Background: The development and pathogenetic substantiation of the new agents used for local therapy of thermal trauma (TT) is an urgent problem in medicine. Melatonin (MT) is an endogenous factor of homeostasis regulation with pleiotropic potential. The aim of our study was to assess the morphology, expression of matrix metalloproteinase-9 (MMP-9) and vascular endothelial growth factor (VEGF), indicators of repair, oxidative destruction of lipids in the skin lesion focus in the dynamics of experimental TT under the conditions of using the original dermal film (DF) with MT. Methods and Results: The experiment was performed on 104 male Wistar rats weighing 200-240 g. For modeling TT II degree according to ICD-10 a relative area of 3.5% of the body surface, an interscapular region isolated from the surrounding tissues, was immersed in distilled water at 98-99 °C for 12 sec. DF based on sodium carboxymethylcellulose with an area of 12 cm2 with MT at a concentration of 5 mg/g was applied daily for five days. The wound area and epithelialization rate were calculated. The content of MMP-9 and VEGF in the burn wound was assessed by an immunohistochemical method. In the homogenate of the burn wound, the content of LPO products was assessed. Morphological and biochemical studies were performed on Days 5, 10 and 20 after TT induction. With experimental TT from Day 5 to Day 20, the absolute area of the burn wound decreases by 35%, the rate of epithelialization increases, the number of neutrophils in the focus of thermal damage decreases, while the representation of lymphocytes, histiocytes, and fibroblasts increases; the expression of MMP-9 and VEGF increases; predominantly secondary and final LPO products in the heptane phase accumulate, the final products of LPO in the isopropanol phase of the lipid extract. The use of MT in the composition of DF daily for 5 days with experimental TT leads to a decrease in the area of the wound defect (by 46% of the original area on Day 20), an increase in the rate of its epithelialization, an increase in the content of lymphocytes and fibroblasts in the burn wound on Days 5, 10 and 20 of TT, a decrease in the representation of neutrophils and macrophages on Days 5 and 10, as well as an increase in VEGF expression on Days 5 and 10, MMP-9 - on Day 5 and a decrease in MMP-9 expression on Days 10 and 20 of TT. In addition, the use of MT in the composition of DF leads to a decrease in the content of predominantly secondary and end products of LPO in the heptane and isopropanol phases of the burn wound on Days 10 and 20 of TT. Correlation analysis revealed that a decrease in the burn surface area under a local application of MT occurs with an increase in the content of VEGF in the wound area and a decrease in the content of MMP-9 and secondary and final LPO products in the heptane phase and the isopropanol phase. On Day 20, there were direct moderate correlations between the absolute burn surface area, on one hand, and secondary and final LPO products, on the other, in the heptane phase (R=0.51, R=0,68; P<0.05) and the isopropanol phase (R=0.44, R=0.46; P<0.05), respectively. Conclusion: The results obtained expand the existing understanding of the role of changes in the expression of MMP-9 and VEGF in the pathogenesis of TT. We believe that the repair-stimulating effect of MT in the DF, which we established during TT at the preclinical stage, is associated with the LPO-limiting effect of MT and a change in the expression of MMP-9 and VEGF in the burn wound and is a prerequisite for further study of the mechanism of action and the effectiveness of MT application in clinical conditions in TT.

