Objectives: To determine the serum calcium levels in mild and severe preeclampsia and compare it with normal pregnancy. Methods: It was a comparative cross sectional study. Thirty five normotensive pregnant women, n=30 women with mild preeclampsia and n=70 with severe preeclampsia were recruited at Liaquat University of Medical and Health Sciences, Hospital. The serum calcium was measured and levels were compared among three groups. The data was analysed on SPSS v. 20. Results: The gestational age was more in the normal pregnancy compared to mild and severe preeclampsia (38.7±2.1 vs 36.7±3.6 and 36.2±2.8, p-value=0.0002). The calcium concentration was less in the both groups of preeclampsia i.e., 8.41± 0.96 mg/dl in mild and 8.02± 0.77 mg/dl in severe preeclampsia vs 9.2± 0.32 mg/dl in normal pregnant women, (p-value <0.001). Conclusion: We found lower levels of serum calcium in preeclampsia, suggesting its possible role in pathogenesis. Further studies to investigate the potential role of dietary supplementation of micronutrients during pregnancy are recommended.

BACKGROUND: Abortion remains a problem in the obstetric care as it is one of the causes of maternal and fetal death. Bleeding or miscarriage threat in the first trimester comprises 20% to 25% of all pregnant women, and 50% of them will end as spontaneous abortion. The major cause of maternal death in abortion is hemorrhage. Some recent studies have indicated that MMP-9 is important in the process of embryo implantation into the endometrium and spontaneous abortion occurs when there is an overwhelming increase of MMP-9. This data indicate the importance of a further research to elucidate the role of MMP-9 in spontaneous abortion.METHODS: This was a cross sectional study, included 70 pregnant women with gestational age <20 weeks who came for examination and treatment at the Department of Obstetrics and Gynecology, Medical Faculty of Padjadjaran University/Hasan Sadikin Hospital, Bandung. RESULTS: There were differences in the MMP-9 levels in spontaneous abortion compared to normal pregnancy and MMP-9 had a cut-off point >1221.7 with the sensitivity of 48.6% specificity of 80%, and accuracy of 60.9%.SUMMARY: The MMP-9 level in spontaneous abortion was higher than in normal pregnancy.KEYWORDS: MMP-9, spontaneous abortion, normal pregnancy

Overview The purpose of this study was to evaluate the role of Tumor Necrosis Factor-α (TNF-α) in insulin resistance (IR) during normal pregnancy. Approach This cross sectional study was carried out on 86 healthy pregnant women including 26, 23 and 37 individuals in the 1st, 2nd and 3rd trimesters, respectively, and in 21 healthy non pregnant women. Serum TNF-α concentration was measured by Enzyme Linked Immunosorbent Assay (ELISA) method. Results There were significant differences between serum TNF-α and IL-6 levels in pregnant women as compared with maternal healthy controls. There was significant correlation between gestational age and Body Mass Index (BMI) (r = 0.28, P = 0.01). There was no significant correlation between gestational age and insulin resistance (IR). We also did not find correlations between IR and TNF-α and IR and IL-6 in pregnant women. Conclusion In conclusion, our findings suggest that TNF-α and IL-6 are not greatly contributed to pregnancy induced insulin resistance in normal pregnancy.

