Healthcare Workers Attitudes, Practices and Sources of Information for COVID-19 Vaccination: An Italian National Survey

Vaccination of healthcare workers (HCWs) is a crucial element to overcome the COVID-19 pandemic. The aim of this survey was to assess attitudes, sources of information and practices among Italian Healthcare workers (HCWs) in relation to COVID-19 vaccination. Methods: From 19 February to 23 April 2021, an anonymous voluntary questionnaire was sent to the mailing list of the main National Health Service structures. Data were collected through the SurveyMonkey platform. Results: A total of 2137 HCWs answered. Hesitancy towards COVID-19 vaccination was more frequent in females, in those with lower concern about COVID-19, and in nurses, auxiliary nurses (AN) and healthcare assistants. Hesitant professionals were more likely to not recommend vaccination to their patients or relatives, while a high concern about COVID-19 was related to an increased rate of recommendation to family members. HCWs were mostly in favor of mandatory vaccination (61.22%). Female sex, a lower education level, greater hesitancy and refusal to adhere to flu vaccination campaigns were predictors influencing the aversion to mandatory vaccination. All categories of HCWs referred mainly to institutional sources of information, while scientific literature was more used by professionals working in the northern regions of Italy and in infection control, infectious diseases, emergencies and critical areas. HCWs working in south-central regions, nurses, AN, healthcare technicians, administrators and HCWs with a lower education level were more likely to rely on internet, television, newspapers, and the opinions of family and friends. Conclusions: Communication in support of COVID-19 immunization campaigns should consider the differences between the various HCWs professional categories in order to efficiently reach all professionals, including the most hesitant ones.

Socio-psychological profile of Latvian inhabitants who did not comply with the COVID-19 epidemiological requirements

Since the beginning of 2020, the world has been affected by the corona virus COVID-19. Since the spring, a number of nations around the world, including Latvia, have declared a state of emergency, with specific government restrictions developed in cooperation with WHO, to reduce spread of the COVID-19. The aim of this study is to examine socio-psychological profile of Latvian inhabitants who did not comply with the COVID-19 epidemiological requirements. A randomized nationally representative sample (N = 2608, aged from 18–74, men 39.8%, women 60.2%) from the online survey, carried out in the frame of the National research program, project Nr. VPP-COVID-2020/1-0011, was used in this study. The results showed that less compliant with COVID-19 epidemiological requirements were males, rural inhabitants, persons aged less than 60 years old, unemployed, with lower education level, and those, who are less afraid of COVID-19 and think that its dangerousness is exaggerated. Compliance with the restrictions did not relate to the marital and parental status. Based on this study, policy suggestions were made regarding a more targeted way of the providing important information on the preventive behaviour to reduce the spread of the virus.

Blood pressure and cognitive function among older adults in India

Objective We aimed to investigate the relationship between blood pressure and cognitive function among older adults in India. Methods In this study, we analyzed cross-sectional data of systolic and diastolic blood pressure (SBP and DBP, respectively) and cognitive testing from 3690 adults aged 60 years and older participating in the Longitudinal Aging Study in India—Diagnostic Assessment of Dementia from 14 states in India. Results After controlling for key sociodemographic, health, and geographic factors, higher SBP and lower DBP were each independently associated with worse cognitive function. Older age, female sex, lower education level, being widowed, residing in a rural area, being a member of a Scheduled Caste or Scheduled Tribe, having a low level of economic consumption, being underweight, and a history of stroke were all independently associated with worse cognitive function scores. Conclusions Both SBP and DBP were independently associated with cognitive function among older adults in India in diverging directions. Clinical interventions targeting high SBP and low DBP may benefit both cognitive health and cardiovascular health.

