Background: The use of tourniquet during TKA aside from having several advantages, is also accompanied by several risks. This study was performed to prove the relation between the duration of tourniquet application to inflammatory response, pro-thrombotic condition, and pain response of the patients undergoing TKA procedures.Methods: This study is a cross-sectional analytical experimental research. A femoral tourniquet was applied to 34 primary osteoarthritis (OA) patients undergoing the standard procedure of TKA and the duration of tourniquet application data was noted and collected. Pain assessment was performed by using Visual Analogue Scale (VAS), whereas blood samples are taken from the patients and IL-6 cytokine and D-dimer levels were identified at 12 hours and 24 hours post-operatively. Statistical analysis (Pearson correlation) was performed to see the correlation of tourniquet duration to the VAS, IL-6, and D-dimer serum level at 12 hours and 24 hours post-operatively.Results: The result of this study showed a positive correlation between the duration of tourniquet use with IL-6 at 12 hours post operation (r=0.359) and 24 hours post operation (r=0.658); with D-dimer at 12 hours post operation (r=0.491) and 24 hours post operation (r=0.483); and with VAS at 12 hours post operation (r=0.647) and 24 hours post operation (r=0.507) with p<0,05. A positive correlation was found between tourniquet time and IL-6, D-dimer and VAS.Conclusions: The duration of tourniquet cut-off time of more than 122.5 minutes elevates IL-6 and D-Dimer which increases the risk for SIRS and DVT.

Aim: The aim of this study was to establish the validity and reliability of the Tswanatranslations of three pain scales.Design: This was a cross–sectional study to validate and test the reliability of threepain scales.Participants:   One hundred subjects participated in the study. They were selectedfrom the back schools of five hospitals in the North -West Province of South A frica andfrom workers in these hospitals who were employed as kitchen workers, laundryworkers and cleaners.Method: Translation of the pain scales and the stages of cross-cultural adaptation were followed as recommended byBeaton et al (2000). Pain tolerance of the subjects was measured using a P.T.M. (pressure threshold meter). The painscales used were the V.A .S. (visual analogue scale) one (nought and ten only), the V.A .S. (visual analogue scale) two(nought through to ten), the W.B.F. (Wong-Baker Faces pain measure) and the V.R.S. (verbal rating scale).  The V.R.S.used came in two forms. The first form was written on cue cards which the subjects arranged in order and the second form was the questionnaire version of the V.R.S.The subjects were interviewed and asked five questions relating to their back pain. Upon completion of the interviewthe pressure threshold of the painful area (back) was tested. Subjects then filled in the three pain scales, namely the (V.A .S. one, V.A .S. two, the V.R.S. and lastly the W.B.F. pain scale). Approximately a third of the sample (37) was retested the following day following the same procedure asdescribed above. Results: There was no correlation between the pressure threshold meter readings and the pain scale measurements.  Conclusion: From the statistical analysis of the results, it became apparent that the subjects tested did not have anunderstanding of any of the three pain scales. Future research needs to be done in developing entirely different scales for peoples of low literacy and differentlanguage and cultural groups in South Africa.

Digit ratio (2D:4D) denotes the relative length of the second and fourth digits. There are contradicting reports on its relationship with ethnicity/race, whereas convincing studies show it is related to obesity. This cross-sectional study was undertaken to demystify ethnic difference in 2D:4D ratio and to analyze its relationship with obesity among adults in Ilorin Nigeria. The cross-sectional study included 701 individuals. Finger lengths were measured with electronic calipers and other anthropometric traits were measured with standard procedure. Student t test and one-way ANOVA were used to detect differences among groups and relationship was computed with Pearson correlation. The receiver operator characteristic curves were used to detect the diagnostic effect of 2D:4D for obesity. The obtained results showed sexual dimorphism in 2D:4D ratio and other anthropometrics at p < 0.01. Obesity was associated with significantly higher mean of 2D:4D in both genders (female 0.9814 ± 0.012:0.9700 ± 0.012; male 0.9700 ± 0.010:0.9592 ± 0.010 at p < 0.001). The area under the curve was 0.753 (95% CI 0.677-0.829, p < 0.01) and 0.798 (95% CI 0.756-0.804, p < 0.01) in female and male R2D:4D respectively for obesity, implying that 2D:4D might be a surrogate marker for obesity in future.  No significant difference was found in 2D:4D among different ethnic groups studied (p >0.05); this result proved that there was no ethnic specificity in 2D:4D ratio among study’ participants. Thus, it can be reported that the digit ratio was related to high 2D:4D, but this cannot be said for different ethnic groups. The results imply that 2D:4D might be a good surrogate indicator for obesity, but not ethnicity.