The aim of the present study was to assess leptin/adiponectin ratio in preeclamptic patients compared with normal pregnant women. A cross-sectional study was designed. The study population consisted of 30 preeclamptic patients and 30 healthy pregnant women. Serum levels of total leptin and adiponectin were assessed using commercially available enzyme-linked immunosorbent assay methods. The oneway ANOVA and Student’s t tests and Pearson’s correlation analysis were used for statistical calculations. Levels of leptin and adiponectin were also adjusted for BMI. A p-value <0.05 was considered statistically significant. The leptin/adiponectin ratio was increased significantly in preeclamptic patients. The leptin/adiponectin ratio was significantly higher in severe preeclamptic patient than in mild preeclampsia. Adjusted leptin/adiponectin ratio was also significantly increased in preeclamptic patients than in normal pregnant women. The findings of the present study suggest that the leptin/adiponectin ratio was increased in preeclamsia and imbalance between the adipocytokines could be involved in the pathogenesis of preeclampsia. ABSTRACT Background Pre-eclampsia remains the most threatening obstetric complication in Pakistan, resulting in high fetomaternal morbidity and mortality. Etiology of pre-eclampsia is not still clearly established. Association of low serum calcium levels with pre-eclampsia may be a factor in poorly nourished women, and some preventive measures may help to reduce the burden of disease. Objective To compare mean serum calcium levels in preeclamptic and normotensive pregnant women at third trimester of gestation. Study design Cross-sectional study. Place and duration of study The Department of Obstetrics and Gynecology, Unit-1, Sir Ganga Ram Hospital, Lahore, from July 2012 to July 2013. Methodology Two hundred patients were enrolled in the study. One hundred each in case and control group. History and examination performed. Proteinuria was checked by dip stick methods. Peripheral venous blood samples were taken to determine mean serum calcium levels of both the groups. Oral calcium supplementation was analyzed through stratification. Data was analyzed through SPSS by applying student's t test and calculating p-value. Results The mean serum calcium levels of the case group were 8.313 ± 0.377 while that of control group were 9.40 ± 0.427 with p-value 0.016. With increasing proteinuria (determined by dipstick method) and so the severity of pre-eclampsia, a gradual fall in serum calcium levels was observed. Conclusion Serum calcium levels in pre-eclamptic pregnant women are lower than in normal pregnant women and so hypocalcemia is associated with pre-eclampsia. Objective: To determine the frequency of hypocalcemia in neonates with unconjugated hyperbilirubinemia receivingphototherapy.Study design and setting: Cross sectional study conducted at neonatal intensive care unit, King Abdullah Teaching Hospital,Mansehra for one year from December 2017 to November 2018.Methodology: Total 213 full term stable neonates of either gender with jaundice were studied in this study. Out of which,143 with unconjugated hyperbilirubinemia were exposed to phototherapy while 70 neonates with exaggerated physiologicalhyperbilirubinemia taken as control were not exposed to phototherapy. Serum calcium level was determined through bloodtest before and after 24 hours of phototherapy. SPSS version 22 was used to analyze the data. Frequency and percentageswere used to describe categorical variables like gender and hypocalcemia. Hypocalcemia was stratified by age and genderto see effect modifiers. Post stratified chi-square test was applied in which p value = 0.05 was considered as significancevalue.Results: In study group, 143 neonates who received phototherapy had mean age of 7 days ± 2.62 SD. Total 65% neonateswere male and 35% neonates were female. Mean serum calcium level of neonates before and after provision of phototherapywas 9.28 mg/dl ± 0.23 and 8.54 mg/dl ± 0.68 respectively, which is statistically significant. The frequency of hypocalcemiawas 40% in neonates with unconjugated hyperbilirubinemia after 24 hours of phototherapy.Conclusion: Hypocalcemia is an important complication in neonates with unconjugated hyperbilirubinemia after continuousphototherapy. Hypocalcemia has clinical impact and adds to morbidity, and if left untreated, can lead to mortality.

Antenatal care or antenatal care is a planned program that is observation, education and medical treatment for pregnant women, to obtain a safe and satisfying pregnancy and childbirth process. The indicator used to assess the access of pregnant women to antenatal care is K1 direction (first visit) is the contact of pregnant women to health workers and K4 (perspective visit) is 4 or more times contact with health workers. From the data of the Padang City Health Office, it was found that the Air Tawar Puskesmas with this low level was caused by various factors. The purpose of this study was to determine factors related to the use of antenatal services. This type of research is a quantitative study with cross sectional study design with 110 respondents of third trimester pregnant women with purposive sampling technique. Data analysis was performed univariate, bivariate with Chi squre test. From the research results obtained from the five factors studied, it was found that the factors related to the use of antenatal services in freshwater health centers were access or distance with pvalue 0.009 = OR 21.676), service availability factors with a p value of 0.001 = OR 9.293, the role factor midwives with a p value of 0.001 = OR 12.302, while the family income factor, disease complaints obtained results have nothing to do with the use of antenatal services. The author's suggestion is that there is a need for good coordination with the Independent Practice Midwife (BPM), revitalize puskesmas to more strategic places and improve better facilities, improve accessibility that can be minimized, strive to reach pregnant women, especially for accessibility that is less affordable.

