In this study, the authors examine patient use of and feelings about wearable technologies for health attainment and management. Based on an online survey of 81 patients using wearable technologies to track and manage health, as well as interviews with three patients utilizing wearables for health management, the authors examine how wearable technologies are being used by patients to attain health, manage health, and/or prevent health issues, and what value users find in these wearable technologies. The authors also examine how such use is impacting communication between medical professionals and patients. Specifically, the authors explore how the inclusion of wearable technologies has changed the “rhetorical relationship” between patients and medical professionals. The study concludes with a discussion of the future of wearable devices in patient-medical practitioner relationships and clinical settings.

In previous studies, various machine-driven decision support systems based on recurrent neural networks (RNN) were ordinarily projected for the detection of cardiovascular disease. However, the majority of these approaches are restricted to feature preprocessing. In this paper, we concentrate on both, including, feature refinement and the removal of the predictive model’s problems, e.g., underfitting and overfitting. By evading overfitting and underfitting, the model will demonstrate good enactment on equally the training and testing datasets. Overfitting the training data is often triggered by inadequate network configuration and inappropriate features. We advocate using Chi2 statistical model to remove irrelevant features when searching for the best-configured gated recurrent unit (GRU) using an exhaustive search strategy. The suggested hybrid technique, called Chi2 GRU, is tested against traditional ANN and GRU models, as well as different progressive machine learning models and antecedently revealed strategies for cardiopathy prediction. The prediction accuracy of proposed model is 92.17% . In contrast to formerly stated approaches, the obtained outcomes are promising. The study’s results indicate that medical practitioner will use the proposed diagnostic method to reliably predict heart disease.

The ancient fishing village of Lower Largo or the Seatoun of Largo stands quietly on Largo Bay along the north side of the Firth of Forth and is famous as being the birthplace of its famous resident, Alexander Selkirk, who inspired Daniel Defoe’s, Robinson Crusoe. However, it has another resident, Dr. John Goodsir, who, for forty-six years served as a medical practitioner and was a Minister of the Gospel at the Largo Baptist Church for twenty years. The current work describes the life of this ordinary early medical practitioner and surgeon, discusses his correspondences, and finally examines his role as serving as Largo’s Baptist minister.

The COVID-19 pandemic affected regular health care for patients with chronic diseases. However, the impact of the pandemic on primary care for patients with coronary artery disease (CAD) who are enrolled in a structured disease management program (DMP) in Germany is not clear. We investigated whether the pandemic affected primary care and health outcomes of DMP-CAD patients (n = 750) by using a questionnaire assessing patients' utilization of medical care, CAD symptoms, as well as health behavior and mental health since March 2020. We found that out of concern about getting infected with COVID-19, 9.1% of the patients did not consult a medical practitioner despite having CAD symptoms. Perceived own influence on infection risk was lower and anxiety was higher in these patients compared to symptomatic CAD patients who consulted a physician. Among the patients who reported chest pain lasting longer than 30 min, one third did not consult a medical practitioner subsequently. These patients were generally more worried about COVID-19. Patients with at least one worsening CAD symptom (chest pain, dyspnea, perspiration, or nausea without apparent reason) since the pandemic showed more depressive symptoms, higher anxiety scores, and were less likely to consult a doctor despite having CAD symptoms out of fear of infection. Our results provide evidence that the majority of patients received sufficient medical care during the COVID-19 pandemic in Germany. However, one in ten patients could be considered particularly at risk for medical undersupply and adverse health outcomes. The perceived infection risk with COVID-19 might have facilitated the decision not to consult a medical doctor.

The Mothers Delivery Kit discusses an innovative healthcare package sold in the poor West African region to aid with the delivery of expectant mothers. The package combines various necessary items used for proper and hygienic delivery of the rural and remote mothers with the help of a local midwife or medical practitioner. Antenatal care in the region is poor due to lack of infrastructure and government funding in healthcare. The Mothers Delivery Kit addresses that issue by providing a cheap and hygienic alternative to conventional delivery methods. The kit has decreased newborn death in the region noticeably. The founders have worked closely with local midwives to train them and enable them to use the Mothers Delivery Kit properly to improve delivery conditions. Regardless of the setbacks caused by local traditional beliefs and culture, the kit has proven to be an active effort to improve delivery conditions.

Background: Prenatal diagnosis enables detection of any disease or disability of the fetus during the pregnancy of a woman. Parents whose fetus is found to have a serious disorder from antenatal testing may terminate the pregnancy if it is permitted by the law or continue with the pregnancy to term. However, the chance of terminating a pregnancy may be denied if there is prenatal negligence by the medical practitioner in terms of diagnosis or failure to advise on the test results correctly. The purpose of this research is to examine the possible legal implications of prenatal diagnosis in Malaysia. Methods: This study adopts doctrinal legal research in which the researcher examines statutes and decided cases in Malaysia, the United Kingdom (UK) and Singapore relating to abortion, wrongful birth and wrongful life claims, in order to determine the legal implications of prenatal diagnosis in Malaysia. Results: In Malaysia, abortion following a prenatal diagnosis is only legally possible if the statutory criteria in the Penal Code are met. Abortion is illegal if it is not done for therapeutic purposes. A wrongful birth action brought by a woman who claims to be deprived of the opportunity to terminate her pregnancy may be successful in Malaysia, if it can be proven that a legal abortion could have been performed if not because of the prenatal negligence of the medical practitioner. However, a wrongful life action brought in the child’s name for being allowed to be born with a disability may not be viable since the claim could hardly be established and it is against the public policy. Conclusions: Theoretically, it is possible to bring a wrongful birth action resulted from negligence in prenatal diagnosis successfully in Malaysia, but the chance is relatively slim for wrongful life action.