Fertility awareness and intentions among young adults in Greece

Background: Greece has a mean age of first motherhood at 31.5 years, higher than the European average age of 29.4. Delaying conception, however, may be an important non-reversible cause of infertility. The aim of this study was to identify possible knowledge deficits regarding fertility in young adults. Methods: This was an online survey of young adults, regarding information on intention to parenthood and knowledge on issues affecting fertility. This study was conducted from February to December 2020, aiming for a representative sample of Greek men and women aged 18 and 26 years. The questionnaire was designed by a multidisciplinary group based on the Cardiff Fertility Knowledge Scale, which contained 22 multiple-choice or Likert-scale questions. Results: We obtained responses from 1875 young adults, whose mean age was 22.1 years. About 91.8% of men and 94.0% of women declared an intention to have children, out of which 44.0% wanted to have two and 29.0% three children. About 52.0 and 50.8% men and women, respectively, aimed to start a family between 31 and 35 years. Residents of rural areas and those with a lower education level more likely aimed to have children before the age of 30. The most prevalent answers for age of ideal parenthood were between 26 and 30 years for a woman and 31–35 years for a man. Smoking, alcohol consumption and sexually transmitted infections were identified as factors affecting both female and male fertility. Half of men and women, respectively, overestimated general success rates of reproductive techniques. Conclusion: The knowledge of fertility, particularly with regards to assisted reproductive techniques’ success rates, may be overestimated as more young adults plan for having children after the age of 30.

HIV Pretreatment Drug Resistance Trends in Mexico City, 2017–2020

In response to increasing pretreatment drug resistance (PDR), Mexico changed its national antiretroviral treatment (ART) policy, recommending and procuring second-generation integrase strand-transfer inhibitor (INSTI)-based regimens as preferred first-line options since 2019. We present a four-year observational study describing PDR trends across 2017–2020 at the largest HIV diagnosis and primary care center in Mexico City. A total of 6688 baseline protease-reverse transcriptase and 6709 integrase sequences were included. PDR to any drug class was 14.4% (95% CI, 13.6–15.3%). A significant increasing trend for efavirenz/nevirapine PDR was observed (10.3 to 13.6%, p = 0.02). No increase in PDR to second-generation INSTI was observed, remaining under 0.3% across the study period. PDR was strongly associated with prior exposure to ART (aOR: 2.9, 95% CI: 1.9–4.6, p < 0.0001). MSM had higher odds of PDR to efavirenz/nevirapine (aOR: 2.0, 95% CI: 1.0–3.7, p = 0.04), reflecting ongoing transmission of mutations such as K103NS and E138A. ART restarters showed higher representation of cisgender women and injectable drug users, higher age, and lower education level. PDR to dolutegravir/bictegravir remained low in Mexico City, although further surveillance is warranted given the short time of ART optimization. Our study identifies demographic characteristics of groups with higher risk of PDR and lost to follow-up, which may be useful to design differentiated interventions locally.

A 10-Year Review of Spatio-Temporal Patterns of Firearm Injury in a Rural Setting

Introduction West Virginia (WV) had the ninth highest rate of firearm mortality of all states in the United States according to the CDC in 2018. Gun violence in WV has been a steady problem over the last decade. The rural population is more vulnerable to unintentional firearm injuries and suicides. Previously published literature from urban settings has demonstrated a link between firearm injuries and modifiable situational variables such as crime, unemployment, low income, and low education. There are very few studies that have utilized geospatial analytic techniques as a tool for injury mapping, surveillance, and primary prevention in rural and frontier zones of the United States. Methods We performed a 10-year retrospective single-institution review of firearm injuries at a rural WV level 1 trauma center between January 2010 and December 2019. The AIS World Geocoding Service was then used to identify specific areas of emerging firearm-related injuries within the service area. Results Specific hot spots of emerging firearm injury were identified in both intentional and unintentional populations. These were located in geographically distinct areas of the WV unincorporated rural and frontier population. These rural WV hotspots were associated with the modifiable variables of crime, unemployment, lower income, and lower education level. Conclusions Emerging hot spots of firearm injury in rural and frontier locations were associated with modifiable social determinants. These areas represent an opportunity for targeted injury prevention efforts addressing these disparities. Further prospective study of these findings is warranted.

