Aim. To determine the association between morphological characteristics of tumor growth and clinical manifestations of progression. Methods. The study included 118 patients with juvenile nasopharyngeal angiofibroma who were examined and treated at the department of head and neck cancer of Cancer Research Center of Uzbekistan from 2003 to 2013. The main group included 69 patients with primary nasopharyngeal angiofibroma, control group - 49 patients with relapsing tumors who underwent surgery. Standard histological examination was performed in all patients. The algorithm for determination of morphological changes degree was developed based on approach proposed by K.M. Mardaleyshvili (1985), which was modified by authors. Results. Seven most important morphological features of juvenile nasopharyngeal angiofibroma were outlined. On this basis, we proposed to determine the degree of morphological changes for the prediction and treatment monitoring, which is a graded scale for the signs in each of the seven groups. In the main group of patients with cell type of juvenile nasopharyngeal angiofibroma growth, the scores ranged from 10 to 13, marking the 1st degree of morphological changes. Patients with vascular fibrous type of tumor growth had 18-19 points, which corresponds to the 2nd degree of morphological changes in the tumor. Tumor samples of patients with vascular fibrous type of juvenile nasopharyngeal angiofibroma had the most evident morphological changes in the tumor tissue, which corresponded to 3rd degrees with the scores reaching 28. Conclusion. The proposed algorithm for determining the grade of morphological characteristics may be used for determining the treatment tactics and monitoring the tumor growth in patients with juvenile nasopharyngeal angiofibroma, as well as the prognostic factor.

Objective. To determine the composition of the the parietal microflora of the colon under the condition of a tumor process and to assess the possibility of using these data from this study in the diagnosis of the colon cancer. Methods. The main group included patients (n=75) operated on in the Trans-Baikal Regional Oncology Center for the colon cancer. Among them are 38 men and 37 women aged 20 to 75 years. Control group consisted of 25 patients. The material of the main group, biopsies of tumor tissue and visually unchanged colon mucosa was taken intraoperatively during tumor removal. In patients of the control group, the material was taken during the colonoscopy. process. The detection of Helicobacter pylori in the biopsy of the mucous membrane was carried out by determining the urease activity, as well as by the method of polymerase chain reaction. The composition of the intestinal microbiota was determined by the bacteriological method. Results. As a result of comparison of the colon microbiota of the cancer patients and the control group, statistically significant differences in the quantitative composition of Lactobacillus spp., Bifidobacterium spp., Bacteroides spp., Clostridium spp., Enterococcus spp., E. coli (typical), E. coli (lactose-negative), Enterobacteriaceae, Staphylococcus spp. (CNS), Candida spp. were revealed. Moreover, a clear tendency of changes in the level of certain microorganisms (Bifidobacterium spp., E. coli (typical), Clostridium sp.) was detected when comparing the mucous membrane of a healthy person, unchanged mucous membrane of a patient with colorectal cancer, tumor tissue. A statistically significant dependence of the level of microorganisms on the sex of a patient, as well as the form of tumor growth, was revealed. Conclusion. Thus, a reliable correlation between the clinical and morphological characterization of the colon malignant tumor and the composition of the intestinal microbiota has been revealed. The microbiological features established within in the course of the study, which arise precisely under the conditions of colon cancer, can be used as an additional diagnostic marker in the diagnosis of a malignant process. What this paper adds It was found that thequantitative composition of the intestinal microbiota during the transformation into mucous and malignant tumor tissue in colon cancer and the correlation of specific microorganisms with the patient’s gender and the form of tumor growth. The state of the intestinal microbiota may represent a promising diagnostic marker for the detection of a malignant tumor.

