Investigating platelet-activating factor as a potent proinflammatory mediator in coronary atherosclerotic patients

The inflammatory reaction is one of the complications in patients with coronary atherosclerosis. This study aimed to determine the diagnostic value of platelet-activating factor (PAF) compared with high sensitivity C reactive protein (hs-CRP) in coronary atherosclerotic patients. Fifty patients with coronary atherosclerosis and 30 subjects with normal angiography were considered as the control group attending Cardiac Center-Surgical Specialty Hospital - in Erbil city / Iraq. The levels of PAF and hs-CRP were estimated quantitatively using a sandwich enzyme-linked immunosorbent assay and a particle-enhanced immune turbid metric assay, respectively. Lipid profiles and some hematological indexes were also used in this study. The levels of the inflammatory biomarkers of PAF and hs-CRP increased significantly in the patients group compared with controls (p<0.05). Although the patients group showed the highest level of low-density lipoprotein (LDL), the difference was not significant (p>0.05) compared with the healthy control. However, the incidence of risk factors such as smoking and obesity showed a significant difference (p<0.05) in the patients group. Additionally, the PAF level correlated positively and significantly with hs-CRP (p<0.05), and negatively with high-density lipoprotein (HDL) (p>0.05). Although hs-CRP was a valuable diagnostic marker for coronary atherosclerosis, the PAF level showed to be a better prognostic indicator than hs-CRP in coronary atherosclerosis patients.

Introduction: Gamma-glutamyltransferase(GGT) plays a catalytic role in degradation of glutathione. Serum GGT is accepted as a marker of oxidative stress.The aim of this study is to investigate the relationship between serum GGT levels and epicardial adipose tissue (EFT) thickness, carotid intima media thickness (CIMT) measurements in patients with psoriasis. Methods: The study population included 89 patients with psoriasis and 79 healthy volunteers. After overnight fasting, blood samples were taken for to determine blood glucose levels and establishing cholesterol profiles including TG, TC, LDL cholesterol and high-density lipoprotein (HDL) cholesterol; GGT; and high- sensitivity C-reaktive protein (hs-CRP) levels. A high-resolution B-mode ultrasound machine (Toshiba, aplio XU) with a 7.5 MHz linear transducer used for examing CIMT.The right common carotid artery (CCA), approximately 1 cm proximal to the bifurcation, was longitudinally selected and CIMT was defined as the distance between the intima and the media. Results: 89 patients with psoriasis (age:41.7±10.9 years;41 women, 48 men), and 71 healthy control subjects (age:40.4±8.2 years;39 women, 32 men) were included. There were no significant variation for age and sex between two groups(p>0.05).The hs-CRP and GGT values were significantly higher in psoriasis, compared with the controls (hs-CRP:1.35(0.9-3.6)mg/l for psoriasis group, 0.45(0.29-0.79)mg/l for control group, p<0.001; GGT:20.6±9.6 U/l for psoriasis group, 16.7±8.0 U/l for control group, p=0.02. In psoriatic patients, CIMT and EFT were significantly inreased (0.60(0.50-0.68)mm vs. 0.50 (0.40-0.60)mm;p=0.007, 0.67±0.20cm; 0.27±0.12cm; p<0.001, respectively) compared with the control group. CIMT significantly positively correlated with EFT, age, BMI, diastolic BP and GGT.EFT significantly positively correlated with GGT, CIMT, age, hs-CRP, systolic BP and TG and negatively correlated with HDL cholesterol. Discussion: The pathophysiology of atherosclerosis in psoriasis is not fully explained.GGT may be used as an indicator of subclinical atherosclerosis like CRP.

