Improving community access to terminal phase medicines in Australia: identification of the key considerations for the implementation of a ‘core medicines list’

During the terminal phase, access to medicines is critical for people wishing to spend their last days of life at home. Yet, access to medicines can be problematic. The aim of this study was to report the perspectives of specialist and generalist health professionals (HPs) on the issues of community access to medicines for this vulnerable group. A qualitative descriptive study design investigated the views of HPs working in palliative care roles in South Australia. Nurses, doctors and pharmacists described their experiences of accessing medicines for management of terminal phase symptoms during semi-structured focus group discussions. Content analysis identified six themes including: ‘Medication Supply’, ‘Education and Training’, ‘Caregiver Burden’, ‘Safety’, ‘Funding’ and ‘Clinical Governance’. Future projects should aim to address these themes when developing strategies for the management of people wishing to die at home.

ObjectivesDifficulties accessing medicines in the terminal phase hamper the ability of patients to die at home. The aim of this study was to identify changes in community access to medicines for managing symptoms in the terminal phase throughout South Australia (SA), following the development of a ‘Core Medicines List’ (the List) while exploring factors predictive of pharmacies carrying a broad range of useful medicines.MethodsIn 2015, SA community pharmacies were invited to participate in a repeat survey exploring the availability of specific medicines. Comparisons were made between 2012 and 2015. A ‘preparedness score’ was calculated for each pharmacy, scoring 1 point for each medicine held from the following 5 classes: opioid, benzodiazepine, antiemetic, anticholinergic and antipsychotic.ResultsThe proportion of pharmacies carrying all items from the List rose from 7% in 2012 to 18% in 2015 (p=0.01). Multiple linear regression demonstrated that a monthly online newsletter subscription (p=0.04) and provision of a clinical service to aged care facilities (p=0.02) were predictors of pharmacies carrying all items on the List. Furthermore, multiple linear regression demonstrated that the provision of an afterhours service (p=0.02) and clinical services to aged care facilities (p=0.04) were predictors of pharmacies with a high ‘preparedness score’. In responding to issues with supply of medicines at end of life, respondents were more likely to contact the prescriber if aware of palliative patients (p=0.03).ConclusionsThese results suggest that there is value in developing and promoting a standardised list of medicines, ensuring that community palliative patients have timely access to medicines in the terminal phase.

<p>Small group discussions (group work) are an important part of the learning process though students are rarely skilled at asking a question. The purpose of this study is to examine student interactions and questioning skills during group work. This research study uses a qualitative descriptive approach. The research subjects consist of eight students who were divided heterogeneously into two groups, with four members for each group. The research instrument consists of the researcher herself, a task in the form of a trigonometric ratio group worksheet, and three video cameras to observe discussion activities. One camera was focused on all class activities and two cameras were pointing at each group being observed. All conversation transcripts during the discussion are deciphered, coded, and then analyzed qualitatively. The results of this study show the interaction of conveying opinion/rebuttals, asking questions, and providing answers, with the percentage of giving opinions/ objections were more dominant than other interactions. The ability to ask questions was obtained by 50% of the students having the medium questioning ability and 50% having the low questioning ability, with the level of questions asked at the LOTS level, namely C1 and C3 levels. None of the students had high questioning skills. Of the two groups observed, group A was more active in interacting both in terms of exchanging opinions/rebuttals, asking questions, and providing answers. Suggestions for further research need to be carried out an in-depth analysis of discussion activities both in terms of asking questions or providing feedback to see the emergence of collaborative reasoning.</p>

Effectiveness of Education and Training in Increasing Auditor Performance at the Lamongan Regency Inspectorate of East Java Province. This study aims to find out and analyze education and training to be the driving factors for improving the performance of auditors at the Inspectorate of Lamongan Regency, East Java Province. The researcher used a qualitative descriptive method with an inductive approach. The conclusion of this study is the effectiveness of education and training carried out through technical training programs held by the Lamongan District Inspectorate and the implementation of functional training and technical training outside which aims to improve the performance of the Lamongan District Inspectorate auditors, including being effective. This can be understood from the results of the overall study through interviewing the author with sources and direct observations. The indicators of effectiveness of auditor participation to carry out technical training carried out by the Inspectorate and functional training and technical training held outside of stating that, every implementation. Education and training can have a positive impact on the auditor of the Lamongan District Inspectorate in improving performance to carry out their duties and functions as internal government supervisors.