Aim. To study the influence of deproteinized dialysate from blood of milk-fed calves on the type of exudative discharge and dynamics of inflammatory reaction after hernia repair with plastics of the anterior abdominal wall with hernioendoprosthesis of polypropylene. Materials and Methods. The study involved 59 patients being on stationary treatment in the surgical department of Kursk Regional Clinical Hospital. Patients were hospitalized for the herniation of small or medium dimension. The patients were divided to two groups: the main (n=30) and control (n=29) groups. After endoprosthetics, the patients of the control group were given complex conservative treatment. The patients of the main group, besides standard treatment, were administered deproteinized dialysate from blood of milk-fed calves intravenously in drips 10 ml + 200 ml of 0.9% sodium chloride solution within 7 days. For cytological examination and determination of the type of cytograms, the traumatic discharge was collected and analyzed using the method of M.F. Kamaev and M.A. Palthsev. Results. Cytomorphometric examination was conducted on the third, fifth and seventh day after endoprosthetics with the aim of studying dynamic changes. Determination of cell composition and also of its changes characteristic of each studied period, was necessary for obtaining further information characterizing inflammatory process in the region of placement of the endoprosthesis. After endoprosthetics in patients who were administered hemodialysate, the inflammatory reaction was less pronounced than in patients who did not receive the preparation. This was associated with a more dynamic change of stages of the inflammatory process. In patients of the main group who received deproteinized dialysate, regenerative type of inflammation first appeared on the fifth day and made 6.9%; by the seventh day the share of patients with the regenerative type rose to 17.5%, while in the patients of the control group no regeneration stage was observed in both periods. Conclusion. Analysis of the efficiency of influence of hemodialysate on the inflammatory reaction in plastics of the anterior abdominal wall with polypropylene endoprosthesis evidences faster course of all stages of inflammation and reduction of its intensity by 10%.

The article presents data on the long-term effect (nine months) of a silicon compound supplied with drinking water – nonahydrate sodium metasilicate (10 mg/l in terms of silicon), on CD68-positive macrophages in the liver and spleen of laboratory rats. Changes in the morphological characteristics of this cell population were found. There was a decrease in the average cell area (in the liver of the control group of rats, the average macrophage area was 179.23±5.94 microns2, and in the group receiving silicon with drinking water – 117.04±3.35 microns2; in the spleen-136.02±3.93 microns2 and 103.44±2.8 microns2, respectively). Macrophages in the liver preparations of the experimental group of rats had a fewer processes and a darker cytoplasmic membrane. The number of macrophages in the liver per unit area was comparable, for the control group of rats it was 18.78±1.24, and for the rats that received with water with the addition of silicon – 19.41±0.75 cells. CD68+ macrophages of the red splenic pulp in laboratory rats that received silicon also underwent the following morphological changes: they were located in a denser way and had fewer processes, while the number of macrophages per unit area was 73.7±2.3 for the control group, 91.6±5.0-for the experimental group, respectively. The distance between them did not change. There was a change in the intensity of CD68 expression on the surface of the cytoplasmic membrane and in the cytoplasm of liver and spleen macrophages. These changes can be interpreted as the adaptive ability of liver and spleen macrophages to silicon introduced with drinking water. Given the heterogeneity of the macrophage population in the liver and spleen, further studies using markers for different subpopulations of macrophages are needed to clarify their role in the response of tissues to silicon supplied with drinking water.

Background. A new strategy of breast cancer management is required for effective disease prevention and control. Immunotherapy is one of promising options.Objective: to evaluate the effect of immunoadjuvant polyoxidonium on pathmorphological changes in breast tumors.Materials and methods. The study was performed in 75 patients with breast cancer: 50 patients received polyoxidonium in a neoadjuvant regimen (main group), 25 patients did not receive polyoxidonium (control group). Polyoxidonium at a dose of 12 mg was administered intramuscularly 1 time per day on the days 1, 2, 3, 5 and 7 before surgery; on the day 8, radical mastectomy was performed in patients of both groups, followed by histological examination and determination of the grade of therapeutic pathomorphosis of the tumor.Results. The proportion of cases with grade I therapeutic pathomorphosis was 58 %, grade II therapeutic pathomorphosis – 6 %. Pathomorphosis in metastatic lymph nodes was observed in 22.7 % of cases.Conclusion. The anti-tumor effect of polyoxidonium was confirmed on the basis of the induction of pathomorphosis of the tumor tissue of breast cancer.

