Aim: Department of Medical Laboratory Science (Medical Microbiology Section), Niger Delta University. Tuberculosis, being an oldest known human disease, is a major cause of mortality. Does gender determine Rifampicin Resistance Mycobacterium tuberculosis among the patients with tuberculosis in Bayelsa State? This work was carried out between February, 2019 and September 2019 to determine the prevalence of Mycobacterium tuberculosis and its resistance to Rifampicin anti-mycobacterium therapy among patients attending Federal Medical Centre, Yenogoa, Bayelsa State. Methodology: A total of 250 sputum samples were collected from both HIV and non-HIV patients attending Federal Medical Centre, Mycobacteriology Laboratory (a referral laboratory in Bayelsa state); (age range 21-71 years) and immediately taken to the laboratory for analysis using GeneXpert machine, an automated cartridge based nucleic acid amplification diagnosis and traditional Ziehl-Neelsen test for the detection of Mycobacterium tuberculosis and Rifampicin resistance gene. Results: Findings showed that 50(100%) sputum samples (of which 24(48%) were from males and 26(52%) from females) out of the 250 samples examined were positive to Mycobacterium tuberculosis. Based on the GeneXpert analysis, the total of 8(16%); {4(8%) females and 4(8%) males}, samples were resistant to Rifampicin. Out of 50(100%) positive samples, the highest percentage prevalence of Mycobacterium tuberculosis was recorded within the age range of 21 and 30 years (52%); followed by 31-40 years (44%) and the least was 71 years (04%). The percentage prevalence of Rifampicin resistance Mycobacterium tuberculosis (RMB) was 16.0% while non-resistance was 84.0%; resistance was higher among the study subjects within the age range of between 21 and 30 years (6%) followed by 31-40 years (4%); 51-60 years (4.0%) and the least was 10-20 years and 70 years above(2%). There is no significant difference between the tuberculosis infected male and female resistance to rifampicin at P value= .88, t=14, df=4. However, out of 50 (100%) positive samples, 24 (48%) patients had HIV co-infection with tuberculosis while 26(52%) had tuberculosis only. There was no statistical significant difference between these two groups at P=.29, t=1.2 and df=4. Conclusion: There is a progressive increase in the frequency of Rifampicin resistant Mycobacterium tuberculosis (RrMTB) in this research compared to the percentage prevalence (4%) reported by World Health Organization (WHO) in 2018. There is therefore a need to establish more medical diagnostic laboratories for rapid detection of tuberculosis infection in all the eight (8) Local Government Areas of Bayelsa state.

Background: Tuberculosis is one of the top 10 cause of death globally. Extra-pulmonary tuberculosis is an important clinical problem. Extra-pulmonary tuberculosis range from 30%-53% in India. Diagnosis of extra-pulmonary tuberculosis is still challenging despite many investigations. World Health Organization recommends Gene-Xpert Mycobacterium Tuberculosis/Rifampicin (Cartridge Based Nucleic Acid Amplification Test-CBNAAT) over conventional tests for diagnosis of extra-pulmonary tuberculosis which permits rapid tuberculosis diagnosis through detection of the genetic sequence of DNA of mycobacterium tuberculosis and simultaneous identification of a majority of the mutations that confirm Rifampicin resistance which is highly predictive of multi-drug resistant tuberculosis.Methods: Study was carried out over a period of one year.  Patients with suggestive of extra-pulmonary tuberculosis were included in study. Diagnosis of extra-pulmonary tuberculosis carried out by clinical, radiological, biochemical analysis, cytological, bacteriological confirmation. Based on mycobacterium tuberculosis result, the study population were divided into ‘Mycobacterium Tuberculosis detected’ and ‘Mycobacterium Tuberculosis not detected’ groups. Mycobacterium Tuberculosis detected group was further divided into ‘Rifampicin resistant’ and ‘Rifampicin sensitive’.Results:  Total 220 patients were included. Among extra-pulmonary tuberculosis, there were 83.64% were pleural fluid. 65.91% patients where be <45 years of age. Mostly patients were from rural areas and illiterate. Diabetes Mellitus found as the most common co-morbidities. CB-NAAT was able to detect mycobacterium tuberculosis in 35% (77) extra-pulmonary samples, out of which 6 were rifampicin resistant. Out of 184 samples of pleural fluid, 53 were rifampicin sensitive and 4 were found rifampicin resistant.Conclusions: CB-NAAT has to be endorsed in every health care centres as the test gives rapid result and also detection of rifampicin resistance which is the major concern for every clinician.

