Background. Menstrual pain syndrome is accompanied by a complex of neurovegetative, metabolic-endocrine and psycho-emotional disorders. Dysplasia of connective tissue is characterized by a high level of functional disorders of the central and autonomic nervous system and emotional disorders. Aim. Detection of features of a clinical current of primary dysmenorrhea in adolescent girls with signs an undifferentiated dysplasia of connecting tissue (UDCT). Materials  and methods. 210 adolescent girls at the age of 11–17 full years suffering primary dysmenorrhea were examined. Depending on the presence or absence of clinical signs of undifferentiated connective tissue, two groups were identified. The main group consisted of 120 patients, the comparison group comprised 90 patients. The severity of the menstrual pain syndrome was assessed using the three scales of the McGill Pain Questionnaire. For psychological testing was used a scale of self-assessment of anxiety of Ch.D. Spielberger – Yu.L. Khanina. Results. In 90.9 ± 2.6% of patients of the main group the debut of a dysmenorrhea has taken place in the period of formation of menstrual function — within the first 2 years after menarche. The average value of the rank index of pain in the main group was higher than in the comparison group (3.5 ± 0.1 and 2.8 ± 0.1, p < 0.001). In an internal picture of perception of pain at patients of the main group the emotional component prevailed. Their psychoemotional status was distinguished by a higher level of the average score of reactive (45.3 ± 0.9 and 36.3 ± 1.4; p < 0.001) and personal (49.9 ± 0.6 and 41.4 ± 1.0; p < 0.001) of anxiety. Conclusions. The obtained results give the grounds to consider the presence of clinical manifestations of an undifferentiated dysplasia of a connecting tissue in adolescent girls as a predictor, which is making heavier a clinical current of a menstrual pain syndrome.

The aim: To evaluate the peculiarity of clinical manifestations of neonatal respiratory distress syndrome (NRDS) in deeply premature infants from mothers with phenotypic markers of undifferentiated connective tissue dysplasia (UCTD). Materials and methods: The study represent the results of a retrospective clinical and statistical analysis of 268 premature birth report card and newborn report sheet. .The main (1 group) included 50 pregnants with obvious phenotypic markers of UCTD, the comparison group (group 2) consisted of 50 pregnant women without phenotypic markers of UDCTD. Results: According to the study, in 12 (24%) pregnant women of the main group at the time of admission to the clinic had contractions,which required specific therapy. Cervical cerclage was performed in 38 (76%) patients of the main group due to the presence of cervical insufficiency (CI). In these cases, the severity of the CI on the Steinber scale was 7.2 &#177; 0.4 points in the main group against 4.4 &#177; 0.2 points in the comparison group (p &#60;0.05). Group I patients were more likely to have complications of labor such as:premature rupture of membranes, uterine contraction abnormalities and fetal distress, which required in most cases cesarean delivery (7% and 2%), respectively (p &#60;0.05). The incidence of neonatal complications requiring respiratory support was 67% in group I and 48% in group II. According to our observations, the clinical manifestations of bronchopulmonary dysplasia were twice as high in infants of the main group (66%) against (44%) of the comparison group (p &#60;0.05). Conclusions:1.Neonatal respiratory distress syndrome in premature infants is more often associated from mothers with UDCTD. 2. The high importance of steroid prophylaxis of NRDS and antioxidant therapy in reducing the frequency of mechanical ventilation and the development of bronchopulmonary pathology, especially in infants from mothers with UDCTD syndrome, has been proven. 3. The possibility of diagnosing disorders of functional maturation of the lungs in the fetal period using a non-invasive method of ultrasonography has been confirmed.

Relevance. Occlusal splints, made for the patients with temporomandibular disorders (TMD), influence proprioceptive sensitivity, sensory characteristics of pain, parameters of microcirculation of the pulp and periodontium of the healthy teeth. Condition of the pulp and periodontium of the healthy teeth should be assessed in patients with TMD associated with connective tissue dysplasia (CTD).Materials and methods. The study examined 36 TMD patients aged 26.3 ± 1.3 лет (М ± m) with CTD (n = 20, main group) and without CTD (n = 16, control group); with natural teeth, without complaints of TMD, masticatory muscles and periodontal condition. Laser doppler flowmetry (LDF) helped to analyze blood flow parameters in the periodontium and pulp of the healthy teeth. Received results were statistically analyzed.Results. Wavelet analysis revealed different microcirculatory flowmotion: high-amplitude irregular type was in patients of the main group, low-amplitude regular type was in controls. In the main group, perfusion is supported by high values of flux and vasomotion, ergotropic sympathetic effects (controlled by catecholamines) prevail, myogenic tone is reduced, elevated microvascular tone reflects possible changes in rheologic blood properties, microcirculatory signs of pain syndrome and relatively elevated ischemic index in pulp microcirculation of healthy teeth.Conclusion. Sensory and regulatory mechanisms, reflected by the condition of periodontium and pulp blood flow, can be regarded as early signs of inflammation and chronic pain in the diagnosis of TMD in patients without complaints.