AbstractBackground:Abnormalities in the hippocampus have been implicated in the pathophysiology of psychosis. However, it is still unclear whether certain abnormalities are a pre-existing vulnerability factor, a sign of disease progression or a consequence of environmental factors. We hypothesized that first-episode psychosis patients who progress to schizophrenia after one year of follow up will display greater volumetric and morphological changes from the very beginning of the disorder.Methods:We studied the hippocampus of 41 patients with a first-episode psychosis and 41 matched healthy controls. MRI was performed at the time of the inclusion in the study. After one year, the whole sample was reevaluated and divided in two groups depending on the diagnoses (schizophrenia vs. non-schizophrenia).Results:Patients who progressed to schizophrenia showed a significantly smaller left hippocampus volume than control group and no-schizophrenia group (F = 3.54; df = 2, 77; P = 0.03). We also found significant differences in the morphology of the anterior hippocampus (CA1) of patients with first-episode psychosis who developed schizophrenia compared with patients who did not.Conclusions:These results are consistent with the assumption of hyperfunctioning dopaminergic cortico-subcortical circuits in schizophrenia, which might be related with an alteration of subcortical structures, such as the hippocampus, along the course of the disease. According with these results, hippocampus abnormalities may serve as a prognostic marker of clinical outcome in patients with a first-episode psychosis.

Background. The effective treatment of the hand-foot syndrome is a necessary component of the complex treatment of patients receiving antitumor therapy with multikinase inhibitors. Aims. The assessment of a new effective method for the treatment of palm-plantar syndrome in patients receiving multi-kinase inhibitors. Materials and methods. There are 27 patients received antitumor treatment with multi-kinase inhibitors and had clinical manifestations of the hand-foot syndrome (HFS) for observation. The main group (14 patients) used a combination of an alpha-lipoic acid at a dose of 600 mg per day per os and an ointment containing 0.005% calcipotriol and 0.05% betamethasone dipropionate 2 times a day. The control group, included 13 patients, used a combination with a cream containing 10% urea and ointment containing 0.05% betamethasone dipropionate. Results. After a week of treatment, the regression of the skin process was observed in both groups, but it was more evident in the main group: significant differences in severity of erythema, paresthesia, pain and burning sensation, the Dermatological Life Quality Index (DLQI) were observed at this stage of therapy (p < 0.05). After 2 weeks of therapy: a more positive trend in the main group in terms of erythema, paresthesia and burning, but the most significant difference is DLQI – 17.4 in the main group and 22.8 in the control group (p < 0.0001). At the end of the 3rd week of therapy: differences with the main characteristics of erythema, desquamation, paresthesia (p < 0.0008), pain (p < 0.0001), DLQI (p < 0.0001). At the end point of the study (after 4 weeks of therapy), significant differences were found in reducing the symptoms and the main parameters for evaluating the effectiveness of DLQI in patients of the main group with a reliability of p < 0.0001. The high correlation of DLQI was found with erythema, pain, burning, paresthesia and desquamation. Conclusions. The treatment of the hand-foot plantar syndrome combined using alpha-lipoic acid inside and topical therapy in the form of an ointment containing calcipotriol and betamethasone dipropionate, can be recommended for patients receiving targeted therapy with multi-kinaseinhibitors.

