Mental Health Caregiver’s Experiences From the Perspective of the Power Threat Meaning Framework

Mental health caregivers experience a range of practical and emotional challenges. The Power Threat Meaning Framework is a recently articulated alternative framework for identifying patterns in distress. This qualitative study explores mental health caregivers’ experiences of power, threat, threat response, and meaning. Ten Australian mental health caregivers participated in semistructured interviews with data analysed using thematic analysis. Participants described positive and negative experiences of power in the form of institutional barriers, empowerment through advocacy, power dynamics, and power through nurturing. The mental health of the care receiver presented as a threat to caregiver’s well-being, as did loss of identity, grief, and community disconnect. Threat responses included seeking interpersonal support, regaining control, engaging in self-care, and using coping strategies. A sense of connection to the care receiver, self-growth, and giving back allowed carers to derive meaning from their role. Utilising the Power Threat Meaning Framework offers insight into mental health caregiver’s experiences beyond the confinements of current clinical practice. Suggestions for supporting mental health caregivers are provided.

Sport organizations have been noted as pivotal to the success or failure of athletes, and sport environments can impact the well-being and development of athletes. In this study, the authors explored stakeholders’ perceptions of how high-performance sport organizations support athlete development. Semistructured interviews were conducted with 18 stakeholders from the United Kingdom’s high-performance sport system and transcripts were analyzed using a semantic thematic analysis. Participants emphasized the importance of performance lifestyle advisors, sport psychologists, and financial assistance for promoting athlete development. Several stakeholders observed that despite the extensive support available to athletes, many do not engage with available support, and the prevalence of a performance narrative has led to an environment that discourages holistic development. It follows that sport organizations could develop alternative strategies for promoting athletes’ access to and engagement with available supports, while funding agencies might broaden existing funding criteria to include well-being or athlete development targets.

Purpose The purpose of this paper is to evaluate the current service provision offered by a mental health service in a young offenders institute (YOI) in England. Design/methodology/approach A qualitative exploratory approach was adopted. Five prison officers (POs) (four male and one female) with direct experience supporting young people through mental health intervention were interviewed using semi-structured interviews. Data was analysed using thematic analysis (Braun and Clarke, 2006), which allowed evaluation of the current mental health provision to evolve. Findings Five overarching themes (process of referrals; intervention and its impact; role and function of the health and well-being team; collaboration and involvement; and feedback, communication and intervention) were found to be highly salient when evaluating service provision. Notably all themes interrelated with one another, demonstrating that changes or improvements made to the service need to consider all factors individually and collectively. Research limitations/implications This study was based on a small sample of POs in only one YOI in England, and therefore cannot be generalised to the entire prison estate. However, it does offer a rich insight into local service provision from the perspective of a group whose opinion is rarely sought. Practical implications POs’ perspectives of service provision are important in informing service developments. Lessons can be learnt from their unique insight into how prison mental health services are run to improve processes and collaborative working in the management of young offenders with mental health difficulties. Originality/value This preliminary and exploratory study is, to the best of the authors’ knowledge, the first study evaluating service provision by using the perspectives of POs. The findings suggest that POs make valuable contributions to the service development and should be involved in similar evaluations in future.

Abstract Currently, Australia’s school counsellors are increasingly being called upon to respond to adolescent mental health needs. Through semistructured interviews with seven school counsellors working with adolescents, this qualitative study aimed to capture the lived experiences of this group of practitioners. By adopting a phenomenological approach and using thematic analysis, this study provides insight into their profession: the current ambiguity surrounding their role; the opportunities and obstacles they face; as well as the often-present tension between stakeholders including parents, other school staff and external agencies. The findings of this study indicate that school counsellors are challenged by the need to be advocates not only for their students but also for themselves and their roles within the school context.

ObjectivesTo explore the lived experience of long COVID with particular focus on the role of physical activity.DesignQualitative study using semistructured interviews.Participants18 people living with long COVID (9 men, 9 women; aged between 18–74 years; 10 white British, 3 white Other, 3 Asian, 1 black, 1 mixed ethnicity) recruited via a UK-based research interest database for people with long COVID.SettingTelephone interviews with 17 participants living in the UK and 1 participant living in the USA.ResultsFour themes were generated. Theme 1 describes how participants struggled with drastically reduced physical function, compounded by the cognitive and psychological effects of long COVID. Theme 2 highlights challenges associated with finding and interpreting advice about physical activity that was appropriately tailored. Theme 3 describes individual approaches to managing symptoms including fatigue and ‘brain fog’ while trying to resume and maintain activities of daily living and other forms of exercise. Theme 4 illustrates the battle with self-concept to accept reduced function (even temporarily) and the fear of permanent reduction in physical and cognitive ability.ConclusionsThis study provides insight into the challenges of managing physical activity alongside the extended symptoms associated with long COVID. Findings highlight the need for greater clarity and tailoring of physical activity-related advice for people with long COVID and improved support to resume activities important to individual well-being.

Introduction: In the past decade, there has been an influx of migrant nurses from India to Australia. Migrant professionals have specific transition needs associated with working in Mental Health (MH). This study aimed to explore the transition experience of overseas trained nurses from India working in Australian MH settings. Method: Hermeneutic phenomenology was the methodological approach used in the study. The participants ( N = 16) were overseas trained nurses from India. Data were collected through in-depth interview and analysed using thematic analysis. Results: The findings resulted in the identification of the following themes: (1) Living in dual culture, (2) Loneliness, (3) Discrimination, and (4) Feeling incomplete. Discussion: It is evident that the transition to work in MH in Australia was a journey of mixed experiences. While certain findings of this study are comparable with experiences of migrant nurses in other settings, it provides insight into those that are working in MH.

