating from the predynastic era until the Greco-Roman period, revealing many positive cultural roles, from practical to conceptual. By assigning them a useful function, they were rendered visible and relevant to Egyptian society. The Egyptians’ example suggests that as necessity forces us to acknowledge the value of invertebrates—from their function as pollinators to becoming future food sources—our respect for them may also grow.

Objective Chronic cough remains a challenging condition, especially in cases where it persists despite comprehensive medical management. For these particular patients, there appears to be an emerging role for behavior modification therapy. We report a series of patients with refractory chronic cough to assess if there is any benefit of adding behavioral therapy to their treatment regimen. Study Design A case series with planned chart review of patients treated for chronic cough. Setting The review was performed with an outpatient electronic health record system at a tertiary care center. Subjects and Methods The charts of all patients treated for chronic cough by a single laryngologist over a 30-month period were analyzed. Patients’ response to treatment and rate of cough improvement were assessed for those with refractory chronic cough who underwent behavior modification therapy. Results Thirty-eight patients with chronic cough were initially treated empirically for the most common causes of cough, of which 32% experienced improvement. Nineteen patients who did not significantly improve with medical management underwent behavior modification therapy with a speech-language pathologist. Of these patients, 84% experienced resolution or marked improvement of their symptoms. Conclusion Behavioral therapy may be underutilized in practice and could lead to improvement of otherwise recalcitrant cough.

The rapid expansion of a novel human infectious disease caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has evolved into a pandemic, affecting thousands of people world wild. Some patients with coronavirus disease 2019 (COVID-19) develop severe infection, which may progress to acute respiratory distress syndrome, multiple organ failure, and death. Increasing studies indicate that abnormal elevation of cytokine levels in response to SARS-CoV-2 may contribute to the pathological process that leads mortality of COVID-19. Thus, application of extracorporeal hemoperfusion (HP) for removal of excessive cytokines from the blood can potentially mitigate or reverse cytokine storm related complications of COVID-19. Here, we presented series of COVID-19 patients, who were treated with HP (HA 380 cartridge, Jafron Biomedocal Co, China). The medical records were evaluated retrospectively to determine the effect of HP on patients’ clinical outcome. Our results showed that HP improvee PO2 and O2 saturation in patients with severe COVID-19. After the last courses of HP, 5 out of 6 patients were extubated and transferred to the general ward with an acceptable medical condition. The following case series demonstrate the promising role of HP in controlling the consequential effect of cytokine storm following a COVID-19 infection, which could facilitate patient survival.

Education, mainly Islamic based learning, can be found in formal schools, non-formal and informal institutions. A person can freely choose which path to use as a way to take talent and potential in him; even when making formal education, even an individual has the right to participate in non-formal education activities. One of the most appropriate places to forge one's abilities is the campus. It is undeniable that students are one of Indonesia's most influential human resources and are also the next generation of nation-building. Therefore the cadre regeneration function is the most basic. As a generation that must carry out the role of regeneration for the sake of the realisation of the good and happiness of the people, nation and country in the future. Therefore education is needed to help towards the better. Islamic culture has a vital role in the education model that exists in Indonesia. Islamic education is used to improve human morals in a better direction with Islamic methods. It can be observed that student organisations within the campus need a strong foundation to improve their quality in faith and loyalty that has both physical and spiritual intelligence. It is here that the need for Islamic education in helping them is better by the mandate of the national education law.
