Treatment with programmed cell death 1 inhibitors is associated with a wide range of cutaneous immune-related adverse events, with lichenoid eruptions representing one of the major cutaneous toxicities. We describe the case of an 81-year-old man with metastatic melanoma treated with pembrolizumab who subsequently developed a delayed-onset generalized lichenoid dermatitis. After failing multiple lines of systemic immunosuppression, narrowband ultraviolet B (NBUVB) phototherapy three times per week for 17 sessions resulted in a significant clinical response in his cutaneous eruption and was well tolerated. NBUVB is a safe, lower-cost modality that induces local, skin-specific immunosuppression without the toxicities of traditional systemic immunosuppressive agents. To date, this is the first report of use of NBUVB in immune-related lichenoid dermatitis resistant to multiple standard therapies.

SUMMARYMeasuring a biomarker in pooled samples from multiple cases or controls can lead to cost-effective estimation of a covariate-adjusted odds ratio, particularly for expensive assays. But pooled measurements may be affected by assay-related measurement error (ME) and/or pooling-related processing error (PE), which can induce bias if ignored. Building on recently developed methods for a normal biomarker subject to additive errors, we present two related estimators for a right-skewed biomarker subject to multiplicative errors: one based on logistic regression and the other based on a Gamma discriminant function model. Applied to a reproductive health dataset with a right-skewed cytokine measured in pools of size 1 and 2, both methods suggest no association with spontaneous abortion. The fitted models indicate little ME but fairly severe PE, the latter of which is much too large to ignore. Simulations mimicking these data with a non-unity odds ratio confirm validity of the estimators and illustrate how PE can detract from pooling-related gains in statistical efficiency. These methods address a key issue associated with the homogeneous pools study design and should facilitate valid odds ratio estimation at a lower cost in a wide range of scenarios.

Abstract The use of triazole, benzotriazole, and naphthotriazole as corrosion inhibitors for brass is briefly reviewed. The corrosion of 70/30 brass immersed in a wide range of solutions is reported both with and without the inhibitors. The inhibitor efficiency of benzotriazole is given as a function of the solution pH and the concentration used. Triazole was only effective in mildly corrosive solutions and benzotriazole and naphthotriazole were much better. Generally naphthotriazole was better than benzotriazole but is much more expensive and a higher concentration of benzotriazole can give the same protection as naphthotriazole at a much lower cost.

There has been an enormous growth in the development of biotechnological applications, where advances in the techniques of microelectronic fabrication and the technologies of miniaturization and integration in semiconductor industries are being applied to the production of Laboratory-on-a-Chip devices. The aim of this development is to create devices that will perform the same processes that are currently carried out in the laboratory in reduced timescales, at a lower cost, requiring less reagents, and with a greater resolution of detection and specificity. The expectations of this Laboratory-on-a-Chip revolution is that this technology will facilitate rapid advances in gene discovery, genetic mapping and gene expression with broader applications ranging from infectious diseases and cancer diagnostics to food quality and environmental testing. A review of the current state of development in this field reveals the scale of the ongoing revolution and serves to highlight the advances that can be perceived in the development of Laboratory-on-a-Chip technologies. Since miniaturization can be applied to such a wide range of laboratory processes, some of the sub-units that can be used as building blocks in these devices are described, with a brief description of some of the fabrication processes that can be used to create them.

Symptoms of active Crohn’s disease may respond to one or more of a number of classes of drug therapies. These include systemic glucocorticoids, budesonide, sulphasalazine, mesalazine (5-aminosalicylates), immunosuppressive agents and antibiotics. More recently, a chimeric mouse-human antibody to tumour necrosis factor (infliximab) has been shown to induce clinical remission and endoscopic improvement in patients with moderately active Crohn’s disease refractory to other therapies. Despite this wide range of existing therapies and the potential of emerging biological therapies, recurrent Crohn’s disease continues to be a major impediment to the fulfilment of a normal lifestyle and optimal quality of life for patients with Crohn’s disease. Many drugs known to be effective for the treatment of active disease have been tried as maintenance therapy to prevent disease relapse or recurrence following medical or surgical therapy. The available evidence suggests that most of these drugs are not as useful in maintaining remission as they are in inducing it. Systemic glucocorticoids, budesonide, mesalazine (5-aminosalicylates), sulphasalazine and antibiotics are all associated with either marginal therapeutic gain in the setting of maintenance therapy or unacceptable long term toxicity. The immunosuppressive agents azathioprine, 6-mercaptopurine and methotrexate have been shown to have a beneficial effect in maintaining remission and may be helpful as steroid-sparing agents. Repeated infusions of antitumour necrosis factor antibody maintain the improvements observed after one or two initial infusions. The relative long term safety, efficacy and cost effectiveness of the various choices of maintenance therapy remain to be determined.

