Aberrant Resting-State Regional Activity in Patients with Postpartum Depression

Abstract The aim of this study is to investigate the alterations of individual local connectivity by regional homogeneity (ReHo) in patients with postpartum depression (PPD) during resting state, and their potential correlations with clinical severity. Thirty patients with PPD and twenty-nine matched healthy postpartum women within 4 weeks after delivery were recruited and performed resting-state functional magnetic resonance imaging (fMRI) scans. The ReHo value was computed as Kendall’s coefficient of concordance (KCC) in the present study, and intergroup differences in the voxel manner were analyzed. Correlations between ReHo values and clinical variables were also analyzed. Compared with healthy postpartum women, patients with PPD exhibited significantly higher ReHo in left precuneus and right hippocampus. ReHo was significantly lower in left dorsolateral prefrontal cortex (dlPFC) and right insula. Furthermore, ReHo values within the dlPFC negatively correlated with the Edinburgh postpartum depression scale (EPDS) score. The ROC curve analysis showed that the area under the curve (AUC) of the above four brain regions are all over 0.7. This study provided evidences of aberrant regional functional activity within brain regions involved in the maternal care network in PPD, and may contribute to the further understanding of neurobiology underlying PPD.

Background: Moyamoya disease (MMD) is a chronic progressive cerebrovascular abnormality characterized by chronic occlusion of large intracranial vessels with smoky vascular development at the base of the skull. In patients with MMD, abnormal spontaneous brain activity would be expected.Purpose: To assess the brain activity changes in patients with MMD by resting-state functional MRI (rs-fMRI), using the percent amplitude of fluctuation (PerAF) analysis method.Materials and Methods: A total of 17 patients with MMD (3 males and 14 females) and 17 healthy control (HC) subjects with matched gender and age were recruited for this study. We used rs-fMRI to scan all the patients with MMD. Spontaneous neural activity was evaluated using the PerAF approach. The receiver operating characteristic (ROC) curve analysis was used to assess the ability of the PerAF to distinguish patients with MMD from HCs. The Hospital Anxiety and Depression Scale (HADS) tests were performed to assess the emotional status of patients with MMD and retinal nerve fiber layer thickness (RNFLT) was measured using high-resolution optical coherence tomography (hr-OCT). The relationship between the HADS scores, RNFLT values, and the PerAF signals was assessed using the Pearson's correlation analysis.Results: Compared with HCs, the PerAF signals in patients with MMD were decreased in the Frontal_Sup_Medial_R and Precentral_L, whereas those in the Caudate_L were increased. The areas under the ROC curves indicated that signals in these brain regions could distinguish between patients with MMD and HCs. The PerAF value of Frontal_Sup_Medial_R was positively correlated with the left and right eye RNFLT values and negatively correlated with the HADS scores.Conclusion: In patients with MMD, reduced PerAF signals in the Frontal_Sup_Medial_R, Precentral_L, and Caudate_L may be associated with psychiatric diseases including anxiety and depression and decreased RNFLT may be associated with ophthalmic complications due to the compression of terminal branches of the internal carotid artery in the retinal fiber layer. The PerAF can be used as an effective indicator of ocular complications of MMD and to study the neural mechanism underpinning emotional complications in patients with MMD.

Abstract Objective: We aimed to identify potential functional network brain-activity abnormalities in patients with orbital fractures (OFs) by using the voxel-wise degree centrality (DC) method.Methods:We selected 20 patients with OFs (12 men and 8 women) and 20 healthy controls (HCs; 12 men and 8 women) matched by gender, age, and education level for this study. Resting-state functional magnetic resonance imaging (fMRI) has been widely used in various disciplines. We calculated receiver operating characteristic (ROC) curves to differentiate characteristics between patients with orbital fractures and HCs; in addition, we applied correlation analyses between behavioral performance and average DC values in different areas. The DC method served to evaluate spontaneous brain activity.Results:The DC values of patients with OFs were higher in the right cerebellum 9 area (Cerebelum_9_R) and left cerebellar peduncle 2 area (Cerebelum_Crus2_L) than those in HCs. The area under the curve (AUC) values for Cerebelum_9_R and Cerebelum_Crus2_L were 0.983 and 1, respectively. The accuracy of our ROC curve analysis result was reliable. Conclusion:Many brain regions seem to show abnormal brain network characteristics in patients with orbital fractures, suggesting potential neuropathic mechanisms.