Objective: to study the effect of nabumetone in oncogynecologic patients in the postoperative period. Materials and methods. There were examined 98 patients of 40-78 years old, who were divided into two groups: the main group - 58 patients, of which 37 - patients with endometrial cancer I-III and 21 - with ovarian cancer of II-IV stage (FIGO); The comparison group included 40 patients (17 and 23 patients respectively). All patients underwent surgical treatment. In the postoperative period, were used NSAIDs - nabumeton. Before the operation and on the 10th day of the postoperative period, the following parameters were studied: a clinical blood test, TNF-a, ceruloplasmin, diene conjugates (DC), malonic dialdehyde (MDA), antioxidant activity (AOA). The content of TNF-a, in the blood serum was determined by radioimmunological, and DC, MDA, AOA by biochemical method. Results. Established: the content of the leukocyte formula before the start of treatment in the compared groups did not reveal any significant differences. After surgical intervention, both groups showed an increase in the absolute values of all leukocyte fractions. The level of ceruloplasmin in the patients of the comparison group increased significantly in the postoperative period, and in the main group the parameters of ceruloplasmin did not differ significantly. The level of TNF-a in the pre- and postoperative period in the compared groups did not differ. On the 10th day of the postoperative period, it significantly increased in the comparison group, and when nabumetone was used during the postoperative period, the level of TNF-a did not increase. The initial indicators characterizing the processes of LPO and AOA in both groups did not differ significantly. Assessing the dynamics of indicators before and after the operation, it was noted that in patients in the blood, the level of DC in both the baseline and in the comparison group increased. The use of nabumethon led to a slight increase in DC. The maintenance MDA in the postoperative period has increased in both groups, however it is unreliable in comparison with parameters before surgical intervention. In the comparison group, after the treatment, the antioxidant defense was reliably reduced. In the main group, the use of nabumetone contributed to a smaller decrease in AOA. The use of nabumeton allowed abandoning of narcotic analgesics in earlier periods of the postoperative period. Conclusion. Use of nabumeton in the postoperative period allows to reduce the pain syndrome, the number of inflammatory complications; contributes to the normalization of LPO processes, inhibits the inhibition of the antioxidant system, which contributes to the reduction of endogenous intoxication and improves the results of treatment of oncogynecologic patients. Background. Primary dysmenorrhea is prevalent in adolescents and young women. Menstrual pain and distress causes poor school performance and physiological damage. Auricular acupressure can be used to treat these symptoms, and Internet-based systems are a flexible way of communicating and delivering the relevant information.Objective. This study investigates the effects of auricular acupressure (AA) alone and combined with an interactive Internet-based (II) intervention for the management of menstrual pain and self-care of adolescents with primary dysmenorrhea.Design. This study adopts a pretest/posttest control research design with a convenience sample of 107 participants.Results. The outcomes were measured using the short-form McGill pain questionnaire (SF-MPQ), visual analogue scale (VAS), menstrual distress questionnaire (MDQ), and adolescent dysmenorrheic self-care scale (ADSCS). Significant differences were found in ADSCS scores between the groups, and in SF-MPQ, VAS, MDQ, and ADSCS scores for each group.Conclusion. Auricular acupressure alone and a combination of auricular acupressure and interactive Internet both reduced menstrual pain and distress for primary dysmenorrhea. Auricular acupressure combined with interactive Internet instruction is better than auricular acupuncture alone in improving self-care behaviors.