AbstractThe postpartum changes in plasma leptin levels of dairy cows ovulated within 35 days after calving were compared to of those with delayed onset of cyclicity in two experiments. The cows were sampled for leptin, insulin, IGF-1, thyroid hormones (T4, T3, rT3), Cortisol and also for some metabolites (glucose, NEFA, BHB and TCH) on day 1-3 after calving and again four times 7 days apart (Exp. 1), or in weeks 1, 2, 3, 5 and 10 postpartum (Exp. 2). In Exp. 1 also the standard low dose ACTH-induced Cortisol response and TRH-stimulated T4/T3 responses were determined on days 1-3 and 28-35. In cows of Exp. 1 the leptin level varied within a wide range with a mean of about 4 ng/ml HE in the first samples. Up to week 5 it remained unchanged in cows ovulated within day 35 but reduced in those with delayed onset of cyclicity, resulting in significant difference between the two group means.In cows of Exp. 2 the week 1 leptin levels were lower and less variable than that of Exp. 1. In weeks 2 and 3 a slightly increasing tendency of leptin pattern was seen in cows resuming their ovarian cyclicity within 35 days whereas in those with delayed onset of cyclicity the leptin level remained unchanged. Up to week 5 this tendency resulted in exactly the same leptin values as seen in Exp. 1 with significant differences between the group means in both of the weeks 5 and 10. It was concluded that the plasma leptin concentration in postpartum dairy cows may interfere with resumption of cyclic ovarian function, but its influence may be only permissive (e.g. a minimal concentration above a supposed threshold seems to be the prerequisite of the onset of cyclicity)

The paper presents the layout of two opposing thyristors working as an Arc Eliminator (AE). The presented solution makes it possible to protect an electrical apparatus against the effects of an arcing fault. An Arc Eliminator is assumed to be a device cooperating with the protected apparatus. Thyristors were used because of their speed of operation and a relatively lower cost compared to other semiconductors with the same current-carrying capacity. The proposed solution, as one of the few currently available, makes it possible to eliminate the fault arc—both at short-circuit currents and current values to which overcurrent protections do not react. A test circuit was designed and made to study the effectiveness of the thyristor arc eliminator. A series of tests was carried out with variable impedance in the arc branch, including the influence of circuit inductance on arc time. It was found that the thyristor arc eliminator effectively protects devices powered from a low voltage power network against the effects of a fault or arc fault. The correctness of system operation for a wide range of impedance changes in the circuit feeding the arc location was demonstrated.

Introduction: Eyelid tumor refers to the primary eyelid skin and accessory tumors, which not only affects the appearance and vision, and even local infiltration and distant metastasis, thereby threatening life. Surgical resection is the most important and effective treatment, but tumor resection often leads to a wide range of eyelid defects, significantly affecting the shape and function of the eyelid. Therefore, the eyelid defect must be repaired after resection to achieve the purpose of aesthetic appearance and protection of the eyeball. The objective of the study was to analyze the clinical results of local flap transfer and scleral transplantation in repairing skin defect after eyelid tumor operation. Methods: Forty patients with postoperative skin defects of eyelid tumors admitted from December 2008 to December 2018 were taken for analysis and divided into two groups, 20 cases in each group. The control group underwent autologous lip mucous membrane transfer and nearby skin flap, while the experimental group received local skin flap rotation and scleral allograft repair. The clinical effect of repair of the two groups were compared. Results: In the experimental group, the effect of clinical repairs were better, the difference of height of palpebralfissure and the length of palpebral fissure were compared with the control group, which showed statistical significance favoring the experimental group. Conclusion: Local flap transfer and scleral transplantation has better clinical outcome to repair the skin defect after operation of eyelid tumor.

Oscillating vortex generator jets have been used to control boundary layer separation from the suction side of a low-pressure turbine airfoil. A low Reynolds number (Re = 25,000) case with low free-stream turbulence has been investigated with detailed measurements including profiles of mean and fluctuating velocity and turbulent shear stress. Ensemble averaged profiles are computed for times within the jet pulsing cycle, and integral parameters and local skin friction coefficients are computed from these profiles. The jets are injected into the mainflow at a compound angle through a spanwise row of holes in the suction surface. Preliminary tests showed that the jets were effective over a wide range of frequencies and amplitudes. Detailed tests were conducted with a maximum blowing ratio of 4.7 and a dimensionless oscillation frequency of 0.65. The outward pulse from the jets in each oscillation cycle causes a disturbance to move down the airfoil surface. The leading and trailing edge celerities for the disturbance match those expected for a turbulent spot. The disturbance is followed by a calmed region. Following the calmed region, the boundary layer does separate, but the separation bubble remains very thin. Results are compared to an uncontrolled baseline case in which the boundary layer separated and did not reattach, and a case controlled passively with a rectangular bar on the suction surface. The comparison indicates that losses will be substantially lower with the jets than in the baseline or passively controlled cases.

The effect of thermal radiation on a steady two-dimensional natural convection laminar boundary layer flow of a viscous incompressible optically thick fluid over a vertical wavy frustum of a cone has been investigated. The boundary layer regime when the Grashof number Gr is large is considered. Using appropriate transformations, the basic governing equations are transformed into a dimensionless form and then solved numerically employing two efficient methods, namely: (a) implicit finite difference method together with Keller-box scheme and (b) direct numerical scheme. Numerical results are presented by streamline, isotherms, velocity and temperature distribution of the fluid, as well as the local shearing stress in terms of the local skin-friction coefficient, the local heat transfer rate in terms of local Nusselt number, and the average rate of heat transfer for a wide range of the radiation—conduction parameter or Planck number Rd and the surface heating parameter θw.