Abstract Background Since the last decade, postpartum depression (PPD) has been recognized as a significant public health problem, and several factors have been linked to PPD. Mothers at risk are rarely undetected and underdiagnosed. Our study aims to determine the factors leading to symptoms of depression using Structural Equation Modeling (SEM) analysis. In this research, we introduced a new framework for postpartum depression modeling for women. Methods We structured the model of this research to take into consideration the Malaysian culture in particular. A total of 387 postpartum women have completed the questionnaire. The symptoms of postpartum depression were examined using the Edinburgh Postnatal Depression Scale (EPDS), and they act as a dependent variable in this research model. Results Four hundred fifty mothers were invited to participate in this research. 86% of the total distributed questionnaire received feedback. The majority of 79.6% of respondents were having depression symptoms. The highest coefficients of factor loading analysis obtained in every latent variable indicator were income (β = 0.77), screen time (β = 0.83), chips (β = 0.85), and anxiety (β = 0.88). Lifestyle, unhealthy food, and BMI variables were directly affected by the dependent variable. Based on the output, respondents with a high level of depression symptoms tended to consume more unhealthy food and had a high level of body mass indexes (BMI). The highest significant impact on depression level among postpartum women was unhealthy food consumption. Based on our model, the findings indicated that 76% of the variances stemmed from a variety of factors: socio-demographics, lifestyle, healthy food, unhealthy food, and BMI. The strength of the exogenous and endogenous variables in this research framework is strong. Conclusion The prevalence of postpartum women with depression symptoms in this study is considerably high. It is, therefore, imperative that postpartum women seek medical help to prevent postpartum depressive symptoms from worsening.

Background: Depression is the most common complication postpartum affecting 10%–15% of women, contributing greatly to maternal mortality and morbidity, but the care availed is very low among the women who suffer. Aim and Objectives: The current study aimed to study the prevalence and associated risk factors of postpartum depression among recently delivered women in a tertiary care hospital of North India. Material and Methods: This was a hospital based cross-sectional study done between May 2019 and January 2020. All women in the study area who had a pregnancy outcome during the past 6 months and have completed 42 days since their last delivery were included in the study. The data on postpartum depression were collected using the Edinburgh Postnatal Depression Scale (EPDS). Results: There were 300 postpartum women in the study area who participated in the study. Mean age of the study participants was 24.5 years. The deliveries were Institutional in 98% of women. The prevalence of depression among the study women (an EPDS score of 10 and above) was 10%. Among women with depression, a history of depression before the last delivery was given by 40% of women. Only 7.5% of women had sought some form of health care for their problem. Conclusion: The study shows that the prevalence of depression among postpartum women is quite high and the health seeking for depression is very low. Health professionals and workers have to be trained to raise awareness, detect, and treat depression among postpartum women promptly

Postpartum depression affects 10-15% of women in the immediate post delivery period. It has adverse effects on the family and her members. There are trigger factors that contribute to the development of depression among mothers. These factors; when identified and addressed, present an opportunity in reducing the prevalence of postpartum depression and its adverse effects on the family. Hence, the objective of the study was to study the relationship between Intimate Partner Violence (IPV) and Postpartum Depression (PPD) among women in Federal Medical Centre Makurdi. This was a cross-sectional study of mothers who presented with their children at the immunization clinic. Three hundred and thirty postpartum women were selected by a systematic random sampling technique. PPD and IPV were assessed using the Edinburgh Postpartum Depression Scale and the HITS (Hit, Insult, Threaten and Scream) scale respectively. The results showed that the mean age of participants was 28±5years. The prevalence of PPD was 27.6% while that of IPV was 22.4%. A statistically significant relationship was demonstrated between IPV and PPD (χ2 = 143.15, p < 0.0001). The prevalence of PPD and IPV were high, affecting approximately a quarter of postpartum women in Makurdi. IPV contributes to the development of PPD (OR=35.35, CI=17.39-71.87). Hence the need for screening of these conditions by physicians. Appropriate prevention modalities against IPV could curb postpartum depression and its effects on the family.

Background: Postpartum depression is a type of mental disorder associated with childbirth during pregnancy or within the first postpartum year. It is reported as a common psychological health problem affecting 10-15% of women worldwide. The duration of postpartum depression frequently depends on its severity and the time of initiation of treatment. This study assessed depression and its associated factors among postpartum period women of Godavari municipality, Lalitpur, Nepal.Methods: A community-based cross-sectional study was conducted using Edinburg Postpartum Depression Scale among 195 mothers who were within six months of the postpartum period. The chi-square and logistic regression were applied to establish the association between postpartum depression and associated factors. Results: Out of the total 195 postpartum women, 37(19%) women suffered from depression and out of those women 2.1% had suicidal thoughts. Among the associated factors, education, occupation, the intent of pregnancy, family support and pregnancy-related problems/complications were found to be significantly associated with Postpartum depression (p<0.05). Conclusions: Nearly one-fifth postpartum women suffered from some type of depression. It is one of the public health concerns which directly or indirectly corresponds to the socio-economic condition of the women. The improved education and economic status of women, intention of pregnancy, family care and support during pregnancy and the postpartum period and early diagnosis and management of health problems could reduce the magnitude of the postpartum depression. Abstract Background: Antenatal and postpartum depression is the most common complication of gestation and childbearing affecting women and their families, and good social support and family function are considered the protective and modifiable factors. This study aimed to investigate the depression status and to clarify the interrelationships between social support and depression considering the influence of family function among antenatal and postpartum women in rural areas of Southwest China.Methods: This is a cross-sectional study. Data were collected from a total of 490 rural antenatal (N=249) and postpartum(N=241) women (age: 28.17± 5.12). A structural equation modeling (SEM) was used to test the hypothesized relationships among the variables. The following instruments were used: the Edinburgh Postpartum Depression Scale (EPDS), the APGAR Family Care Index Scale, and the Social Support Rate Scale (SSRS).Results: We found that the prevalence of depression was 10.4%. Path analysis showed that family function had a direct negative correlation with depression (β =-0.251, 95%CI: (-0.382)– (-0.118)). Social support had a direct positive correlation with family function (β =0.293, 95%CI: (0.147– 0.434)) and had an indirect negative correlation with depression (β =-0.074, 95%CI: (-0.139)– (-0.032)), family function fully mediated the relationship between social support and depression. Conclusions: Findings of this study highlight that family function should be considered as the key target for interventions aiming to lower the prevalence of antenatal and postpartum depression. Family members interventions are critical to reduce depression among antenatal and postpartum women.