The purpose of this study was to investigate the function of the renin–angiotensin–aldosterone system (RAAS) in normal tension glaucoma (NTG) patients by measuring the level of renin and angiotensin II (AngII) in the plasma. Twenty-four patients with NTG and 38 control subjects were included in this study. Renin and AngII were measured in the blood samples of all subjects by enzyme-linked immunosorbent assay (ELISA). No significant differences were found in the complete blood count, fasting glucose, low-density lipoprotein (LDL), and high-sensitivity C-reactive protein (hs-CRP) levels between the control and NTG groups. The systemic concentration and variability of the renin concentration in the blood was significantly higher in the NTG group (p = 0.005 and 0.005, respectively). According to multivariate logistic regression analysis, the variability of the renin concentration was associated with NTG (p = 0.006). In conclusion, the systemic concentration and variability of renin levels were elevated in NTG patients. An altered renin concentration could represent a difference in RAAS function in NTG patients.

Objective: The purpose of this study was to examine the effects of a 6-month therapeutic lifestyle modification (TLM) program on chemokines related to oxidative stress, inflammation, endothelial dysfunction, and arterial stiffness in subjects with metabolic syndrome (MetS). Methods: The authors performed a randomized controlled trial, assigning 52 women (mean age 62.7 ± 9.0 years) with MetS to a TLM intervention group ( n = 31) or a control group ( n = 21). The authors provided the TLM intervention group with health screening, exercise, low-calorie diet, and health education and counseling for 6 months and instructed the control group to maintain their usual lifestyle behaviors. Outcome variables included levels of myeloperoxidase (MPO), oxidized low-density lipoprotein (LDL), adiponectin, leptin, resistin, high-sensitivity C-reactive protein (hs-CRP), interleukin-1β, interleukin-6, tumor necrosis factor-alpha (TNF-α), CD40L, monocyte chemotactic protein-1 (MCP-1), retinol-binding protein 4 (RBP-4), endothelin-1, and brachial-ankle pulse wave velocity. The authors used generalized estimating equation (GEE) analyses to estimate the effects of the TLM program. Results: After the 6-month TLM program, hs-CRP levels decreased significantly, and MCP-1 levels increased at a significantly slower rate in the TLM group than they did in the control group (all p < .05). Conclusion: These results indicate that a TLM program could be effective for improving patient inflammatory states and may also be effective in preventing cardiovascular complications in subjects with MetS.

Introduction: Injection of intraperitoneal gentamicin at a dose of 60 mg/kg BW intra-peritoneal in rats for 7 days has been shown to trigger renal tubular degeneration and increase in high sensitivity C-reactive protein (hs-CRP) level. The use of purslane (Portulaca oleracea) to reduce hs-CRP levels and total score of renal tubular degeneration has not been reported.Objective: to determine the effect of purslane ethanol extract on hs-CRP levels and renal tubular degeneration score in gentamicin induced-renal damage in rats.Methods: This was a study using posttest only control group design. Twentyfive male Wistar rats aged 8-12 weeks, weighed 150-200 grams were allocated into 5 groups: normal (Nor-G), without treatment; negative (Neg-G), gentamicin (60 mg/kg BW), intraperitoneally; P-200; P-300 ; P-400 (gentamicin plus purslane extract at a dose of 200, 300, and 400 respectively) for 7 days. The hs-CRP was evaluated using the ELISA method. Total score of renal tubular degeneration was evaluated using modified score of Sarjadi.Results: there was a significant difference in mean hsCRP level and total score of renal tubular degeneration between groups (p <0.05). Posthoc analysis showed that hsCRP level and total score group of P-200, P-300 and P-400 were significantly lower than those of Neg-G, p <0.05. Meanwhile, the levels of hsCRP and total score of renal tubular degeneration in P-200, P-300, P-400, and Nor-G groups were not significantly different (p> 0.05).Conclusion: the administration of purslane ethanol extract at doses of 200, 300, and 400 mg/kg BW for 7 days improve hs-CRP level and total score of tubular degeneration similar to normal.