Abstrak Penelitian bertujuan untuk analisis penggunaan Schoology pada Mata Pelajaran PPKn terhadap minat belajar siswa di SMA Negeri 1 Srandakan Bantul Tahun 2020. Penelitian ini dilakukan di SMA Negeri 1 Srandakan pada bulan Juli sampai Agustus tahun 2020. Penelitian ini menggunakan metode deskriptif kualitatif. Data dikumpulkan melalui observasi, wawancara dan dokumentasi. Subjek penelitian ini adalah : (1) satu orang Kepala Sekolah SMA N 1 Srandakan Bantul (2) satu orang Guru Mata Pelajaran Pendidikan Pancasila dan Kewarganegaraan (3) Siswa SMA N 1 Srandakan sejumlah 8 orang. Teknik analisis data melalui pengumpulan data, reduksi data, penyajian data dan penarikan kesimpulan. Pemerikasaan keabsahan data dilakukan dengan triangulasi data. Hasil dari penelitian ini adalah: (1) Schoology memiliki kemudahan dalam mengatur administrasi pembelajaran siswa di SMA Negeri 1 Srandakan karena siswa dapat presensi, mendapat materi hingga evaluasi di Schoology; (2) Schoology dapat mengendalikan minat belajar siswa dalam aspek preventif dengan cara kegiatan belajar menyenangkan, persuasif dengan cara membujuk melalui fitur massages, dan represif dengan cara didatangi rumahnya berdasarkan informasi yang disajikan dalam aplikasi tersebut; (3) Schoology memiliki tampilan seperti media sosial Facebook ditandai warna dasar yang sama yaitu biru dan putih serta menu yang identik; (4) Schoology memiliki keunggulan untuk diskusi pada pembelajaran daring, siswa dapat berdiskusi kelompok dengan guru sebagai pemantik materi dalam fitur Groups; (5) Schoology mengakomodasi aneka macam media pembelajaran antara lain media gambar visual, audio, dan audio visual atau video. Kata Kunci: Schoology, PPKn, Minat Belajar Siswa Abstract The research aims to analyze the use of Schoology in PPKn Subjects on student interest in learning at SMA Negeri 1 Srangkan Bantul in 2020. This research was conducted at SMA Negeri 1 Srangkan from July to August 2020. This study used a qualitative descriptive method. Data were collected through observation, interviews and documentation. The subjects of this study were: (1) one principal of SMA N 1 Srangkan Bantul (2) one teacher of Pancasila and Citizenship Education (3) 8 students of SMA N 1 Srangkan. Data analysis techniques through data collection, data reduction, data presentation and drawing conclusions. Data validity was checked by data triangulation. The results of this study are: (1) Schoology has the convenience of managing the administration of student learning at SMA Negeri 1 Srangkan because students get presence, receive material and evaluation in Schoology; (2) Schoology can control student interest in preventive aspects by means of fun, persuasive learning activities by persuading through massages, and repressive features by visiting their house based on the information presented in the application; (3) Schoology has a display similar to Facebook's social media marked with the same basic colors, namely blue and white and identical menus; (4) Schoology has the advantage of discussions on online learning, students can have group discussions with the teacher as material triggers in the Groups feature; (5) Schoology accommodates various kinds of learning media, including visual images, audio, and audio-visual or video media. Abstract Aim: The purpose of this study was to investigate how old persons perceived their life to be, how they viewed the ageing process and their need of health care and societal support. Background: The purpose of WHO’s Healthy Ageing strategy and development of age-friendly environments is to support physiological and psychosocial changes in old persons by facilitating basic needs. Interventions to operationalize these needs in older people living at home are often developed from a professional perspective and to a small extent involves the perceptions, experience and expectations of the older persons. Method: This qualitative study has an explorative design using focus group discussions to collect data. In all, 34 persons between 69 and 93 years of age participated in seven group discussions. The interviews were analyzed using inductive manifest content analysis. Findings: The main results suggest that most old persons enjoyed life and wished it to continue for as long as possible. Important was to sustain networks and to feel useful. Unexpected changes were described as threats and the need to use health care services was associated with illness and being dependent. The result is presented in three categories with sub-categories: ‘Embracing life’, ‘Dealing with challenges’ and ‘Considering the future’.

