Abstract Background: Digital optic disc images are integral in remote telehealthcare, yet no quality control standards exist for the exposure setting of the images. This study evaluated the relationship between exposure setting and cup/disc ratio (c/d) grading among glaucoma specialists. Methods: Color disc photos were taken of 50 eyes of pediatric patients under anesthesia at 3 light exposure settings: dark, medium, and bright. 9 glaucoma specialists evaluated the c/d of the imaged discs in random order. The relationships between the exposure levels and the c/d estimates as well as between c/d size and variability were evaluated.Results: A total of 150 photos of 50 optic discs were graded. The c/d estimates were significantly larger among bright when compared to photos taken at the medium exposure setting(0.53 vs 0.48, P < 0.001) as well as the dark exposure setting (0.47, P < 0.001). In addition, images with larger mean c/d did not show more variability amongst readers (P = 0.59). Conclusion: Image exposure affects c/d grading of nonstereoscopic disc images. The brighter exposure is associated with larger c/d grading. Exposure consistency is key in longitudinal care.

Objective To assess the relationship between infection risk and abatacept exposure levels in patients with polyarticular-course juvenile idiopathic arthritis (pJIA) following treatment with subcutaneous and intravenous abatacept. Methods Data from two published studies (NCT01844518, NCT00095173) of abatacept treatment in pediatric patients were analyzed. One study treated patients aged 2–17 years with subcutaneous abatacept and the other treated patients aged 6–17 years with intravenous abatacept. Association between serum abatacept exposure measures and infection was evaluated using Kaplan–Meier plots of probability of first infection versus time on treatment by abatacept exposure quartiles and log-rank tests. Number of infections by abatacept exposure quartiles was investigated. Results Overall, 343 patients were included in this analysis: 219 patients received subcutaneous abatacept and 124 patients received intravenous abatacept. Overall, 237/343 (69.1%) patients had ≥1 infection over 24 months. No significant difference in time to first infection across four quartiles of abatacept exposure levels was observed in the pooled (p = 0.4458), subcutaneous (2–5 years p = 0.9305; 6–17 years p = 0.4787), or intravenous (p = 0.4999) analyses. Concomitant use of methotrexate and glucocorticoids (at baseline and throughout) with abatacept did not increase infection risk across the abatacept exposure quartiles. There was no evidence of association between number of infections and abatacept exposure quartiles. No opportunistic infections related to abatacept were reported. Conclusion In patients aged 2–17 years with pJIA, no evidence of association between higher levels of exposure to intravenous or subcutaneous abatacept and incidence of infection was observed.

This study evaluated the relationship between brightness setting and cup/disc ratio (c/d) grading among glaucoma specialists. Methods Optic disc photographs obtained during routine examinations under anesthesia were collected to construct an image library. For each optic disc, photographs were obtained at 3 light intensity settings: dark, medium, and bright. From the image library, photograph triads (dark, medium and bright) of 50 eyes (50 patients) were used to construct the study set. Nine glaucoma specialists evaluated the c/d of the study set photographs in randomized order. The relationships between the brightness levels and the c/d grading as well as graders’ years in practice and variability were evaluated. Results The c/d were graded as significantly larger in bright photographs when compared to photographs taken at the medium light intensity (0.53 vs 0.48, P < 0.001) as well as those taken at the dark setting (0.47, P < 0.001). In addition, no relationship was found between ophthalmologists’ years in practice and the variability of their c/d grading (P = 0.76). Conclusion Image brightness affects c/d grading of nonstereoscopic disc photographs. The brighter intensity is associated with larger c/d grading. Photograph brightness may be an important factor to consider when evaluating digital disc photographs.

The endoplasmic reticulum (ER) maintains an environment essential for secretory protein folding. Consequently, the premature transport of polypeptides would be harmful to the cell. To avert this scenario, mechanisms collectively termed “ER quality control” prevent the transport of nascent polypeptides until they properly fold. Irreversibly misfolded molecules are sorted for disposal by the ER-associated degradation (ERAD) pathway. To better understand the relationship between quality control and ERAD, we studied a new misfolded variant of carboxypeptidase Y (CPY). The molecule was recognized and retained by ER quality control but failed to enter the ERAD pathway. Systematic analysis revealed that a single, specific N-linked glycan of CPY was required for sorting into the pathway. The determinant is dependent on the putative lectin-like receptor Htm1/Mnl1p. The discovery of a similar signal in misfolded proteinase A supported the generality of the mechanism. These studies show that specific signals embedded in glycoproteins can direct their degradation if they fail to fold.

