Background: Vitiligo is a chronic cutaneous disease characterized by depigmented patches that leave psychological impact on the patients. New treatment modalities have been developed to improve the results vitiligo with less side effects. Objective: To evaluate intralesional injection of 5-fluorouracil in treatment of vitiligo. Patients and Methods: The study included 20 localized stable vitiligo patients. Each patient was treated with intralesional injection of 5-fluorouracil every 2 weeks for 3 sessions followed by narrow band sessions twice weekly for 3 months. Results: There was statistically significant repigmentation after treatment with intralesional 5-FU (mean of 50.30 ± 34.60, P value =0.001 Wilcoxon signed ranks test between before and after). 55% of patients showed >50% repigmentation. after 3 months therapy. Side effects were minimal and transient. Conclusion: Intralesional injection of 5-FU is safe and effective in the treatment of vitiligo.

Background: Vitiligo is a chronic cutaneous disease characterized by milky white depigmented patches that leave psychological impact on the patient's quality of life. New treatment modalities have been developed to shorten the duration of treatment of vitiligo with the least side effects. Objective: To evaluate the safety & efficacy of microneedling in combination with narrow band-UVB in the treatment of vitiligo. Patients and Methods: This study included 20 patients with stable vitiligo. They were treated by microneedling (one session every 2 weeks) in combination with narrow band-UVB (3 sessions weekly) for 3 months. Results: The studied patients reported statistically significant degree of clinical improvements as follow;10%reportedgoodimprovement, 25% showe dmoderate improvement, 45% showed mild improvement and 20% showed noimprovement,after3months therapy. The reported side effects were minimal and transient in the form of minor pain, burning sensation and erythema at site of microneedling that disappeared spontaneously within few hours. Conclusion: Microneedling in combination with narrow band-UVB phototherapy could be considered as effective treatment of vitiligo. Microneedling is a tolerable technique, harmless with negligible side effects.

AbstrakKanker payudara adalah penyebab kematian terbesar pada wanita. Salah satu hal yang penting untuk diperhatikan adalah pada pasien kanker payudara yang menjalani kemoterapi akan mengalami efek samping mual muntah, rambut rontok, nyeri, kehilangan nafsu makan dan lain- lain. Penelitian saat ini bertujuan untuk mengetahui pengaruh konseling terhadap tingkat pemahaman penyakit dan efek samping kemoterapi pasien kanker payudara Penderita kanker payudara perlu mengetahui penyakitnya dengan baik agar kualitas hidupnya dapat tetap terjaga. Edukasi yang dilakukan secara perseorangan maupun kelompok pada penderita tentang efek samping obat kemoterapi sangat diperlukan supaya penderita tidak merasa cemas apabila mengalami efek dari kemoterapi tersebut. Pemberian konseling oleh Apoteker pada pasien kemoterapi kanker payudara sangat penting karena dapat meningkatkan pengetahuan pasien, kepatuhan pasien dalam pengobatannya serta dapat meningkatkan kualitas hidup pasien. Tujuan penelitian ini untuk mengetahui pengaruh pemberian konseling oleh Apoteker terhadap tingkat pemahaman penyakit dan efek samping kemoterapi pada pasien kanker payudara di Unit Kemoterapi Salah Satu Rumkit Pemerintah Di Kota Malang..Penelitian ini merupakan studi kuantitatif dengan desain Pre-Experimental berupa pretest-postes design. Banyaknya sampel yang diambil didasarkan atas waktu yaitu selama satu bulan dan sampel diambil dengan metode consecutive sampling. Hasil penelitian menggunakan uji Wilcoxon menunjukkan terdapat perbedaan nilai sebelum dan sesudah diberikan konseling oleh apoteker terhadap pasien kemoterapi kanker payudara dengan nilai signifikansi sebesar 0,000 (P value< 0,05). Kesimpulan pada penelitian ini adalah pemberian konseling oleh apoteker dapat meningkatkan tingkat pemahaman penyakit dan efek samping dari kemoterapi kanker payudara.Kata kunci : konseling, pemahaman, penyakit kanker payudara, efek samping kemoterapi Abstract Breast cancer is the leading cause of death in women. One important thing to note is that breast cancer patients who undergo chemotherapy experience side effects of nausea and vomiting, hair loss, pain, loss of appetite and others. The current study aims to determine the effect of counseling on the level of understanding of the disease and the side effects of breast cancer chemotherapy patients Breast cancer patients need to know the disease well to the quality of life can be maintained. Education is done individually or in groups to the patient about the side effects of chemotherapy drugs is indispensable so that the patient does not feel anxious when experiencing the effects of chemotherapy. Provision of counseling by pharmacists in patients with breast cancer chemotherapy is very important because it can improve patient knowledge, patient compliance in the treatment and can improve the quality of life of patients. The purpose of this study to determine the effect of counseling by pharmacists to the level of understanding of the disease and the side effects of chemotherapy in breast cancer patients on Chemotherapy Unit One Government Hospital In Malang .. <p><span>Technology such as smartphones is growing rapidly. Development of applications such as instant messengers, social networks even easier for everyone to communicate, the distance is not an obstacle now. In addition to ease of access, the smartphone can also give confidence to users, in addition to models of smartphones increasingly stylish, applications that can be installed therein can make smartphone users are always updated with the latest news. The psychological impact caused by the use of smartphones on individuals and society in general about changing behavior and habits before and after use. Nomophobia (no-mobile-phone-phobia) is a type of phobia characterized by excessive fear if someone loses or does not exist near his smartphone. People who experience nomophobia always live in fear and are always anxious to put or keep his smartphone, so always take it anywhere to go. The questionnaire used to nomophobia is an adaptation of the questionnaire Yildirim (2014), composed of 4 dimensions with 20 statements. As for the questionnaire confidence, based on the theory of Justice (2002) derived into 3 dimensions with 46 statements. Results of the reliability values obtained for instruments confidence of 0.944, while the result of the reliability values for nomophobia instrument amounted to 0,939. The results of the data analysis of the correlation between nomophobia and confidence showed a p-value = 0.626&gt; α = 0.05. Poppers are legal and largely used in France despite severe side effects, such as methaemoglobinaemia (MetHbia). Our work aimed to assess the prevalence of poppers consumers among patients with a MetHbia higher than or equal to 5% in French university hospitals and its evolution before and after the legalization of poppers in France. We conducted a national multicentre observational retrospective study. All patients for whom at least one MetHbia measurement was performed from 2012 to 2017 in university hospitals where the French addictovigilance network (FAN) is implanted were included. For each MetHbia measurement exceeding or equal to 5%, a return to the clinical file was made by the FAN to assess poppers consumption. We calculated the prevalence of MetHbia exceeding or equal to 5% and 25% and the prevalence of poppers consumption before and after the legalization. A total of 239 (0.14%) patients had a MetHbia level exceeding or equal to 5% with 25 (10.46%) cases of poppers consumption. Poppers consumption represented 68.4% (13 out of 19) of cases with MetHbia greater than or equal to 25%. Poppers consumption among patients with MetHbia exceeding or equal to 5% increased after the legalization from 4.76% to 11.67% (prevalence ratio PR = 2.45, 95% CI = [0.98–8.37], p-value = 0.190). The proportion of patients with a MetHbia level of 25% or more increased after the legalization from 4.76% to 8.63% (PR = 1.81, 95% CI = [0.68–6.82], p-value = 0.374). The use of poppers is very frequently reported by patients with MetHbia greater than or equal to 25%.

