Background: The thyroid gland produces two key metabolic hormones which regulate metabolic rate, growth and development. They play vital roles in digestion, heart and muscle function, brain development and maintenance of bones. People suffering from thyroid disorders may have autoimmune disease, ranging from primary hypothyroidism, Hashimoto’s thyroiditis, to hyperthyroidism caused by Graves’ disease. Objectives: To study clinical profile of thyroid disorders in endocrinology clinic of BPKIHS, Nepal. Methods: This is a hospital based retrospective study of past five years (2012 – 2017) done in department of internal medicine at B.P. Koirala Institute of Health Sciences, in which thyroid disorder patient records from the endocrinology clinic will be compiled together will be analyzed after classifying them according to the guidelines of the American Thyroid Association (ATA). Results: Among 584 thyroid disorder cases that were sampled, higher prevalence of thyroid disorders was seen in females, and the Male: Female ratio was 1:4.13. Most common type of thyroid disorder was Hypothyroidism (29.6%) followed by Subclinical hypothyroidism (28.3%). Conclusions: Thyroid disorders are more common in females than males and hypothyroidism being commonest thyroid disorder in our setup. Subclinical hypothyroidism is second commonest followed by hyperthyroidism. In Subclinical hypothyroidism Anti TPO antibody is most commonly found to be positive.

Dengue is a viral febrile illness, which has become a major international public health concern including Bangladesh, spectrum of illness ranging from dengue fever (DF) to life threatening dengue haemorrhagic fever (DHF), dengue shock syndrome (DSS) and ultimately death. The clinical profiling and outcome during epidemic dengue outbreak was observed in Dhaka Medical College Hospital in 2000 to 2001. One hundred and fifty patients were systematically observed. Male female ratio was 5:1 with severity of illness in younger patients. Hundred percent patients had fever with 83% having generalized weakness. The severe cases(DHF) were having sudden sustained temperature of 103 degree farenheight. Majority have abdominal symptoms(>80%) while 10% had respiratory only. Sixty percent have hepatomegaly while 40 % spleenomealy while 80% of DHF had ascites. Above 75% cases of Dengue haemorhagic fever had positive tourniquet test while melaena was the commonest mucosal bleeds. No fatality was observed in this study although variability of discharge was seen in different groups. Objective: depression among adolescents is a serious public problem. The majority of genetic and clinical studies showed that early — onset depressions etiologically may be distinct from adult — onset depressions. Itis possible to assume some differences in aged-dependent risk profile between depressions at adolescence and adulthood.Aim: to study adolescent depressions for identification a number of novel risk factors for increase in frequency and early manifestation of the disorder.Patients and methods: outpatient adolescents registered in one of the Moscow psychoneurological dispensary at 1999–2019 years (total 1704 boys and girls). From the entire cohort we selected all participants appealed for medical aid in connection with depression (F3 of ICD-10) during last decade. Results: the significant increase in the incidence and early manifestation of adolescent depressions during last decade were revealed especially in girls with shift of male/female ratio from 3,1/1 in 2009 year to 1,8/1 in 2019 year. Some specific features of nowadays adolescent depressions are noted: beginning at an earlier age, increasing part of severe depressive episodes, growth of the autoaggressive behavior and suicidal potential. Novel links between adolescent depressions and risk factors are described. Some endocrinological changes resulting from premature pubertal timing and disordered circadian rhythms, social deprivation owing to socio-economic reforms with a lack of social support, low social expectations, absence of positive perspectives leading to low self-esteem and unwarranted self-criticism are turned out to be the most important factors heightening vulnerability to adolescent depressions. Conclusion: identification of pathogenetic mechanisms of adolescent depressions creates an opportunity for early target psychosocial intervention and might help to reduce risk for depression among youth.

Background: Dengue viral infection caused by arthropod borne flavivirus has become endemic worldwide. India contributes to a tune of 20% of global burden. The clinical manifestation ranging from benign dengue fever (DF) to life threatening dengue hemorrhagic fever (DHF)/ dengue shock syndrome (DSS), has varied nonspecific features in infants. study was undertaken during our peak dengue season to know the clinical profile and outcome of dengue viral infection in infants. Methods: This is a prospective observational study. All infants with features suggestive/ clinical suspicion of dengue were enrolled with parental consent. Their demographic detail, history, clinical features were collected, categorised and treated according to WHO 2012 dengue guidelines. Results: A total of 86 infants were enrolled. Male female ratio was 2.44:1. Majority of infants were in Category B (63.9%) Fever being the most common presentation, Gastrointestinal and Respiratory symptoms, CNS manifestation like convulsion/ altered sensorium was noticed in 3.4% cases. Majority of cases were in critical phase (80%), recovery phase with fluid overloaded (edema) features were present in 7% cases. Thrombocytopenia was seen in all infants. Infants in Category C needed ionotropes, ventilator support and blood products, whereas those in other two categories did not. Recovery was seen in 94.25% and mortality was 4.6%.Conclusions: This study highlights the varied presentation of dengue in infants and to have a high index of clinical suspicion for dengue in infants presenting with nonspecific symptoms, for effective care of this dynamic disease.

The author examined completed suicides occurring over a period of 25 years in a county of Hungary with a traditionally low (relatively speaking) suicide rate of 25.8. The rates are clearly higher in villages than in the towns. The male/female ratio was close to 4:1, among elderly though only 1.5:1. The high risk groups are the elderly, divorced, and widowed. Violent methods are chosen in 66.4% of the cases. The rates are particularly high in the period April-July. Prior communication of suicidal intention was revealed in 16.3% of all cases. Previous attempts had been undertaken by 17%, which in turn means that 83% of suicides were first attempts. In our material 10% the victims left suicide notes. Psychiatric disorders were present in 60.1% of the cases, and severe, multiple somatic illnesses (including malignomas) were present in 8.8%. The majority of the data resemble those found in the literature.
