Summary Acne vulgaris is among the commonest inflammatory skin diseases affecting pilosebaceous units. It occurs mainly in puberty and affects adolescents at the age of 14-19 years both females and males, in relation to sebum production of hair follicles under the action of sex hormones. Clinically the disease is presented with comedones, papules, pustules, nodules and scars in some cases. Seborrheic areas face, chest and back are affected. Recent epidemiologic studies have shown significant number of female patients aged over 25 years with acne and the term Adult onset acne (AOA)was established.The latter is manifested clinically in the lower third of the face. Primary etiological cause of AOAisahormonal imbalance, mainly hyperandrogenemia. Acase ofa25-year-old woman with adult acne and elevated levels of testosterone and prolactine is presented in this paper.

Acne vulgaris is a chronic skin disease of the pilo-sebaceous unit and develops due to blockages in the skin's hair follicles. It is characterized by blackheads or whiteheads, pimples, oily skin, and possible scarring It primarily affects areas of the skin with a relatively high number of oil glands, including the face, upper part of the chest, and back. These blockages are thought to occur as a result of the following four abnormal processes: a higher than normal amount of oily sebum production (influenced by androgens), excessive deposition of keratin protein leading to comedo formation, colonization of the follicle by Cutibacterium acnes (C. acnes) bacteria, and the local release of pro-inflammatory chemicals in the skin. The resulting appearance can lead to anxiety, reduced self-esteem and, in extreme cases, depression or suicidal thoughts. In  Ayurveda, it is termed as Mukhadushika. In Ayurveda, Mukhadushika is described under the heading of Kshudra rogas. These diseases are called as Kshudra because of their lesser severity. According to Sushruta Samhita Nidan Sthana, these are Shalmali thorn like eruption found on the face of adolescents are called as Mukhadushika or Yuvan Pidika. These are impregnated with Meda. Vitiated Vata Dosha, Kapha Dosha and Rakta Dosha are considered to be the main Samprapti Ghatak of Mukhadushika. In Ayurveda treatment of diseases is mainly of two types of Chikitsa are described i.e. Shodhana Chikitsa and Shamana Chikitsa. Similarly in Mukhadushika also Shodhana and Shamana Chikitsa is done.

ABSTRACT The trichoepithelioma is a benign cutaneous neoplasm which is derived from hair follicles. It is common in the face, but there are only three reports of the solitary occurrence on the nose. It is often not recognized because of its rarity, controversial classification, origin and biological potential. The objective of this paper is to present a case of solitary trichoepithelioma on the nose, histopathological examination and treatment. It should be considered as a differential diagnosis of a solitary lesion of nose which is confused with basal cell carcinoma. The confirmation by histopathological examination is essential. Alopecia areata (AA) is an autoimmune skin disease caused by chronic inflammation of hair follicles. Chronic inflammatory skin diseases such as psoriasis and lupus erythematosus can increase the risk of cardiovascular diseases. However, the relationship between AA and heart diseases (HDs) remains unclear. Therefore, we conducted this retrospective cohort study to evaluate the risk of subsequent HDs in patients with AA. We reviewed 3770 cases of AA and from 18,850 age, sex, and income level-matched controls from the National Health Insurance Service-National Sample Cohort. In the subgroup analysis, patients who suffered from alopecia totalis, alopecia universalis, and ophiasis were designated as patients with severe AA and patients having the disease for over a year were designated as patients with long-standing AA. As a result, we found that AA was not associated with a higher risk of heart failure, angina pectoris, or myocardial infarction. There was no significant increase in the risk of overall HD associated with AA (adjusted hazard ratio: 1.17; 95% confidence interval: 0.93–1.48; p = 0.177). Neither the severity nor the duration of AA was related to an increased risk of HDs. During the study period, AA patients did not show a significantly higher cumulative incidence of HDs than controls (log-rank p = 0.157). In conclusion, AA does not increase the risk of HD.

Acne vulgaris is one of the most common chronic inflammatory skin diseases. Bergamot and sweet orange are rich in nutritional and functional components, which exhibit antioxidant, anti-inflammatory, and antiapoptotic effect. The aim of this study was to evaluate the potential effect of bergamot and sweet orange (juice and essential oil) on acne vulgaris caused by excessive secretion of androgen. Eighty male golden hamsters were randomly divided into 10 groups and received low or high dose of bergamot and sweet orange juice and essential oil, physiological saline, and positive drugs for four weeks, respectively. Results showed that all interventions could improve acne vulgaris by reducing the growth rate of sebaceous gland spots, inhibiting TG accumulation, decreasing the release of inflammatory cytokines (notably reducing IL-1α levels), promoting apoptosis in the sebaceous gland, and decreasing the ratio of T/E2. Among them, bergamot and orange essential oil may have better effects (dose dependent) on alleviating acne vulgaris than the corresponding juice. In view of the large population of acne patients and the widespread use of sweet orange and bergamot, this study is likely to exert an extensive and far-reaching influence.

