Saltwater Fish but not Freshwater Fish Consumption is Positively Related to Handgrip Strength: the TCLSIH Cohort Study

PurposeThe Caribbean Consortium for Research in Environmental and Occupational Health prospective environmental epidemiologic cohort study addresses the impact of chemical and non-chemical environmental exposures on mother/child dyads in Suriname. The study determines associations between levels of environmental elements and toxicants in pregnant women, and birth outcomes and neurodevelopment in their children.ParticipantsPregnant women (N=1143) were enrolled from December 2016 to July 2019 from three regions of Suriname: Paramaribo (N=738), Nickerie (N=204) and the tropical rainforest interior (N=201). Infants (N=992) were enrolled at birth. Follow-up will take place until children are 48 months old.Findings to dateBiospecimens and questionnaire data on physiological and psychosocial health in pregnant women have been analysed. 39.1% had hair mercury (Hg) levels exceeding values considered safe by international standards. Median hair Hg concentrations in women from Paramaribo (N=522) were 0.64 µg/g hair (IQRs 0.36–1.09; range 0.00–7.12), from Nickerie (N=176) 0.73 µg/g (IQR 0.45–1.05; range 0.00–5.79) and the interior (N=178) 3.48 µg/g (IQR 1.92–7.39; range 0.38–18.20). 96.1% of women ate fish, respective consumption of the three most consumed carnivorous species, Hoplias aimara, Serrasalmus rhombeus and Cichla ocellaris, known to have high Hg levels, was 44.4%, 19.3% and 26.3%, respectively, and was greater among the interior subcohort. 89% frequently consumed the vegetable tannia, samples of which showed presence of worldwide banned pesticides. 24.9% of pregnant women had Edinburgh Depression Scale scores indicative of probable depression.Future plansFish consumption advisories are in development, especially relevant to interior women for whom fish consumption is likely to be the primary source of Hg exposure. Effects of potentially beneficial neuroprotective factors in fish that may counter neurotoxic effects of Hg are being examined. A pesticide literacy assessment in pregnant women is in progress. Neurodevelopmental assessments and telomere length measurements of the children to evaluate long-term effects of prenatal exposures to toxicant mixtures are ongoing.

Introduction: Trimethylamine N-oxide (TMAO) may play a key mediator role in the relationship between the diet, gut microbiota and cardiovascular diseases, particularly in people with kidney failure. The aim of this review is to evaluate which foods have a greater influence on blood or urinary trimethylamine N-oxide (TMAO) levels. Methods: 391 language articles were screened, and 27 were analysed and summarized for this review, using the keywords “TMAO” AND “egg” OR “meat” OR “fish” OR “dairy” OR “vegetables” OR “fruit” OR “food” in December 2020. Results: A strong correlation between TMAO and fish consumption, mainly saltwater fish and shellfish, but not freshwater fish, has been demonstrated. Associations of the consumption of eggs, dairy and meat with TMAO are less clear and may depend on other factors such as microbiota or cooking methods. Plant-based foods do not seem to influence TMAO but have been less investigated. Discussion: Consumption of saltwater fish, dark meat fish and shellfish seems to be associated with an increase in urine or plasma TMAO values. Further studies are needed to understand the relationship between increased risk of cardiovascular disease and plasma levels of TMAO due to fish consumption. Interventions coupled with long-term dietary patterns targeting the gut microbiota seem promising.

There is evolving evidence for an association between dysphagia and sarcopenia in older adults. For optimizing the acute health care initiative across health care settings, this study investigated prevalence and time-course of dysphagia in older patients admitted to an emergency department (ED) as well as its association with parameters for probable sarcopenia, inactivity, malnutrition, disease status, and systemic inflammation. A secondary analysis of data from the FAM-CPH cohort study on acutely admitted older medical patients (n = 125). Data were collected upon ED admission as well as four and 56 weeks after discharge. Using the Eating Assessment Tool cut-off score ≥ 2, signs of dysphagia were present in 34% of the patients at ED admission and persisted in 25% of the patients 56 weeks after discharge. Signs of dysphagia at 56-week follow-up were significantly (p < 0.05) associated with probable sarcopenia (low handgrip strength (OR = 3.79), low leg muscle strength (OR = 8.14), and low physical performance (OR = 5.68)) and with baseline swallowing inactivity (OR = 5.61), malnutrition (OR = 4.35), and systemic inflammation (OR = 1.33). Signs of dysphagia in older patients admitted to an ED was prevalent, persisted 56 weeks after discharge, and was associated with probable sarcopenia and related conditions; all modifiable targets for management of dysphagia in older patients.

Abstract Eighty-four samples (fish, milk, eggs, vegetables) were collected from within a 25 mile radius of 10 perchloroethylene or trichloroethylene manufacturing plant sites and analyzed for hexachloro-1,3-butadiene (HCBD) by a gas chromatographic method. Residues of HCBD were found mainly in freshwater fish caught in the lower Mississippi River. The levels ranged from 0.01 to 1.2 ppm. Two of the 20 saltwater fish contained residues of 0.01 and 0.02 ppm. No HCBD residues were found in eggs and vegetables. One of 20 milk samples contained 1.32 ppm HCBD.

Abstract Background although handgrip strength (HGS) is considered an excellent predictor of morbidity and mortality, there are few reference data available, especially across life from young to old populations. Objective to investigate the reference values and determinants for HGS in a large Chinese adult population. Design this cross-sectional study used data from the Tianjin Chronic Low-Grade Systemic Inflammation and Health Cohort Study ranging from 2013 to 2017. Setting and subjects in total, 37,707 Chinese adults aged 18–93 years with measures of HGS were included. Methods HGS was measured using a handheld digital dynamometer. The percentile distribution of HGS, as well as mean (standard deviation) values of HGS, was calculated according to sex and age categories. The Pearson’s or Spearman’s correlation coefficient and multiple linear regression were also used. Results the peak mean value of HGS was 45.2 ± 6.88 kg in men and 26.8 ± 4.65 kg in women. Different reference values for each sex and age category are present. The prevalence of low HGS was 7.69% in men and 6.46% in women. Multiple regression analysis showed that almost 72.5% of HGS variability can be explained by sex, age, height and weight. Conclusions the HGS reached peak at 40 years old, maintained or slightly lowered between ages 40 and50 and then start an accelerated decline after 50 years old. Reference values of HGS should be stratified by sex and age group. HGS variability can be largely explained by sex, age, height and weight.
