Background: Keloids and hypertrophic scars are difficult to treat. There are many modalities available with variable success rate. Triamcinolone is still the most commonly used drug but recurrence and partial response to this conventional treatment is still a major concern. Many studies have proved the role and efficacy of intralesional bleomycin and this study was done to assess the role of intralesional bleomycin in recurrent and residual lesions.Methods: The patients with recurrent lesion or residual lesions after the injection of triamcinolone were enrolled in the study. Patients received local infiltration of bleomycin. The injections were scheduled at monthly intervals for three consecutive months. They were followed at 1st, 2nd, 3rd, and 12th months. Scar assessment was done by calculating its volume, Vancouver scar scale (VSS) and visual analogue score (pain and pruritis). Side effects were also recorded. Final assessment was done at 1year for any recurrence.Results: Twenty-five patients completed the study. The most common location of scars was presternal (52%) and trauma was the most common cause in scars (20%). We observed the decrease of volume of 75.85% which was highly significant on statistical analysis (p=0.001). Similarly, statistically significant difference was noticed in VSS, VAS for pain and pruritis. No systemic side effects were noticed. No recurrence was noted at the end of 12 months.Conclusion: Intralesional bleomycin is found to be useful in management of recurrent or residual lesions. It can be a potential option in the management of recurrent and residual hypertrophic scars and keloids.

Introduction: Post-operative improved pain control of patients has made arthroscopy as a day care procedure, reducing patient’s expenses and hospital staffs work load. Various analgesic drugs have been administered into the joint following Arthroscopy. Intra-articular injection of morphine has been practiced in many centres all over the world. We aimed to compare the efficacy of intra-articular tramadol and morphine. Tramadol is an opioid drug with similar analgesic properties like morphine. Tramadol unlike morphine is readily available over the counter not being abused and has more favourable side effects. Methods: It was a prospectively randomized double-blind study in which sixty patients having elective arthroscopic surgery of the knee were randomized into two groups. Group A (Tramadol Group) received intra-articular tramadol 50mg and Group B (Morphine Group) received morphine 5mg in equivalent volumes. Post-operative pain using Visual Analogue Score (VAS) between 0 and 10, (0 no pain to 10 worst pain) requirement of first analgesic, and incidence of side effects were recorded postoperatively at intervals of 3,4,5,6 and 24 hours. Results: The assessment of VAS score among the two groups in 3,4,5,6 and 24 hours of IA injection showed a p value of 0.349, 0.807, 0.676, 0.271 and 0.163 respectively, suggesting non significant difference in two groups. There was statistically significant result (p=0.005) for request of first analgesia (Ibuprofen 400mg+ Paracetalmol 500mg) at 6 hours of IA injection with tramadol group, requiring less analgesics. There are no other clinically important differences between the groups, including preoperative duration of symptoms, postoperative pain scores and side effects irrespective of the diagnosis and the procedure performed. While clinical trials have now solidified the role of thrombectomy in emergent large vessel occlusive stroke, additional therapies are needed to optimize patient outcome. Using our previously described experimental ischemic stroke model for evaluating adjunctive intra-arterial drug therapy after vessel recanalization, we studied the potential neuroprotective effects of verapamil. A calcium channel blocker, verapamil is often infused intra-arterially by neurointerventionalists to treat cerebral vasospasm. Such a direct route of administration allows for both focused targeting of stroke-impacted brain tissue and minimizes potential systemic side effects. Intra-arterial administration of verapamil at a flow rate of 2.5 µl/min and injection volume of 10 µl immediately after middle cerebral artery recanalization in C57/Bl6 mice was shown to be profoundly neuroprotective as compared to intra-arterial vehicle-treated stroke controls. Specifically, we noted a significant (P ≤ 0.05) decrease in infarct volume, astrogliosis, and cellular apoptosis as well as a significant increase in neuronal survival and functional outcome over seven days. Furthermore, intra-arterial administration of verapamil was well tolerated with no hemorrhage, systemic side effects, or increased mortality. Thus, verapamil administered intra-arterially immediately following recanalization in experimental ischemic stroke is both safe and neuroprotective and merits further study as a potential therapeutic adjunct to thrombectomy.