Assessment of hygiene status of poultry slaughtering facilities and meat handling practices of butchers by using a hygiene assessment tool

In the present study, hygiene conditions and meat handling practices of poultry butchers are assessed by using a hygiene assessment tool. The study was conducted in the Hyderabad and Jamshoro districts of Pakistan from April 2019 to February 2020. A total of fifty slaughtering facilities were selected based on convenient sampling techniques and butchers from those shops were interviewed by using a hygienic assessment tool. Moreover, socio-demographic details of butchers were also recorded by questionnaire survey. The hygiene assessment tool comprises three domains, personal hygiene index (PHI), meat hygiene index (MHI) and slaughtering facility hygiene index (SFHI). Each of the indexes consists of certain observations and each of the positive observations was scored one, while negative observation was scored zero. It has been revealed from this study that more than 50% of butchers were without any education and none of the butchers had received any formal training in slaughtering and meat handling practices. Basic prerequisites of hygiene such as handwashing facilities and clean water were missing in all of the slaughtering facilities. More than 30 and 50 percent of butchers scored the least (between zero – one) in PHI MHI and SFHI. However, the total score of butchers with a higher level of education was greater (p < 0.05) than with a lower education level. To improve hygiene levels and reduce transmission of diseases due to the consumption of contaminated meat, regular surveillance of the poultry shops and formal training of butchers are necessary.

Smoking Behavior Changes during COVID-19 among Korean Adults

Objectives: In this study, we investigated the changes in smoking behavior during the COVID-19 outbreak in Korea in 2020. We also examined the influence of general characteristics and mental health problems due to COVID-19 on changes in smoking behavior. Methods: We collected data from August to October 2020 from adults aged 19 years and older from the Korea Community Health Survey (N=229,269). Results: Among current smokers (N=39,534), 11.0% reported an increase in smoking, and 12.8% reported a decrease in smoking. Overall, 33.3% reported perceived stress and 3.7% reported depression. Participants reported COVID-related anxiety about infecting family (83.0%), economic damage (76.3%), blame or harm from others (66.5%), infection (60.6%), and death (31.3%). Lower education level, living alone, and poor subjective health status were associated with increased smoking. Being a woman and being of older age were associated with decreased smoking. Stress, depression, and anxiety about economic damage due to COVID-19 were more likely to result in increased smoking. Anxiety related to death due to COVID-19 was more likely to result in decreased smoking. Conclusion: These findings provide insight into the changes in smoking behavior caused by COVID-19.

Cognitive Influences in Parkinson's Disease Patients and Their Caregivers: Perspectives From an Australian Cohort

Objectives: Cognitive impairment impacts negatively on Parkinson's disease (PD) patient and caregiver quality of life (QoL). We examined cognitive impairment in PD patients and their caregivers to determine if caregiver cognition affected their PD relative.Methods: Validated cognition and clinical outcome measures were assessed in 103 PD patients and 81 caregivers.Results: PD patients showed more cognitive impairment than their carers, with 48.6% having possible Mild Cognitive Impairment (MCI) and 16.5% having PD dementia. Increasing age, male gender, lower education level, various non-motor symptoms and certain therapies, associated with poorer cognition in PD. Eighteen and a half percent of caregivers were found to have MCI, in association with a lower physical and mental QoL. This reflected in lower QoL and mood for the respective PD patients.Conclusion: Impaired cognition and QoL in caregivers was associated with decreased QoL and mood for respective PD patients, suggesting MCI in caregivers is an important consideration for the management of PD.

Resilience: A Protective Factor from Depression and Anxiety in Mexican Dialysis Patients

Depression and anxiety are highly prevalent psychological disorders in end-stage renal disease (ESRD) that have a negative clinical impact. The purpose of our study was to identify factors associated with the presence of depression and anxiety, in a sample of ESRD patients treated with hemodialysis. We included 187 patients from two dialysis facilities, age 18–65 years. Beck’s depression and anxiety inventories, KDQOL36 questionnaire, the cognitive distortion scale and the Mexican scale of resilience were used. Socio-demographic and clinical information was obtained from medical records. Depression was present in 143 (76.4%) patients. Patient with depression were older (33 (26–52) years vs. 30 (24.43) years, p = 0.025), had a lower education level (36% vs. 9%, p = 0.001), used more medications (67% vs. 36%, p = 0.001), had a comorbidity (75% vs. 41%, p = 0.001), and a higher proportion were waiting for a kidney transplant. Anxiety was present in 112 (59.8%) cases. By multivariate analysis, depression was independently associated with lower education, absence of previous kidney transplant, anxiety, higher cognitive distortion, lower psychological resilience, and lower quality of life scores. In conclusion, lower psychological resilience, lower education level, and higher cognitive distortions are factors associated with depression and anxiety in ESRD patients.
