Self-Efficacy and Balance Correlates of Fear of Falling in the Elderly

This study examined relationships among physical activity patterns, self-efficacy, balance, and fear of falling in older adults. Fifty-eight older adults (52-85 years) completed measures of physical activity, self-efficacy, and fear of falling. Subjects then performed the items found in the Berg Balance Scale (Berg, Wood-Dauphinee, Williams, & Maki, 1992). More physically active adults were less fearful of falling, had better balance, and had stronger perceptions of efficacy. Those with better balance were less fearful of falling, and females were more fearful than males. Balance and self-efficacy had significant independent effects on fear, whereas the contribution of history of physical activity was nonsignificant. The findings suggest that behavioral, social cognitive, and biological factors may be important correlates of fear of falling. Further support is provided for the utility of self-efficacy measures in the prediction of fear of falling, although reliance on any one measure to assess this construct may underestimate the role of self-efficacy.

This study investigated attrition in a 6-month physical activity intervention for older adults. The program was based on the social-cognitive theory incorporating self-efficacy factors. Two hundred forty-eight insufficiently active 65- to 74-year-olds were recruited from the Australian federal electoral roll. The intervention comprised walking and strength and flexibility exercises and was conducted in 30 local neighborhoods where the participants resided. Characteristics of individuals lost to attrition (n= 86, 35%) were compared with those of program completers (n= 162, 65%). Logistic-regression analysis showed that those lost to attrition came from areas of lower socioeconomic status, were overweight and less physically active, and had lower walking self-efficacy scores and higher loneliness scores. The results suggest that early assessment of these characteristics should be undertaken to identify individuals at risk of attrition, to improve retention, and to avoid potential bias.

Background Physical activity (PA) is essential for older adults with osteoporosis, and health care professionals play important roles in promoting PA and encouraging patients to make healthy choices. However, many factors influence habitual PA, and there is only limited knowledge about the perceptions and experiences of PA among older women with osteoporosis. Objectives The purpose of this study was to describe perceptions and experiences of PA and the factors that influence habitual PA among older adults with osteoporosis, impaired balance, and fear of falling. Design This was a qualitative interview study applying interpretive content analysis with an inductive approach. Methods Informants were a purposeful sample of 18 women, aged 66 to 86 years, with osteoporosis, impaired balance, and fear of falling. Individual, semistructured, face-to-face interviews were recorded, transcribed, condensed, and coded to find subthemes and themes. Results The overall theme found was “Physical activity—a tool for staying healthy with osteoporosis.” This overall theme comprised 2 main themes interpreting the challenges and possibilities of being physically active with osteoporosis. These themes were not separate but rather linked to each other like 2 sides of the same coin, with factors that could act as both barriers to and facilitators of PA. Personal preferences and osteoporosis-related concerns influenced habitual PA, and individualization was perceived as important. Limitations Some results may be context specific and limit the transferability to people with other cultural or socioeconomic backgrounds. Conclusions The women perceived that PA was an important tool to maintain health with osteoporosis and believed that they had a responsibility to use this tool. They had adapted to disease-specific limitations and developed strategies to overcome challenges and barriers to PA. Lack of PA promotion and conflicting advice about PA from physicians created uncertainty. Encouragement and guidance from physical therapists, individually or in groups, were very important.

Abstract Background Physical inactivity can be associated adversely with markers of eye health, but little is known about how ophthalmic disease (OD) may be related to sedentary behaviour (SB) and different intensities of physical activity in older adults. We compared device-assessed time spent in SB, light-intensity PA (LPA), and moderate-to-vigorous physical activity (MVPA) between older adults with and without OD. Methods A randomly-recruited sample of 512 community-dwelling older adults (47% male; aged 65–84 years) in Japan wore a valid tri-axial accelerometer for seven consecutive days in 2017. Prevalence of OD was assessed by medical interview. Times spent in SB, LPA, and MVPA were compared between those with and without OD by compositional multivariate analysis of covariance, adjusting for potential confounders (gender, age, residential area, living arrangement, working, body mass index, self-rated health, and cognitive function). Results Overall, percent times spent in SB, LPA, and MVPA during waking hours were 51.0, 44.6, and 4.4, respectively; and, 167 (36.5%) had OD. Activity patterns differed significantly between those with and without OD: in those with OD, the proportion of time spent in MVPA relative to other behaviours was lower, and the proportion of time spent in SB relative to other behaviours was higher. Conclusions Older adults with OD were found to be less physically active than those without. Key messages While these cross-sectional findings need to be confirmed by evidence from prospective studies, the reciprocal influences of OD and inactivity in older adults should be addressed as scientific, clinical and public health considerations.

