The aim of our research was to study the indicators changes in antioxidant protection and malonic dialdehyde (MDA) levels in the oral fluid in patients with  GP and rheumatoid arthritis (RA) who live in environmentally unfavorable areas and  assessment of the dynamics of changes in the studied indicators in the oral fluid under the influence of comprehensive treatment. Materials and methods. 105 patients with GP were examined and treated, of which 70 patients were with RA. Depending on the ecological living conditions of patients with RA was divided into two groups: group I (n = 35) - live in environmentally unfavorable areas; group II (n = 35) - live in environmentally favorable areas. Group III (n = 35) was formed by patients with GP without concomitant pathology. Each of the groups, depending on the severity of GP was divided into subgroups: A - GP of the I degree; B - GP of the II degree of severity. The control group consisted of 18 virtually healthy individuals with intact periodontium and preserved dentition. The state of the prooxidant system of the oral cavity was determined by the level of MDA. To study the state of the antioxidant defense system (AOS), the activity of superoxide dismutase (SOD) and catalase was determined. Research results.  Determining the level of MDA in oral fluid in all subgroups of patients with GP and RA and without concomitant pathology revealed an increase in this indicator before treatment. It increased most significantly in the IB subgroup to 1.19 ± 0.02 nmol/l and in the IA subgroup to 1.16 ±0.01 nmol/l, compared with 0.34 ± 0.17 nmol / l in healthy individuals. (* pIB-3 <0.001, * pIA-3 <0.001), which exceeded it by 3.5 and 3.4 times, respectively, which may be due to the negative impact of the environment. During all follow-up periods after conducted treatment, the level of MDA decreased and most closely approached the rate of healthy individuals after 6 months. However, before treatment, depletion of the antioxidant system was detected in all subgroups, as evidenced by the weakening of the AOS. In particular, in the IB subgroup there is the greatest decrease in the activity of SOD to 35.6 ± 0.56%, compared with the group of healthy individuals 49.06 ± 0.03% (p <0.001) and catalase to 4.42 ± 0.03 с.u along with healthy - 6.64 ± 0.01 c.u (p <0.001). The activity of certain antioxidant protection enzymes tends to increase after a comprehensive treatment in all subgroups after 6 months. And in the IB subgroup the activity of SOD after 6 months was 44.77 ± 0.35%, catalase 6.29 ± 0.03c.u. Conclusions. The results of the conducted biochemical researches of oral fluid in patients with GP and  RA, who live in environmentally unfavorable areas, showed a decrease in the activity of SOD enzymes, catalase and increased levels of MDA in these patients. After the conducted complex treatment there is a normalization of the AOS system, which is manifested in increased activity in the oral fluid SOD and catalase, as well as a decrease in MDA, which indicates a decrease in inflammation process and the effectiveness of our treatment and prevention complex on the antioxidant system, which provided remission GP in these patients.

The aim of the work was to study the dynamics of changes in the immune system indices and antioxidant protection when fluoroquinolones are included in the treatment regimen for acute edematous pancreatitis. The study included 86 patients with a diagnosis of acute edematous pancreatitis. Group I (n=40) consisted of patients who received treatment according to national and local standards and protocols, group II (n=46) — patients who additionally received fluoroquinolones as part of a comprehensive treatment. The control group consisted of 48 conditionally healthy people in whom laboratory and instrumental diagnostics were carried out similarly to those in patients with acute pancreatitis. According to the purpose and objectives of the study, the state of the immune system and the antioxidant defense system was studied. In patients with acute pancreatitis, changes in the indicators of the immune status were revealed, manifested by the formation of a secondary immunodeficiency with the addition of an autoimmune component. The traditional scheme of pharmacotherapy of acute pancreatitis without the use of antibiotics made it possible to partially correct the indicators of immune status. The use of ciprofloxacin in the complex pharmacotherapy of acute pancreatitis contributed to the normalization of the studied parameters. Thus, the use of fluoroquinolones in the complex pharmacotherapy of acute pancreatitis made it possible to effectively normalize the state of the immune system, cytokine and antioxidant statuses.