Objective. To carry out a comparative analysis of the results of epithelial coccygeal course treatment using laser-induced interstitial thermotherapy LIT) under ultrasound control and traditional open excision. The high trauma rate of traditional operations in the epithelial coccygeal course ECC), the duration of hospital stay, pain syndrome force surgeons to search for new technologies in the treatment of this pathology. Materials and methods. A retrospective analysis of the treatment results of 369 patients with ECC who were admitted to the clinic for elective surgery in the period from 2009 to 2019 was carried out. There were 264 men 71.5 %) and 105 women 28.5 %). The mean age was 26.9 7.2 years. The patients were divided into 2 groups: 190 patients of the main group underwent surgery using a fiber-optic laser under ultrasound guidance, 179 patients of the comparison group underwent traditional radical excision of ECC with application of primary sutures or various types of plastic. Results. The severity of pain syndrome after surgery in patients of the main group was 1.7 0.6 points, in the comparison group 3.5 1.3 points by a five-point visual analogue scale VAS) for pain. The duration of inpatient treatment in the main group was 1.8 0.4 days, in the comparison group 11.5 1.7 days. Moreover, in 197 76.1 %) patients of the main group, the operation was performed on an outpatient basis. The restoration of working capacity in the main group occurred after 5.2 1.2 days, and in the comparison group after 17.4 2.8. After traditional operations, the relapse of the disease was noted in 11.7 %, and after laser treatment, slightly more than 12.1 %. Satisfaction with the results of treatment in the main group was noted in 95.1 %, and in the comparison group in 90 % of patients. Conclusions. The obtained results of treatment allow us to recommend the technique of interstitial laser obliteration of the coccygeal cyst under ultrasound navigation developed in the clinic for widespread use in practical work of surgeons.

The aim of the study was to investigate radiofrequency ablation (RFA) effect on histo- and fibroarchitectonics of plantar aponeurosis in dogs with fasciopathy simulated by alprostan.Material and methods. Methods of light microscopy (Van Gieson, Weigert–Van Gieson and picro-allory staining with alcian and toluidine blue) and morphometry were used in the study. The study involved sections of plantar aponeurosis in animals of the control group (without fasciopathy, n=8), the main group (with RFA, n=8) and the comparison group (without RFA, n=8).Results. The study demonstrated that a single exposure to RFA after simulated alprostane fasciopathy (four-time injection of the medication during 35 days) did not cause an increase or occurrence of additional pathological changes in the injection area after 7 days. Edema, microbleeding, infiltration by lymphocytes, plasmocytes and leukocytes, dystrophy in the form of mucoid swelling, collapse and rupture of collagen fibers were revealed in animals of the main group, as well as in animals of the comparison group. Processes of adaptation and regeneration of the connective tissue in both groups were manifested in the form of a large number of activated fibrocytes, fibroblasts, microvessels, and the formation of collagen fibers. All these together resulted in the same type of heterochronous and heteromorphic spatial histotopographic changes in the aponeurosis – its structure became more cellular, but less ordered in space. Foci of the chaotic distribution of damaged and newly formed collagen fibers appeared. However, exposure to RFA under moderate power (energy) conditions destroyed cells, thin and damaged collagen fibers, and prevented the development of irreversible fibrinoid swelling and granulation tissue around the foci of dystrophy. Together, all these changes reduced the possibility of gross cicatricial rearrangements in the foci of pathologically altered collagen networks.Conclusion. A single exposure to RFA prevented the development of irreversible stromal-vascular dysproteinoses, inflammatory processes and gross cicatricial changes in the connective tissue of the plantar aponeurosis in dogs. These changes can be determined as reasons for the positive clinical effect of RFA on the foci of pathologically altered collagen networks in fasciopathies.

Summary. High prevalence, unfavorable effects of periodontal infection centers on the oral cavity condition determine the relevance of the problem of the use of phytopreparations in the treatment of generalized periodontitis.The aim of the study – to evaluate the clinical efficacy of the drug Resverazin in the complex treatment of generalized periodontitis.Materials and Methods. The study was attended by 43 patients, patients with chronic generalized periodontitis of primary and stage I. The main group was 25 people, the comparison group was 18. To assess the hygienic state of the cavity, the indices Green-Wermillion, ONI-S, Silness-Loe, Stallard were used. The assessment of the periodontal condition was conducted according to clinical signs; the severity of gingivitis was determined by the PMA index, the state is clear – according to the index of bleeding Muhlleman, the stages of destructive changes in the periodontium – according to the periodontal index (PI). For the treatment of periodontitis in both groups, traditional therapy was used. The patients in the main group were prescribed Resverazin for 1 capsule 2 times a day after eating. The course of treatment was 2 months.Results and Discussion. In an objective survey conducted before treatment, the average value of the index of hygiene in the main group was (1.64±0.31), in the comparison group – (1.59±0.23) points. The PMA index in the main group was (63.12±1.17), in the comparison group – (62.39±1.13) points. The ONI-S, Silness-Loe and Stallard indexes in the patients in the main group were (2.36±0.13), (1.54±0.08), (1.200±0.062) points, in the comparison group – (2.30±0.12), (1.51±0.06) and (0.890±0.032) points, respectively. The values of PI and the Muhlleman index in the groups were (1.22±0.09) and (2.21±0.13) and (1.24±0.13) and (2.18±0.11) points, respectively. During the treatment, a positive dynamics of the studied parameters was observed. The values of the PMA index (%) and PI were (10.23±1.11) and (0.63±0.11) in the main group and (11.22±1.19) and (0.84±0.12) in the comparison group.Conclusions. The obtained results confirm the high clinical efficiency of the natural complex Resverazin, the inclusion of which in the scheme of basic therapy of generalized periodontitis contributes to positive dynamics and stable changes in the tissues of periodontal disease.

