The Combined Laser Therapy in Patients with Psoriasis

One of the known methods of treatment of patients with psoriasis is laser therapy, however, the task of improving the efficiency of this method is still relevant. The objective of this study was the evaluation of the effectiveness of treatment technique of patients with psoriasis, including the intravenous laser light of blood and the local effect of pulse infrared laser radiation. 42 patients with widespread forms of psoriasis in the stage of progression (12 women and 30 men) aged 35 to 60 years (the average age 47), having a disease duration of 5 to 20 years, were observed. In the complex of therapeutic measures of the main group the laser therapy was included. It is the original technique by means of laser therapeutic apparatus ʺLasmik‐VLOKʺ (Registration certificate № RZN 2014/1410 from 06.02.2014) with the laser emitting heads KL‐VLOK‐525‐20 for intravenous laser light of blood and ML‐635‐ 40 for external impact. For this technique the disposable sterile lights KIVL ‐01 on TU 9444‐005‐72085060‐ 2008 made in Research centre ʺMatrixʺ (Russia, Moscow) were used.It is shown that the combined laser therapy of patients with moderate form of psoriasis (PASI index 40‐ 50), including local effects on psoriatic lesions by pulsed radiation of the red spectrum (635 nm) of 40 watts with the change of the pulse repetition frequency to 10000 Hz and VLOK‐525, allows effectively to stop the inflammatory changes, to decrease of erythema, infiltration and desquamation. The rapid decrease in the area affected of the skin is observed.

One of the known methods of treatment of patients with atopic dermatitis is laser therapy, however, the task of improving the efficiency of this method is still relevant. The objective of this study was the evaluation of the effectiveness of treatment technique of patients with atopic dermatitis, including alternation in a day the intravenous laser light of blood by low-intensity laser radiation with a wave length of 365 nm and 525 nm (green spectrum). 37 patients with atopic dermatitis (10 women and 27 men) aged 18 to 56 years (the average age 36,2), having a disease duration of 17 to 54 years, were observed. In the complex of therapeutic measures of the main group the laser therapy was included. It is the original technique by means of laser therapeutic apparatus "Lasmik- VLOK" (Registration certificate № RZN 2014/1410 from 06.02.2014) with the laser emitting heads KLVLOK- 365-2 (for LUFOK®) and KL-VLOK-525-2 for intravenous laser light of blood. For this technique the disposable sterile lights KIVL -01 on TU 9444-005-72085060-2008 made in Research centre "Matrix" (Russia, Moscow) were used. It is shown that the combined intravenous laser light of blood with a wavelength of 365 nm (VLOK-365 or LUFOK®) and radiation with a wavelength of 525 nm (green spectrum, VLOK-525) in a day, 10 sessions in combined treatment of patients with atopic dermatitis – allows to achieve complete regression of all acute inflammatory symptoms of disease - erythema, papules, desquamation, excoriations in 87,5% of patients with moderate-severe disease (the average SCORAD index – 57,5±4,0) and to lower at 3,4 times the average SCORAD index (to 21,3±4,0) in patients with severe disease (initial value – 72,8±3,0) with an overall positive dynamics.

Currently, physical treatment methods, in particular use of photochromotherapy in the treatment of sinusitis, are of interest. Is to investigate the local effect of light in the visible range (450 nm) on the factors of local infection protection of the nasal mucosa. The study involved 50 patients with a diagnosis of acute rhinosinusitis at the age of 27.23±3.29 years. Exposure of the mucous membrane of the nasal cavity was carried out in accordance with the parameters: 450 nm wavelength (monochromatic non-polarized blue light), during 5 minutes for each nostril. Sessions of photochromotherapy were carried out according to the "Sanitary norms and rules for the design and operation of lasers" No. 5804-91. Inclusion of photochromotherapy in the complex of therapeutic measures for patients with acute rhinosinusitis helps to reduce the total number of neutrophils in nasal secretion, normalize their lysosomal activity, NBT-reducing activity, activity and intensity of phagocytosis. Restoration of local immunity factors in nasal mucosa, expressed in normalizing the functional and metabolic status of neutrophil granulocytes of the nasal secretion, can serve as one of the criteria for the effectiveness of treatment using photochromotherapy

In recent decades, great strides have been made in perinatal neurology in the development of criteria for early diagnosis and treatment of diseases. Further study of the biochemical foundations of perinatal lesions of the nervous system with the definition of objective markers of the severity of the lesion will reveal new links in its pathogenesis and develop more effective methods of treatment.

