Theoretical Approaches to Ensuring Effective Interaction of State and Non-State medical institutions in the Format of Public-Private Partnership

Expanding the sphere of interaction between the state and the medical business is one of the priorities for the development of the medical services market in the Russian Federation. The healthcare reform, based on the principles of evidence-based medicine, digital healthcare, and high medical technologies, is primarily aimed at improving the quality of medical services and the availability of highly qualified medical care for all segments of the population. Accessibility, quality and timeliness of medical care are the main postulates of the daily activities of a private medical institution. during the period of increasing crisis phenomena in the economy and social sphere, private medicine can provide significant assistance to the state health service, become the “lifesaver” for the domestic health protection system, which today is mostly represented by the public sector. One of the most effective mechanisms of interaction between the state and private business, adopted all over the world, is the system of public-private partnership. The purpose of the study is to analyze possible forms and mechanisms of interaction between the medical business and the state within the framework of public-private partnership. M a t e r i a l s a n d m e t h o d s . The assessment of the current regulatory framework was carried out, a significant volume of domestic and foreign literary publications was analyzed, the main theoretical hypothesis was formulated on the basis of the systematic scientific method and a research plan was built. R e s u l t s . In the course of the conducted research, objective main cause-and-effect factors affecting the development of an adequate system of public-private partnership in the field of medicine and healthcare have been identified and analyzed, in terms of the distribution of benefits, costs and risks carried out on the principles of long-term, voluntary, manageability, legal validity between a private medical organization and federal and/or municipal executive authorities. The main causal factors include insufficient funding, an unfavorable demographic situation, low efficiency of using the resource base, a reduction in the bed fund under the guise of “optimization”, a decrease in the social level of the population and the redistribution of the medical and diagnostic structure due to the COVID‑19 infection pandemic. It should be recognized that the state system of medical care is not always able to fully meet the needs of societies for high-quality qualified medical care. The analysis of foreign literature has shown that medical services abroad successfully use the principle of public-private partnership, redistributing efforts to form the necessary institutional environment in healthcare between private business and the state. Such a form of interaction as concession, leasing, franchise, outsourcing, aufstaffing allows us to optimally balance the interests of the medical business and public health, significantly optimize the budget costs for health protection, especially in a difficult epidemiological period. The result of the research was the development of a conceptual and theoretical model of various forms of interaction between private medical business and government agencies within the framework of public-private partnership programs. C o n c l u s i o n s . Thus, public-private partnership is an effective economic tool that ensures the coincidence of the interests of private medical business and the state, allowing to make a profit for both subjects of medical activity, with a reduction in state budget expenditures for healthcare purposes, but with the preservation of high quality medical services and resource provision.

The role of the state in the regulation of natural monopolies in the developed and transition economies is also analyzed. With reference of crisis’s deepening processes at the present stage of national economy’s development there is emerge a reduction in investment by both the private sector and public institutions. At the same time, one of the important components of creating a favorable climate for Ukraine’s economic growth is the development of infrastructure. Up today a set of problems related to the fixed assets` obsolescence in almost all areas of economic activity, physical and moral deterioration of equipment, lack of investment in infrastructure, lack of budget funding for infrastructure investment and innovation projects, etc. Consequently, there is a need to find qualitatively new tools and mechanisms for investment development of Ukraine’s economy, also forms and methods of investment interaction between the state and business based on public-private partnership (hereinafter – PPP). The effective interaction between the state and the private sector in PPP together with well-organized risk management system will allow investing in the development of production capacity, accelerate industrial growth, expand domestic and foreign markets, improve the quality of goods, works and services, improve public services, improve investment attractiveness and business activity. The research in the article is devoted to public-private partnership, which arises as a result of partnership between the state and business. PPP today is one of the qualitatively new tool and mechanism for investment development of Ukraine's economy. The world practice of PPP projects’ application, their most widespread types in different countries is investigated in the work. At present PPP is quite widely, especially in Europe, in the implementation of socio-economic tasks, such as ensuring effective governance in the field of PPP, reducing burden on the budget, strengthening the social responsibility of business, improving the quality of life of the country`s population, etc.

In the modern economy, there are problems associated with investing in infrastructure development. Its facilities are public goods. Therefore, private business is not interested in their creation and development. And the state does not have sufficient resources to solve the problems of infrastructure development. Therefore, the mechanism of interaction between the state and business on the basis of public-private partnership (PPP) has spread in this sphere. The analysis shows that the forms of PPP depend on the country (region) and industry. Therefore, increasing the effectiveness of PPP, as the authors have shown, should be based on the specificity of projects. The article analyzes the experience and perspectives of PPP development in the Russian transport industry. The authors concluded that the interaction of power and private capital is the most effective mechanism in modern conditions for the creation and development of transport infrastructure in Russia. At the same time, the article focuses on the financial component of PPP in transport.