Background: Osteoid osteoma is a benign bone tumor; diagnosed using x-ray and Computer Tomography (CT). It shows a nidus and cortical thickening. When the nidus is not well visualised especially in uncommon locations; Bone Scan (BS) can be performed for evaluation of osteoid osteoma.Methods: A retrospective observational study was done where 21 subjects presenting with suspicion of osteoid osteoma underwent BS using 10-20mCi (370 to 740MBq) of Technetium-99m Pertechnetate with perfusion, delayed cortical and Single Photon Emission Tomography/Computer Tomography (SPECT/CT). Increased perfusion and delayed focal cortical uptake was assessed on BS to locate osteoid osteoma. Response evaluation to Radiofrequency ablation (RFA) was also performed using BS.Results: Osteoid osteoma was detected in femur (8), spine (3), forearm bones (2), humerus (2), tibia (2), fibula in one and iliac bone in one patient using BS. BS detected more lesions (18) than CT scan where nidus as confirmatory sign was seen in 13 lesions. The McNemar test showed no significant difference (p=0.22) in the detection of osteoid osteoma using CT and BS in common location of femur. However there was significant difference noted between BS and CT in uncommon site (p=0.023). 3/8 patients showed persistent increased cortical activity after RFA ablation on BS.Conclusions: BS was more useful for confirmation of diagnosis of Osteoid osteoma for lesions in uncommon sites. BS also helped to assess response to RFA ablation therapy. SPECT/CT improved interpretation of BS to locate the osteoid osteoma.

The aim of the present study is to investigate the positive role of Allicin in reducing the side effects induced by cyclophosphamide drug in reproductive system of male rats. In this experiment, fifty adult male Wister rats were used, about four month old, with average weight about (162.5±13gm). They were divided randomly into five equal groups (10 animals for each group) and drenched for 60 consecutive days as follows: The results of this study revealed significant differences (P ≤ 0.05) represented by increasing in concentration of testosterone, SSH and ICSH hormones in T1 group as compared with other groups, while there was a significant decrease in T2 group as compared with other groups. Also, there was a significant difference represented by increasing levels of these hormones in T3 and T4 groups as compared with T2 group ,and there was a significant difference in concentration of testosterone, SSH and ICSH between T3 and T4 groups represented by increasing the concentration of these hormones in T4 group as compared with T3 group. It is concluded that allicin has in dose of 50 mg/kg/B. W. both a preventive and a therapeutic role in ameliorating cyclophosphamide toxicity in adult Wister male rats. The use of allicin as a therapeutic agent showed more amelioration in cyclophosphamide toxicity than a preventive agent.

Abstract: The control and planning of the treatment of hypertensive patients need specific attention. As regards concomitant diseases and treatments, glaucoma and the use of eye drops should be taken into consideration. The ingredients of the administered eye drops get through the nasolacrimal canal and can be absorbed by the nasal mucosa. Because of the lack of enterohepatic ‘first pass’ effect, they can act systemically – like after intravenous administration. This way they can cause systemic side effects. The authors present a case of a patient, too, who was examined and medically checked regularly for years with negative results because of repeated syncope. It became clear only at the Hypertension Centre that the timolol-containing combined eye drops caused the symptoms. The authors draw attention to the fact that in the case of systemic side effects which can be connected to beta-blocking agents (blood pressure fall, bradycardia, breathing disturbance, depression), the role of the eye drops should be taken into consideration. At the same time, the possibility of the systemic drug interactions should not be forgotten either. The interaction with dihydropyridine-type calcium-channel blockers can be of great importance. In these cases, after consultation with an ophthalmologist, the glaucoma treatment with eye drops containing beta-blockers should be modified. Orv Hetil. 2019; 160(8): 309–313.