According to Bandura’s social-cognitive theory, perceptions of somatic and affective barriers are sources of self-efficacy. This longitudinal study compares general indicators of health barriers with measures of perceived somatic and affective barriers to predict self-efficacy and accelerometer-assessed physical activity in a subsample of n = 153 (selected at random from N = 310) community-dwelling German older adults. Perceived somatic and affective barriers longitudinally predicted physical activity mediated by self-efficacy, whereas general health barriers did not. Perceived health barriers to physical activity might be more important than more objective health barriers for older adults’ physical activity levels.

Abstract Background: Interaction of physical activity and overall immune profile is very complex and depends on the intensity, duration and frequency of undertaken physical activity, the exposure to cytomegalovirus (CMV) infection and the age-related changes in the immune system. Daily physical activity, which particularly influences immunity, declines dramatically with age. Therefore, the aim of the study was to explain whether physical activity sustained throughout life can attenuate or reverse immunosenescence. Methods: Ninety-nine older adults (60-90 years) were recruited for the study. According to the 6-min walk test (6WMT), the Åstrand-Ryhming bike test (VO2max) and Community Healthy Activities Model Program for Seniors (CHAMPS) questionnaire, the individuals were classified as physically active (n=34) and inactive (n=20) groups. The analysis of T lymphocytes between active vs. inactive participants was performed using eight-parameter flow cytometry.Results: Analysis of the baseline peripheral naïve and memory T lymphocytes showed a significant relationship of lifestyle exercise with CD4/CD8 ratio. Above 50% of physically active participants demonstrated the CD4/CD8 ratio ≥1 or ≤2.5 contrary to the inactive group who showed the ratio <1. The elderly with the result of 6WMT >1.3 m/s and VO2max >35 mL/kg/min had a significantly higher CD4+CD45RA+ T lymphocyte percentage and also a higher ratio of CD4+CD45RA+/CD4+CD45RO+.Interestingly, in active older adults with IgG CMV+ (n=30) the count of CD4+CD45RA+ T lymphocytes was higher than in the inactive group with IgG CMV+ (n=20). Conclusion: Based on the flow cytometry analysis, we concluded that lifestyle exercise could lead to rejuvenation of the immune system by increasing the percentage of naïve T lymphocytes or by reducing the tendency of the inverse CD4/CD8 ratio.

In the sport, physical activity, and aging literature, much attention has been given to the importance of physical activity and sport involvement for the elderly. Most of the literature, however, has focused on the continuity of physical activity among older adults. The purpose of this study was to extend the understanding of older sport participants by conducting a case study of Max Springer, a male, White master runner (88 years old). We assumed that continuity in sport would represent a primary adaptive strategy for coping with the aging process. In addition to two in-depth interviews with Max, the authors interviewed various other “participants” regarding their perceptions of Max as an older runner. From deductive analysis of the interview material, the following themes emerged as figural to Max’s experience as an older runner: tradition of always being physically active, I’m not an athlete, being of senior age, meaning and philosophy of running, and significance of social support.

Abstract Older adults are insufficiently physically active, and therefore, are at an increased health risk. However, less is known about the moment-to-moment physical activity behavior patterns that change across the day. The current study utilized accelerometry to better understand diurnal physical activity patterns. Minority older adults (N=91, age M=70.14, 96% Black/African American) participated in an 8-day study where they wore an ActivPAL accelerometer to measure physical activity. Physical activity was operationalized as stepping in the 60-minute window (±30-min) around a given moment. Time varying-effect modeling was used to determine how physical activity patterns change over the course of weekdays and weekend days. Results are rounded to the 5-min interval. On weekdays, time spent stepping increased from 8:00am (B=7.16, 95%CI: 5.04, 9.27) until peaking at 11:10am (B=8.46, 95%CI: 6.59, 10.33), slowly decreased to the lowest point at 6:10pm (B=6.72, 95%CI: 4.82, 8.62), and then increased slightly in the evening. On weekend days, time spent stepping was lowest at 8:00am (B=4.47, 95%CI: 2.14, 6.80), peaked at 12:20pm (B=5.50, 95%CI: 3.22, 7.78), gradually decreased until 5:50pm (B=5.21, 95%CI: 2.90, 7.52), and then increased slightly in the evening. Minority older adults engage in more stepping time around mid-day, but less stepping in the late afternoon to early evening on both weekdays and weekend days. Self-care activities (e.g., mealtime, bed-time routines) may influence older adults’ physical activity. This work identifies vulnerable times during the day when older adults engage in relatively low levels of physical activity which may be of interest for interventions.