Introduction. Nowadays anti-inflammatory and immunosuppressive therapy has significantly improved the quality of life and prognosis of rheumatoid arthritis (RA). Nevertheless, there are still many patients with progressive rheumatoid inflammation, resulting in the destruction of joints. Cell therapy seems like a promising direction in rheumatology. The aim of our research was to evaluate the efficacy of fetal chondrocyte transplantation in patients with RA.Methods. We examined 60 patients with rheumatoid arthritis (I - III stages) between 20 and 63 years of age. They were divided into 2 groups: the first group underwent the fetal chondrocytes transplantation (n = 40), and the second was a control group who got conservative therapy (n = 20). Donor cells were taken from the chondrogenic layer of the humerus or femur heads and hip condyles of human embryos in gestation for 17-20 weeks. A suspension of fetal chondrocytes injected into affected areas of the articular surfaces under X-ray control. Cell viability was determined before the injection. Efficacy of the therapy was assessed by clinical, instrumental, and laboratory tests. This clinical trial was allowed by The Ministry of Public Health and Ethics Committee. All of our patients gave informed consent for the fetal chondrocytes transplantation.Results. Evaluation of the clinical manifestations of RA in the first group of patients showed 3.7 times decrease in pain and 1.6 times relief of synovitis. Complete reduction of contracture was observed in 82% of patients in the first group. Morphometric changes in X-ray demonstrated inhibition of the destruction in articular cartilage and surfaces of bones after transplantation of fetal chondrocytes. The dynamics of morphological changes in synovium showed 2.5 times reduction of the inflammatory reaction. Transplantation of fetal chondrocytes led to a significant reduction in ESR, CRP, fibrinogen , γ-globulin after a period of 12 months (p < 0.03). Furthermore, patients in the second group had 2.7 times higher risk of ankylosis compared to the first group. We did not observe any complications of fetal chondrocytes transplantation.Conclusions. Application of fetal chondrocytes therapy had the desired clinical effect, which was confirmed by reduction of the RA activity and decrease of cartilage and bone destruction.

Goal. Improving the scheme of treatment and prevention of periodontal diseases in workers of the woodworking industry of Chernivtsi region, who in the process of professional activity have long-term contact with unfavorable factors of the production environment. Materials and methods. 70 workers of the woodworking industry with diagnosed chronic generalized periodontitis of the I degree were treated (35 people – the main group (A); 35 people – comparison group (B)), age – from 25 to 45 years. To compare the results of laboratory studies, an additional survey of 25 healthy individuals of the same age with an intact periodontium were conducted. They formed the control group (C). Clinical examination of patients was performed according to standart methods: subjective (complaints, medical history, life history) and objective (examination, periodontal indices, determination of the level of gingival attachment). Laboratory methods of research included determination of urease and lysozyme activity in saliva, degree of dysbiosis of oral cavity. As maintenance therapy, patients of the main group were prescribed the proposed composition DEPE. Results. After the treatment, a significant improvement in the hygienic status of patients in both groups was observed, but difference between groups A and B in the indicators of oral hygiene after treatment was no statistically significant (pA2–B2 > 0.05). There was a significant improvement in periodontal indices after treatment in patients of both groups (PMA index according to C. Parma, bleeding index according to Muhlemann in Cowell I. modification, Russell periodontal index, PSR-test), but the indicators in patients of the main group were significantly better – pA2–B2 < 0.05. In patients of the main group (A) the level of attachment loss decreased by 1.83 times, comparison group (B) – by 1.71 times. The difference in levels of attachment loss after treatment between groups A and B is statistically significant (pA2–B2 < 0.05). The activity of the enzyme urease in patients of the main group (A) and comparison group (B) after treatment was significantly reduced (pA1–A2 < 0.001, pB1–B2 < 0.001), but only in the main group it reached the level in the control group (C) (pA2–C > 0.05). The level of lysozyme after treatment was significantly increased in patients of both observation groups (pA1–A2 < 0.001, pB1–B2 < 0.001), and reached the level of lysozyme in patients of