Although advanced diagnostic and treatment methods are available, congenital heart disease (CHD) holds an important place among the causes of death within the first year of age. Therefore, several prognostic factors are needed for diagnosis and monitoring of these patients. In this study, which includes 66 CHD patients and 38 healthy control children, serum cardiac troponin-I (cTnI), high-sensitivity C-reactive protein (Hs-CRP), and N-terminal prohormone brain-type natriuretic peptide (NT-proBNP) levels were analyzed for their prognostics values. The patient groups were categorized and then evaluated as cyanotic ( n = 16), acyanotic ( n = 50), symptomatic ( n = 23), asymptomatic ( n = 43), and isolated ventricular septal defect (VSD)-isolated atrial septal defect (ASD) groups. Cyanotic group was statistically compared with acyanotic group, symptomatic group with asymptomatic group, and VSD group with ASD group. Between the cyanotic, acyanotic, and control groups; between symptomatic and asymptomatic groups; and between the VSD and ASD groups, significant difference was not showed for age ( p > 0.05). NT-proBNP was found to be significantly higher in the cyanotic group than acyanotic and control group, in the symptomatic group than asymptomatic group; and in the patient group than healthy control group ( p < 0.05). Between the groups of VSD and ASD, significant difference was not showed ( p > 0.05). The same comparison results for TnI and Hs-CRP were not significant ( p > 0.05). TnI and Hs-CRP were only found significantly higher in the patient group than healthy control group ( p < 0.05). Eventually, we think that NT-proBNP, Hs-CRP, and TnI might be used for clinical management and estimation of outcome of these disorders in the future and these also might be able to modify existing strategies, but much more studies are needed.

Objective. The present study was planned to examine the relationships between obstructive sleep apnea syndrome (OSAS) and the newly revealed adipokines adropin and adiponectin concentrations that display significant metabolic and cardiovascular functions and the levels of proinflammatory cytokine levels. Method. A total of 166 overweight and obese male patients with a body mass index (BMI) >27 kg/m2 were included in the study. Among study participants, 84 were recently diagnosed with OSAS by polysomnography with an apnea-hypopnea index (AHI) ≥5, and 82 were nonapneic with normal polysomnography (AHI<5) findings. The serum adropin and adiponectin levels of all cases were analyzed via the enzyme-linked immunosorbent assay method. Serum interleukin-1 (IL-1) beta and tumor necrotizing factor-alpha (TNF-alpha) levels were determined using Luminex cytokine multiplex analyses. Results. The mean age of the OSAS patients was 50.9 ± 5.7 years and BMI was 32.4 ± 6.0 kg/m2, and there was no statistically significant difference determined with the control group (49.3 ± 5.8 years and 30.6 ± 5, 6 kg/m2) (p>0.05). There were no statistically significant differences between the OSAS and control groups concerning total cholesterol, triglyceride, low-density lipoprotein (LDL), high-density lipoprotein (HDL), and glucose levels. Adiponectin was lower in the OSAS group at a statistically significant level in comparison with the control group and was related at a statistically significant level to OSAS intensity. Adropin concentration was determined to be higher in the OSAS group at a statistically significant level in comparison with the control group. Conclusion. The results of our study suggest that increased adropin concentration may be an indicator of endothelium dysfunction in OSAS patients. Serum adropin and adiponectin levels may be new bioindicators used for diagnosis and risk assessment in OSAS patients.

It is postulated that high-sensitivity C-reactive protein (hs-CRP) and tumor necrosis factor α (TNF-α) are diagnostic utilities for pleural effusion. This study was designed to explore the detection and significance of TNF-α and hs-CRP in the pleural effusion of patients with diabetes and pulmonary tuberculosis. A total of 60 patients with diabetes and pulmonary tuberculosis pleural effusion were selected as the study group, while 60 patients with pulmonary tuberculosis pleural effusion were considered as the control group. The expression of TNF-α and hs-CRP in the two groups was determined from pleural effusion by enzyme-linked immunosorbent assay (ELISA). The expression levels of TNF-α and hs-CRP in pleural effusion of the study group were significantly ( P < 0.05) higher than the control group, and the sensitivity and specificity of the combined detection were significantly ( P < 0.05) higher than those of the separate detection. The expression of TNF-α and hs-CRP in the pleural effusion of patients with diabetes and pulmonary tuberculosis increased remarkably, which plays an important role in the diagnosis and treatment helping with differential diagnosis and evaluation of severity and prognosis by related detection of changes of these indexes, especially the combined detections.