SummaryA suggestion that schizophrenic speech may be harder to understand than normal speech was tested by a technique of reconstruction. Ten schizophrenic and ten normal passages were typed onto cards, one sentence per card. Each passage was then presented with the sentences in random order, and students were asked to reconstruct what they believed was the original order. Fewer correct strings of three or more sentences were achieved for the schizophrenic material than the normal material. It is concluded that thereisa detectable abnormality in the structure of schizophrenic speech, but that it stems from the relationshipbetweensentences rather than the content of individual sentences.

BackgroundGlaucoma may cause physiological and behavioural circadian misalignment because of the loss of intrinsically photosensitive retinal ganglion cells, the primary receptors of environmental light. Although studies have suggested a high prevalence of depression in patients with glaucoma, it is unclear whether the association is independent of the light exposure profiles as an important confounding factor.MethodsIn this cross-sectional study of a community-based cohort of 770 elderly individuals (mean age, 70.9 years), glaucomatous optic discs were assessed using fundus photographs and depressive symptoms were assessed using the short version of the Geriatric Depression Scale (GDS). Daytime and night-time ambient light exposures were objectively measured for 2 days.ResultsDepressive symptoms (GDS score ≥6) were observed in 114 participants (prevalence, 14.8%) and glaucomatous optic discs were detected in 40 participants (prevalence, 5.2%). The prevalence of depressive symptoms was significantly higher in the group with glaucomatous optic disc than in the group without it (30.0% vs 14.0%, respectively; p=0.005). Multivariable logistic regression analysis adjusted for potential confounding factors, including daytime and night-time light exposures, revealed that the OR for depressive symptoms was significantly higher in the group with glaucomatous optic disc than in the group without it (OR 2.45, 95% CI 1.18 to 5.08; p=0.016).ConclusionsIn this general elderly population, glaucomatous optic disc was significantly associated with higher prevalence of depressive symptoms independent of a number of potential confounding factors, including daily light exposure profiles.

Context:Underwater treadmill (UTM) exercise is being used with increased frequency for rehabilitation of injured athletes, yet there has been little research conducted on this modality.Objective:To determine the cardiorespiratory responses of UTM vs land treadmill (LTM) exercise, particularly with respect to the relationship between heart rate (HR) and oxygen consumption (VO2).Design and Setting:This quantitative original research took place in sports medicine and athletic training facilities at Wake Forest University.Participants:11 Wake Forest University student athletes (20.8 ± 0.6 y, 6 women and 5 men).Intervention:All participants completed the UTM and LTM exercise-testing protocols in random order. After 5 min of standing rest, both UTM and LTM protocols had 4 stages of increasing belt speed (2.3, 4.9, 7.3, and 9.6 km/h) followed by 3 exercise stages at 9.6 km/h with increasing water-jet resistance (30%, 40%, and 50% of jet capacity) or inclines (1%, 2%, and 4% grade).Main Outcome Measures:A Cosmed K4b2 device with Polar monitor was used to collect HR, ventilation (Ve), tidal volume (TV), breathing frequency (Bf), and VO2 every minute. Ratings of perceived exertion (RPE) were also obtained each minute.Results:There was no significant difference between UTM and LTM for VO2 at rest or during any stage of exercise except stage 3. Furthermore, there were no significant differences between UTM and LTM for HR, Ve, Bf, and RPE on any exercise stage. Linear regression of HR vs VO2, across all stages of exercise, indicates a similar relationship in these variables during UTM (r = .94, y = .269x − 10.86) and LTM (r = .95, y = .291x − 12.98).Conclusions:These data indicate that UTM and LTM exercise elicits similar cardiorespiratory responses and that HR can be used to guide appropriate exercise intensity for college athletes during UTM.

