Giardia is a ubiquitous intestinal protozoan parasite of vertebrates and the most common intestinal pathogen of humans and domestic animals with a worldwide distribution including both temperate and tropical regions.Giardia was first observed in 1681 by Antony van Leeuwenhoek in his own faeces (Dobell 1920), and the organism has intrigued biologists and clinicians ever since. However, the first detailed description of the parasite was not given until two centuries later by Lambl (1859). Koch’s postulation was proven by Rendtorff in 1954 when he successfully transmitted symptomatic Giardia infection to human volunteers following orally administered cysts. The first symptoms of clinical giardiasis were reported in the early 1920s, although the significance of Giardia as a cause of diarrhoeal disease was controversial for many years (see Farthing 1994; Cox 1998), and it is only recently that the significance of Giardia as a cause of chronic disease in children and its association with failure to thrive, wasting and malabsorption syndromes has been fully realised (reviewed in Farthing 1994; Hall 1994; Gracey 1994; Rabbani and Islam 1994; Hesham et al. 2005; Savioli et al. 2006; Thompson 2008).The question of Giardia ’s role as a source of zoonotically transmitted disease again has been controversial. The World Health Organization (WHO) recommended that Giardia should be considered as a zoonotic agent in 1979 (Anon. 1979). Since that time, increasing circumstantial epidemiological evidence from waterborne outbreaks, the results of some cross-infection experiments and molecular characterization studies of Giardia isolates from humans and other animals has led most authorities to conclude that Giardia should be considered a zoonotic parasite (Acha and Szyfres 2003; Savioli et al. 2006; and reviewed in Thompson 2004). However, as discussed below, the frequency of zoonotic transmission is uncertain.

The leishmaniasis is a complex disease system, caused by the protozoan parasite Leishmania and transmitted to humans by the vector Lutzomyia spp. Since it is listed as a neglected disease according to the World Health Organization, the aim of this study was to determine the current and future niche of cutaneous and visceral leishmaniasis in the Neotropical region. We built the ecological niche model (ENM) of cutaneous (N= 2 910 occurrences) and visceral (N= 851 occurrences) leishmaniasis using MaxEnt algorithm. Nine bioclimatic variables (BIO1, BIO4, BIO5, BIO6, BIO7, BIO12, BIO13, BIO14, BIO15 (downloaded from the Worldclim) and disease occurrences data were used for the construction of ENM for three periods (current, 2050 and 2070) and four climate change scenarios (RCP 2.6, 4.5, 6.0 y 8.5). We analyzed the number of pixels occupied, identity niche, modified niche (stable, loss, and gain) and seasonality. Our analyses indicated the expansion for cutaneous leishmaniasis (CL), a comparison for visceral leishmaniasis (VL). We rejected the null hypothesis of niche identity between CL and VL with Hellinger’s index = 0.91 (0.92-0.98) and Schoener’s Index = 0.67 (0.85-1.00) but with an overlap niche of 56.3 %. The differences between the two leishmaniasis types were detected in relation to RCP scenarios and niche shifts (area gained / loss). Seasonality was more important for CL. We provided a current picture of CL and VL distributions and the predicted distributional changes associated to different climate change scenarios for the Neotropical region. We can anticipate that increasing range is likely although it will depend locally on the future trends in weather seasonality.

Trichomoniasis is the most common non-viral sexually transmitted disease (STD) globally and yet is not a reportable disease. Trichomonas vaginalis is an important source of reproductive morbidity and may increase risk of acquisition and transmission of human immunodeficiency viruses (HIV). The World Health Organization (WHO) and the Control Disease Center (CDC) recommend various regimens of nitroimidazole s for treatment. The common nitroimidazoles used for trichomoniasis are metronidazole and tinidazole, which vary in their cost, efficacy, and side effect profile. It is relevant to study these factors for better management of the patients. This study aimed to compare and study the efficacy, compliance of various treatment regimens, their outcomes, and side-effects for trichomoniasis, among STI clinic attendees in Trinidad. A clinical trial study was designed, and after obtaining the informed consent, a routine clinical examination was conducted and the swabs for trichomoniasis tests were collected for diagnosis from the 692 participants. Out of 692 participants, 82 patients with positive diagnosis of Trichomonas infection were treated according to the patient’s choice, using different drug regimens. Compliance to treatment, side effects, and outcome were evaluated. The prevalence of trichomoniasis in the population attending our STI clinic is 11.9% and prevalence of HIV is 9%. Of the total 82 participants for the treatment, 80% were females; nearly 90% of the patients belonged to age group 15–45 years, and over 60% were below 30 years. Among those diagnosed for Trichomonas vaginalis, 14.6% had coexistent HIV infection. The compliance with respect to single dose treatment was significantly better than the long-duration oral regimen and has a significant relation with side effects of the treatment. The outcome is generally better and comparable and shows no significant difference between different treatment regimens used in the study. Metronidazole and tinidazole are commonly used drugs in various regimens. Compliance is better with those treated with tinidazole and metronidazole single dose than with other groups. Outcome is comparable between these regimens, especially when combined with other important factors like abstinence and treatment of the partners. The treatment regimens mainly differed in the compliance side effects profile and duration of therapy, which suggests that to improve the compliance of the drugs with fewer side effects, short course regimen would be a preferred choice.