Introduction: The workload that exceeds individual work capacity will increase the pulse rate rapidly. The purpose of this research was to study the correlation between physical workload and the increase in workers’ pulse rate. Methods: This research utilized cross-sectional design by using the observational method with 23 workers from the BRF area of PT X Sidoarjo as respondents. This research was conducted by conducting the direct observation by calculating the workload received by the worker by using %CVL method and calculating the resting and working heart rates. Results: The result showed that all workers were male and had the productive age distribution of 60.9% in the age range of 25-50 years. Based on the calculation of workload, the majority of workers have the workload on the moderate %CVL category, amounting to 56.5%. Based on the measurement of the resting and working heart rates, it was found that all workers experienced the pulse rate increase. Based on the result of Pearson correlation statistical test, the result was p<0.05, which means that there was a significant correlation between the workload and the increase in the workers’ pulse rate. Moreover, the correlation coefficient was 0.911, that indicated a very strong and positive correlation. Conclusion: This research concludes that there is a strong and positive correlation between workload and the increase in the workers’ pulse rate. Keywords: %CVL, cardiovascular load, workload, pulse rate

Syndactyly release may be done by skin graft or graftless techniques. We prospectively examined bilateral syndactyly releases in the same patient at one operation. The grafted side was randomized and the contralateral side was done graftless. Fourteen patients had surgery at a mean age of 27 months (range 7–166). The mean follow-up was 52 months (range 6–111). The mean tourniquet time was 97 minutes (range 66–135) for graft and 84 minutes (55–120) for graftless. The mean finger abduction was 57° (32°–80°) for graft and 54° (38°–80°) for graftless. The mean web creep score was 1.2 (0–3) for graft and 2.1 (0–3) for graftless. The mean scar score was 1.9 (1–3) bilaterally. The mean parents’ visual analogue scale for graft cosmesis was 7.1 (5–9) and 6.2 (4.3–8) for graftless. The surgeon’s visual analogue scale for graft was 7.9 (6.4–9.5) and 6.2 (4–8.7) for graftless. The therapist’s visual analogue scale was 7.9 (6.5–10) and 6.4 (4.7–8) for graftless. Although there is a longer tourniquet time with grafting, there may be advantages in appearance and web creep. Pain is the most common symptom of oral diseases. Pain perception in children is highly variable and unreliable due to poor communication. Therefore we designed a study to compare pain measurement techniques, that is, visual analogue scale (VAS) and Wong-Baker faces pain rating scale (WBFPS) among Delhi children aged 3 to 14 years undergoing dental extraction. Method. A cross-sectional study was conducted on 180 patients aged 3 to 14 years who had undergone dental extraction. Children were assessed for their pain sensitivity using visual analogue scale (VAS) and Wong-Baker faces pain rating scale (WBFPS ). Result and Conclusion. Pain threshold tends to decline, and the self-management of pain becomes more effective with increasing age. Genderwise result shows that communication ability of boys and girls is similar in all age groups.

Introvert people tend to have distress. Psicosomatic manifestation from distress on women can be appeared as premenstrual syndrome. The aim of this study is to know the correlation between social introversion stage and premenstrual syndrome. This study is an analitic observasional study with cross sectional approach, which was held on November 2006 on Medical Faculty UNS Solo. Samples were taken by purposive sampling with restriction on new university studentsclass 2006 Medical Faculty of UNS. The requirements are : (1) on premenstrual or menstrual phase, and (2) does not have reproduction organ disease. Subject  fi  lled (1) Kuesioner Riwayat Menstruasi dan Ginekologi (Gynecology and Menstrual History Questionair) to know subject’s menstrual cycle, (2) Kuesioner Skala L-MMPI (L-MMPI Scale Questionair) to state honesty in answering questions given, (3) Kuesioner Sindroma Pramenstruasi (Premenstrual Syndrome Questionair) consisted of pramenstrual syndrome’s aspects, (4) Kuesioner Skala-0 MMPI (MMPI 0-Scale Questionair) to state social introversion stage. It’s obtained 42 subjects data. Social introversion stage (SI) and premenstrual syndrome (PMS) data were then analyzed with (1) Normality test by Shapiro Wilk, and (2) Hypothetic test by Pearson correlation used SPSS 11.0 for Windows computer program.Results of this study are (1) Premenstrual subjects are bigger amount than non-premenstrual subjects (88,1% vs 11,9%), (2) there is signifi  cant positive correlation between social introversion stage and premenstrual syndrome with strong correlation (r=0,551; P<0,05). The conclusion of this study is there is signifi  cant positive correlation between social introversion stage and premenstrual syndrome.Keywords: Social introversion, premenstrual syndrome

