Abstract Background Most interventions for conditions with a small cohort size, such as transplantation, are unlikely to be part of a clinical trial. When condition-specific evidence is lacking, expert consensus can offer more precise guidance to improve care. Management of cardiovascular risk in liver-transplant recipients is one example for which clinicians have, to date, adapted evidence-based guidelines from studies in the general population. However, even when consensus is achieved, implementation of practice guidance is often inadequate and protracted. We report on a novel mixed-methods approach, the Northwestern Method©, for the development of clinical-practice guidance when condition-specific evidence is lacking. We illustrate the method through the development of practice guidance for managing cardiovascular risk in liver-transplant recipients. Methods The Northwestern Method© consists of (i) adaptation of relevant, existing, evidence-based clinical-practice guidelines for the target population; (ii) consensus by experts of the proposed practice guidance; (iii) identification of barriers to guidance adherence in current practice; and (iv) recommendation for implementation and dissemination of the practice guidance. The method is based on an iterative, user-centered approach in which the needs, wants, and limitations of all end users, including patients, are attended to at each stage of the design and development process. Conclusions The Northwestern Method© for clinical-practice-guidance development uses a mixed-methods approach to bring together broad representation from multiple disciplines and practice settings to develop consensus considering the unique needs and preferences of patients, caregivers, and practitioners who are directly impacted by clinical-practice-guidance recommendations. We hypothesize that a priori involvement of end users in the guidance-development process will lead to sustainable implementation of guidance statements into clinical practice.

Abstract The articles in this supplement address key questions on syphilis diagnostics, provide reference tables of test performances, and discuss optimal specimens and knowledge gaps. Laboratory-developed genetic direct detection tests could be most useful at the point of care and add to the currently available serologic methods of nontreponemal and treponemal tests.

Regenerative medicine is a new and expanding field in biomedical research. Organ and tissue loss through disease and injury, propel the development of a treatment that can regenerate tissue and help for less relaying in organ transplantation. Regenerative medicine has the potential to heal tissue and damaged organ. Currently, a patient suffering from diseased and injured organs can be treated with transplantation organs, but there is a shortage of donor organs. Dermatology is estimated to have a larger share in the market for regenerative medicine as skin being an organ with great cell replicate characteristics. US and Japan play a major role in the market for regenerative medicine. Food and drug administration (FDA) regulations for a medical device used in regenerative medicine are covered here to support risk-based, flexible regulatory methods to help and support the potential to bring novel treatment possibilities to market further. But due to incorrect FDA regulations, it stands as a barrier for the marketing regenerative medicines. Better and clear guidance development or a clear regulatory framework for both regenerative medicine and medical device can leads the product a best fit clinical development and product access in the market.

In Law 12 of 1995 concerning Correctional, it is explained that the convict is someone who has obtained permanent legal force from the court for violating the law he commited and placed in a correctional institution. In undergoing their criminal term in correctional institution they receive guidance in order to realize their mistakes, improve themselves, and no longer to repeat their criminal acts, so that they can be readmitted into the community. In the process of their return to the community, many of them were struggled to adapt again, because of  negative stigmatization of the community. The impact is they will find hard to get a decent job, will be exclude from the community, and even worse they will did their crime again. In correctional institution there is a term for recidivist or a term for those ex-convict who have been released but go back again into correctional institution  for repeating their crime. In this case many opinions from outside assume if the coaching process was given did not go well or failed. So in the study will discuss how to disperse the negative stigma of ex-convicts through the implementation results of the guidance that has been given into the community.

The purpose – of this article is to assess the vocational guidance situation and development opportunities in Lithuania. Research methodology – Systematic analysis of the peculiarities of vocational guidance; identification of the main problems and possibilities for vocational guidance through the development of a qualifications system based on the results of statistical analysis, expert assessment, data grouping and interpretation. Findings – Vocational guidance as an important subsystem of the educational system is being underemphasised and underfinanced, career counselling is pursued in a fragmentary manner, mainly through project-based initiatives which are not be based on systematic information. A way to improve vocational guidance is to organise it as an integrated information platform linked to the qualifications system. Research limitations – The main limitation is the lack of official statistics in vocational guidance. The systematic collection and publication of statistics would make it possible to quantify and analyse the factors of the current vocational guidance situation and their impact on the development of human resources. Practical implications – The obtained results are useful for social and economic and educational policy-making. Originality/Value – The article contributes to the scientific literature by presenting a model of vocational guidance development related to the development of the qualifications system, which would allow providing the necessary access to information.

Abstract I test the proposition that interest groups achieve greater policy success when they lobby during the agency guidance document development process as opposed to the notice and comment process. Policymaking via guidance documents often receives lower levels of public attention, which provides greater flexibility to accommodate lobbying requests. I analyze the hypothesis during the creation of 41 rules by one US government agency—20 of which were promulgated using the notice and comment process and 21 via the guidance document process. I measure regulatory policy change using a content analysis of government documents and lobbying texts, and I also incorporate a telephone survey of interest groups who lobbied on these same rules. I find that interest groups perceive—and achieve—greater policy success when lobbying during the agency guidance process. The results yield new insights into the relationship between lobbying and regulatory policymaking.

In the dental literature, the association between the occlusion and hypersensitive teeth is poorly explained. Quantified occlusal contact force and timing parameters have been largely ignored in studies assessing hypersensitive teeth. This chapter introduces a novel occlusal concept, frictional dental hypersensitivity (FDH), after systemically simplifying the existing and often confusing terminology used in the literature to describe the variant clinical presentations of the hypersensitive dentition. Clinical evidence from combining computerized occlusal analysis and electromyography is presented linking opposing posterior tooth friction and muscular hyperactivity to FDH. This chapter will outline how occlusion, many muscular TMD symptoms, and FDH are all interrelated. Both a pilot study and a 100 subject cold ice water swish follow-up study are presented and used a numerical visual analog scale (NS/VAS) to quantify cold response dental hypersensitivity resolution observed in occlusally symptomatic patients that underwent the immediate complete anterior guidance development coronoplasty (ICAGD). This computer-guided occlusal adjustment procedure eliminated pretreatment FDH symptomatology, further supporting that dental hypersensitivity often has an occlusally-based, frictional etiology. Additionally, consideration for the orthopedic influences that may directly affect the occlusion and neurology of the system are outlined, as well as the medical concept of tooth allodynia. Furthermore, trigeminal neurological influences are compared and contrasted to autonomic sympathetic inputs in relation to the influence that they each have upon the hypersensitive dentition. Lastly, the greater auricular diagnostic nerve block is discussed, as is the influence that this nerve may have upon the hypersensitive mandibular posterior dentition.