Background & Objectives:The use of insecticide treated nets has been advocated for the prevention of the vector borne transmitted disease (malaria) by the World Health Organization and UNICEF for more than a decade. In spite of this, there is no significant reduction in the transmission of the disease. Main objectives of study were to find out the real pattern of using it, to find out the physical integrity of the bed nets being used, and to prove the correlation in between the physical integrity of bed nets and the disease outcome. Torn bed nets with holes size more than 1.2 mm were considered as “holes” in this study.Materials & Methods:A community based cross- sectional study was carried out in Lakhantari VDC within the duration of two weeks. This VDC has been named recently as Gramthan Gaupalika one of State one.  Sample size of 384 was determined by the WHO sample size calculator. Face to face interview technique was used after taking consent from individual. Confidentiality was maintained. It was ethically approved by the IRC (Institutional Review Committee) of Nobel Medical College.Results:A total of 384 household were studied. Total household had bed nets but the physical integrity of bed nets was not intact. Almost 73% of the bed nets were torn having more than four holes in them. Nearly 94% of household used bed nets only for three to four days a week. Nearly half of the   Malaria was found among 22% and encephalitis in 17% of household. Conclusion:Use of bed nets do not prevent and provide guarantee from vector borne disease unless it is properly used. Torn bed nets are of almost no use unless people are using other preventive measures.

SummaryIn order to understand current and changing patterns of population health, there is a clear need for high-quality health indicators. The World Health Organization Study on Global AGEing and Adult Health (SAGE) survey platform and the International Network for the Demographic Evaluation of Populations and Their Health in developing countries (INDEPTH) generated data for this study. A total of 4300 people aged 50 years or older were selected randomly from the Matlab Health and Demographic Surveillance System of the International Centre for Diarrhoeal Disease Research, Bangladesh. The health indicators derived from these survey data are self-rated general health, overall health state, quality of life and disability levels. The outcome of the study is mortality over a 2-year follow-up since the survey. Among the four health indicators, only self-rated health was significantly associated with subsequent mortality irrespective of sex: those who reported bad health had higher mortality than those who reported good health, even after controlling for socio-demographic factors. For all other three health indicators, such associations exist but are significant only for males, while for females it is significant only for ‘quality of life’.

When any new product is introduced to the market it is extremely important to evaluate all the experimental evidence needed to support the claims. In this regard, we take strong exception to the comments of Dr J. Tulloch who is concerned that a recent advertisement run in Pediatrics for Mead Johnson's newly introduced product, Ricelyte, is misleading. After more than 4 years of research and development, stimulated in part by the extensive efforts of the World Health Organization Diarrhoeal Disease Control Programs (CDD), the Mead Johnson Research Center successfully produced from rice flour, a refined, highly specific glucose polysaccharide, "rice syrup solids."

Pilomyxoid astrocytoma (PMA) is a freshly described figure of low-grade neoplasms encountered in early childhood. Nevertheless, its precise classification by the World Health Organization (WHO) is still debatable. Making an exact diagnosis relies on histological and immunohistochemical pathognomonic features with specific radiological findings. PMA behaves aggressively with a shorter progression-free survival, and its management is unfortunately still arguable. We describe a rare case of PMA involving the suprasellar region who displays symptoms consistent with diencephalic syndrome. The diagnosis was made by magnetic resonance imaging (MRI) focused on the hypothalamic-pituitary axis, and the patient underwent a subtotal tumor resection combined with chemotherapy. Diagnosis of brain tumors should be kept in mind in young children with generalized and severe unexplained loss of subcutaneous fat with failure to thrive after ruling out classical causes.

Background. Trichomoniasis is a common sexually transmitted disease caused by Trichomonas vaginalis. It is a major health problem worldwide. The World Health Organization (WHO) has estimated that 180 million infections are acquired annually worldwide. Methodology. Vaginal swabs (1207) were cultured for T. vaginalis on Trichomonas Medium no. 2 (Oxoid) soon after specimen collection. The cultures were examined daily using a light microscope to detect the presence of T. vaginalis. Results. The prevalence of T. vaginalis was 13.6% (164/1207). The infection rate was the highest during pregnancy, 28.1%, and the lowest among women whose spouses use condoms, 8.6%. Conclusions. The culture method was used in this study to accurately determine the prevalence of this parasite in the West Bank, Palestine. The results of the study will eliminate ambiguities concerning trichomoniasis in this country and will contribute to better management and proper treatment.

The novel Corona Virus Disease popularly known as COVID-19 brought all sectors to a temporary standstill since its first outbreak in Wuhan China in 2019. The rapid spread of this disease to all countries of the world made it to be declared a pandemic by the world health organization in the first quarter of the year 2020. This pandemic exposed the world population to the understanding of immunity since that was the only remedy at present to this new pandemic. The morbidity and mortality of this new virus were estimated across the globe by many institutions and organizations. The tropical regions where malaria epidemic is more pronounced is less affected by COVID-19 probably due to high immunity already developed by the population as a result of frequent malaria disease and regular use of anti-malarial drug.

AbstractPneumonia is a highly transmitted disease in children. According to the World Health Organization (WHO), the most affected regions include South Asia and sub-Saharan Africa. 15% deaths of children are due to pneumonia. In 2017, 0.88 million children were killed under the age of five years. An analysis of pneumonia disease is performed with the help of a delayed mathematical modelling technique. The epidemiological system contemplates subpopulations of susceptible, carriers, infected and recovered individuals, along with nonlinear interactions between the members of those subpopulations. The positivity and the boundedness of the ongoing problem for nonnegative initial data are thoroughly proved. The system possesses pneumonia-free and pneumonia existing equilibrium points, whose stability is studied rigorously. Moreover, the numerical simulations confirm the validity of these theoretical results.