Background: Neovascular glaucoma (NVG) is a serious eye disease that causes irreversible damage to the eye. It can significantly increase intraocular pressure and cause severe pain, as well as abnormal activity in the cortical and pre-cortical visual systems. However, there are few studies in this area. This trial assessed the altered regional brain activity in patients with NVG using the percentage of fluctuation amplitude (PerAF) method.Methods: Resting-state functional MRI (rs-fMRI) scans were conducted in 18 individuals with NVG and 18 healthy controls (HCs), matched for education level, gender, and age. The PerAF method was applied to assess brain activity. Mean PerAF values of brain regions in NVG and HCs were compared using receiver operating characteristic (ROC) curves.Results: Lower PerAF values were found in the NVG group than in controls in the right anterior cingulate and paracingulate gyri (ACG.R), right superior occipital gyrus (SOG.R) and left superior frontal gyrus (orbital part) (ORBsup.L) (p &lt; 0.001). In contrast, PerAF value was higher in NVG patients than in controls in the left inferior temporal gyrus (ITG.L) (p &lt; 0.001). The hospital anxiety and depression scale (HADS) and visual analog score (VAS) were significantly and positively correlated with PerAF in ITG.L (r = 0.9331, p &lt; 0.0001; and r = 0.7816, p = 0.0001, respectively).Conclusion: Abnormal activity in the patient’s brain regions further confirms that the NVG affects the entire brain, not just the visual pathways and posterior retinal mechanisms (including the hypothalamic lateral geniculate nucleus and the primary visual cortex). This strengthens our understanding of the NVG and provides potential diagnostic and therapeutic support for patients who are difficult to diagnose and treat early.

Differences in the correlated activity of networked brain regions have been reported in individuals with generalized anxiety disorder (GAD) but an overreliance on the null-hypothesis significance testing (NHST) framework limits the identification and characterization of disorder-relevant relationships. In this preregistered study, we applied a Bayesian statistical framework as well as NHST to the analysis of resting-state fMRI scans from females with GAD and demographically matched healthy comparison females. Eleven a-priori hypotheses about functional correlativity (FC) were evaluated using Bayesian (multilevel model) and frequentist (t-test) inference. Reduced FC between the ventromedial prefrontal cortex (vmPFC) and the posterior-mid insula (PMI) was confirmed by both statistical approaches. FC between the vmPFC-anterior insula, the amygdala-PMI, and the amygdala-dorsolateral prefrontal cortex (dlPFC) region pairs did not survive multiple comparison correction using the frequentist approach. However, the Bayesian model provided evidence for these region pairs having decreased FC in the GAD group. Leveraging Bayesian modeling, we demonstrate decreased FC of the vmPFC, insula, amygdala, and dlPFC in females with GAD. Exploiting the Bayesian framework revealed FC abnormalities between region pairs excluded by the frequentist analysis, as well as other previously undescribed regions, demonstrating the benefits of applying this statistical approach to resting state FC data.

IntroductionWomen during the postpartum period experience many physiological, psychological, and social changes. Quality of life (QOL) is a sense of well-being and arises from satisfaction or dissatisfaction with various aspects of life including health, employment, socioeconomic state, psychological-emotional state, and family.ObjectivesThe purpose of this study was to identify influence of childbirth experience and postpartum depression on QOL.MethodsThis is a descriptive cross-sectional study regarding 150 postpartum women receiving cares in the hospital of Sfax and examined during the first and the sixth week post-delivery. Data collection tools in this study were demographic questionnaire, Edinburgh Postnatal Depression Scale (EPDS), and world health organization quality of life-bref (WHOQOL-bref). Data were analyzed using SPSS.ResultsThe mean age of our sample (n = 150) was 29.61 years. During the sixth week study period, 126 of 150 were examined.A personal psychiatric history of depression was found in 9.3% of cases.The current pregnancy was undesired in 15.3% of cases.The prevalence of postpartum depression in the first week was 14.7% and 19.8% in the sixth week after delivery.The mean score of quality of life was 81.62 ± 9.09.Scores of quality of life and all its dimensions were significantly lower in depressive women.ConclusionBecause enormous changes develop in postpartum women, we suggest supportive measures for mother by her mother-in-law family, and caregivers to improve the QOL and health status of the mother and her child and to prevent postpartum depression.Disclosure of interestThe authors have not supplied their declaration of competing interest.