Introduction: Demodex mites are associated with various symptoms pertaining to facial and eyelid skin in humans. Demodicosis is often reported in elderly people, above 50 years of age. In young adults, the Demodex spp. infection is less common and is asymptomatic or the symptoms are mild. Aim: The aim was to evaluate the occurrence of Demodex spp. in healthy young adults in Poland and analyze the associations between the presence of mites, participants gender and skin type, with regards to hygienic practices and symptoms. Material and methods: The content of facial sebaceous glands and eyelash follicles was examined in a group of 94 people, aged 18–32 years. Metric data and information about face and eyelid skin symptom occurrence, skin type and hygienic practices were collected by diagnostic survey. Results and discussion: Demodex spp. was identified in 21.3% of subjects. In 45% and 30% of carriers Demodex folliculorum and Demodex brevis occurred, respectively. A mixed infestation in 25% of subjects was detected. Demodex infestation was slightly higher in people with oily and mixed skin (23.5%), compared to those with dry and normal skin (18.6%). No significant influence of shared hygienic accessories on the level of Demodex spp. infection were observed. The skin symptoms of the face and eyelids were reported at 75% for subjects positive for Demodex. Conclusions: Demodex mites are often prevalent in healthy young adults in Poland in both the sebaceous glands of the face and hair follicles. While diagnosing face and eyelid skin diseases in young people, Demodex spp. infection should be considered.

Background: There are various therapeutic modalities for acne and post acne scarring, but the best option is to have a safe, effective with affordable cost such as monotherapy or combination therapy. One of the recommended combination therapy for acne vulgaris with atrophic acne scars is microneedling radiofrequency which can improve skin structure. Case report: A 25 year old male with moderate comedonal acne vulgaris and severe scarring. The patient came with complaints of uneven skin surface on both cheeks, forehead and chin which had been felt to increase since 3 years ago. The appearance of acne on the face has been felt since 12 years ago. The patient's parents and sister also had acne complaints. On dermatological examination, on the forehead, temples, cheeks, nose and chin found blackheads, whiteheads, papules, hyperpigmented macules, multiple atrophic acne scars. The treatment choice for this patient is microneedling radiofrequency in combination with topical therapy. Discussion: The combination of microneedling with radiofrequency is one of the therapeutic modalities for acne vulgaris and also post-acne scarring with minimal side effects. In the evaluation for 2 weeks after the procedure, this patient did not complaint of any adverse events.

ABSTRACT: RELATIONSHIP OF STRESS AND FACE CLEANLINESS TO THE EVENT OF VULGARIS IN MEDICAL STUDENTS OF UNIVERSITAS MALAHAYATI Background: Acne vulgaris is a disorder of the sebaceous follicles specifically associated with hair follicles and sebaceous glands are the most common found on the face, chest, and back. Acne vulgaris associated with facial cleanliness and psychological stress. Objective: This research is aiming to know  reletionship of stress and hygiene face of acne vulgaris in malahayati university medical faculty student.Methods: This study using cross sectional method. Subjects consisted of 324 respondents using questionnaires and physical examination.Result: Statistical test results are obtained p-value=0,000 which means p<α (Ho rejected and Ha accepted) so it can be concluded that there is a stress relationship to the onse of acne vulgaris. An OR score of 26,414, cause stress with respondents had a 26,414 times greater chance of akne vulgaris than those who did not experience stress. Akne vulgaris berhubungan dengan kebersihan wajah dan stres psikologis.Selain stres, kebersihan wajah juga merupakan salah satu faktor timbulnya akne vulgaris.Tujuan :Penelitian ini bertujuan untuk mengetahui hubungan stres dan kebersihan wajah terhadap kerjadian akne vulgaris pada mahasiswa fakultas kedokteran universitas malahayatiMetode :Penelitian ini menggunakan metode crossectional. Subjek penelitian terdiri dari 324 responden dengan menggunakan kusioner dan pemeriksaan fisik pada responden.Hasil : Hasil uji statistik diperoleh p-value=0,000 yang berarti p<α (Ho ditolak dan Ha diterima) maka dapat disimpulkan bahwa ada hubungan stress terhadap terjadinya acne vulgaris.Dengan nilai OR 26,414 berarti responden stress memiliki peluang 26,414 kali lebih besar untuk terjadinya akne vulgaris  dibandingkan responden yang tidak mengalami stress.Hasil uji statistik diperoleh p-value=0,000 dengan nilai OR 3,452.Kesimpulan :Adahubungan stres dan kebersihan wajah terhadap kejadian aknevulgaris pada mahasiswa fakultas kedokteran universitas malahayati Kata kunci: akne vulgaris, stres, kebersihan wajah

Acne vulgaris is a disease of the pilosebaceous follicle characterized by noninflammatory (open and closed comedowns) and inflammatory lesions (papules, pustules, and nodules). There are four major factors involved in the disease production viz. increased sebum production, cornification of pilosebaceous ducts, microbial involvement and inflammation. This condition is found commonly in puberty. In Ayurveda, it is termed as Mukhadushika In Ayurveda Mukhadushika is described under the heading of Kshudrarogas. The Shalmali thorn like eruption on the face due to vitiation of Kapha, Vata and Rakta which are found on the face of adolescent are called as Mukhadushika or YuvanPidika. Although vata, kapha and rakta are mentioned as dosha-duṣya involved in the pathogenesis of the disease, pittaja symptoms are also found in the disease like daha and paka.  Patient was suffering from Acne over face (Pidika), Burning sensation over face (Daha), Itching over face (Kandu), and Discolouration of skin (Vaivarnya) since 6 months. Patient was treated with jalukavacharan and mahamanjishtadi kwath.

There are various skin diseases affecting human beings but acne is the most commonly reported one. It is because face is the reflection of personality and acne affects mostly the skin of face. In Ayurveda face is one of the 56 Upangas mentioned and described at the top. Acharya Sushruta mentioned youvan pidika for the first time which can be correlated with acne. Youvan pidika means disease of young age. The main dosha vitiated in this disease is Rakta. So, Raktamokshan is done for sodhana of Rakta dosha. In the following case described, Raktamokshana is done using Jalauka Avacharana (Leech Therapy) was done to treat acne. Remarkable result was observed in the particular case.