Ritual circumcision is a custom. But, when it is done by inexperienced person, it may cause painful situation for parents.: We are presenting a case of ritual circumcision done by inexperienced person. Complete amputation of glans was occurred during circumcision. That was very painful situation for parents. To avoid such complication circumcision should be done by experienced medical practitioner or training should be provided to the persons who are doing ritual circumcision.

Background: Rural clinics still have X-ray facilities that produce physical films, which are sent to the nearest hospital for evaluation.  Purchasing digitalization facilities is costly, thus, sending digitized films to the radiologist may be a solution.  This can be achieved via digital photo capture.  However, there can be different output resolutions that may not be optimized for online diagnosis.  This paper investigates if digitized X-ray films can be enhanced using image processing techniques of Contrast-Limited Adaptive Histogram Equalization (CLAHE), Normalized-CLAHE (N-CLAHE) and Min-Max Normalized-CLAHE (MMCLAHE).        Methods: We collected and digitized 21 X-ray films with low, medium, and high resolutions and implemented the CLAHE, N-CLAHE and MMCLAHE image enhancement. These methods introduced a limit to clip the histogram of image intensities so as to reduce any noise amplification before file compression with the Fast Fourier Transform (FFT) and Discrete Cosine Transform (DCT).  Quantitative metrics of the Peak Signal-to-Noise Ratio (PSNR) and Mean-Squared Error (MSE) were used to compare the accuracies between digitized and processed X-ray films.  A qualitative evaluation was performed by a medical practitioner to validate the accuracy of enhanced digitized X-ray.  Results: It had been found that both CLAHE and MMCLAHE provided good average PSNR values of 31dB - 32dB and produced low MSE values compared to N-CLAHE.  The results of qualitative evaluation attained 89.9% correct diagnosis on nine randomly selected images.  Generally, the evaluation indicated that the results fulfill the acceptable criteria for further evaluation and there seemed to be no pathological differences observed. Conclusion: This paper presented a proof of concept on an implementation of the CLAHE technique and its variations on digitized X-ray films.  This paper had shown potential improvements with the proposed enhancement methods that may contribute to an increase efficiency in healthcare processes at rural clinics.

Background & Objective: Ayurveda, the “Mother of all healing”, has existed for over 5,000 years and hence is considered to be the oldest healing science. Ayurveda states that the mind can heal and transform a person's whole being as the mind and body are associated. Herbs are the heart of Ayurvedic belief. They are used to boost defense against diseases and viruses and keep the brain, body, and soul in complete balance. Although ayurvedic medicines and herbs have natural components, they should still be used with certain precautions under the supervision of a medical practitioner. This study aims to manually curate information for the various ayurvedic medicinal herbs that have antiviral activity against harmful viruses. Methods: Detailed information is collected from the literature regarding the following (a) types of viruses (b) which particular category they belong to(c) the respective components of herbs that are responsible for curing viruses. We developed a web interface with the help of php and mysql to get the desired output. Results: The database consists of 104 viruses and 704 natural components. The web server is available at: http://ayurvir.com. Interpretation & Conclusion: We believe that AyurVirDB database will be extremely beneficial for the research community. It not only aids in investigations of Ayurvedic medicinal plants and their components. On the emergence or re-emergence of a virus, one could be able to predict the ayurvedic plants/herbs used for viral treatment based on virus similarity or disease symptoms.

Femoral fracture is continuity disconnected of the femoral shaft occurring due to direct trauma (traffic accidents, falls from the height), and it is usually experienced by the adult males. Fractures of this area affects heavy bleeding and impacts the shock. This study aimed to find out whether there is the influence of pre-operative health education to the self-concept improvement on femoral fracture patients At Haji Medan Hospital of North Sumatera Government in 2021The research type used was Quasi Experiment design. This study used one group pre-test and post-test design. The research location was at Haji Medan Hospital of North Sumatera Government in 2021. The population were all femoral fracture patients having treatment at Haji Medan Hospital of North Sumatera Government. Based on RSU Haji Medical record data, there were 30 patients in July- December 2020. The sampling technique was purposive sampling. The total sample were 20 femoral patients.The outputs described that there is a significant correlation between pre operative health education and self-concept improvement on femoral fracture patients At Haji Medan Hospital of North Sumatera Government in 2021. The value is p < α as 0.000 < 0.05. It is proved that pre operative health education to be effective in improving self-concept on femoral fracture patients At Haji Medan Hospital of North Sumatera Government in 2021.The suggestion of the study is required to be applied by medical practitioner in the hospital, particularly the nurses providing the nursing care to the femoral fracture patients and support the patients in overcoming the anxiety of the disease by maximizing the existing social supports.