Background Reports on pediatric electroencephalogram of Nepalese patients are rare. Objective We aimed to study the relationship between provisional clinical and electrophysiological diagnoses of pediatric patients with documentation of demographic profiles, and type and frequency of the disorders/diseases. Methods Electroencephalographic reports of 634 children from 2006 to 2009 were analyzed at neurophysiology laboratory, department of Basic and Clinical Physiology, B. P. Koirala Institute of Health Sciences, Dharan, Nepal, retrospectively. Chi-Square test was applied after detail descriptive statistics. Results Male and female were 72.2 % (n=458/634) and 27.76 % (n=176/634) respectively. Most frequent EEG abnormality was seizure disorder (n=370, 59.39%), then febrile seizure (n= 94, 15.08%) and birth asphyxia with hypoxic-induced encephalopathy (n=68, 10.91%). Electroencephalogram showed significant epileptiform discharges in seizure disorder (p=0.001, OR= 2.26, 95 % CI= 1.61 to 3.18) and in cerebral palsy (p=0.049, OR=6.88, 95 % CI=0.89 to 145.95), specifically in 6 to 12 (p=0.001, OR=2.94, 95 % CI=1.43 to 6.06) and one to five (p=0.019) years, respectively. Electroencephalogram detected significantly less epileptiform discharges (p=0.001, OR=0.25, 95 % CI= 0.15 to 0.42) in febrile seizure specifically in 1 to 5 years (p=0.003, OR=0.16, 95 % CI= 0.04 to 0.63). Conclusion Predominant Electroencephalographic abnormality was seizure disorder, followed by febrile seizure and birth asphyxia with hypoxic-induced encephalopathy respectively. Electroencephalographic abnormality was highly associated with seizure disorder and cerebral palsy but was not associated with febrile seizure. This paper provides the first data about physiological responses to stress in jundiá (Rhamdia quelen) exposed to different light colours. Jundiá is a species for fish production in the southern part of South America - and suitable for any region with a temperate or subtropical climates. In order to develop a light management for jundiá fingerlings during indoor maintenance and to understand the relationship between welfare and light colour in the jundiá, fingerlings were exposed to white, blue and green light. At the 10th day of light exposure an acute stressor was imposed. One hour after the application of the stressor, fish were sampled. Stress was assessed by means of cortisol determination. Our results show that green light seems to be the worst alternative to illuminate jundiá indoor experimentation facilities, or even fish transportation. The results also suggests that colour affects the stress response of jundia, and may be usefull for the management of this species.

AbstractHaving treatment in a hospital (hospitalization) is an unpleasant and threatening experience for everyone, especially for children who are still in the process of growth and development. One way to resolve this problem is to use therapeutic communication effectively which will and will be carried out nursing actions. To communicate with children, special approaches or techniques are needed so that the relationship can run well according to the child's growth and development. One of the predisposing factors according to Lawrencen Green's theory is knowledge, when a nurse has good knowledge, her ability in therapeutic communication will be good too. The aim of study is to determine the relationship of knowledge with the therapeutic communication skills of nurses in pediatric patients. This research uses survey analytic methode with crosssectional. Sample in the study were 66 nurses in inpatient rooms of Graha Husada Hospital,technique purposive samplingfor sampling.Data analysis using the Spearman Rank (Rho) correlation test with a significance level (α = 0.05)  is obtained p value 0.025 (p <0.05). There is relationship of knowledge with therapeutic communication ability of nurses in pediatric patients treated at Graha Husada Hospital  Bandar Lampung. Nurses are expected to be able to improve services in providing nursing care to pediatric patients, especially preschool age pediatric patients. AbstrakBerobat di rumah sakit (hospitalisasi) merupakan pengalaman yang tidakmenyenangkan dan mengancam bagi setiaporang,terutama bagi anak – anak yang masih dalam proses tumbuh kembang. Salah satu cara untuk mengatasi masalah tersebut adalah dengan menggunakan komunikasi terapeutik secara efektif yang akan dilakukan tindakan keperawatan. Untuk berkomunikasi dengan anak diperlukan pendekatan atau teknik khusus agar hubungan dapat berjalan dengan baik sesuai dengan tumbuh kembang anak. Salah satu faktor predisposisi menurut teori Lawrence Green adalah pengetahuan, bila perawat memiliki pengetahuan yang baik maka kemampuannya dalam komunikasi terapeutik juga akan baik. Tujuan penelitian ini untuk mengetahui hubungan pengetahuan dengan keterampilan komunikasi terapeutik perawat pada pasien anak. Penelitian ini menggunakan metode survey analitik dengan pendekatan cross sectional. Sampel dalam penelitian ini adalah 66 perawat ruang rawat inap Rumah Sakit Graha Husada dengan teknik pengambilan sampel purposive sampling. Analisis data menggunakan uji korelasi Spearman Rank (Rho) dengan tingkat signifikansi (α = 0.05) diperoleh dari niai p 0.025 (p<0.05). Ada hubungan pengetahuan dengan kemampuan komunikasi terapeutik perawat pada pasien anak yang di rawat di RS Graha Husada Bandar almpung. Perawat diharapkan mampu meningkatkan pelayanan dalam memberikan asuhan keperawatan kepada pasien anak khususnya pasien anak usia prasekolah.

Single-sided lapping is one of the most effective planarization technologies. The process has relatively complex kinematics and it is determined by a number of inputs parameters. It has been noted that prediction of the tool wear during the process is critical for product quality control. To determine the profile wear of the lapping plate, a computer model which simulates abrasive grains trajectories was developed in MATLAB. Moreover, a data-driven technique was investigated to indicate the relationship between the tool wear uniformity and lapping parameters such as the position of conditioning rings and rotational speed of the lapping plate and conditioning rings.
