Summary Objectives To determine what paediatric specialist registrars think of the educational supervision they have received and what advice they would give to a consultant who wanted to be a more effective educational supervisor. Design A questionnaire study. Setting The North Thames Deanery, UK. Participants 129 year 3, 4 or 5 paediatric specialist registrars in the North Thames Deanery. Main outcome measures Reported value of educational supervision on a Likert scale; what elements of educational supervision were reported to be most useful; what elements of educational supervision were reported to be done poorly; what advice would specialist registrars give to a consultant who wanted to be a more effective educational supervisor. Results 86/129 specialist registrars responded (67%). The mean score on the Likert scale (0-a complete waste of time; 100-excellent) was 57 with 37% of respondents giving a score of less than 50. The most valued aspects of educational supervision were: feedback on performance-cited by 50 respondents (56% of respondents); career advice-cited by 43 (48%); objective setting-cited by 36 (40%); pastoral support-cited by 25 (28%). Aspects of educational supervision that were reported to be often not done well were: commitment to educational supervision- cited by 44 respondents (49% of respondents); ensuring sessions are bleep-free-cited by 43 (48%); listening rather than talking-cited by 23 (26%); being encouraging-cited by 18 (20%). Advice to consultants about how to improve educational supervision included: listen rather than talk; be encouraging; treat the trainee as an individual with individual needs. Conclusions We can find no other study of trainees’ views about how educational supervision can be improved. Although some trainees found educational supervision very valuable, many did not. Educational supervision should only be carried out by consultants who are committed to the task. An educational supervisor should listen carefully in order to understand the trainee's individual ambitions and needs, should provide specific feedback on performance and should be encouraging.

Abstract Background The purpose of this study was (1) to develop a virtual reality (VR) intervention program based on the psychological needs of patients residing in nursing facilities in South Korea to alleviate their behavioral and psychological symptoms and (2) to confirm the possibility of utilizing VR in patients with dementia. Methods In the first phase, patients with dementia residing in nursing homes and experiencing behavioral and psychological symptoms were recruited. Surveys and questionnaires were used to identify activities that alleviated the behavioral and psychological symptoms of dementia (BPSD) among the patients. These activities were classified into five types of psychological needs. In the second phase, a fully immersive, interactive, easy-to-use VR platform was developed that reflected these psychological needs. Patients with dementia experienced the VR content. The researchers assessed the level of the participants’ immersion, preference, and interaction with the VR using a 5-point Likert scale. Results In the feasibility test, 10 nursing home residents were recruited. The mean immersion score was 4.93 ± 0.16 points, the mean preference score was 4.35 ± 0.41 points, and the mean interaction score was 3.84 ± 0.43 points using a 5-point Likert scale. Higher mean scores indicated a more positive outcome. Six of the 10 participants required assistance while using the VR. The mean VR experience duration was 10.00 ± 3.46 min. Conclusions The VR-based intervention program that was developed to reduce BPSD was feasible for the participants and provided them with a high degree of satisfaction and immersion. Furthermore, this study also confirmed the convenience and safety of the program. These findings support the potential use of VR-based BPSD intervention programs to treat patients with dementia.

ObjectivesTo estimate the cost of human papillomavirus (HPV)-based screening through community health campaigns (CHCs) and home-based testing.SettingCHCs and home-based testing in six communities in rural Western Kenya.ParticipantsCHCs and home-based screening reached 2297 and 1002 women aged 25–65 years, respectively.Outcome measuresOutcome measures were overall cost per woman screened achieved through the CHCs and home-based testing and the cost per woman for each activity comprising the screening intervention.ResultsThe mean cost per woman screened through CHCs and home-based testing were similar, at $37.7 (range $26.4–$52.0) and $37.1 (range $27.6–$54.0), respectively. For CHCs, personnel represented 49% of overall cost, supplies 25%, services 5% and capital goods 23%. For home-based testing, these were: personnel 73%, supplies 25%, services 1% and capital goods 2%. A greater number of participants was associated with a lower cost per participant.ConclusionsThe mean cost per woman screened is comparable for CHC and home-based testing, with differences in type of input. The CHCs generally reached more eligible women in the six communities, whereas home-based strategies more efficiently reached populations with low screening rates.Trial registration numberNCT02124252.