AbstractThe high prevalence of both Helicobacter.pylori infection and coronary atherosclerosis in our country prompted us to assess the probable association between both conditions. This cross-sectional study recruited 153 patients scheduled to undergo coronary artery angiography. Patients were divided into two groups on the basis of coronary angiography results. Sixty-nine patients had coronary atherosclerosis and the 84 remaining patients were normal. Characteristics and pre-angiographic serum levels of triglyceride, low-density lipoprotein cholesterol, high-density lipoprotein, and Helicobacter.pylori IgG antibody were assessed in the patients and compared between the groups. Helicobacter.pylori infection occurred in 88 (57.51%) patients: 40 (58%) in the atherosclerotic group and 48 (57.1%) in the control group with no significant differences (P=0.918). Our multivariable analysis revealed that Helicobacter.pylori infection was not an independent predictive factor for coronary artery disease (P =0.915). Also, the prevalence of atherosclerosis risk factors with respect to the seropositive and seronegative Helicobacter.pylori infection was assessed in the case group, which showed no significant difference. Furthermore, the prevalence of seropositive Helicobacter.pylori infection in terms of the number of diseased coronary vessels was evaluated, this demonstrated no significant association between the number of the diseased vessels and Helicobacter.pylori infection. This study demonstrated that Helicobacter.pylori infection was not an independent predictive factor of atherosclerosis.

Latar Belakang: Komposisi makanan tinggi lemak dapat menjadi faktor terjadinya obesitasyang menyebabkan oksidasi lemak. Oksidasi lemak dapat menyebabkan inflamasi yang dikarakterisasikan dengan tingginya kadar High Sensitivity C-Reactive Protein (hs-CRP). Ubi ungu kaya akan antioksidan terutama antosianin yang mungkin dapat menurunkan kadar hs-CRP. Penelitian ini bertujuan untuk mengetahui pengaruh sari ubi ungu terhadap kadar hs-CRP tikus sprague dawley dengan pakan tinggi lemak.Metode: Jenis penelitian ini adalah true experimental dengan  pre-post test control group design. 24 tikus sprague dawley jantan dibagi menjadi 4 kelompok yaitu kontrol negatif, kontrol positif, perlakuan 1 dan perlakuan 2. Perlakuan 1 dan 2 diberikan sari ubi ungu dengan dosis 2 gram/200grBB dan 3 gram/200grBB berturut-turut selama 6 hari. Sebelum dan sesudah perlakuan, kadar hs-CRP dianalisis dengan menggunakan metode ELISA (Enzyme Linked Immunosorbent Assay).Hasil: Selisih kadar  hs-CRP pada keolompok kontrol negatif, kontrol positif, perlakuan 1 dan perlakuan 2 adalah 0.4±0.20, -0.07±4.70, -4.3±0.79 dan -8.1±0.45. Terdapat perbedaan signifikan kadar  hs-CRP antar kelompok sesudah intervensi (p=<0.001). Pada kelompok perlakuan 2 terdapat penurunan yang paling tinggi (persen delta 33,33% dengan nilai p=<0.001).Simpulan: Sari ubi ungu dapat menurunkan kadar  High Sensitivity C-Reactive Protein (hs-CRP) secara signifikan.

The aim of this study is to examine uric asid (UA) and high sensitive C-Reactive protein (Hs-CRP) levels in patients with coronary artery ectasia (CAE). Ninety-eight patients with isolated CAE (mean age 57.5±10.3), (group-I), 110 patients with CAD but without CAE (mean age 56.3±10.7), (group-II), and 105 patients with normal coronary angiographies (mean age 58.1±10.8), (group-III) were included in the study. Blood samples of all individual were taken after coronary angiography from an antecubital vein, the patients uric acid and Hs-CRP levels were assessed. The severity of ectasia was evaluated and categorized according to Markis. A significant difference was not seen in serum uric acid and Hs-CRP levels between CAE and CAD groups. However, relative to the control group, uric acid and Hs-CRP levels in CAE and CAD groups were higher to a significant degree (p=0.001, p