Разработка и патогенетическое обоснование новых подходов к локальной терапии термической травмы (ТТ) является актуальной и востребованной проблемой. В частности, представляет интерес разработка дермальных пленок (ДП), содержащих эндогенные регуляторы гомеостаза мультитропного действия. Цель исследования - оценка эффекта мелатонина (МТ) в составе оригинальной ДП на процессы репарации и содержание продуктов перекисного окисления липидов (ПОЛ) в коже очага повреждения при локальной термической травме. Методика. Эксперимент выполнен на 126 крысах-самцах Wistar. ТТ ІІІА степени площадью 3,5% моделировали погружением участка межлопаточной области кожи в очищенную воду с температурой 98-99 ºС на 12 с. МТ в составе ДП (0,005 г/г) на основе натрия карбоксиметилцеллюлозы наносили ежедневно после ТТ в течение 5 сут. На 5-е и 10-е сут после ТТ оценивали макроскопическую картину, площадь и глубину ожоговой раны, скорость ее эпителизации. Содержание продуктов ПОЛ в гомогенате кожи ожоговой раны определяли экстрационно-спектрофотометрическим методом в гептановой и изопропанольной фазах липидного экстракта. Результаты. Установлено, что накопление вторичных и конечных продуктов ПОЛ в гептановой и изопропанольной фазах липидного экстракта на 5-е и 10-е сут ассоциировано с площадью ожога. Применение оригинальной ДП с мелатонином приводит к снижению абсолютной и относительной площади ожога, увеличению скорости эпителизации ожоговой поверхности. На 5-е сут обнаружено снижение содержания вторичных и конечных продуктов ПОЛ в изопропанольной фазе, на 10-е сутки - снижение вторичных продуктов ПОЛ в гептановой фазе, конечных продуктов ПОЛ в изопропанольной фазе липидного экстракта. Заключение. Полученные результаты демонстрируют ускоряющий репарацию кожи в очаге ТТ эффект МТ в составе дермальной пленки за счет его ПОЛ-ограничивающего действия, расширяют представления о мультитропных эффектах МТ в организме и являются предпосылкой для применения ДП с МТ в клинической практике. Methods. Experiments were performed on 126 Wistar male rats. Grade IIIA TT with an area of 3.5% was modeled by immersing a section of interscapular skin in purified water at a temperature of 98-99oC for 12 s. MT formulated into DF (0.005 g/g) based on sodium carboxymethylcellulose was applied after TT daily for 5 days. The macroscopic picture, area and depth of the burn wound, and the wound epithelization rate were evaluated on days 5 and 10 after TT. Concentration of LPO products in the injured skin homogenate was measured by extraction spectrophotometry in heptane and isopropanol phases of the lipid extract. Results. The accumulation of secondary and final LPO products in the heptane and isopropanol phases of the lipid extract on days 5 and 10 was associated with the burn area. The use of the original DF with MT resulted in a decrease in the absolute and relative areas of the burn and an increase in the rate of burn surface epithelialization. On day 5, a decrease in the content of secondary and final LPO products in the isopropanol phase was observed, and on day 10 decreases in secondary peroxidation products in the heptane phase and end LPO products in the isopropanol phase were detected. Conclusion. The results of this study demonstrated that MT formulated into DF accelerates skin repair in the TT focus due to its LPO-limiting effect, expands the understanding of MT pleiotropic effect, and represents a prerequisite for the clinical use of DF with MT.

Cauterization (or cautery) is medical practice or technique of burning a part of the body to remove or close off a part of it. As after cauterization, the wound which is caused is a type of burn wound. As per Ayurvedic text, Dhatakyadi yog has been used as a local application for burn wound. The present study entitled,” To study the efficacy of Dhatakyadi yog in the local application on post-op fissurectomy cauterized wound” and aims & objectives were decided according to the present study. a single case study was done. The clinical assessment was done on the basis of grading criteria with specific symptomology of patients like pain, swelling, discharge. It can be concluded that patient applying Dhatakyadi yog in Cauterized wound is quite effective in reducing the pain, swelling, discharge. And as per Ayurveda it causes shodhan and ropan effect on wound.

The relationship between body height and body weight has been described by using various terms. Notable among them is the body mass index, body surface area, body shape index and body surface index. In clinical setting the first descriptive parameter is the BMI scale, which provides information about whether an individual body weight is proportionate to the body height. Since the development of BMI, two other body parameters have been developed in an attempt to determine the relationship between body height and weight. These are the body surface area (BSA) and body surface index (BSI). Generally, these body parameters are described as clinical health indicators that described how healthy an individual body response to the other internal organs. The aim of the study is to discuss the use of BSI as a better clinical health indicator for preclinical assessment of body-organ/tissue relationship. Hence organ health condition as against other body composition. In addition the study is `also to determine the best body parameter the best predict other parameters for clinical application. The model parameters are presented as; modeled height and weight; modelled BSI and BSA, BSI and BMI and modeled BSA and BMI. The models are presented as clinical application software for comfortable working process and designed as GUI and CAD for use in clinical application.

The aim of this study was to investigate the clinical efficacy of vanadium complexes on triglycerides (TG), total cholesterol (Chol), uric acid (UA), urea (U), and antioxidant parameters: nonenzymatic (FRAP—ferric reducing ability of plasma, and reduced glutathione—GSH) and enzymatic (glutathione peroxidase—GPx, catalase—CAT, and GPx/CAT ratio) activity in the plasma of healthy male Wistar rats. Three vanadium complexes: [VO(bpy)2]SO4⋅2H2O, [VO(4,4′Me2bpy)2]SO4⋅2H2O, and Na[VO(O2)2(bpy)]⋅8H2O are administered by gavage during 5 weeks in two different diets such as control (C) and high fatty (F) diets. Changes of biochemical and antioxidants parameters are measured in plasma. All three vanadium complexes statistically decrease the body mass growth in comparison to the control and fatty diet. In plasma GSH was statistically increased in all vanadium complexes-treated rats from control and fatty group in comparison to only control group. Calculated GPX/CAT ratio was the highest in the control group in comparison to others.