The article is devoted to the study of theoretical and methodological aspects of public-private partnership in the field of capital construction. In the context of globalization and the development of European integration processes, the need to improve the quality of life of the population and the introduction of European and world standards of life support makes the actual problem of improving the efficiency of housing and communal services and capital construction. The article defines that public-private partnership in today's conditions is one of the most progressive mechanisms of effective cooperation between the state (territorial community) and private business, which is carried out in order to increase the effectiveness of their functions by state and municipal authorities. Thus, based on the analysis of domestic practice of public-private partnership, the promising experience in implementing partnership projects of local governments and the private sector on the example of Ivano-Frankivsk is separated. In particular, the main aspects of cooperation between the local authorities of Ivano-Frankivsk and NEFCO in the implementation of energy saving programs are presented. It is noted that the basis of public-private partnership, as a rule, is the financial relationship between public authorities and representatives of private business. That is, it is about investing private capital in the development of state and communal property, necessarily on mutually beneficial terms. It is obvious that in the current conditions the implementation of projects in the field of capital construction by local authorities is practically impossible without the involvement of private partners in this process. At the same time, there are a number of factors and obstacles that in practice significantly complicate the process of establishing effective cooperation between the state and business. It is substantiated that public-private partnership is a kind of mechanism that provides certain benefits simultaneously for all its participants: the state, private business and the community. However, it has been proven that, as in any other economic relationship, there are some risks in a public-private partnership that are likely to create a threat to each party to the partnership.

Introduction: The article discusses the problems of joint activities of public and private sectors to ensure the proper quality of medical services and jointly overcome financial and production risks. It is emphasized that if the financial and administrative issues of such interaction have been studied to a certain extent, then a quantitative assessment of the quality of medical care provided in a public-private partnership has so far raised questions The aim: Identify ways to use industrial quality criteria for providing medical care to patients in public-private partnership conditions. Materials and methods: The results of rehabilitation of 300 patients with gastrointestinal pathology who underwent rehabilitation in the resort “Kvitka Poloniny” of the TOV “Suzirya” were studied. 160 patients underwent rehabilitation in the private part of the resort, 140 – in the state. Results and conclusions: The theoretical approaches to the use of scientific and technological tools for the implementation of public-private partnership in medical institutions, in particular in a rehabilitation center, are discussed. They are based on instrumental approaches to assessing the quality of medical care. The use of Pareto assessment techniques and the Taguchi loss function allows us to quantitatively characterize the improvement in the quality of care provided with the help of public-private partnership.

The article examines the nature and world experience of public-private partnership (PPP). The advantages of PPP for the state and for private business, types of contacts and the most popular spheres of application of public-private partnership are considered. The essence of concession is considered as one of the most widespread types of public-private partnership, as well as features of concession contracts. Prospects of application of concession relations in Russia are analyzed. The article analyzes the conditions of occurrence and the main stages of development of partnerships between the state and the private sector. The features of functioning of public-private partnership (PPP) in developed and developing countries are investigated. The world experience of PPP projects implementation is summarized and practical recommendations on the establishment and development of the public-private partnership institute in Russia are presented.

The article deals with the state of public-private partnership in the field of waste management. The concept of waste was given, and the importance of managing the waste management system was justified, taking into account the resource orientation of many regions of the country. Legislative changes forming a new system of waste management are reflected. The author presents the dynamics of growth of waste generation, as well as their utilization and disposal in Russia. The main areas of expenditure related to the implementation of projects in the field of waste management were Identified. The problems of implementing these costs in the implementation of projects are highlighted, first of all, the lack of sources of funding for both the state and private business. The concepts of public-private partnership and the mechanism of its implementation, given by different authors, are considered. It was found that cooperation between the state and the private sector in this area has not yet been sufficiently developed, and only in the field of solid waste management. Projects in the field of production are not implemented. It is noted that the main initiator of the projects is the state, private business is not sufficiently interested in implementing projects in the field of waste management in the form of public-private partnership. The main forms of implementation of PPP projects, stages of implementation of projects and their degree of development in the Federal districts of the Russian Federation are considered. It was revealed that the projects do not use debt financing, which indicates a weak interest of banks in this sector of the economy in view of significant investments, long payback period of projects and the risks of their implementation.

Public-private partnership (PPP) in the conditions of the set strategic tasks by the President of the Russian Federation until 2030 can become one of the sources of attracting financial resources for implementation of the large-scale projects. In this regard, it is relevant to systematize the forms of PPPs and the scope of their application. Based on a study of different views on the essence of PPP, as well as taking into account the development of public administration in Russia, the author proposed the use of public-public-private partnership as a form of development of cooperation between the state, private business and society. The polyformism of PPPs is reflected in the presented classification. Based on the analysis of PPP development in the regional context, hypothesis on the correlation between the level of PPP and the socio-economic development of the subject of the Russian Federation was confirmed.

THE road to better child health has been discussed in relation to the doctor and his training, health services and their distribution. We have dealt with the unavoidable question of costs. Particular attention has been given to some of the advantages and dangers of decentralization of pediatric education and services. Each of the various subjects has been discussed from the point of view of its bearing on the ultimate objective of better health for all children and the steps necessary to attain this goal. Now, we may stand back from the many details of the picture, view the whole objectively and note its most outstanding features. First is the fact that the improvement of child health depends primarily upon better training for all doctors who provide child care, general practitioners as well as specialists. This is the foundation without which the rest of the structure cannot stand. The second dominant fact is the need for extending to outlying and isolated areas the high quality medical care of the medical centers, without at the same time diluting the service or training at the center. The road to better medical care, therefore, begins at the medical center and extends outward through a network of integrated community hospitals and health centers, finally reaching the remote and heretofore isolated areas. Inherent in all medical schools is a unique potential for rendering medical services as well as actually training physicians. The very nature of medical education—whereby doctors in training work under the tutelage of able specialists in the clinic, hospital ward, and out-patient department—provides medical services of high quality to people in the neighboring communities.