Aim: to study the state of local immunity and oral hygiene in patients with GP suffering from AN.Materials and methods: 35 patients with AN were examined at the age of 19-35 years. In the study we used immunological methods, an index assessment of the state of oral hygiene (PMA, API, GI).Results and discussion: in patients of I group poor oral hygiene was established. The amount of lysozyme in the oral fluid in I group was 0.014 ± 0.07 g/l. A decrease SIgA (11SIgA) was established in patients of I group: I, II and control (135.31 ± 23.17, 130.26 ± 24.21 and 300.34 ± 27.38 mg/l, respectively). A tendency to an increase in the level of IgG in the oral fluid in patients of group II compared with I and the control group (5.35 ± 0.36, vs. 4.57 ± 0.04 and 4.98 ± 0.23 g/l, respectively) was established. Conclusions: poor oral hygiene is an objective indicator which reflects low motivation to maintain oral health. The observed immunodeficiency (11SIgA) in patients with GP is a mirror image of changes occurring in the body of patients with AN.

Aim: to investigate the intensity of proteins and lipids peroxidation, the state of the antioxidant system and biochemical indices of endogenous intoxication in patients with snoring. Materials and Methods: Blood serum of 20 patients with snoring and 19 healthy persons without any manifestation of respiratory complications at sleeping were studied. All patients were divided into two groups. Group I included 10 patients with moderate manifestation of snoring, and group II with the hard one. Results: An increase of the proteins’ oxidative modifications in patients with snoring without obstructive apnea at sleep were moderate and severe, that is reflected by an increase of the levels of neutral- and basiccharacter aldehyde-dinitrophenyl-hydrazones and basic-character ketone-dinitrophenyl-hydrazones has been established. The level of thiobarbiturate-positive products in patients with the moderate degree of snoring was in the value of control group. At progression of this pathology this index increased, too in compare to control group. The level of catalase activity in patients of both group was increased where as the content of free thiol groups was decreased. The content of medium weight molecules in patients of both groups was on the level of control, but at progressing of the disease it increased. The content of tyrosine-containing peptides was increased in the serum of patients in both groups. Our data prove for the intensification of the proteins’ and lipids’ peroxidation, for the changes in antioxidant system and for the development of endogenous intoxication in patients with severe level of snoring.

The effectiveness of complex drug “Bifidum BAG” for status correction of large intestine microbiocenosis and antioxidant properties of colonocytes in experimental dysbiosis has been studied. Acomplex drug was administered to experimental animals, which includes Bifidobacterium and Dihydroquercetin. Quantitative and qualitative study of large intestine was done in mices by bacteriological method. The state of lipid peroxidation system was evaluatedaccoding to the content of acylhydroperoxide and malonic dialdehyde. The state of antioxidant protective system was reached by means of catalase and superoxide dismutase activity. Experimental dysbiosis was shown as significant changes in mucosal microflora, changes colonocytes antioxidant properties. The use of the complex preparation “Bifidum BAG”, led to the normalization of the colon microbiota (11 of 16 microorganisms were recovered). After correction gentamicin-associated dysbiosis with a complex probiotic, a positive effect of the drug on the colonocytes antioxidant defense was noted. So the activity of catalase increased 1.1 times, compared with the determined index in the group “dysbiosis”. The activity of superoxide dismutase increased 2 times in comparison with the group “dysbiosis”, exceeding the value of the control group. The concentration of LPO products in colonocytes of experimental animals decreased significantly. The content of malonicdialdehyde and acylhydroperoxide decreased 1.6 times and 5.6 times in comparison with the determined index of the group “dysbiosis”, respectively.