Background: Polycystic ovary syndrome (PCOS) is a common disease that affects up to 10% of women of reproductive age, in which hyperandrogenism (HA), enlarged cystic ovaries, and chronic anovulation often co-exist with obesity, dyslipidemia and insulin resistance (IR). There is a need for an alternative to metformin with minimal side effects to improve insulin sensitivity and correct dyslipidemia in PCOS patients.Methods: It was a prospective, Randomized controlled clinical trial. 116 PCOS patients, 58 each in two groups received either Metformin 500 mg TDS or N-acetylcysteine (NAC) 600 mg TDS for 3 months. Clinical and biochemical parameters contributing to metabolic syndrome (MS) and insulin resistance (fasting blood sugar (FBS), fasting insulin (FI), FBS:FI, HOMA-IR and QUICKI) were assessed at the start and end of the study. Results were compared between the two groups.Results: Both treatment modalities resulted in a significant reduction in number of cases with IR (p=0.001) and MS. Fasting hyperinsulinemia improved in 15 % (p=0.12) and 30% (p=0.001) of patients while 12% (p=0.23) and 18% (p=0.049) improvement was seen in FBS:FI in MET and NAC group respectively. Similarly, improvement in HOMA-IR was 12% (p=0.30) and 32% (p=0.001) in MET and NAC group which is significant with NAC. QUICKI and impaired glucose tolerance showed significant improvement in both the groups with a p-value of 0.04 and 0.006, 0.035 and 0.046 respectively. Significant reduction was seen in triglycerides (p=0.048) in NAC group.Conclusions: NAC is equally efficacious as metformin in improving parameters of insulin resistance and metabolic syndrome with minimal occasional side effects ensuring better compliance for a long-term therapy.