AbstractFour years (September 2012 to August 2016) of simultaneous current observations across the Yucatan Channel (~21.5°N) and the Straits of Florida (~81°W) have permitted us to investigate the characteristics of the flow through the Gulf of Mexico. The average transport in both channels is 27.6 Sv (1 Sv = 106 m3 s−1), in accordance with previous estimates. At the Straits of Florida section, the transport related to the astronomical tide explains 55% of the observed variance with a mixed semidiurnal/diurnal character, while in the Yucatan Channel tides contribute 82% of the total variance and present a dominant diurnal character. At periods longer than a week the transports in the Yucatan and Florida sections have a correlation of 0.83 without any appreciable lag. The yearly running means of the transport time series in both channels are well correlated (0.98) and present a 3-Sv range variation in the 4 years analyzed. This long-term variability is well related to the convergence of the Sverdrup transport in the North Atlantic between 14.25° and 18.75°N. Using 2 years (July 2014–July 2016) of simultaneous currents observations in the Florida section, the Florida Cable section (~26.7°N), and a section across the Old Bahama Channel (~78.4°W), a mean northward transport of 28.4, 31.1, and 1.6 Sv, respectively, is obtained, implying that only 1.1 Sv is contributed by the Northwest Providence Channel to the mean transport observed at the Cable section during this 2-yr period.

Purpose This paper aims to analyze the situation of the financial system in the Spanish-governed cities of Melilla and Ceuta, Christian and Muslim cities located on the north coast of Africa, and compared it with the mean bankarization level in the rest of Spain in 2000-2015. Design/methodology/approach Although different calculation methods have been proposed, most authors agree that the bankarization level of a country or a territory reflects the development of the society as a whole and has a positive correlation with economic growth. The indicators of financial depth proposed by these researchers are not only the ratio between variables such as loans, deposits, etc., but also the ratios of these variables to the population and the gross domestic product (GDP) of the country or territory. Findings The results obtained revealed that there are differences between these two North African Spanish cities. Furthermore, the financing gap between the mean bankarization levels of these cities and those of mainland Spain was found to be even larger than most of the other economic indicators (GDP per capita and the unemployment rate). Practical implications The authors are convinced that the manuscript is a contribution of great interest for serving pilot experience in cities wishing to offer a development of traditional banking and Islamic banking. The paper should be of interest to readers in the areas of finance systems and commercial banks where two different cultures coexist. Originality/value This is the first research study on the financial framework of European cities whose populations have an approximately equal percentage of Christians and Muslims. The data reflected the existence of savings and loan methods parallel to conventional banking. The conclusion was that in the near future, it would be advisable for European banks to take into account the cultural customs and religious practices of potential Muslim clients.


In this paper the author investigates the periodical variations of the winds, rain and temperature, corresponding to the conditions of the moon’s declination, in a manner similar to that he has already followed in the case of the barometrical variations, on a period of years extending from 1815 to 1832 inclusive. In each case he gives tables of the average quantities for each week, at the middle of which the moon is in the equator, or else has either attained its maximum north or south declination. He thus finds that a north-east wind is most promoted by the constant solar influence which causes it, when the moon is about the equator, going from north to south; that a south-east wind, in like manner, prevails most when the moon is proceeding to acquire a southern declination ; that winds from the south and west blow more when the moon is in her mean degrees of declination, going either way, than with a full north or south declination ; and that a north-west wind, the common summer and fair weather wind of the climate, affects, in like manner, the mean declination, in either direction, in preference to the north or south, and most when the moon is coming north. He finds the average annual depth of rain, falling in the neighbourhood of London, is 25’17 inches.

AbstractThe number of predators inhabiting nests of Hyphantria cunea Drury was recorded annually for 13 years in four areas in New Brunswick and two areas on the coast of Nova Scotia. The most common groups were the pentatomids and spiders, which sometimes reproduced within the nests, but the mean number per nest was low in relation to the number of H. cunea larvae in the colonies. The rate of predation on fifth-instar larvae was low. Small or timid predators appeared to prey largely on moribund larvae or small saprophagans during the principal defoliating instars of H. cunea.No relationship could be detected between the number of larvae reaching the fifth instar and the number of predators in the colony; nor could any functional or numerical response of the predators to either the initial number of larvae per colony or the population density of colonies be found. It is concluded that the influence of the nest-inhabiting predators is small and relatively stable, and may be treated as a constant in the development of models to explain the population dynamics of H. cunea.H. cunea is a pest in parts of Europe and Asia, where it has been accidentally introduced from North America. The introduction to other continents of the North American predator, Podisus maculiventiis (Say), is discussed briefly.

