Orofacial cleft is one of the commonest congenital abnormalities which impacts negatively on the life of the individual and to a large extent affects the family. Caused by the interaction of environmental and genetic factors, this abnormality brings about decreased quality of life. Management of this abnormality entails a team involving a cleft surgeon, speech therapist, dentist, orthodontists, and so forth. This study involves the review of the various literatures on orofacial clefts, discussing the problems on the genetic basis, associated syndromes, and their management. Counseling of prospective mothers should be promoted to ensure that the abnormality is prevented at the early stages. Education on orofacial clefts should be promoted to create awareness on its preventive measures. Much attention must be geared towards cleft genetics studies to identify potential risk factors which might be predisposing individuals to the anomaly.

The existing legislation determines the education as "an integral and focused process of teaching and upbringing, which represents a socially important value and shall be implemented so as to meet the interests of the individual, the family, the society and the state". However, even in this part, the meaning of the notion ‘socially significant benefit is not specified and allows for a wide range of interpretation [2]. Yet the more inconcrete is the answer to the question – "who and how should determine the interests of the individual, the family and even the state?" The national doctrine of education in the Russian Federation, which determined the goals of teaching and upbringing, the ways to attain them by means of the state policy regulating the field of education, the target achievements of the development of the educational system for the period up to 2025, approved by the Decree of the Government of the Russian Federation of October 4, 2000 #751, was abrogated by the Decree of the Government of the Russian Federation of March 29, 2014 #245 [7]. The new doctrine has not been developed so far. The RAE Academician A.B. Khutorsky believes that the absence of the national doctrine of education presents a threat to national security and a violation of the right of citizens to quality education. Accordingly, the teacher has to solve the problem of achieving the harmony of interests of the individual, the family, the society and the government on their own, which, however, judging by the officially published results, is the task that exceeds the abilities of the participants of the educational process.  The particular concern about the results of the patriotic upbringing served as a basis for the legislative initiative of the RF President V. V. Putin, who introduced the project of an amendment to the Law of RF "About Education of the Russian Federation" to the State Duma in 2020, regarding the quality of patriotic upbringing [3]. Patriotism, considered by the President of RF V. V. Putin as the only possible idea to unite the nation is "THE FEELING OF LOVE OF THE MOTHERLAND" and the readiness for every sacrifice and heroic deed for the sake of the interests of your Motherland. However, the practicing educators experience shortfalls in efficient methodologies of patriotic upbringing, which should let them bring up citizens, loving their Motherland more than themselves. The article is dedicated to solution to this problem based on the Value-sense paradigm of upbringing educational dynasty of the Kurbatovs [15].

Wolfram Syndrome (WS) is a rare disease (RD) with an estimated prevalence of 1/770,000 inhabitants. It is considered a multisystemic, chronic and progressive disease.WS diagnosis implies devastating consequences at physical, educational and emotional levels. WS is also known by the acronym DIDMOAD, derived from the first letters of the main clinical symptoms: Diabetes Insipidus, Diabetes Mellitus, Optic Atrophy, and Deafness. Rare diseases are characterized by their low prevalence and the lack of knowledge on the pathophysiology and treatment of them. Interestingly, around 80% of RD have a genetic basis, and this fact causes doubts and uncertainties in the couples, about the idea of having another child. The existence of a RD in a family, alters significantly the relationships and the quality of life within the family. The present work remarks the huge value of psychosocial aspects in order to pay an adequate attention to these patients, not only taking care of the clinical aspects. The main purpose of this study has been to ascertain the quality of life of Wolfram’s syndrome affected patients, and its impact in the daily life.

The purpose of this study is to analyze Moroccan TIMSS 2011 data using the multi-level approach in order to detect the entanglement of factors that influence the academic performance of students in the second year of college secondary education (grade eight ). The main question is, what is the influence of factors related to the individual characteristics and characteristics of the school attended on the performance of students in mathematics? This question can be answered in many ways: 1. Does student achievement in mathematics vary by school? 2. Is there a significant relationship between the socioeconomic and cultural characteristics of the students and their performance in mathematics? the main question is whether the effect of socio-cultural variables linked to the family context (in particular family educational resources) tends to worsen when they interact with the contextual variables of the school. This raises the question of both the efficiency of schools and the equitable distribution of performance in mathematics. The question can be formulated in the following way: Does the effect of socio-cultural variables at the individual level increase with contact with socioeconomic variables at school level? 3. To what extent do the contextual variables of the school (school climate, educational resources available to schools, etc.) have a moderating or amplifying effect on the relationship between the socio-cultural variables linked to the family context of students and academic performance in math? 4. And what are the determinants of the quality of academic performance defined according to TIMSS standards? Added to this is the question of what has been the impact of the educational reform on the dynamics of school organizations and on the quality of academic performance.