Context: Pain in sport has been normalized to the point where athletes are expected to ignore pain and remain in the game despite the possible detrimental consequences associated with playing through pain. While rehabilitation specialists may not have an influence on an athlete’s competitive nature or the culture of risk they operate in, understanding the consequences of those factors on an athlete’s physical well-being is definitely in their area of responsibility. Objective: To explore the factors associated with the experiences of subelite athletes who play through pain in gymnastics, rowing, and speed skating. Design: The authors conducted semistructured interviews with subelite athletes, coaches, and rehabilitation specialists. They recruited coach participants through their provincial sport organization. Athletes of the recruited coaches who were recovering from a musculoskeletal injury and training for a major competition were then recruited. They also recruited rehabilitation specialists who were known to treat subelite athletes independently by e-mail. Setting: An observation session was conducted at the athlete’s training facility. Interviews were then conducted either in a room at the university or at a preferred sound-attenuated location suggested by the participant. Participants: The authors studied 5 coaches, 4 subelite athletes, and 3 rehabilitation specialists. Interventions: The authors photographed athletes during a practice shortly before an important competition, and we interviewed all the participants after that competition. Our photographs were used during the interview to stimulate discussion. Results: The participant interviews revealed 3 main themes related to playing through pain. They are: Listening to your body, Decision making, and Who decides. Conclusion: When subelite athletes, striving to be the best in their sport continue to train with the pain of an injury, performance is affected in the short-term and long-term consequences are also possible. Our study provides some insight into the contrasting forces that athletes balance as they decide to continue or to stop.

Research studies have identified various different mechanisms in the effects of nature engagement on well-being and mental health. However, rarely are multiple pathways examined in the same study and little use has been made of first-hand, experiential accounts through interviews. Therefore, a semi-structured interview was conducted with seven female students who identified the role of nature engagement in their well-being and mental health. After applying thematic analysis, 11 themes were extracted from the data set, which were: “enjoying the different sensory input,” “calm nature facilitates a calm mood,” “enhancing decision making and forming action plans,” “enhancing efficiency and productivity,” “alleviating pressure from society's expectations regarding education,” “formation of community relations,” “nature puts things into perspective,” “liking the contrast from the urban environment,” “feel freedom,” “coping mechanism,” and “anxious if prevented or restricted.” The results indicate complementary mechanisms for how nature-related activities benefit mental health and well-being that may occupy different levels of experience within a hierarchical framework informed by perceptual control theory.

Abstract Background A subset of adolescents with mental disorders are likely to have decision-making capacity that facilitates their therapy engagement. However, there are high rates of drop-out in mental health settings. Aim This study aims to identify perceived barriers to or facilitators of mental health care engagement among adolescents with decision-making competence in Greece. Methods A qualitative study was conducted using semi-structured interviews of adolescents with a wide range of mental health problems. In addition, two psychometric assessment measures were used to define who to include or exclude from the study sample. Results Positive attitudes and experiences with therapy were reported as strong (“major”) facilitators of therapy engagement for adolescents with mental disorders, whereas negative experiences with therapy were reported as strong barriers to it. Furthermore, and most importantly, a “good” adolescent-therapist relationship was reported as a strong facilitator, whereas negative experiences of participants with their therapist were reported as strong barriers. Moreover, goals such as getting rid of symptoms, improving personal well-being, and improving social skills and relationships (especially with peers) emerged as strong facilitators of therapy engagement. Importantly, the early remission of symptoms emerged from the study as a strong barrier to therapy engagement for participants. Among the weaker (“minor”) perceived facilitators were goals such as confessing to a trustworthy person, becoming able to achieve personal expectations and life goals, enhancing independence and self-esteem, and developing a positive self-image. The (active or supportive) role of family emerged as a facilitator. The stigma related to mental health emerged as both a (“minor”) facilitator of and barrier to therapy engagement for participants. Friends were reported as having a role ranging from neutral to mildly supportive. Conclusion A number of more or less strong barriers and facilitators were identified that, for the most part, were consistent with prior literature. However, the authors identified some nuances that are of clinical importance. For instance, adolescents are most likely to terminate the treatment prematurely if they experience early symptom remission. Highlighting the role of therapy in achieving their goals or improving their families’ well-being might be used by therapists to reduce the attrition rate.

The paper strives to indicate the constructs that are conducive to the development of optimism-pessimism in adolescents, by presenting the research on optimism-pessimism conducted on a sample of adolescents from Kosovo and Metohija. After an insight into the findings of the research presented (which were conducted from 2017 to 2019) on the sample of more than 700 adolescents aged between 18 and 29, it was found that the correlation of optimism comprises the following: adolescents' personality traits, interpersonal orientation, experience of beauty of the human face, hope, satisfaction with life and well-being. Conclusions drawn from the research will enable the conception and organization of further research. Involving other variables, such as the context of upbringing, family relations, relationship with peers, in research to be conducted in the future is necessary for the examination of their contribution to the development of optimism-pessimism, bearing in mind the major significance optimism has for adolescents' mental health.

Abstract. Objectives: Previous literature demonstrated that, even when mental health and psychological support services are available for refugees, there may still be obstacles in accessing services. This is the first known study to explore the experiences of mental-health and well-being services for Syrian refugees in Coventry and Warwickshire, United Kingdom. The research investigates the views and perceptions of service providers on the current mental-health and well-being services provided for this population. Methods: Eight service providers participated in semistructured interviews and focus groups, and the data were analyzed using thematic analysis. Results: Three main themes emerged from an analysis of the data: “positive aspects of service delivery,” “service challenges,” and “recommendations for service improvements and quality.” Conclusion: The findings bring to the fore specific gaps in current provision and interpreting services. Recommendations for proposed improvements in service provision and policy as well as clinical implications are included in this article.
