Renal biopsy tissues were taken from 142 suspected glomerulonephritic patients who were admitted into the Department of Nephrology of Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka and Combined Military Hospital (CMH), Dhaka Cantonment, Dhaka. The tissues were processed for both Light Microscopy (LM) and Direct Immunofluorescence (DIF) studies. The study was done in the Department of Anatomy, Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka and Armed Forces Institute of Pathology (AFIP), Dhaka Cantonment, Dhaka, from March to December 1999. Seven histopathological types of glomerulonephritis were identified with LM and another one type i.e. IgA Nephropathy was identified exclusively by using DIF. Diffuse immunofluorescence positivity was found in 44.36% cases. C3 components were found in all cases irrespective of the histopathological type of glomerulonephritis. Immune complex deposits were observed in immunofluorescence both in the mesangium and the glomerular basement membrane (GBM) with more generalized and less scattered distributions. Immunoglobulins (Ig) were tested for IgG, IgA and IgM. IgG was found the most common (74.60%) among immune complex deposits. Notable LM features include proliferation of mesangial cells, expansion of mesangial matrix, thickening of GBM, infiltration of glomerular macrophages, platelets and neutrophil and crescent formation. The presence of IgG in the mesangium of the kidney of the glomerulonephritic patient suggests a role of IgG in the inflammatory process. There is also evidence that C3 is synthesized within the glomeruli of the patients with glomerulonephritis. Finding the role of the complement components in pathogenesis of glomerulonephritis, a keen observation is needed to determine the extent of local complement synthesis and their involvement in tissue injury process. Key words: Complement synthesis; immune complex; glomerulonephritis; renal biopsy. DOI: http://dx.doi.org/10.3329/jdmc.v20i1.8566 J Dhaka Med Coll. 2011; 20(1) : 15-19

The paper presents the results of a study of reflexive techniques impact on the educational motivation of medical university students. In the context of the implementation of the humanistic paradigm in pedagogy, the role of motivation in educational activity regulation is analyzed. Reflexion is considered by the subjects of pedagogical process as a factor of facilitation of "objectification" of needs in the context of educational activity and construction of its subjective sense. It was found that for ISMA students professional motives are dominant in educational motivation and avoidance motives are the least declared. As a result of a quasi-experiment with two non-randomized groups, the impact of reflexive techniques on educational motivation was assessed. The techniques made it possible to keep the structure of educational motivation unchanged in the experimental group, while in the control group a decrease in professional motives and an increase in communicative motives were found.

Abstract Background IgA Nephropathy (IgAN) is one of the most common glomerular diseases. However, the effect of uric acid on the prognosis of IgAN is still unclear, especially between males and females. Therefore, the present study aimed to explore the role of hyperuricemia in IgAN patients and the differences in gender. Consequently, the study conducted a retrospective analysis of the prognosis of IgAN in patients from the General Hospital of Tianjin Medical University. Methods: A total of 1,022 patients with primary IgAN, diagnosed through renal biopsy were enrolled from the Department of Nephrology, General Hospital of Tianjin Medical University. However, after applying the exclusion criteria, only 463 patients remained and were regularly followed up in the hospital. In addition, the relationship between Time Average Uric Acid (TA-UA) and prognosis of IgAN was analyzed. Results: The findings showed that TA-UA was an independent risk factor for the prognosis of IgAN in the 463 patients studied (OR=1.752,95%CI=1.348-2.277,p<0.001). Additionally, higher TA-UA values were associated with a faster progression of disease and worse prognosis (p < 0.05). Elevated TA-UA was also shown to be an independent risk factor for the progression of disease in female patients but not in their male counterparts (women: HR=1.998,95%CI=1.398-2.854,p<0.001,men: HR=1.405,95%CI=0.869-2.274, p=0.166). Conclusion: Increased TA-UA is an independent risk factor for the progression of IgAN, especially in women.

Abstract Background: Human papillomavirus (HPV) testing is more sensitive than cytology for detecting cervical cancer and its precursors. This study aimed to analyze the prevalence of high-risk HPV genotypes and evaluate the role of HPV genotyping triage for detecting high-grade squamous intraepithelial lesions, adenocarcinoma in situ and cervical cancer (HSIL+) in HPV-positive women with normal cytology. Methods: A retrospective study was performed in women who had undergone co-screening at the China Medical University-affiliated Shengjing Hospital between 2012 and 2014. Results: Of the 34,587 women, 2,665 HPV-positive women with normal cytology who had received colposcopy were eligible for analysis. In HSIL+ groups of 204 women, the common genotypes were HPV16, HPV52, HPV58, HPV33, HPV31 and HPV18 in order of prevalence. The proportion of histological HSIL+ in women infected with HPV33 or HPV31 was not significantly different compared to women infected with HPV16 (P= 0.30, P= 0.19, respectively). The odds ratios for histological HSIL+ were 3.26 (95% confidence interval [CI]: 2.41–4.40) in women with HPV16/18, 4.21 (95% CI: 2.99–5.93) in those with HPV16/18/31/33, and 5.73 (95% CI: 3.30–9.97) in those with HPV16/18/31/33/52/58. Including HPV31/33 genotyping together with HPV16/18 significantly increased the proportion of HSIL+ detection from 63.2% to 77.5% (P = 0.002) without significantly increasing the colposcopy per HSIL+ detection ratio (7.7 to 8.1, P = 0.66). Conclusion: HPV genotyping played an important role in managing HPV-positive women with normal cytology. Genotyping for HPV31/33 should be added to the previously recommended HPV16/18 genotyping in triaging HPV-positive women in northeastern China.
