A cross-sectional study was conducted from October 2014 to July 2015 to determine the prevalence and populations ofE. colias well as the prevalence and antimicrobial susceptibility ofE. coliO157:H7 isolated from raw milk. Biochemical and serological tests methods were used to confirmE. coliandE. coliO157:H7 and isolates were subjected to antimicrobial susceptibility test using the agar disc diffusion method. Out of 380 raw milk samples examined, 129 (33.9%) and 11 (2.9%) were contaminated withE. coliandE. coliO157:H7, respectively. The highest prevalence was recorded in samples obtained from vendors (39.1%, 4.978 ± 0.180 log10/ml) compared with samples from farmers (28.1%, 3.93 ± 0.01 log10/ml) with significant differences(P=0.02). The frequency of contamination was higher in the samples collected from milk that was stored and transported in plastic containers (39.4%) than in the containers made of stainless steel (23.0%)(P=0.002). The antimicrobial susceptibility profile showed thatE. coliO157:H7 were resistant to tetracycline (81.8%), streptomycin (81.8%), and kanamycin (63.6%). Milk samples were produced and handled under poor hygienic conditions, stored, and transported in inappropriate containers and under temperature abuse conditions leading to high health risk to the consumers. Additional studies would be needed to establish association between the occurrences ofE. coliO157:H7 in raw milk and all the risk factors involved in and around Asosa town.

Escherichia coli is one of the leading causes of early-onset neonatal sepsis in many industrialized countries. However, there is a lack of studies on Escherichia coli colonization in women and neonates. The study aimed at determining the prevalence Escherichia coli among pregnant women and newborns, perinatal transmission, antimicrobial susceptibility, and risk factors for neonatal colonization. Material and Methods. In this prospective, cross-sectional study, 827 infants born to 808 mothers were enrolled. The study was carried out from October 1, 2006, to June 30, 2007. Women were screened for E. coli carriage at 35–37 weeks of gestation or on admission for premature rupture of membranes and delivery; neonates, within 15 minutes of their lives. Risk factors for colonization were collected by a questionnaire and were recorded during labor. Results. Maternal E. coli colonization rate was 19.9%; neonatal, 14.4%; and transmission rate, 21.4%. Less than one-fourth (22.7%) of neonatal E. coli strains were resistant to ampicillin. Logistic regression analysis revealed that anal sexual intercourse (OR, 3.91; 95% CI, 1.87–8.19), one sexual partner (OR, 2.01; 95% CI, 1.30–3.11), maternal vaginal Escherichia coli colonization (OR, 1.81; 95% CI, 1.12–2.93), maternal body mass index of ≤27 (OR, 1.77; 95% CI, 1.15–2.73), and maternal education lower than university level (OR, 1.70; 95% CI, 1.06–2.74) were associated with neonatal Escherichia coli colonization. Conclusions. The prevalence of maternal Escherichia coli colonization was higher in this study than other studies (19.9%). Neonatal Escherichia coli colonization was 14.4%. The resistance of Escherichia coli isolates to ampicillin was not high (22.7%). Improvement of maternal education and modification of mothers’ sexual habits need to be undertaken to prevent neonatal Escherichia coli colonization.

The serrano artisanal cheese is a typical product from South region of Brazil, which is produced by skilled cheesemakers using raw milk. The contamination of this food by Escherichia coli has a great impact on public health, since it could threat the consumers’ health. The study evaluated the presence of virulence genes, antimicrobial susceptibility profiles and bofilm-production ability of Escherichia coli isolates obtained from raw milk and artisanal cheese produced in Southern Brazil. A total of 117 isolates of E. coli were characterized by multiplex PCR to detect the following virulence genes: eae for enteropatogenic E. coli (EPEC), lt and st for enterotoxigenic E. coli (ETEC), stx for shiga toxin-producing E. coli (STEC), stx and eae for enterohemorrhagic E. coli (EHEC), ipaH for enteroinvasive E. coli (EIEC) and aggR for enteroaggregative E. coli (EAEC). In addition, antimicrobial susceptibility profile to 22 antimicrobial agents was also performed by disk diffusion method, and we searched for extended-spectrum beta-lactamases (ESBL) and/or carbapenemase- producing isolates. Isolates that were positive for ESBL and carbapenemase were further investigated for the presence of the genes: blaTEM, blaSHV, blaOXA, blaCTX-M, for ESBL and blaOXA-48 for carbapenemase. Further, isolates had their ability to form biofilms investigated by the red Congo agar method. Virulence genes of E. coli were identified in 21.37% of the tested isolates, which were classified as EPEC (the most prevalent pathotype) and ETEC or EAEC. Ten (8.55%) of the total studied E. coli isolates revealed a multidrug-resistant profile, since they were resistant to three or more antimicrobial classes; whereas four isolates (3.42%) were classified as ESBL-producers and showed the presence of blaTEM gene. None of the isolates exhibited carbapenemase activity nor did they carry carbapenemase genes. From the total of E. coli isolates, 79 (67.52%) were considered potential biofilm producers. These results address a serious public health issue, since artisanal cheeses pose a risk to consumers’ health, since may be sources of dissemination of diarrheogenic E. coli, that can cause from subclinical to severe and fatal infections in children and adults, and also emphasize the need to improve adaptations/adjustments in the manufacturing processes of these products.