Improvement and quality assurance of public services is very needed, therefore the Government through Law Number 25 of 2009 concerning Public Services and Minister of Administrative Reform and Bureaucratic Reform Number 15 of 2014 concerning Guidelines for Service Standards requires that every public service provider be obliged to establish and apply Public Service Standards for each type of service that it provides. One of them is the Yogyakarta Education and Training Agency as a public service provider in the form of education, training, and competency development for the State Civil Apparatus. Public service standard documents that have been prepared by the Yogyakarta Education and Training Agency in 2017 need to be evaluated because they allegedly did not meet the method in the preparation process. The research conducted is a literature review with data mining techniques using observation techniques, interviews, mini focus group discussions and public hearings. Based on the research results it is known that the public service standard documents that have been owned by the Yogyakarta Education and Training Agency are still not comprehensive, but this solution is then obtained after an analysis and discussion with stakeholders at the Yogyakarta Education and Training Agency has also successfully identified various types of services that need to be it is prioritized to develop public service standards.

The purpose of this study is to find and describe (1) the use of Mandar ethnic values in the Passayang-Sayang songs of the 1960-1970 period, (2) the use of the Mandar ethnic values in the Passayang-Sayang songs period of 1970-1980, (3) the use of the Mandar ethnic values in the Passayang-Sayang song period of 1980-1990, (4) the use of Mandar ethnic values in the Passayang-Sayang song period of 1990-2000, (5) the use of the Mandar ethnic value in the Passayang-Sayang song period of 2000-2010, and (6 ) the use of the Mandar ethnic value in the song of the Passayang-Sayang period 2010-Present. The theory used in this research is the theory of literary geography put forward by Herder, Stael, and Ricardson. This research is a qualitative descriptive study. The research data are in the form of Passayang-Sayang Mandar poetry texts, informant statements, and facts in the field. Data collection activities carried out by observation and observation techniques, interviews, recording, recording, and documentation. In this study the researcher acted as a key instrument guided by data collection guidelines. Data analysis procedures use the spiral model, while data analysis uses objective hermeneutics. To avoid research bias, triangulation was carried out for each stage of the study, research tools, and clarification of research findings to the informants as poet owners, Focus Group Discussions, and external audits. The findings of this study are the development of ethnic values in the Passayang-Sayang Mandar poem in the form of (16) ethnic values found in the period 1960-1970, (5) ethnic values found in the period 1970-1980, (7) ethnic values found in the 1980 period -1990, (6) ethnic values were found in the period 1990-2000, (7) ethnic values were found in the period 2000-2010, (8) ethnic values were found in the period 2010-Now.

Pelatihan pembuatan bento bertujuan untuk meningkatkan kreativitas menciptakan bekal makan yang menarik dan unik. Metode yang digunakan adalah sosialisasi, praktik dan pelatihan. Kegiatan pelatihan ini dilaksanakan satu kali pertemuan dengan beban 5 jam praktek. Tempat pelatihan bento diadakan di aula Kecamatan Danurejan dan dihadiri oleh 50 peserta di wilayah Kecamatan Danurejan. Hasil pelatihan pembuatan bento terlihat peserta mulai terampil dalam menciptakan kreativitas menata makanan sehingga tercipta bekal makan yang menarik, unik dan layak jual. Rekomendasi selanjutnya peserta diharapkan mengulang dan berlatih kembali dirumah. Bento making training aims to increase creativity to create interesting and unique lunch. The method used is socialization, practice and training. This training activity was held once with a load of 5 hours of practice. The bento training venue was held in the Danurejan Region hall and was attended by 50 participants in the Danurejan Region area. The results of the training on making bento show that participants began to be skilled in creating creativity in arranging food so as to create interesting, unique and worth selling lunch. The next recommendation is that participants are expected to repeat and practice again at home.

The government has implemented the Healthy Living Community Movement (GERMAS) program through promotion of the pharmaceutical sector related to drug storage at home. However, there are still many problems in storing household medicines in the community. This activity was carried out through a community empowerment approach using the community capacity building method for a community organization, Aisyiyah, which consists of Muslim women in the Grendeng village, Purwokerto. This activity ws carried out in stages, namely pre-test methods, lectures, group teaching, self-empowerment through peer teaching and training, and measuring the success of activities using post-tests. Based on the whole series of activities, the knowledge of partners about storing medicines at home has increased in a good category.