Social media use: The role of parents' perceptions about social media impact in early marriageBackground: Early marriage is a marriage that is conducted at adolescence, the factors causing early marriage are socio-cultural factors, economic pressure, level of education, difficulty in getting a job, social media, religion and views and beliefsPurpose: To find out the influence of social media and the role of parents on the incidence of early marriageMethod: A quantitative research using descriptive analytical research method with cross sectional study design with variable use of social media with the criteria for the results "active and inactive". For the variable of the role of parents with 2 outcome criteria, namely "influential and not influential". And for the variable incidence of early marriage, there are 2 criteria, namely age 14-16 years and age 17-19 years, the questionnaire used has been through the validity of previous researchers. Then the results of the data obtained were analyzed in the SPSS program using the Chi Square testResults: From the Chi Square test results for social media variables obtained p value = 0.001, then the value of p = 0.001 <0.05 (α) while the role of parents variable Chi Square test results obtained p value = 0.022, therefore the value of p = 0.022 <0.05 (α).Conclusion: Based on the results of the study it can be concluded that, there is a significant influence between the use of social media and the role of parents in the event of early marriage Keywords: Social media; Parents, Early marriagePendahuluan: Pernikahan usia dini adalah perkawinan yang dilakukan pada usia remaja, faktor penyebab pernikahan usia dini adalah faktor sosial budaya, desakan ekonomi, tingkat pendidikan, sulit mendapat pekerjaan, media sosial, agama serta pandangan dan kepercayaan.Tujuan: Untuk mengetahui pengaruh media sosial dan peran orang tua terhadap kejadian pernikahan dini di Kecamatan Marioriawa Kabupaten Soppeng Metode: Penelitian kuantitatif dengan menggunakan metode penelitian deskriptif analitik dengan rancangan cross sectional study dengan variabel penggunaan media sosial dengan kriteria hasil “aktif dan tidak aktif”. Untuk variabel peran orang tua dengan 2 kriteria hasil yaitu “berpengaruh dan tidak berpengaruh”. Dan untuk variabel kejadian pernikahan dini ada 2 kriteria  yaitu umur 14-16 tahun dan umur 17-19 tahun,  kuesioner yang di gunakan sudah melalui uji validitas peneliti sebelumnya. Kemudian hasil data yang di dapatkan dianalisis di program SPSS dengan menggunakan uji Chi SquareHasil: Dari hasil uji Chi Square untuk variabel media sosial didapatkan nilai p=0,001, maka nilai p=0.001<0.05 (α) sedangkan variabel peran orang tua hasil uji Chi Square didapatkan nilai p=0,022, oleh karena itu nilai p=0.022<0.05 (α).Simpulan: Berdasarkan hasil penelitian dapat di simpulkan bahwa, Ada pengaruh yang signifikan antara penggunaan media sosial dan peran orang tua terhadap kejadian pernikahan dini