Objective: Tuberculosis is an airborne infection caused by Mycobacterium tuberculosis. Timely diagnosis and treatment are important to prevent the spread of infection. Cartridge-based nucleic acid amplification test (CBNAAT) provides a valuable tool in the early detection of TB. This study is undertaken to evaluate the utility of CBNAAT for the detection of MTB. Comparison of cartridge-based nucleic acid amplification testing with ZN staining. Methods: This prospective observational study was carried out in the Department of Microbiology, BLDEDU’s Shri B. M. Patil Medical College, Hospital and RC and Dr. Karigoudar Diagnostic Laboratory, Vijayapur. A total of 129 samples from patients with the presumptive diagnosis of TB based on history, clinical presentation, and radiological findings were included in the study. All samples were subjected to ZN staining, and Cartridge-based nucleic acid amplification test and data were analyzed. Results: The present study showed ZN smear positivity of 7.75% and CBNAAT positivity of 19.38%. CBNAAT sensitivity and specificity were 90% and 86.55, respectively, compared with ZN staining with a significant P value of <0.001. Conclusion: CBNAAT helps diagnose TB and detect rifampicin resistance within 2-3 h with high sensitivity and specificity. Rifampicin resistance detection is of great concern, which otherwise leads to treatment failure and on time spread of multidrug resistance TB, leading to increased morbidity and mortality.

Tuberculosis (TB) is a pulmonary disease caused by Mycobacterium tuberculosis. Globally in 2018 theestimated number of people affected by TB was estimated at 10.0 million population and 484,000 cases ofmultidrug-resistant TB (MDR-TB). This study aims to determine the correlation between microscopic testresults with RMT on TB and MDR-TB patients at RSUD Dr. H Chasan Boesoirie Ternate. This type of researchis analytical research using a retrospective approach. The sample in this study were patients with suspected TBand MDR-TB who had performed microscopic tests and TCM in February – April at 2020 in the ClinicalPathology Laboratory of RSUD Dr. H Chasan Boesoirie Ternate, who met the inclusion and exclusion criteria.Data were analyzed using the SPSS program analysis was carried out in stages, namely by univariate andbivariate using chi-square test. From 100 samples, the results of RMT examination with TB suspects were 30rifampicin sensitive samples with 2 rifampicin resistance and 5 rifampin sensitive samples for TB MDR-TBsuspects RMT examination results. Chi-square test results obtained the value of p = 000 (p <0.05). There is stilla significant difference between the microscopic test results with RMT in TB and MDR-TB suspect patients atRSUD Dr. H Chasan Boesoirie Ternate.

ABSTRACTMolecular diagnostics have revolutionized the management of health care through enhanced detection of disease or infection and effective enrollment into treatment. In recognition of this, the World Health Organization approved the rollout of nucleic acid amplification technologies for identification ofMycobacterium tuberculosisusing platforms such as GeneXpert MTB/RIF, the GenoType MTBDRplusline probe assay, and, more recently, GeneXpert MTB/RIF Ultra. These assays can simultaneously detect tuberculosis infection and assess rifampin resistance. However, their widespread use in health systems requires verification and quality assurance programs. To enable development of these, we report the construction of genetically modified strains ofMycobacterium smegmatisthat mimic the profile ofMycobacterium tuberculosison both the GeneXpert MTB/RIF and the MTBDRplusline probe diagnostic tests. Using site-specific gene editing, we also created derivatives that faithfully mimic the diagnostic result of rifampin-resistantM. tuberculosis, with mutations at positions 513, 516, 526, 531, and 533 in the rifampin resistance-determining region of therpoBgene. Next, we extended this approach to other diseases and demonstrated that aStaphylococcus aureusgene sequence can be introduced intoM. smegmatisto generate a positive response for the SCCmecprobe in the GeneXpert SA Nasal Complete molecular diagnostic cartridge, designed for identification of methicillin-resistantS. aureus. These biomimetic strains are cost-effective, have low biohazard content, accurately mimic drug resistance, and can be produced with relative ease, thus illustrating their potential for widespread use as verification standards for diagnosis of a variety of diseases.

