Subject. With increasing age of patients, in many cases, the severity of periodontal diseases also increases, and dystrophic ones join the inflammatory processes. Local predisposing factors in the oral cavity are aggravated by concomitant somatic diseases and a decrease in compensatory processes against the background of physiological aging. Literature data indicate that the manifestations of the physiological process of aging and pathological processes (inflammatory and dystrophic) can be clearly observed on the example of buccal epithelial cells. The aim is to identify the relationship between the age of patients, their periodontal status and the cytological characteristics of buccal epithelial cells. Methodology. All study participants were divided into two groups: the first included patients with a conditionally healthy periodontium (72 people), the second - with a chronic inflammatory process in the periodontium (57 people). In accordance with the WHO classification, a gradation of age was carried out: young (18―44 years old), mature (45―59 years old), elderly (60―74 years old), senile (75―90 years old). All patients underwent a comprehensive dental examination, sampling and cytological examination of the buccal epithelium, calculation of the integral indices of the buccal cytogram. Results. In healthy patients, a weak positive correlation was established between the cytogenetic index, the index of the accumulation of cytogenetic disorders and age, a weak negative correlation was found between the proliferative index and age. In patients with an inflammatory process in the periodontal tissues, a multidirectional change in the values of the buccal cytogram indices was observed, which may indicate an imbalance in the processes of regeneration, apoptosis, and cellular damage in inflammatory periodontal diseases in older age groups. Conclusions. In patients with healthy periodontal disease, a regular accumulation of cytogenetic disorders occurs with increasing age, while proliferative activity, on the contrary, decreases. Patients with inflammatory phenomena in the periodontal tissues are characterized by imbalance, impaired coordination of regeneration and apoptosis processes, combined with the most pronounced reactivity in the middle age period.

Summary. Prevention and treatment of periodontal diseases remains one of the most important tasks of modern dentistry in connection with their high prevalence among all age groups of the population. Recently, much attention has been paid to the study of gum recession, as one of the clinical forms of periodontal disease. The paper presents data on paraclinical indexes in patients with gum recession, depending on age and biotype of periodontal disease. The aim of the study – to evaluate the intensification of inflammatory processes in periodontal tissues in patients with gum recession, depending on age and periodontal biotype using paraclinical indices. Materials and Methods. The article presents a comparative assessment of the intensity of inflammatory processes in periodontal tissues, depending on age and periodontal biotype in 204 somatically healthy patients who had a recession of the gums caused by different etiological factors. Results and Discussion. As a result of our research, it was found that with increasing age, there was a clear tendency to increase the indexes of all paraclinical indexes analyzed by us. Also, attention was drawn to the fact that the carriers of biotype periodontal A1-Thin S flow of inflammatory processes proceeded more pronounced and more intense.Conclusions. In persons with recession gum, the intensification of inflammatory processes in periodontal tissues increased with age, but depended on the periodontal biotype.Conclusions. Relevance. Before starting the therapeutic treatment of various periodontal diseases, it should be clearly understood that a high-quality treatment is possible only with complex therapy, excluding local irritating adverse factors in the oral cavity, elimination of inflammatory processes in the periodontal tissues, as well as violations of its function. One of the main tasks of the treatment of periodontal diseases is the preservation of the dentition with a single functional system.Purpose – to assess the use of the drug Kollapan in tooth extraction and treatment of periodontitis.Materials and methods. The clinical observation group consisted of 40 patients aged 19 to 65 years. The gender ratio was 3 : 2 (60% men, 40% women). The largest percentage of patients (65%) came to the clinic with complex tooth extraction. The remaining 35% – with generalized periodontitis of moderate and mild severity.Results. Clinical observation showed that in the first group of patients, whose therapy included the use of Collapan, after a complex tooth extraction, physiological healing of the hole was observed, the disappearance of inflammation within 24–36 hours after the surgery