A ternary ontological model in which the living being is a triad of I – form – substrate is described. I is an intangible subject, contemplating the content of consciousness and controlling the material body, which is the unity of the form and the substrate. The contents of consciousness are connected both with the form of the body, which I contemplate in the inner “mental space” in the form of in­formation, and with the substrate, which embodies the forms of the body and is responsible for sensations and intentions. The problem of control of the material body by the non-material self is solved under the assumption that the human brain is a quantum object. The ternary model of a living being is inscribed in an absolute ontology, in which the Absolute also has a threefold structure and is the unstitched unity of the absolute I, the absolute Form and the absolute Sub­strate. The Absolute creates the other world with its threefold energies, which provides the threefold structure of a living being. The created world arises from the timeless world of the potential possibilities of the Universe, which modern cosmology associates with its wave function. Created entities arise in the process of alienation from the Absolute, resulting in free will.

The vibration suppression of the proposed pitch-resistant hydraulically interconnected suspension system for the tri-axle straight truck is investigated, and the vibration isolation performances are parametrically designed to achieve smaller body vibration and tire dynamic load using increased pitch stiffness and optimized pressure loss coefficient. For the hydraulic subsystem, the transfer impedance matrix method is applied to derive the impedance matrix. These hydraulic forces are incorporated into the motion equations of mechanical subsystem as external forces according to relationships between boundary flow and mechanical state vectors. In terms of the additional mode stiffness/damping and suspension performance requirements, the cylinder surface area, accumulator pressure, and damper valve’s pressure loss coefficient are comprehensively tuned with parametric design technique and modal analysis method. It is found the isolation capacity is heavily dependent on installation scheme and fluid physical parameters. Especially, the surface area can be designed for the oppositional installation to separately raise pitch stiffness without increasing bounce stiffness. The pressure loss coefficients are tuned with design of experiment approach and evaluated using all conflict indexes with normalized dimensionless evaluation factors. The obtained numerical results indicate that the proposed pitch-resistant hydraulically interconnected suspension system can significantly inhibit both the body and tire vibrations with decreased suspension deformation, and the tire dynamic load distribution among wheel stations is also improved.

Intoduction: Acute and chronic exposure to toluene at high doses is known to affect all organs of the body including the spermatogenesis process. In the industrial sector, the use of toluene as a solvent is still widely used, up to 10 million tons per year. The control over health problems that may occur is carried out by applying work exposure threshold values. This research aims to explore the effect of toluene exposure at the threshold value range on spermatogenesis.Method: This research used laboratory experiment on 30 male Wistar rats which were divided into five groups of different exposure levels, namely 12.5 parts per million (ppm], 25 ppm, 50 ppm, 100 ppm, and no exposure (control). Exposure was given for 4 hours daily over 14 days through a hood with measured release in the glass cage. The toluene exposure markers observed were Malondialdehyde (MDA) in the blood tissue and testicles using the Thiobarbituric Acid Reactive Substances (TBARS) method. The effect on the spermatogenicity process was assessed by counting the spermatogonia A cells of male Wistar rats with Periodic Acid Schiff (PAS) staining and is calculated by the Abercrombie formula. Analysis of the correlation between the level of exposure and its effect on the increase in malondialdehyde, and spermatogenesis was carried out using the Spearman correlation analysis.Result: There was a moderately positive correlation between levels of toluene exposure and plasma MDA levels (r = 0.42; p = 0.025). Meanwhile, on [the issue of] the quantity of spermatogonia cells, a high level of negative correlation with exposure levels was obtained (r = -0.68; p = 0.001).Conclusion: Toluene exposure in male Wistar rats within the range of threshold values influenced the increase in plasma MDA levels and decreased the Spermatogenia A cells. However, toluene exposure did not affect the testicular MDA levels of male Wistar rats.

This articles describes the “neuronal crisis,” the epidemic of psychosomatic illnesses observed all over the world, particularly in the West. The paper looks into the deeper real causes and seeks the most effective kind of cure for this malady. This leads to rational consideration of the metaphysical dimension of the human being and the fundamental problems (those of evil, of freedom, of God, of the soul, and of the body), where lack of sufficiency plays a major part in the etiology of these pathologies, as the desire for the Absolute is the basis of the unconscious. This approach presumes the Freudian model but denies its purely libidinal interpretation that substitutes desire for the Absolute with libido. Hence, an explanatory system applied to increasingly serious pathologies: ailments, neuroses, depressions, and psychoses. Frustration of one’s desire for the Good gives rise to a sublimation of finite goodness. The inevitable desublimation, caused by anguish because of the Evil, intense guilt, and the dramatization of evils, causes neuroses as awkward but inevitable solutions to the existential problem that is still unresolved, due to lack of functional and experimental knowledge. Psychiatry and even medicine must take into account the metaphysical layer, and, therefore, operate within an existential dynamic, aiming to progress in wisdom and to discover man, man’s brain and body, as these are structured around the axis of his desire.