Abstract Background: World Health Organization (WHO) and United Nations Children’s Fund (UNICEF) “Exclusive breastfeeding (EBF)” is explained as “the infant receives only breast milk, no other liquids or solids. Its recommended for the first 6 months of life, and then complementary feeding should be started. EBF helps to reduce child morbidity and mortality from several diseases such as diarrhea, respiratory and /or ear infections and other infectious diseases, shielding lactation women from breast and ovarian cancer. In addition to offers societal economic and environmental benefits, The objective of this study was to assess the knowledge and awareness of exclusive breast feeding among mothers with a child < 2 years of age. Methods : A descriptive cross-sectional study was carried out in Health Directorate of Holy Karbala city. The study consist of (487) mothers how attendant to six Primary Health Care center (PHC). The collected data was done by direct interview with mothers and using a questionnaire form that designed based on the study objectives according to WHO criteria. Data were analyzed by SPSS, descriptive statistics included numbers (N) and percentage (%), and the inferential statistical test was done by using Chi-Square test , P-value was equal or less than 0.05 considered as statistically significant to find for any association between the results variables. Result : A total of 487 participants were included in the study, the rate of response was 92.7%. The highest percentage (66.3%) of study sample were full in the age group (21-30) years, and more than three quarters were lived in urban area. Nearly half of the study sample 47% had primary education and 60% those housewives, and the main source of information being family and friends (84.4%). As an overall assessment, 61.8% of the study sample had good knowledge and awareness about exclusive Breastfeeding, with highly significant differences to education level ,occupation residence of the mother. Conclusion: Over half of the subjects had good knowledge and awareness of exclusive Breastfeeding. And, on the other hand, there was a very significant difference between education level, occupation and mother's residence with a level of knowledge.

Tuberculosis is an infectious disease caused by Mycobacterium tuberculosis. This disease is a health problem in the community. Minimal checks that need to be done to confirm pulmonary tuberculosis diagnostics are smear examinations. ZiehlNelseen method is an inspection method recommended by WHO. Sputum used to make preparations is a mucopurulent sputum. This can lead to infected laboratory workers if inhaled droplets containing Mycobacterium tuberculosis. This study aims to determine the effect of fuchsin carbolic acid and sputum heating before the preparation of the smear result. Temperature used 60oC, 70oC, 80oC and 90oC and control is sputum staining with Ziehl Neelsen method. The samples used positive smear sputum. The experimental research design and data were analyzed with Kruskal Wallistest. The research was conducted at the Bacteriology Laboratory of Medical Laboratory Technology Poltekkes Kemenkes Jakarta III in February to October 2016. The result of statistical test showed p> 0,301 which means there was no significant difference between the dyeing result by using sputum which has been given carbolic fuchsin and heating before the preparation with the result staining method ZiehlNeelsen. The conclusion of this method can coloring Mycobacterium tuberculosis bacteria.

This work assessed the presence of heavy metals in water contaminated with petroleum products in Keffi Local Government Area. Two water sampling station were selected based on the stratified method of sampling of Antau River. Station A is located along Federal Government College Keffi popularly known as Antau Bridge where Agricultural and mechanical activities, with sales of petroleum product take place. Station B was located at Dadin Kowa known as Gada Manu where human activities such as bathing and washing takes place which served as control. The distance between stations is 1km. The procedural plan of this study was monthly sampling of water from both Stations. Materials such as funnel, sampling container, distilled water, conical flask, filter paper, 50ml polypropylene centrifuge tube, cooler were used. Heavy metal concentration was determined using X – ray Fluorescent Spectroscopy (XRF) at the Center for Energy Research and Training (CERT), Ahmadu Bello University Zaria. The water parameters of the four samples were determined using the Extech instrument. The result on Physico – chemical parameters revealed the values obtained from contaminated water samples to be pH (7.31), Dissolved Oxygen (1.14), Electrical Conductivity (8.22), Total Dissolved Solid (4.13), Temperature (27.45) and Salinity (0.091) while the controlled Station at point B were pH (7.43), DO (7.35), EC (3.14), TDS (1.54), Temperature (26.75), Salinity (0.o94) respectively. From the result obtained, the heavy metal contaminated samples had the following values: Mg (29.31±), Al (218.69±), Si (230.12±), S (1153.65±), Cl (54.25±), Ca (106.56±), Ti (23.66±), Cr (3.15±), Fe (15.92±), Cd (7.54±). For the control sample at point B, the following values were obtained: Mg (27.49±), Al (223.71±), Si (243.21±), S (94.84±), Cl (23.52±), Ca (28.44±), Ti (32.99±), Fe (16.63±), Cd (7.67±) respectively. The P-value was 0.8105 (p>0.05) for the water quality parameters while for the concentration of heavy metal P-value was 0.2054 (p>0.05). This implies that there was no significant difference between the concentrations of elements. However, the concentration values of heavy metals in the contaminated water at Station A and control water samples at Station B were higher than the World Health Organization values (Standard) for drinking water