Obesity prevalence has significantly increased especially in young adults, which is caused by a particular lifestyle, food quality and dietary behavior. Obesity leads to development of huge array of comorbid conditions such as arterial hypertonia, heart stroke, arthritis and other diseases. We conducted standard clinical examination of oral cavity of 154 young patients (18-21 years old) – all of them were students of Ukrainian Medical Stomatological Academy. It has been determined that the majority of patients with BMI >30 kg/cm2 have aggravated family heredity, in 66,2% cases one parent of obese students had obesity and 32,43% patients have both parents with obesity that is significantly higher compared with persons with normal BMI and overweight. There was a high prevalence of periodontal diseases about 74% and carious lesions of teeth - 97.4% among young people despite the age and contingent of examined patients - medical students. In patients with BMI >30 kg/cm2 prevalence of generalized forms of gingivitis and periodontitis was by three times higher compared with individuals with normal BMI. The values of oral hygienic indexes were poor in all examined groups, but their values were slightly lower in patients with normal BMI than in those with overweight and obesity. The prevalence of inflammatory changes in gums was higher in persons with obesity: all of them had a mild degree of lesions in periodontal tissues. Inflammatory processes in the gums were the most intense in patients with the second degree of obesity. According to the results of the study, the presence of the first and the second degree of obesity should be considered as a risk factor triggering periodontal tissues diseases. For persons with BMI >30 kg/cm2 with periodontal disease measures for the secondary prevention of inflammatory and inflammatory dystrophic periodontal diseases should be carried out and in persons without periodontal disease on the background of obesity measures primary prevention should be done.

Biomarkers of inflammation can be used as indicators for monitoring the treatment of periodontal diseases, as well as for finding the safest and most effective drugs with different mechanisms of action. The concentration of biomarkers in saliva was determined by enzyme immunoassay and chemiluminescent analysis. The obtained results reflect the degree of activity of inflammatory processes in periodontal tissues that occur in gingivitis and reveal an imbalance in the processes of free radical oxidation and antioxidant protection of the oral cavity.

Subject. Despite the constant efforts of practicing physicians and scientists, the prevalence proportion of inflammatory and destructive periodontal diseases in the world is not subsiding. Previously scientists have examined several different theories on the development of pathological conditions in periodontal tissues, ranging problems from the neurotrophic theory of occurrence to the effect of occlusion injury on periodontal tissues. Theories dealing with the influence of specific and non-specific bacterial plaque have also been studied. None of the theories has become reasonable and single valued. It is believed that the mechanism of the development of the disease is complex, autoimmune, damaging primarily collagen, elastic fibers of the capillary endothelium, gingival ligament and bone tissue. Over the past decade, it has been proven that inflammation in periodontal tissues is initiated and proceeds in response to the introduction of pathogenic bacteria. However, it can be said that not all patients develop an inflammatory response to bacterial invasion. The second important factor in the development and progression of the inflammatory-destructive process is the immune response of a particular individual. Scientists have noted the influence of the non-microbial factor and have come to the conclusion that in the surrounding tissues of the inflamed area there are not only enzymes secreted by bacteria, but also leukocyte proteinases, which in turn are mediators of the tissue destruction. Purpose ― to examine the role of leukocyte proteinases in the pathogenesis of inflammatory periodontal diseases. Methodology. The number of indigenous and foreign literature sources dealing with the study of the role of leukocyte proteinases in the development and maintenance of a destructive inflammatory process has been reviewed by us. Conclusion. The role of the aggressive destructive effect of leukocyte proteinases on periodontal tissues has been analyzed. Leukocyte proteinases are mediators of progressive destruction of connective tissue and can be used as markers to assess the severity of the course and predict the development of the inflammatory process in periodontal tissues.

The purpose of our study was to increase the level of treatment of periodontal diseases in children with rheumatic diseases through the use of injections of homeopathic drugs in complex treatment. To do this, they conducted a dental examination of children with rheumatological diseases, determined the degree of damage to periodontal tissues using the indices PMA, GI, CPITN and compared with the control group of children. The relationship between inflammatory processes in in periodontal tissues and rheumatic diseases in children was revealed and the effectiveness of homeopathic drugs in the complex treatment of periodontal tissues was assessed.

Today, more than ever, it’s important to be a well-groomed and healthy person, in whom “...everything should be fine...” as the classic said. The vast majority of patients tend to lead a healthy lifestyle, which includes dental health as well [3]. Health of periodontal tissues directly depends not only on the proper oral hygiene carried out by patients at home, but also on an individually selected set of therapeutic measures performed by the dentist [4, 5]. Thus, dentists and related specialists should provide highly professional medical care in the treatment of inflammatory diseases of periodontal tissues to achieve maximum results, stop the inflammatory process and restore the periodontium as a whole [6, 7].