Rural-residing older adults (OA) are not meeting physical activity (PA) recommendations, such that identifying methods of increasing PA among OA remains an ongoing challenge. This study evaluated the effect of a community-based exergaming program on PA readiness-to-change and self-efficacy among rural-residing OA (n = 265). There was a significant (p = .008) increase in readiness-to-change classification from PRE to POST. Significant increases in self-efficacy, or confidence in their ability to be physically active for a prescribed period of time, were detected for 35 (p = .011) and 40 min (p = .035) of continuous PA. PA self-efficacy change for 35 min of continuous PA (F [3,137] = 3.973, p = .010) and 40 min of continuous PA (F [3,137) = 2.893, p = .038) were influenced by the interaction between PRE self-reported health and PRE PA readiness-to-change levels. Results suggest that an exergaming-themed PA intervention is effective at increasing PA participation and self-efficacy for PA among rural-residing OA.

Abstract Background: The expanding population of older adults with multiple sclerosis (MS) likely experience many of the same benefits of physical activity as younger and middle-aged adults with MS. However, participation in physical activity is exceedingly low in this particular segment of the MS population. This study examined variables from social cognitive theory (SCT) as correlates of physical activity in older adults with MS, as this might inform the subsequent development of behavioral interventions. Methods: Older adults with MS (≥60 years, N = 180) completed an online survey including demographics and clinical characteristics, SCT variables (exercise self-efficacy, exercise goal setting, social support, and outcome expectations), and physical activity (total physical activity (PA) and moderate-to-vigorous physical activity (MVPA)). Results: Bivariate correlation analyses indicated that all SCT variables were significantly associated with both PA and MVPA (all P values ≤ .001). Hierarchical linear regression analyses indicated that disability status was a significant correlate of PA (β = −0.48; R2 = 0.23) and MVPA (β = −0.44; R2 = 0.19) in Step 1; disability and self-efficacy were significant correlates of PA (disability β = −0.20; self-efficacy β = 0.59; R2 = 0.50) and MVPA (disability β = −0.16; self-efficacy β = 0.60; R2 = 0.47) in Step 2; and disability, self-efficacy, and exercise goal setting were significant correlates of PA (disability β = −0.21; self-efficacy β = 0.50; exercise goal setting β = 0.14; R2 = 0.55) and MVPA (disability β = −0.17; self-efficacy β = 0.51; exercise goal setting β = 0.15; R2 = 0.51) in Step 3. Conclusions: Our results suggest that behavioral interventions targeting self-efficacy and exercise goal setting as targets from SCT may be appropriate for increasing physical activity in older adults with MS.

Abstract Aging results in a chronic, pro-inflammatory state which can promote and exacerbate age-associated diseases. In contrast, physical activity in older adults improves whole body health, protects against disease, and reduces inflammation, but the elderly are less active making it difficult to disentangle the effects of aging from a sedentary lifestyle. To interrogate this interaction, we analyzed peripheral blood collected at rest and post-exercise from 68 healthy younger and older donors that were either physically active aerobic exercisers or chronically sedentary. Subjects were profiled for 44 low-abundance cytokines, chemokines and growth factors in peripheral blood. At rest, we found that regular physical activity had no impact on the age-related elevation in circulating IL-18, eotaxin, GRO, IL-8, IP-10, PDGF-AA or RANTES. Similarly, there was no impact of physical activity on the age-related reduction in VEGF, EGF or IL-12 (p70). However, older exercisers had lower resting plasma fractalkine, IL-3, IL-6 and TNF-α compared to sedentary older adults. In contrast to our resting characterization, blood responses following acute exercise produced more striking difference between groups. Physically active younger and older subjects increased over 50% of the analyzed factors in their blood which resulted in both unique and overlapping exercise signatures. However, sedentary individuals, particularly the elderly, had few detectable changes in response to exercise. Overall, we show that long term physical activity has a limited effect on age-associated changes in basal cytokines and chemokines in the healthy elderly, yet physically active individuals exhibit a broader induction of factors post-exercise irrespective of age.