A fissura de lábio e ou de palato não sindrômicas são anomalias congênitas craniofaciais mais frequentes. Elas ocasionam problemas estéticos e funcionais que requerem tratamento em longo prazo, envolvendo reabilitação multidisciplinar incluindo a fonoaudiologia e fisioterapia. Este estudo propõem apresentar alguns dos princípios de atuação do fonoaudiólogo e do fisioterapeuta nas fissuras orofaciais não sindrômicas. Realizou-se uma revisão de literatura narrativa com busca na Bireme e Scielo e nas bases Lilacs, Pedro e  PubMed em outubro de 2020, envolvendo a atuação do  fonoaudiólogo e do fisioterapeuta nas fissuras orofaciais não sindrômica, no idioma inglês e português, sem recorte temporal. Os estudos encontrados observaram que a intervenção fonoaudiológica e fisioterapêutica deve ser mais precoce e de acordo com a disfunção apresentada. De modo geral, a atuação do fonoaudiólogo favorece a alimentação oral  e o desenvolvimento global referente à linguagem, a fala, audição e neuropsicomotor, para evitar atrasos e favorecer o melhor desenvolvimento infantil. A atuação fisioterapêutica visa diminuir a hospitalização prolongada, melhorar a qualidade de vida e funcionalidade, bem como assistir as crianças que cursarem com problemas motores, posturais e respiratórios. Conclui-se que a atuação fonoaudiológica nas diferentes fases da reabilitação de indivíduos com fissuras labiopalatinas contribui para alimentação e inteligibilidade da fala, beneficiando assim a comunicação verbal e consequentemente a interação com o meio social; e a assistência fisioterapêutica ajuda e melhorar a sintomatologia e as disfunções respiratórias apresentadas, prevenindo e tratando complicações de forma a melhorar a qualidade de vida e restabelecer a independência funcional.   Palavras-chave: Fissura Palatina. Fenda Labial. Aleitamento Materno. Fala. Fisioterapia.   Abstract Non-syndromic cleft lip and or palate are the most frequent congenital craniofacial anomalies. They cause aesthetic and functional problems that require long-term treatment, involving rehabilitation including speech therapy and physiotherapy. This study proposes to present some of the principles of performance of the speech therapist and physiotherapist in non-syndromic orofacial clefts. A narrative literature review was carried out with searches in Bireme and Scielo and in the Lilacs, Pedro and PubMed databases in October 2020, involving the performance of the speech therapist and physiotherapist in non-syndromic orofacial clefts, in English and Portuguese, with no time frame. The studies observed that speech therapy and physiotherapy intervention should be as earlier and in accordance with the presented dysfunction. In general, the performance of the speech therapist favors oral feeding and the global development related to language, speech, hearing and neuropsychomotor, to avoid delays and favor best child development. Physiotherapeutic action aims to reduce the prolonged hospitalization, improve quality of life and functionality, as well as assist children who are experiencing motor, postural and respiratory problems. It is concluded that the speech therapy performance in the different phases of rehabilitation of individuals with cleft lip and palate contributes to feeding and speech intelligibility, thus benefiting verbal communication and consequently the interaction with the social environment; and physiotherapeutic assistance helps and improves the symptoms and respiratory disorders presented, preventing and treating complications in order to improve the quality of life and restore functional independence.   Keywords: Cleft Palate. I fully appreciate and understand the nature of Dr. Bates' request. The many patients seeking treatment at our clinics make me uncomfortably aware of the problem. I wish I were able to give a more satisfactory answer. Many of the orthodontic problems, especially the severe ones, are best explained on a genetic basis. Therefore, prevention, except for a rather indelicate perusal of the family albums prior to the nuptial vows, is often not possible. In specific instances where the environment is unfavorable, such as crossbite and late thumbsuckers, the orthodontists and pedodontists can and are eliminating more extensive treatment by instituting preventive measures. However, these constituite a minority of the cases.