Background. Nosocomial infections occur among patients during their stay in hospitals. The severity of infection depends on the characteristics of microorganisms with a high risk of being acquired when the environment is contaminated. Antibiotic-resistant bacteria are emerging rapidly around the globe creating a serious threat. Methods. A cross-sectional study was conducted from December 2016–February 2017 at Mizan-Tepi University Teaching Hospital, Southwest Ethiopia. Samples were collected from the equipment and hospital surfaces. The isolated bacteria were checked for susceptibility by the Kirby–Bauer disc diffusion method following the standards of CLSI 2014. Health professionals and sanitary team members were included in the study which assessed the disinfection practice of objects from which samples were taken. Data were analyzed using SPSS version 20.0. Results. A total of 201 swab samples were taken, and most bacteria were recovered from thermometer and floor consisting of 21.6% S. aureus, 19.3% CoNS, 15.9% E. coli, 14.8% Klebsiella species, 11.4% P. aeruginosa, 10.2% Proteus species, and 6.8% Serratia species. The most multidrug resistant organisms were S. aureus (79%), Klebsiella species (53.8%), CoNS (47%), and Proteus species (44.4%). Only 6.45% of health professionals disinfect their stethoscope consistently. Conclusion. S. aureus, CoNS, and E. coli were the predominant isolates. Most isolates showed highest susceptibility to ciprofloxacin and least to ampicillin and penicillin. There is no regular sanitation and disinfection of hospital equipment and surfaces.

Abstract Background Tonsillitis is the third most frequently diagnosed infection in the pediatrics age group around the world. It causes significant morbidity and loss of school attendance in children. The emergence of drug resistance in bacterial tonsillitis is getting higher every year. However, data on the drug resistance profiles of bacterial causes of tonsillitis among children within five years of age is not available in Somaliland.Therefore; this study determined the bacterial causes of tonsillitis and their antimicrobial resistance profiles among children within five years of age at Hargeisa Group of Hospital, Somaliland. Methods A cross-sectional study was conducted from March to July 2020. A total of 374 children within five years of age were included using convenient sampling method. Throat swabs were collected from children, processed and bacterial species were identified using standard bacteriological procedures. Antimicrobial susceptibility was done using disc diffusion method. Data on demographic variables and clinical profiles were collected using structured questionnaires. Logistic regression analysis was computed to identify factors associated with bacterial tonsillitis. P-values < 0.05 were taken as statistically significant. Results The median age of children included in the study was 4 years. Overall, 120(32.1%)(95% CI 27.4–36.8%) of children had culture confirmed bacterial tonsillitis. Of them,23(19.2%) had mixed infections.The most frequent bacterial isolates were Streptococcus pyogenes 78(55%), Staphylococcus aureus 42 (29%) and Streptococcus pneumoniae 10(7%). Isolates revealed 83.3–100 % rate of resistance to ampicillin. S. aureus was resistant to clarithromycin (38%) while 60% of S. pneumoniae isolates were resistant to gentamicin. The overall multidrug resistance (MDR) was 50.4% and 52.6% of S.pyogenes and 60% of S.pneumoniae were MDR. History of tonsillitis(AOR = 0.12; 95% CI = 0.06–0.21), difficulty of swallowing(AOR = 6.99; 95% CI = 3.56–13.73), weight loss (AOR = 0.33; 95% CI = 0.186–0.597) and attending school (AOR = 2.98; 95% CI = 1.64–5.42) were found to be associated with tonsillitis among children within five years of age. Conclusions Bacterial tonsillitis with high degree of ampicillin resistance, mixed infections and MDR isolates are major concerns in children within five years of age at Hargeisa, Somaliland. Therefore, treatment of cases should be guided by regular culture and antimicrobial susceptibility testing to prevent disease consequences and critical drug resistance.

