The impact on healthcare organization on the territory of Russian Arctic of unique natural and climatic, demographic, ethnic, settlement and professional factors of influencing the health of population, constantly or temporarily living on this territory is studied. The necessity is substantiated of various forms and resource provision with healthcare services such real and potential patients of Arctic medical institutions, as representatives of indigenous small peoples of the North, workers of mining and metallurgical industry, military personnel, sailors and shift workers. In this connection a correction of a number of All-Russian normative acts is proposed.

Increased risk of obesity and diabetes in shift workers may be related to food intake at adverse circadian times. Early morning shiftwork represents the largest proportion of shift workers in the United States, yet little is known about the impact of food intake in the early morning on metabolism. Eighteen participants (9 female) completed a counterbalanced 16 day design with two conditions separated by ~1 week: 8 h sleep opportunity at habitual time and simulated early morning shiftwork with 6.5 h sleep opportunity starting ~1 h earlier than habitual time. After wake time, resting energy expenditure (REE) was measured and blood was sampled for melatonin and fasting glucose and insulin. Following breakfast, post-prandial blood samples were collected every 40 min for 2 h and the thermic effect of food (TEF) was assessed for 3.25 h. Total sleep time was decreased by ~85 min (p < 0.0001), melatonin levels were higher (p < 0.0001) and post-prandial glucose levels were higher (p < 0.05) after one day of simulated early morning shiftwork compared with habitual wake time. REE was lower after simulated early morning shiftwork; however, TEF after breakfast was similar to habitual wake time. Insufficient sleep and caloric intake during a circadian phase of high melatonin levels may contribute to metabolic dysregulation in early morning shift workers.

A possible association between night shift work and musculoskeletal disorder has been suggested. This study aimed to evaluate the impact of decreased night work on musculoskeletal pain. Difference-in-difference estimation was used to compare changes in musculoskeletal pain between shift workers (N = 122) and non-shift workers (N = 170) in a manufacturing company before and after the introduction of a new shift system eliminating overnight work. Musculoskeletal pain was measured by a questionnaire asking if workers had symptoms in specific body parts, including the neck, shoulder, arm/elbow, wrist/hand, back, and leg/foot, over the past year. Generalized estimating equation models were used to estimate changes in pre- versus post-intervention musculoskeletal pain rates between the treated and control group. In the difference-in-difference (DID) models, prevalence of musculoskeletal pain for shoulder (−10.3%), arm (−12.9%), all sites combined (−9.2%), and upper extremity combined (−14.8%) showed significant decreases from pre- to post-intervention among the treated group (shift workers) compared to the control group (non-shift workers) after controlling for age and weekly working hours. Decreasing night work was related to improvement in musculoskeletal pain in shift workers.

There is evidence to indicate that the central biological clock (i.e., our endogenous circadian system) plays a role in physiological processes in the body that impact energy regulation and metabolism. Cross-sectional data suggest that energy consumption later in the day and during the night is associated with weight gain. These findings have led to speculation that when, as well as what, we eat may be important for maintaining energy balance. Emerging literature suggests that prioritising energy intake to earlier during the day may help with body weight maintenance. Evidence from tightly controlled acute experimental studies indicates a disparity in the body’s ability to utilise (expend) energy equally across the day and night. Energy expenditure both at rest (resting metabolic rate) and after eating (thermic effect of food) is typically more efficient earlier during the day. In this review, we discuss the key evidence for a circadian pattern in energy utilisation and balance, which depends on meal timing. Whilst there is limited evidence that simply prioritising energy intake to earlier in the day is an effective strategy for weight loss, we highlight the potential benefits of considering the role of meal timing for improving metabolic health and energy balance. This review demonstrates that to advance our understanding of the contribution of the endogenous circadian system toward energy balance, targeted studies that utilise appropriate methodologies are required that focus on meal timing and frequency.

The article focuses on the manifestations of the modern «pandemic» crisis in the key resource industries (mineral resources and forest complexes) of the Far Eastern macroregion. The features of the crisis dynamics of production are shown depending on the predominance of some types of natural resources in the structure of the regional economy, their connection with Russian and world markets, the character of the impact of market factors. The influence of external shocks and national institutional imperatives on the change in the parameters of different branches of the natural resource sector has been investigated. There were found new crisis manifestations associated with the peculiarities of staff formation in the resource sector («shift workers factor»), their consequences for the regional economy and raw materials companies, the need to update the relevant scientific and methodological assessments and management decisions. It was revealed that with all the novelty and multidirectionality of manifestations of the crisis in 2020, the key characteristics of the situation are determined by the traditional set of fundamental reasons that define the development of a region with a resource specialization both in crisis and recovery periods

The Goal. To analyze the literature on the impact of high latitude climate on human health, as well as consider some aspects of adaptation to Arctic conditions and nutritional issues for the population and shift workers.Methods. Review of the articles placed in the bibliographic and abstract databases of VINITI, CyberLeninka, RSCI, Scopus, WoS, on the study of environmental and hygienic problems in the Arctic territories, where intense industrial activity is carried out. The following keywords were used: Arctic territories, adaptation, watch, food.Results. Long-term fundamental research has established that a complex of unfavorable and extreme natural and climatic factors in the Arctic causes two main syndromes in humans – “polar tension” and “northern tissue hypoxia”. Adaptation of new settlers in the Arctic territories proceeds in three stages. The duration of the shift period must have a physiological and hygienic justification and correspond to the duration of the period of stable working capacity. In high latitudes, it is especially important to develop diets and food products for various groups of the population and shift workers in relation to the specifics of their work in the Arctic regionsConclusions. Among the main environmental and hygienic problems of the Arctic territories, where intense industrial activity is carried out, it is necessary to highlight the tasks of scientific substantiation of rational modes of work and rest of workers in rotational forms of work. Also it’s necessary to define the quantitative and qualitative composition of food rations recommended for nutrition of various groups of the population, in relation to the specifics of production factors.

Eating at night time, as is frequent in shift workers, may contribute to increased cardiovascular disease (CVD) risk through a disruption in usual lipid metabolism, resulting in repeated and sustained hyperlipidemia at night. This systematic review aimed to investigate the impact of eating a meal at night compared with the same meal eaten during the day on postprandial lipemia. Six databases were searched: CINAHL Plus, Cochrane Library, EMBASE, Ovid MEDLINE, Informit, and SCOPUS. Eligible studies were original research cross-over design with a minimum fasting period of 5 h before testing preceded by a standardized control meal; measured postprandial triacylglycerol (TAG) for 5 h or greater; had meal time between 0700 h and 1600 h for day time and between 2000 h and 0400 h for night time; and had within-study test meals (food or drink) that were identical in macronutrient composition and energy. Two authors independently completed eligibility and quality assessment using the American Dietetic Association Quality Criteria Checklist for Primary Research. After removing duplicates, 4,423 articles were screened, yielding 5 studies for qualitative synthesis. All studies identified at least one parameter of the postprandial TAG response that was different as a result of meal time (e.g., the total concentration or the time course kinetics). Two studies reported a greater total TAG concentration (area under curve) at night compared with day, and 3 studies found no difference. Four studies reported that the kinetics of the postprandial time course of TAGs was different at night compared with during the day. Inconsistent reporting in the primary studies was a limitation of the review. Night eating may negatively affect postprandial lipemia and this review shows there is a need to rigorously test this using standardized methods and analysis with larger sample sizes. This is critical for informing strategies to lower CVD risk for shift workers.