Bone metastases are common complications of solid tumors, particularly those of the prostate, breast, and lungs. Bone metastases can lead to painful and devastating skeletal-related events (SREs), such as pathological fractures and nerve compressions. Despite advances in treatment for cancers in general, options for bone metastases remain inadequate and generally palliative. Anticancer drugs (chemotherapy and radiopharmaceuticals) do not achieve therapeutic concentrations in the bone and are associated with dose-limiting side effects to healthy tissues. Nanomedicines, with their tunable characteristics, have the potential to improve drug targeting to bone metastases while decreasing side effects for their effective treatment. In this review, we present the current state of the art for nanomedicines to treat bone metastases. We also discuss new treatment modalities enhanced by nanomedicine and their effects on SREs and disease progression.

(1) Background: There are limited data on the success of conservative treatment of high-grade cervical squamous intraepithelial lesions (HSIL) with imiquimod directly compared to standard of treatment with LLETZ. (2) Methods: Patients aged 18–40 with histological HSIL (with high-grade cervical intraepithelial neoplasia, CIN2p16+ and CIN3), were randomly assigned to treatment with imiquimod or LLETZ. The primary outcome was defined as the absence of HSIL after either treatment modality. The secondary outcomes were the occurrence of side effects. (3) Results: 52 patients were allocated in each group and were similar regarding baseline characteristics. In the imiquimod group, 82.7% of patients completed treatment, which was successful in 51.9%. All patients in the LLETZ group completed treatment, which was successful in 92.3% (p < 0.001). In the subgroup of CIN2p16+ patients, treatment with imiquimod was not inferior to LLETZ (73.9% vs. 84.2%, p = 0.477). During and after treatment, no cases of progression to cancer were observed. Side effects and severe side effects (local and systemic) were more prevalent in the imiquimod than in the LLETZ group (88.5% vs. 44.2% (p-value < 0.001) and 51.9% vs. 13.5% (p-value < 0.001), respectively). (4) Conclusion: Generally, in patients with HSIL, LLETZ remains the gold standard of treatment. However, in a subgroup analysis of patients with CIN2p16+, the success rate was comparable between the two treatment modalities. Due to the prevalence of side effects, the treatment compliance with imiquimod use may, however, present a clinically important issue.

Objective: To evaluate the efficacy and safety of a novel tomato-based food supplement on the lower urinary tract symptoms (LUTS) of patients with benign prostatic hyperplasia (BPH). Methods: Twenty patients with BPH were enrolled in this observational study. They were assigned to consume daily a sachet of Lycoprozen® (5 grams) dissolved in water for two months. Results: All patients successfully completed the Lycoprozen scheduled regimen and the IPSS (International Prostatic Symptom Score) questionnaire before and after treatment. No side effects due to treatment were noticed. In this preliminary study, we have found that Lycoprozen® significantly reduced the LUTS severity (paired t-test, two-tailed p value < 0.0001). The IPSS mean values before and after the treatment were 16.95+6.0 SD (range 31-6) and 12.2+4.9 SD (range 20-2), respectively. Conclusions: Based on these data, Lycoprozen® may represent a suitable alternative option for the treatment of symptomatic BPH patients which worth of further testing in a phase 2 prospective randomized double blind placebo controlled study. The treatment was without side effects and acceptance among patients was high.

Apathy is a disabling condition with a large burden of disease and lack of effective treatments. Over the past decades, new treatment modalities have been explored, including neuromodulation with repetitive transcranial magnetic stimulation (rTMS). An accepted treatment for depression in many countries, rTMS is well tolerated and has few side effects. The published evidence indicates that rTMS can reduce apathy/negative symptoms, although effects may be modest and transient. Less evidence is available for transcranial direct current stimulation, though it may also have potential to improve apathy. Thorough studies are needed to establish moderators of effect in order to optimize treatment parameters and thereby achieve more durable and clinically relevant effects.

A 27-year-old patient suffering from deeply infiltrating rectovaginal endometriosis was treated with 2 mg/day dienogest vaginally for 3 months. The therapy was tolerated very well. The patient reported less side effects compared to the oral use of dienogest. After 3 months of dienogest treatment, the rectovaginal gynecological examination identified the visible vaginal part of endometriosis in remission. The firm endometriosis node approximately 3 cm in size and approximately 10 cm ab ano was still palpable, but it was much less painful. The laboratory values for luteinizing hormone (LH) and follicle-stimulating hormone (FSH) were unremarkable, with an LH/FSH quotient of 0.7 during dienogest treatment, while 17-β estradiol and progesterone were suppressed. At palpation and vaginal ultrasonography, there was no change in the findings before and after 3 months of dienogest treatment, but the patient was now de facto asymptomatic. To the best of our knowledge, this is the first report of a vaginal dienogest treatment in symptomatic deeply infiltrating rectovaginal endometriosis. Vaginal administration of dienogest should receive further investigation in pharmacokinetic and clinical studies.