Escherichia coli is an emerging public health concern in most countries of the world. It is an important cause of food-borne human disease. The present study assessed the prevalence and determined the antibiotic resistance patterns of E. coli from raw milk marketed in Chittagong, Bangladesh. Of the raw milk marketed in Chittagong ~33(18%) of the 186 raw milk samples of it contains E. coli, indicator bacteria for any enteric pathogens. The mean viable count of total bacteria was 4.04×108 cfu/ml and the mean viable count of E. coli in the contaminated raw milk was 1.88×106 cfu/ml. E. coli from only six (18.2%) of the 33 positive samples yielded colourless colonies across the CT-SMAC, suggesting the probable presence of populations belonging to the serotype O157 and rest of the isolates 27 (81.82%) produced coloured colony on CT-SMAC considering the probable presence of populations belonging to the serotype non-O157. Growth of probable E. coli O157, as evidenced by the colourless colonies on CT-SMAC compared to coloured colonies from other bacteria. Confirmed isolates were further subjected to antimicrobial susceptibility test using the Agar disc diffusion technique. Antibiotics susceptibility profile showed that all the isolates in case of E. coli O157, penicillin (100%), tetracycline (100%), amoxicillin (83.33%) and erythromycin (83.33%) were the most resistant whereas ciprofloxacin (66.67%), gentamicin (50.0%), and streptomycin (50.0%) were the most sensitive antibiotics. In case of E.coli non-O157 susceptibility profile showed that chloramphenicol (40.74%), erythromycin (40.74%) and oxacillin (37.04%) were the most resistant whereas ciprofloxacin (70.37%), sulphamethoxazole/trimethoprim (S/T) (59.26%) and gentamycin (55.55%) were the most sensitive antibiotics. The antimicrobial resistance exhibited by E. coli O157and non-O157 strains in this study is an indication of possible antibiotic abuse.

Objectives.In 2013, Clermont classifiedE. colistrains into eight phylogenetic groups using a new quadruplex PCR method. The aims of this study were to identify the phylogenetic groups ofE. colibased on this method and to assess their antibiotic resistance patterns in Bushehr, Iran.Methods. In this cross-sectional study, 140E. coliisolates were subjected to phylogenetic typing by a quadruplex PCR method. Antimicrobial susceptibility testing was performed by disk diffusion method.Results. Phylogenetic group B2 was most predominant (39.3%), followed by unknown (27.1%), E (9.3%), C and clade I (each 6.4%), B1 (5%), F and D (each 2.9%), and A (0.7%). The most common antibiotic resistance was related to amoxicillin (82.1%) and the least to meropenem (0.7%). 82.14% of isolates were multiple drug resistant (MDR). Antibiotic resistance was mainly detected in group B2 (50%).Conclusions.Our findings showed the high prevalence of MDRE. coliisolates with dominance of group B2. About 25% ofE. coliisolates belong to the newly described phylogroups C, E, F, and clade I. Such studies need to be done also in other regions to provide greater understanding of the antibiotic resistance pattern and the prevalences of different phylogenetic groups.

A total of sixty raw milk samples were collected from (street vendors and shops) from Baghdad city, Iraq. The samples were inoculated into peptone water and, then, subcultured onto McConkey agar and Blood agar. Identification of isolates was confirmed by microscopic examination, cultural characteristic, biochemical tests, Vitek (VITEK®2 system), and Biolog GN substrate reactions followed by 16S rRNA and specific genes sequencing. Of 60 raw cow’s milk samples, Providencia spp. were identified only in 4 samples (6.67%) and P. rettgeri was the most common, 2/4 (50%), followed by P. stuartii and P. vermicola, 1/4 (25%) . Antimicrobial susceptibility tests were conducted against ten antibiotics by the disc diffusion method. All Providencia isolates showed multidrug resistance (MDR), and the absolute resistant was 100% to tetracycline, erythromycin, and doxycycline and 50% against ampicillin\sulbactam and amoxicillin/clavulanic acid. They were highly susceptible (100%) to trimethoprim, imipenem, and chloramphenicol. These findings indicate that milk might be contaminated with Providencia spp. leading to transmission to humans causing poisoning, diarrhea, and other infections. This is the first study of isolated Providencia spp. from raw cow’s milk.