Introduction: In developing countries, which is an endemic region in terms of tuberculosis, there is an urgent need for fast, accurate, and inexpensive serological testing. The aim of this study was to determine the diagnostic value of patient serum IgG antibodies by ELISA in the diagnosis of Mycobacterium tuberculosis. Method: This case-control study was performed on patients with pulmonary tuberculosis in 2017-2020. After selecting the case (n = 30) and control (n = 30) subjects according to inclusion criteria, their blood samples were obtained and analyzed in the reference laboratory by standard kits for immunoglobulin G against 16, 36, and 40 kDa antigens of mycobacterium tuberculosis. Results: The mean age of the subjects was 47.07 (15.57%). The majority of participants were 46 (51.1%) women. There was no significant difference between the two groups regarding sex and age. serological examination of patients with pulmonary tuberculosis showed 25 positive results and only 4 of the control group had a positive result. Sensitivity, specificity, positive and negative predictive values of serology test were 83.3%, 86.67%, 86.20% and 87.88% respectively. Conclusion: Despite the acceptable sensitivity of the serologic immunoglobulin G test, according to the statement of World health organization (WHO), it did not possess an acceptable specificity. It is recommended that a a wider range of different antigens to be studied also it is essential to evaluate the diagnostic value of the other immunoglobulins inpatient in different stages of the disease.

Background: According to World Health Organization (WHO), self medication is use of medicines to treat self recognized symptoms, or use of a prescribed medicine for chronic symptoms. It is associated with use of prescription drugs, alternative medicines and over the counter drugs (OTC). Methods: This was a prospective, observational, questionnaire based study where two groups of students were interviewed with a prevalidated questionnaire. The first group comprised of students from MBBS while the second group consisted of students of paramedical courses namely physiotherapy, operation theatre technicians and lab technicians. Results: A total of 200 students with an average age 19.62±0.65 years participated in the study. Among these students, 100 were medical students. All the students had practiced self medication however, only 51.00% agreed with the concept of self-medication. Considering the significant „p‟ value to be <0.05 there was a significant difference among the 2 groups with respect to awareness of generic and brand names of the drugs and importance of completing the course of treatment. 51.00% of medical and 29.00% paramedical students were aware about generic and brand names of drugs while knowledge about importance of completing course of treatment was present in 62% medical and 42% paramedical students. (p value<0.05). Also, there was no significant difference regarding the knowledge about the content, dose, duration of treatment and frequency of drug administration among the two groups. Conclusion: The present study shows a high prevalence of selfmedication among medical and paramedical students of the institution. The knowledge about self-medication was encouraging among the students of both the groups. Keywords: Self medication, student, knowledge

Recurrent abortion consider as one of the most common problem in world. There are different causes associated with recurrent abortion, these may be occur due to complication of microorganism infection or associated with immunological disturbance like abortion caused by Anti-phospholipid syndrome, or associated with SLE. The anti-b2glucoprotien antibodies have a role in abortion. In this study, we wish to determine whether there is a relationship between the levels of these anti-b2glucoprotien antibodies and abortion. Thirty females patients with spontaneous abortion (10 of them with Antiphospholipid syndrome,10 of them with systemic lupus erthymatosus and final 10 with toxoplasmosis with ages ranged between (20-46) where taken from (Al-Hussein Medical City/Kerbala). Control group consisted of 20 healthy people who were free from history of abortion who matched in age and gender with patients. B2gp(IgMandIgG) EASIA Kit, generic assay) and was studied using the enzyme-linked immunosorbent assay (ELISA) method. T-test and ANOVA and Pearson correlation used to analyze results by using SPSS version 24. P-value ≤ 0.05 was considered significant. The patients with age range blow 30 years show high percent(63.3%) among the other groups .most of the patients have high percent of 3 times abortion were reported in all different groups .The mean value of anti-B2gP-I IgG (45.354) show highly a significant difference results in aborted patients with APS and significant difference in patients with SLE when compared with controls groups. the Anti-B2gPI IgM not give statistically significant difference when camper to health cases. we conclude that the Anti-B2GPI IgG levels were increased significantly in aborted females with APS and SLE .