the control group (C) (pA2–C > 0.05, pB2–C > 0.05). The degree of oral dysbiosis in patients of the main group (A) was significantly reduced by 5.43 times (pA1–A2 < 0.001), and its difference from the degree of dysbiosis in patients of the control group (C) is statistically insignificant (pA2–C > 0.05 ). The degree of oral dysbiosis in patients of the comparison group (B) was significantly reduced by 3.04 times (pB1–B2 < 0.001), but still remained significantly different from oral dysbiosis in patients of the control group (C) (pB2–C < 0.05). Conclusion. The proposed pharmacological composition DEPE is an effective antiseptic solution and can be used in the phase of maintenance therapy to improve the treatment and prevention of periodontal disease in workers of woodworking industry. Objective: To study the features of clinical manifestations, cellular and humoral immunity of newborns with intrauterine mixed infections (IUMI). Methods: A comprehensive survey of 45 infants with IUMI was carried out. Depending on the classification of IUMI they were divided into 3 main groups: group I – 24 patients (53%) with the viral-bacterial association, group II – 12 patients (27%) with the viral-viral association, and group III – 9 patients (20%) with the viral-parasitic association. The control group consisted of 10 newborns born from uninfected, somatically healthy mothers. Diagnosis of IUMI was based on the detection of specific antibodies of the IgA, IgG and IgM classes, as well as phagocytic activity and phagocytic index of leukocytes. In addition, general clinical, biochemical, bacteriological and instrumental research methods have been conducted. Results: The blood serum IgA and IgM indices in patients of the main group were significantly higher, and the mean IgG values were lower compared to the control group. Analysis of cellular immunity parameters in the main group showed a decrease in the number of mature T-lymphocytes (CD3), B-lymphocytes (CD20), the number of T-helpers and cells that produce IL-2 in the peripheral blood, compared with the control group. In the main group, there was also an increase in the number of apoptosis cells (CD95), cells with high cytotoxic activity (CD25, CD71) and the percentage of natural killer cells (CD16). A decrease in the phagocytic activity and phagocytic index of neutrophils was recorded, which indicates the insufficiency of the nonspecific component of immunity. Conclusions: In newborns, various changes were found both on the part of specific and nonspecific components of immunity. This indicates the development of secondary immunodeficiency in this category of patients and makes it necessary to add to the main treatment of immune corrective therapy. Relevance. One of the main causes of periodontitis is oral dysbiosis, in which changes in the quantitative and qualitative composition of the microflora occur, the systems of interaction between the microorganism and the microbiota are disturbed, and immunity is reduced. Is to study the comparative anti-disbiotic efcacy of various immunobiological preparations in patients with chronic apical periodontitis against the background of oral dysbiosis.Materials and methods. The study included 66 patients who were divided into 2 groups: comparisons (n = 34) and main (n = 32). Each group was divided into three subgroups depending on the degree of dysbiosis of the oral cavity according to the method of A.P. Levitsky (2007): determination of the ratio of the relative activities of urease and saliva lysozyme. All patients underwent traditional endodontic treatment and oral hygiene with parallel correction of oral microbiocenosis through the use of a complex of immunomodulatory drugs to patients of the main group.Results. In chronic apical periodontitis, urease activity was signifcantly increased, and in those examined with grade II-III dysbiosis almost 2 times compared with patients with grade I severity of clinical manifestations. The proposed treatment reduced the activity of urease in patients in all three subgroups of both groups (main and comparison). The results indicate a signifcant decrease in the activity of lysozyme in all patients more than 2 times. Conducted traditional treatment (comparison group) increased activity, but did not lead to a recovery in the rate of healthy individuals. The use of immunomodulatory drugs allowed to increase the level of lysozyme to the indicators of the control group.Conclusions. The inclusion of immunobiological preparations in the complex therapy of chronic apical periodontitis against the background of oral dysbiosis eliminates the dysbiotic phenomena in the oral cavity and has a therapeutic effect on destructive processes in the periapical tissues that are difcult to treat and often recur.