Epidemiological studies in recent years have shown a high prevalence of periodontal tissue diseases in children and adolescents in the world and in Ukraine in particular. One of the main general factors in the development of periodontal tissue diseases is the presence of concomitant pathology, one of which is an excessive body weight.The aim of the study – to learn the prevalence, structure and peculiarities of the clinical course of periodontal tissue diseases in overweight children.Materials and Methods. 278 children with overweight (the main group) were examined. The comparison group was made of 292 children with normal body weight. The study was conducted in age groups of 7, 12, 15 years old. The state of the periodontal tissues of the examined persons was objectified according to the classification of Danylevskyi (1994) and by using of the periodontal indices of PMA, CPI and hygienic index OHI-S. On the basis of the anthropometric examinations, the body mass index (BMI) was calculated.Results and Discussion. The results of the dental examination showed that the number of overweight children with intact periodontium was significantly lower (32.01 ± 2.8 %) compared with children with normal body mass (53.08 ± 2.92 %) (p <0.01). On the other hand, the inflammatory process in the tissues of periodontium in overweight children, on average, was 67.99 ± 2.80 %, which was much higher in relation to children with normal body mass – 46.92 ± 2.92 % (p <0.01).Conclusions. The results of the examination of periodontal tissues in overweight children indicate a high incidence and severity of inflammatory processes in periodontal tissues compared with normal weight children.

Summary. Today it is known that among maxillodental abnormalities crowded teeth have a leading position and often accompanied by periodontal diseases, lesions of the hard dental tissues, functional disorders of maxillodental system and psycho-emotional disorders of patients is a relevant problem in dentistry. The researchers ascertained the fact of increasing the activity of the course of periodontal disease in patients with maxillodental abnormalities.The aim of the study – to estimate the intensification of inflammatory processes in periodontal tissues of young individuals with crowding of the teeth using periodontal indices.Materials and Methods. The article presents the comparative estimation of the intensity of inflammatory processes in periodontal tissues from 104 people in the age of 18−35 years with chronic catarrhal gingivitis (CCG) on the background of crowding of teeth (main group) and 33 people with CCG without orthodontic pathology (comparison group). For the estimation of the state of periodontal tissues the indexes PMA, bleeding gums and СPI were used. The obtained data were worked out statistically. Results Discussion. It is set on the results of studies that in dependence on the degree of severity of CCG and with increase of the age of examined increases the intensity of inflammatory processes in tissues of teeth-restraint apparatus in both study groups, but in patients with CCG on the background of crowding of teeth this tendency was more pronounced. The average value of the PMA index by its criteria of index revealed to gingivitis of the average severity in the main and comparison groups of the study. With the increase of age of patients in both study groups the increase of the index of bleeding was set. In patients with crowding teeth the values of CPI index exceeded the values of the index in individuals without orthodontic pathology.Conclusions. In patients with crowded teeth on the background of chronic catarrhal gingivitis the intensification of inflammatory processes in periodontal tissues increased with age and depended on the severity of chronic catarrhal gingivitis and was higher than in patients without orthodontic pathology.

The expression of toll-like and adhesive receptors on epithelial cells of the oral mucosa changes in different pathological conditions, both local and systemic levels, in particular, in chronic periodontitis. The long-term presence of periodontal pathogenic microorganisms in the gingival furrow stimulates and supports the inflammatory process. The interaction of periodontal pathogens with epithelial cells of the oral mucosa is the first stage of the development of periodontitis. The pathological process affects the function of epithelial cells, in particular their ability to interact with representatives of microbiocenosis. Therefore, the natural colonization of normal oral microbiota on buccal epitheliocytes, reflecting the ability of epithelial cells to microbial adhesion, is a sensitive indicator of various destabilizing processes. Determining the level of expression of toll-like TLR2 and TLR4 receptors on epithelial cells also allows us to assess the functional state of cells and the severity of the inflammatory process at the level of the oral mucosa, in particular, in chronic periodontitis. In this paper, we studied the receptor-dependent reactions of buccal epithelial cells in chronic periodontitis using flow cytofluorometry and by determining the level of natural (microbial) colonization. The authors also compared these methods for determining the functional state of mucosal cells in chronic periodontitis. The results showed that in patients with periodontitis, the activity of receptors involved in adhesive reactions with the oral microbiota changed slightly and was little higher than in healthy donors. At the same time, the expression of TLRs on epithelial cells in periodontitis changed significantly. Thus, the percentage of cells expressing TLR2 significantly increased, while TLR4 decreased. Concurrently, the percentage of mucosal cells that do not have TLRs increased significantly in oral pathology. Thus, the study of TLR2 - and TLR4-expression on buccal epithelial cells is a more representative test in assessing the severity of the inflammatory process in chronic periodontitis than determining the level of natural colonization.