A cross-sectional study was conducted to detect the presence ofSalmonellaspp.,Candida albicans,Aspergillusspp., and antimicrobial residues in raw milk (n=120) and processed cow milk (n=20) from smallholder dairy farms from three sites in Zimbabwe. Culture and isolation ofSalmonellaspp.,C. albicans, andAspergillusspp. were performed using selective media, while antimicrobial residues were detected by a dye reduction test. NoSalmonella, butC. albicans(17.5%; 21/120),Aspergillusspp. (0.8%; 1/120), and antimicrobial residues (2.5%; 3/120) were detected from raw milk.C. albicanswas isolated from all three sites, whileAspergillusspp. and antimicrobial residues were detected from sites 1 and 3, respectively. From processed milk, onlyC. albicans(5%) was isolated whileAspergillusspp. and antimicrobial residues were not detected. These results suggested low prevalence ofSalmonellaspp. andAspergillusspp. and a relatively high prevalence ofC. albicansin raw milk from the smallholder farms. The potential public health risks ofC. albicansand the detected antimicrobial residues need to be considered. Thus, educating farmers on improving milking hygiene and storage of milk and establishing programmes for monitoring antimicrobial residues may help to improve the safety of milk from smallholder farms.

Abstract Objectives Food borne diseases are one of the main important issues in the world. Escherichia coli are considered as the most prevalent food borne pathogen. A cross sectional study was conducted from January 6- 2018 to September 10-2019 on cloacae swab; farm and market’s egg at Jimma town to isolate, identify and determine antimicrobial susceptibility patterns. Result Out of 415 total samples, 156 (37.59%) were identified as E. coli from farm eggshell 21/83 (25.30%), farm egg content 12/83 (14.46%), cloacae swab 42/83 (50.60%) and market egg shell 45/83 (54.23%). In the current study antimicrobial susceptibility test of E. coli isolates from different sample types revealed varying degree of susceptibility to antimicrobial. Isolated E. coli was highly susceptible to Ciprofloxacin (100%), Gentamicine (100%), Streptomycin ( 96.4% ) , sulfonamides (94%), Kanamycin (91%), Chloramphenicol ( 89.5% ) , tetraccine (78.9%) , Trimethoprim (75%) these are considered appropriate for empirical treatment of E. coli in the study area. Moreover, resistance of isolates with 100%, 75% 50%, 25%, 1.6% and 0.5% was developed to ampicillin, neomycin, cefoxitin oxytetracycline , Streptomycin and tetraccine respectively. The presence of E.coli within egg/and chicken can pose serious public health problems. The appropriate hygienic practices of eggs at both farms and markets should be under taken.

Diarrheal diseases continue to be the major cause of morbidity and mortality among children under 5 years. This study aimed to isolate, identify and determining the prevalence, antimicrobial susceptibility profile of Shigella sp associated with acute diarrhea among children in Kano, Northern Nigeria. A cross sectional study was conducted among children less than 5 years diagnosed with acute diarrhea and admitted to paediatric ward of Murtala Muhammad Specialist Hospital Kano. Stool samples from a total of 37 (20 male and 17 female) subjects were used to isolate and identified the pathogen. Antimicrobial susceptibility test was conducted using disc diffusion method. The result showed 12 out of 37 samples were positive for Shigella sp which accounted for 32.4%. Higher incidence of Shigella sp was found among subjects of age between 2 – 3 years. The isolates were 100% resistant to Ampicillin. High resistance was also observed in Amoxicillin (83.33%), Chloramphenicol (58.33%) and Tetracycline (25%). The isolates are 100% sensitive to ciprofloxacin, 66.7% to Levofloxacin and Gentamicin each and 58.33% to Erythromycin. Three (3) isolates were resistance to Ampicillin and Amoxicillin, 5 isolates were resistance to Ampicillin, Chloramphenicol and Amoxicillin while 2 isolates were resistance to Ampicillin, Chloramphenicol, Tetracycline and Amoxicillin. It is concluded that Shigella sp is one of the etiological agent of diarrhea in children. Ciprofloxacin, levofloxacin and Gentamicin are drugs of choice for treating diarrhea caused by Shigella sp.