The way we treat our wastewater nowadays is not always favourable for improving the quality of our environment. Indeed, the quality of the surface waters has increased by the high investments we have made in our treatment facilities, but it cannot be denied that these same facilities have a major impact on other environmental issues. From ibis point of view the question is raised as to which treatment techniques are favourable not only for the water phase, but also from the viewpoint of energy, waste production and renewal of resources. Over thirty techniques, all of them chemical/physical treatment techniques, have been reviewed, especially for the treatment of industrial wastewaters. More importantly, these techniques have been analysed thoroughly in a multi-criteria-analysis, not only for their environmental impacts, but also on criteria influencing the implementation of these techniques. From ibis multi-criteria-analysis the most favourable treatment technique to be developed and implemented may be chosen.

The article describes the experience of application of minimally invasive methods of treatment chronic hemorrhoids stages II-III. The analysis of the effectiveness of methods and stand-alone when combining methods on the basis of the treatment and dynamic follow-up of 493 patients. The analysis of the indications for use, clinical effectiveness and disadvantages of minimally invasive techniques. Substantiated the clinical efficacy of venotonics (micronized flavonoid fraction) in the pre - and postoperative treatment in the application of minimally invasive methods of treatment chronic hemorrhoids. Identified the need for a differentiated approach to the definition of indications and choice of a method of minimally invasive surgery depending on the clinical picture of the disease (type, stage, presence of inflammatory changes, bleeding).

Comprehensive diagnostics, accepted classifications, and selection of pathogenetic methods of treatment (including surgery) for axis injuries, odontoid process fractures, axis traumatic spondylolisthesis and hangman’s fractures are discussed in the paper. The presented data prove the necessity and possibility of early exhaustive diagnosing to differentiate the type of a craniocervical disorder and to provide a timely choice of pathogenetic treatment technique, especially of a surgical one.

In the Ayurvedic classics, Kasa is considered as an independent disease. It may also occur as Lakshana (symptom) or Upadarava (complication) in other diseases. It is an example of Pranavaha Srotas Dushti. According to Acharya Charaka, the Moola of Pranavaha Srotas are Hridya (Brain, Heart) and Mahasro-tas. Acharyas have described definition, etiological factors, prodromal symptoms, symptoms, types of Kasa along with all Pathya (wholesome) & Apathya (unwholesome) and therapeutic measures. Kasa is manifested with the vitiation of Vata & Kapha. Understanding and differentiating Kasa is important for its effective treatment. The study of Kasa is necessary as it can be associated with many complications if left untreated. In this context different Lakshanas (symptoms) of Doshaja Kasa explained by Acharyas can be used for diagnosis as well as prognosis of the disease. Thus, with help of this study alternate safe methods of treatment can be employed. So, its identification as Doshaja Kasa can be a ray of hope for the diagno-sis.

The aim: Is to determine the tactics and methods of treatment of bezoars of the gastrointestinal tract. Materials and methods: From 2001 to 2019, 17 patients were diagnosed with “bezoar”. Results: Due to the “weariness” of the clinic, the diagnosis was made in the first 3 days only for 3 (17,6 %) patients. On the basis of the obtained average pH values for 4 (23,5 %) patients established moderate hypoacid, for 4 (23,5 %) - pronounced hypoacid, for 6 (35,3 %) - anacid. For 5 (29,4 %) patients, the bezoars were withdrawn on the first attempt, while the other 5 (29,4 %) were “lumped”. In the course of fibrogastroscopic examination, all patients were diagnosed with impaired motor-evacuation function of the stomach: gastroesophageal and duodenogastric refluxes, presence of passive discharge of the contents of the stomach into the esophagus. Conclusions: Therefore, the preconditions for the development of bezoars may be: hypo- and anacid, impaired motor-evacuation function of the stomach, chronic gastric ulcer,cognitive impairment. Endoscopic method should be preferred in the treatment, on condition of its failure - laparoscopic gastrotomy with bezoar extraction.

Of the latter, laser therapy is of particular interest, in particular, the use of low-intensity laser radiation is promising, which is quickly gaining a place among the known non-drug methods of treatment in therapeutic practice.

A histopathological and immunohistochemical study of the brain and heart was made in 50 patients with the chronic cardiac form of Chagas' disease. The immunohistochemical technique used was the peroxidase-antiperoxidase method adapted for the demonstration of the T. cruzi amastigotes. Histological and immunohistochemical examination of the brain showed encephalitis in multiple foci, although sparse, in four patients (8%). In one of the patients the process was recent, active, and containing parasites. In the other three patients, the lesions were of minor intensity, with light exudative inflammatory changes, suggesting a process becoming inactive, or already inactive. The search for parasites in these three patients was negative, even with use of immunoperoxidase. The heart histological and immunohistochemical study showed, besides the chronic myocarditis in multiple foci associated with interstitial fibrosis, amastigotes in seven patients (14%). The absence of parasites and of inflammatory changes in the brain in the great majority of patients with chronic Chagas' disease, contrasting with the constant finding of inflammatory changes and the occasional finding of amastigotes in the myocardium of the same patients, allows us to state, in the same way other authors did, that there is no histopathological basis to support the existence of the chronic nervous form of Chagas' disease.