Background: The role of serum calcium in coronary artery disease (CAD) patients with or without first incident acute myocardial infarction has not been studied previously. This study aimed to assess the relationship between serum calcium and first incident acute myocardial infarction. Methods: This cross-sectional study was conducted from Jan 2014 to Dec 2016. All the participants were from our database, described in detail elsewhere including 1609 cases and 3252 controls. Multiple logistic regression was carried out to explore the effect of serum calcium on first incident acute myocardial infarction. Interaction between serum calcium and risk factors were evaluated. Results: Patients with first incident acute myocardial infarction have significantly lower serum calcium concentrations than those without acute myocardial infarction (2.18 (0.21) vs 2.24 (0.19) mmol/L, P<0.0001). After adjusting for sex and age, logistic regression showed that serum calcium was significantly associated with first incident acute myocardial infarction (odds ratio (OR): 1.50, 95% confidence interval (CI): 1.41-1.60). Further adjusted for potential confounders, serum calcium was associated with first incident acute myocardial infarction (OR: 1.32, 95% CI: 1.22-1.42). Moreover, the association still existed when patients were divided into subgroups according to gender and age. A significant interaction was found between serum calcium and diabetes mellitus (DM), lipoprotein (a) (Lp (a)), and serum albumin. Conclusion: Serum calcium was associated with first incident acute myocardial infarction among CAD patients in both sexes and in age categories. This study provides further evidence showing the value of serum calcium levels in clinical practice.

Background: Preeclampsia is the multi-systemic hypertensive pregnancy disorder accompanied by proteinuria. Objectives: To determine the frequency of different presentations of preeclampsia in tertiary care hospital and find out its risk factors. Materials and Methods: Present study was hospital based cross-sectional study and conducted from 1st August 2015 to 31st July 2016 in Gynaecology and Obstetrics department, Liaquat University of medical and health sciences (LUMHS), Hyderabad after ethical approval. A total of 112 pre-eclamptic women were enrolled during the study period by non-probability consecutive sampling. Patients were divided on the basis of their presentations into mild preeclampsia, severe preeclampsia, antepartum eclampsia, intrapartum eclampsia, HELLP syndrome, postpartum preeclampsia, postpartum eclampsia and atypical preeclampsia/eclampsia. All the socio-demographic factors and clinical variables were noted. Frequency and percentage were calculated for categorical variable and mean/standard deviation (SD) for continuous variables. Results: Of the 112 preeclamptic women, 54.5% admitted with antepartum eclampsia, 12.5% with severe preeclampsia, 8.9% with atypical preeclampsia/eclampsia, 8% with mild preeclampsia, 8% with postpartum eclampsia, 3.6% with HELLP syndrome, 2.7% with intrapartum eclampsia and 1.8% with postpartum preeclampsia. Over all, majority of the patients were primigravida (57%), had gestational age >34 weeks at presentation (58.9%) and <7 antenatal visits (88.3%) during their pregnancy. Over all 17.8% had previous bad obstetrical event, 11.6% had previous history of preeclampsia and 64.3% had consanguineous marriages. Conclusion: Different presentations of preeclampsia may help obstetricians to rule out high risk pregnancies and provide antenatal care to patients earlier to prevent complications to both mother and fetus.

Introduction: Respiratory physiotherapy is one of the therapeutic methods in various respiratoryviral infections. Breathing exercises with other interventions, could enhance the total lung capacityand symptom alleviation in patients with virus-related acute respiratory distress syndrome (ARDS).Aim: To study the clinical evaluation of the role of physiotherapy in hospitalised patients of covid-19diseases. Method: This was an Observational (prospective, cross-sectional) study. 187 patientswere recruited and diagnosed as COVID-19 as per RTPCR at NSCB Medical College, Jabalpur (MP).Patients were randomised into the physiotherapy group and controlled group. Patients in thephysiotherapy group were undergone positional changes, prone ventilation, Breathing exercise,Resistive training, Passive joint motion, Muscle stretching, Bedside standing training, enduranceexercise by a specialized physiotherapist. Mean cough severity index, MMRC grade, respiration rateand peripheral blood saturation were recorded before and after intervention and compared with thecontrolled group. Result: There was significant improvement found in MMRC grade and respirationrate after physical therapy in the physiotherapy group (p-value < 0.05). Exertional dyspnoea andcough severity index significant deceased in physiotherapy group. The positive change was found inperipheral blood saturation. There was decreased in hospitalization stay in the physiotherapy groupas compared to the controlled group. Conclusion: Our study revealed the positive effect ofphysiotherapy on covid-19 disease in terms of symptoms and hospitalization duration among thephysiotherapy group. Physiotherapy exerts a beneficial role in the management of the covid-19disease.