Objective: to compare the effectiveness of two approaches to the treatment of patients with low back pain syndrome: the traditional approach based on the classification of I.P.Antonov (1984) and more modern one based on the criteria of nonspecific pain. Material and methods. The work gives the data of complex examination and treatment of 628 patients of the main group and 79 patients of the comparison group. Results and discussion. Both the groups detected abnormalities from normal values with greater frequency in laboratory and tool methods of research. The duration of inpatient treatment of the patients of the main group was statistically significantly shorter due to a differentiated traditional approach to the treatment of patients with vertebrogenic lumbosacral pain syndrome. Conclusion. The traditional approach to the concretization of pain syndrome in lumbar osteochondrosis substantially in terms of effectiveness exceeds the concept of nonspecific pain in the lower back, references for which dominate in English-language sources.

The article describes the offered by us multicomponent remedy based on chitosan, to provide analgesic and wound-healing effect in patients with acantholytic lesions of the prosthetic bed tissues. Objective of the study. To improve the conditions of removable dentures use for patients with acantholytic pemphigus. Materials and methods. The study involved 56 patients with acantholytic pemphigus, aged from 45 to 63 years old, who were divided into 2 clinical groups. The main group used the proposed remedy; the comparative group received the standard local pemphigus treatment. Methods for determining the area of ​​mucus lesion and the method for determining the intensity of pain syndrome were used to evaluate the final results. Results of the study and their discussion. The average value of visual analogue scale (VAS) was 4.76±0.56 cm in patients of the main group during the first day. During the 7th and 14th day, the value decreased to 2.4±0.28 cm and 1.87±0.12 cm, respectively. In patients of the comparison group, the average value of VAS was 5.07±0.36 cm as for the first day and 4.45±0.45 and 2.36±0.17 cm during the 7th and 14th days, respectively. The ratio of the mean value of the affected area in patients of the main group and the comparison group was 1.04 during the first day, and during the 7th and 14th day the area of ​​injury in patients of the main group was lower 1.29- and 1.84 -fold, respectively. Conclusions The results of the study confirm the necessity of using a multicomponent remedy based on chitosan together with the removable dentures in patients with acantholytic pemphigus, due to its clearly expressed epithelial and analgesic action.

<p>Primary dysmenorrhea is pain during menstruation period due to high levels of prostaglandins, especially prostaglandin F2α in epithelial cells. MungBean Extract Drinks contain several nutrients including vitamin C, calcium, carbohydrates and flavonoids as anti-depressants and anti-inflammatory agent. Stretching is a relaxation technique that can helps relieve menstrual pain in the abdominal part by increasing the production of endorphins which function as neurotransmitters. To determine the effect of Mung Bean ExtractDrinks and Stretching on primary dysmenorrhea (primary menstrual pain) in adolescent girls stressed. This type of open-label non-randomized controlled trial used 87female adolescents who experienced primary menstrual pain with moderate and severe stress levels from two Islamic boarding schools in Mataram City, West Nusa Tenggara, Indonesia, which were divided into 3 groups, one group as the control and two groups as the experiment. The experiment of 29people with pre and post-test. Mung bean extract drinks were given 250ml/day, otherwise stretching was given 1x/day for 7days before menstruation. Control and experimental group was observed for 1 month. Measurement of menstrual pain scale using the Numeric Rating Scale (NRS) assessment sheet. Data analysis uses T-test. The mean primary menstrual pain in adolescents for MungBean Extract Drinks with moderate stress levels was 1.31±1.32 and the difference before and after mung bean extract drinks experience is significant at p=0.004 while at severe stress levels was 1.69±1.54 and the difference between before and after mung bean extract drinks experience is significant at p=0.001. The mean primary menstrual pain in adolescents for stretching experience with moderate stress levels was 2.50±1.93 and the difference before and after stretching experience is significant at p=0.008 while the severe stress level was 2.05±1.80 and the difference before and after stretching experience is significant at p≤0.001. The multivariate test with linear regression revealed that mung bean extract drinks contributed y= (-)138X1+4.897(p=0.001) and related stretching contributed y= (-)2.517X1+6.276(p=0.001). MungBean ExtractDrinks and Stretching will affect to the reduction of primary dysmenorrhea in adolescent girls both at moderate and severe stress levels</p>