Sustainable Development (SD) is a rich, challenging and thought-provoking construct in social sciences. The main purpose of this paper was to identify and explore the role played by primary school teachers in building up the idea of sustainable development (SD) among students. This paper was intended to identify that how a teacher can successfully execute the concept of SD by influencing students’ minds at the primary level. Quantitative survey technique were utilized for data collection. All the primary school teachers of Lahore division comprised the population of the study. Through multistage sampling technique, 352 primary school teachers were selected as participants of the study. A self-developed SD questionnaire incorporating four major factors (teachers’ awareness, pedagogy, curricular and co-curricular activities) with Cronbach’s alpha value = .93 was used to measure the role of teachers in building the sustainability concept among students at primary level. The results indicated a significant mean score difference among SD scores of teachers, sector wise (private and public). Furthermore, the results also reconnoitered the significant difference (p=.04) between the mean scores of female and male teachers in building up the SD concept in students’ minds.

Introduction: The role of virgin coconut oil in the treatment of allergic rhinitis is controversial. Thus, the aim of the present study is to determine the effects of virgin coconut oil ingestion, in addition to standard medications, on allergic rhinitis. We also studied the side effects of consumption of virgin coconut oil. Methods: Fifty two subjects were equally divided into test and control groups. All subjects received a daily dose of 10mg of loratadine for 28 days. The test group was given 10ml of virgin coconut oil three times a day in addition to loratadine. The symptoms of allergic rhinitis were scored at the beginning and end of the study. Results:, the symptom score were divided into nasal and non-nasal symptom scores. Sneezing score showed a significant difference, however the score was more in control group than test group, indicating that improvement in symptom was more in control group. The rest of the nasal symptom and non-nasal symptom score showed no significant difference between test and control groups. Approximately 58% of the test subjects developed side effects from consumption of virgin coconut oil, mainly gastrointestinal side effects. Conclusion: In the present study, ingestion of virgin coconut oil does not improve the overall and individual symptoms of allergic rhinitis, furthermore it has side effects.

ABSTRACT Toilet training is an effort to train children to be able to control and urinate (BAK) and defecate (BAB). Toilet training is one of the main tasks of children at toddler age. Toilet training is one of the main tasks of children in toddler age which is very important to be done to create independence in children in controlling BAK and BAB and children know the parts of the body and their functions. Data in 2012 shows that ± 60% of parents do not teach toilet training to children from an early age. The aim of the study was to find out the effectiveness of toilet training education on maternal behavior and toilet skills in toddler age training (18-36 months). The study was conducted in July-August 2018. This type of quantitative research used the design of the Quasy pretest and posttest experiment with non-equivalent control group design. Samples were 36 mothers and 36 children with purposive sampling technique. Data analysis used Paired t test, Wilcoxon test, Man-Whitney test an Independent t test. The results showed that toilet training education through lecture methods, modules and maze games was more effective than toilet training education through lecture and leaflet methods on children's knowledge and abilities. Conversely, for the role of mothers in supervision there is no significant difference in effectiveness. Health education is recommended in health promotion programs to increase maternal knowledge, the role of mothers and the ability of toilet training children independently. The present study was aimed to study the effect of Sublingual Vitamin D3 on Serum Vitamin D level in Vitamin D deficiency patients. This was a cross-sectional and interventional study. All the Vitamin D deficiency patients of age 18-60years and either gender, willing to participate in the study were included. Patients who had greater than 20 ng/ml were excluded from the study. The total number of participants in our study was 200, out of these 111 males and 89 females, the mean age in our study was 51.07 ± 7.39Yrs. All volunteers were given sublingual vitamin D3 (60,000IU) in six doses every fifteen days of follow up for 3 months. The subject’s serum 25(OH)D levels were estimated before and after the treatment of sublingual vitamin D3. There was a statistically significant difference in serum vitamin D3 level before 16.61±6.71 ng/ml and after 35.80±7.80 ng/ml after treatment with Sublingual Vitamin D3. Six doses of 60,000IU of Vitamin D3 sublingual route having improved the role of serum 25(OH)D levels in the treatment of Vitamin D3 deficiency patients.Keywords: Vitamin D3; Sublingual route