Progress in medical technology and treatment has resulted in more people with neurodegenerative conditions surviving for longer periods of time. This increased lifespan means that these individuals have a longer period of dependency on others, with a heightened need to maintain quality of life for both the individual and the family. Our paper reports on the findings of a study involving in-depth interviews with 16 carers to determine their perceived needs during the final stage of caring for someone with muscular dystrophy (MD) or motor neurone disease. Results suggest that the palliative care model has much to offer individuals with degenerative neuromuscular conditions and their families, but it is not yet recognized as an important part of care for young people with MD. Three major themes emerged in the analysis: reactions and responses, health system crossing points, reaching forward.

In India, the SARS-CoV-2 COVID-19 pandemic has grown to 112,359 cases and 3,435 deaths as per 21st May, 2020. The severe acute respiratory syndrome (SARS) affected the world with 4,893,186 cases and 323,256 deaths as of 21st May, 2020. The WHO declared the outbreak as a public health emergency of international concern on 30th January, 2020 and it was declared as a pandemic in March 2020. Clinical studies conducted on hospitalized cases show that the onset of COVID-19 is associated with symptoms commonly associated with viral pneumonia, most commonly fever, cough/sore throat and myalgia/fatigue. COVID-19 world pandemic imposed a new set of challenges for the individual to maintain a healthy diet. When compared to the USA, India is currently facing lesser cases of the pandemic because of early implementation of the preventive measures such as a lockdown. Because of the lockdown imposed, India has and is facing a downfall in every aspect, such as the economy, the agriculture, the health sector, the IT and management sectors and also tourism. This in turn, will have a negative impact on the quality of life and economy in India. The present paper aims to find out the impact of COVID-19 pandemic on the health sector and is based on the results obtained from a survey about the eating patterns of participants from Mulund, Mumbai, India. It was seen that the mental stress caused due to the pandemic had an effect on the dietary behaviors of the participants.

The aim is to identify parents’ (legal representatives’) satisfaction with the quality of medical care provided to their disabled children to improve this type of service further. Materials and methods. A sociological survey was conducted of 506 legal representatives of minors (aged from birth to 17 years) with the status of a disabled child. The study design is single-center, non-randomized, uncontrolled. Results. An analysis of the living conditions of a disabled child in the family, the parents’ assessment of his health status, the problems arising in the registration of disability, in the provision of medical and rehabilitation assistance, and issues of medical and social support made it possible to determine the position of this part of the child population in current legal and medical and social conditions. The main problems were the collection of a large number of documents when registering a disability, a long wait for the day of examination, the remoteness of the medical and social examination bureau, the lack of specialist doctors, problems with obtaining subsidized drugs, the lack of consideration of the individual needs of the child when conducting unique rehabilitation programs, the need to apply at the same time to various organizations and departments, violation of rights in the provision of medical services to a disabled child. Conclusion. The obtained information is very significant for further improving the provision of medical and social assistance to disabled children and children with disabilities. The main task today is to develop mechanisms for implementing the declared rights and freedoms of persons with disabilities, the obligations assumed by the State concerning them.

In the wake of the Pope Francis' encyclical Laudato sì which talks about the Earth as a common home of all people, the author offers a draft of ecological spirituality. The beginning of true eco-spirituality is ecological conversion, which is nothing else but a renewed consciousness that from our faith and encounter with Jesus Christ we also ought to change our relationship with the world around us. Ecological conversion should help the faithful to repeatedly and correctly develop their awareness of having been created, of their own immersion into the world of creatures with which they are interconnected. Correct ecological attitude leads to internal balance of the man himself, to solidarity with others, to natural balance of all living beings and to spiritual balance with God. Ecological awareness can help not only to preserve the environment, which is one of the basic goals, but also to change the way of behaviour, which can raise the quality of life of both the individual and the family. Ecological spirituality understands and lives the sacraments in a new way, as a means of salvation and sanctification, because in the sacraments God uses the visible matter in order to communicate his (invisible) grace. Eco-spirituality calls for upbringing and adopting new attitudes in the lives of contemporary Christians which need to help them to properly treat the created world.


ABSTRACT:The goal of the Canadian Migraine Forum was to work towards improving the lives of Canadians with migraine by reducing their migraine-related disability. This paper reviews the epidemiology and diagnosis of migraine, and the effects of migraine on health related quality of life. Many patients with migraine do not consult a physician for their headaches, and when they do they often do not receive a correct diagnosis. The discussion at the Forum concluded that better education, both for physicians and the public, on issues relating to migraine was a necessary step in improving migraine diagnosis. The degree of disability caused by migraine is often not recognized by society, and can be substantial for individuals with migraine. Once again, education of the public and of the health professionals who see these patients is key, so that the best migraine management can be instituted to minimize the impact of migraine on the individual, the family, and society at large.