Aims: To evaluate D-Dimer serum levels in patients with chronic urticaria and its correlation with disease activity. Settings and Design: Single centre Cross sectional prospective observational age & sex matched case-control study at Dermatology OPD of a tertiary referral centre. Methods and Material: This study was conducted from January 2018 to June 2019. We in-cluded 33 patients with CU and 30 controls . They were recruited from urticaria clinic. All cases were subjected to history taking, general and dermatological examination. The serum levels of D-Dimer were measured by Semiquantitative, immunofiltration kits. Statistical analysis: Data was analysed by Statistical Package for Social Sciences (SPSS) ver-sion 21.0. Tests used were Independent t test/Mann-Whitney Test, Chi-Square test/Fisher’s Exact test, Spearman rank correlation coefficient, Kolmogorov- Smirnov test.. Results: Patients with active CU had elevated D-Dimer serum levels (p<0.0001) when com-pared with the control group (papulo-squamous disorder). Of 33 CSU patients, D-dimer level was elevated in 19 patients (57.58%). There was statistically significant positive correlation between disease severity (UAS7) and plasma D-dimer level (p <.0001, r =0.935). Conclusions: This study showed elevated D-dimer levels in more than half of Indian patients with CSU. There was a positive correlation between plasma D-dimer levels and the severity of disease activity. Investigation for plasma D-dimer level may be an alternative objective way to evaluate disease severity in patients with CSU. Limitations: Low sample size . Semi quantitative method was used instead of ELISA for D-Dimer.

Introduction: The Coronavirus Disease-2019 (COVID-19) has caused a serious threat to people’s mental health causing psychosomatic disorders such as panic disorder, anxiety, and depression. Aim: To find out the correlation between the occurrence of psychosomatic disorders and age during the COVID-19 lockdown phase. Materials and Methods: This cross-sectional, observational study was conducted in Department of Physiotherapy at RK University, Rajkot, Gujarat, India. Males and females of age ≥21 years, with an ability to understand and fill Google form were recruited for the study. The subjects were divided into four groups according to age 21-34 years, 35-54 years, 55-64 years, and 65 years and older. There were 250 participants in each group. The subjects were asked to fill the COVID-19 Peritraumatic Distress Index (CPDI). Data were analysed by using software Statistical Package for the Social Science (SPSS) version 20. The normality of data was checked by the Shapiro Wilk test. Data followed parametric type so; Pearson correlation test was applied to find out the correlation between age and CPDI score. Results: The mean age of the population was 49.24±18.14 years. The mean CPDI scale scores were 27.572, 31.948, 31.364, and 35.328, respectively from groups 1 to 4. There was a significant positive correlation (p-value=0.031) in group 4, while no significance was found in group 1 (p-value=0.074), group 2 (p-value=0.067), and group 3 (p-value=0.062) Conclusion: There was a significant positive correlation between age and psychosomatic disorder occurrence in the geriatric age group (65 years and older) in the Rajkot city.

Objective: To compare steroids with other treatment options like NSAIDS and other analgesics for management of postchikungunya arthritis based on visual analogue scale (VAS). Study Design: Comparative cross-sectional study. Place and Duration of Study: Department of Rheumatology, Liaquat National Hospital, Karachi, from Jan to Jul 2018. Methodology: A total of 112 patients were included in this study, of both genders. Patients with history of fever >38.5°C and polyarthralgia who had positive chikungunya IgM serology or PCR were included in this study. Patients fulfilling inclusion criteria and who were treatment naïve were given NSAIDs or other analgesics if contraindication to NSAIDs was present. Those who had received 2 weeks of NSAIDs were given a single dose of intramuscular steroids. Response to treatment was characterized by clinical examination according to visual analogue scale (VAS) taken at baseline, 2 weeks and 4 weeks. All analyses will be conducted by using SPSS-20. Results: Overall, 66 (58.9%) received steroids, 37 (33%) patients were given NSAIDs, and other forms of analgesics were prescribed to 9 (8%) patients. In nearly half of the patients, i.e. 57 (50.89%) partial response was seen, 13 (11.6%) showed complete response, while 42 (37.5%) had persistent arthralgia, which was statistically significant p-value <0.001. Conclusion: This study shows that improvement in symptoms is seen with a single dose of intra-muscular steroid and it helped bring down the VAS score.