Abstract Objective Orbital roof fractures are more likely to occur in younger children, specifically younger than 7 years. Cranium to face ratio decreases with age; however, there is no definition for measurement of the neurocranium or face. We propose using the length of the orbital roof as a measurement of the neurocranium and length of the orbital floor as a tool to estimate midface size. The purpose of this study is to test this measurement as a correlation rate of orbital roof fractures within the pediatric population. Design This is a retrospective study. Setting This study was done at the LeBonheur Children's Hospital. Participants Sixty-six patients with orbital roof fractures were identified and stratified by gender and age, specifically younger than 7 years and 7 years or older. Main Outcome Measures The main outcome measures were orbital roof length, floor length, and ratio thereof. Results Mean orbital roof length was 43.4 ± 3.06 and 45.1 ± 3.94 mm for patients <7 and ≥7 years, respectively (p = 0.02). Mean orbital floor length was 41.3 ± 2.99 and 47.7 ± 4.19 for patients <7 and ≥7 years, respectively (p < 0.00001). The mean roof to floor ratio (RTFR) for patients <7 years was 1.051 ± 0.039 and for patients ≥ 7 years was 0.947 ± 0.031 (p < 0.00001). Conclusion As children age, the relative length of the orbital roof decreases when compared with the orbital floor. The RTFR was more than 1.0 in children younger than 7 years. These differences were statistically significant when compared with children 7 years and older. This measurement shift follows the differences noted in orbital fracture patterns during childhood.

Abstract. Although the temporal changes of aerosol properties have been widely investigated, the majority focused on the averaged condition without much emphasis on the extremes. However, the latter can be more important in terms of human health and climate change. This study uses a previously validated, quality-controlled visibility dataset to investigate the long-term trends of extreme surface aerosol extinction coefficient (AEC) over China, and compare them with the median trends. Two methods are used to independently evaluate the trends, which arrive at consistent results. The sign of extreme and median trends are generally coherent, whereas their magnitudes show distinct spatial and temporal differences. In the 1980s, an overall positive trend is found throughout China with the extreme trend exceeding the mean trend, except for Northwest China and the North China Plain. In the 1990s, AEC over Northeast and Northwest China starts to decline while the rest of the country still exhibits an increase. The extreme trends continue to dominate in the south while it yields to the mean trend in the north. After year 2000, the extreme trend becomes weaker than the mean trend overall in terms of both the magnitude and significance level. The annual trend can be primarily attributed to winter and fall trends. The results suggest that the decadal changes of pollution in China may be governed by different mechanisms. Synoptic conditions that often result in extreme air quality changes might dominate in the 1980s, whereas emission increase might be the main factor for the 2000s.

Objectives: The Priorities and Evaluation Committee (PEC) funding recommendations for new cancer drugs in British Columbia, Canada have been based on both clinical and economic evidence. The British Columbia Ministry of Health makes funding decisions. We assessed the association between cost-effectiveness of cancer drugs considered from 1998 to 2008 and the subsequent funding decisions.Methods: All proposals submitted to the PEC between 1998 and 2008 were reviewed, and the association between cost-effectiveness and funding decisions was examined by (i) using logistic regression to test the hypothesis that interventions with higher incremental cost-effectiveness ratios (ICERs) have a lower probability of receiving a positive funding decision and (ii) using parametric and nonparametric tests to determine if a statistically significant difference exists between the mean cost-effectiveness of funded versus not funded proposals. A sub-analysis was conducted to determine if the findings varied across different outcome measures.Results: Of the 149 proposals reviewed, 78 reported cost-effectiveness using various outcome measures. In the proposals that used life-years gained as the outcome (n = 22), a statistically significant difference of nearly $115,000 was observed between the mean ICERs for funded proposals ($42,006) and for unfunded proposals ($156,967). An odds ratio indicating higher ICERs have a lower probability of being funded was also found to be statistically significant (p < .05).Conclusions: Economic evidence appears to play a role in British Columbia cancer funding decisions from 1998 to 2008; other decision-making criteria may also have an important role in recommendations and subsequent funding decisions.

This study aims to test the influence of the senior management’s support as a moderating variable on the relationship between the independent factors (Training, Empowerment, Motivation and Communication) and the dependent variable (Performance of Employees). (SEM-AMOS) is used to test the impact of the moderating variable. Where it is depended on the method of sampling or analysis of what is known as multiple-groups analysis. The paragraphs of the senior management’s support variable are collected and divided into two groups according to the mean of the total paragraphs. In addition, according to the relative weights given to the paragraphs of the questionnaire, using a five- point’s Likert scale: 1= strongly disagree to 5 = strongly agree. The first group consisted of the grades less than the mean and it is considered as the group which is non-supporters of the existence of support. While the second group consisted of the grades higher than the mean and considered as the group which is a supporter of the existence of support. The study found that the model of study in the presence of the support of the senior management’s is appropriate for the second group and inappropriate in light of the lack of support by the senior management’s support for the first group.