AbstractTo evaluate the changes of left atrial (LA) geometry and function in patients with rheumatoid arthritis (RA) by conventional echocardiography and two-dimensional speckle tracking imaging (2D-STI). We enrolled 46 RA patients with a duration of < 5 years as Group I, 40 RA patients with a duration of ≥ 5 years as Group II, and 40 normal subjects as the control group. Conventional echocardiography was conducted to measure traditional parameters. The LA strain during reservoir phase (LASr), LA strain during conduit phase (LAScd), LA strain during contraction phase (LASct), and LA global longitudinal strain (LAGLS) were obtained from 2D-STI. Related ultrasound results were compared. The LASct was significantly higher in Group I than in control group (P < 0.05). The LASr, LAScd, and LAGLS were significantly lower in Group I than in control group (all P < 0.05). The LASr, LAScd, LASct, and LAGLS were significantly lower in Group II than in control group and Group I (all P < 0.05). The function of LA impaired in RA patients, and the impairment aggravated with the clinical course of RA patients. 2D-STI technology can early and accurately evaluate the LA function of RA patients by evaluating LASr, LAScd, LASct, and LAGLS.

The objective of the study is to clinically assess the condition of periodontal tissues in patients with rheumatoid arthritis, living in anthropogenically loaded and environmentally friendly areas and in people with generalized periodontitis without concomitant somatic diseases. Materials and methods. There were examined 137 patients, including 82 patients with generalized periodontitis of the I degree (subgroups A) and the II degree (subgroups B) with rheumatoid arthritis, living in anthropogenically loaded areas (group I), environmentally friendly areas (group II) and without concomitant somatic diseases living in environmentally friendly areas (group III). Control group – included 18 healthy people. Periodontal tissues were evaluated according to the indices: Greene-Vermillion, PMA, periodontal index offered by Russel and the depth of periodontal pockets.  Results. Women (80%) dominated in each group, patients with generalized periodontitis of the II degree of severity dominated, too.  The highest depth of periodontal pockets was 5.02±0.11 mm in the IB subgroup and differed significantly from this figure in the IIB subgroup 1.07-fold (pIB-IIB<0.05) and from that in the IIIB subgroup 1.2-fold (pIB-IIIB<0.001). The value of the Greene-Vermilion index in patients with GP of the I degree of development in all subgroups A corresponded to “unsatisfactory”. In patients with GP of the II degree of development, the state of hygiene corresponded to “bad” in subgroups IB, IIB and was 3.04±0.11 points (pIB-IIB˂0.01, pIB-IIIB˂0.001, pIB-K˂0.001); 2,63 ± 0.07 points (pIIB-IB˂0.01, pIIB-IIIB˂0.001, pIIB-K˂0.001), in ІІІB – “unsatisfactory”. According to the indicators of the PMA index in patients with GP of the I degree of severity, we’ve found the average severity degree of gingivitis (within the range of 43.25±2.02 – 48.06±1.46%) and severe degree of gingivitis in patients with the GP of the II degree >50 %. The highest indicator of periodontal index was found in the IB subgroup – 6.16±0.10 points (pIB-IIB˂0.01, pIB-IIIB˂0.001, pIB-K˂0.001), which indicated a severe form of periodontitis. Conclusion. Most often, GP in patients with RA was diagnosed in women, most of whom were patients with GP of the II degree (most in group I – 69.04%). The highest depth of periodontal pockets was 5.02±0.11 mm in the IB subgroup. The performed clinical and index assessment of periodontal tissues in patients of three groups indicates a more severe course of GP of the I and II degree in patients with rheumatoid arthritis living in anthropogenically loaded areas (IA, IB subgroups).

Background & Objectives: The hallmark of rheumatoid arthritis is the inflammation that is mediated by the macrophages and monocytes that cause release of pro-inflammatory cytokines like interleukin-18. It is highly expressed in serum of patients suffering from rheumatoid arthritis and has a positive association with disease activity. The aim of this study was to analyze the gene expression of interleukin-18 in rheumatoid arthritis patients on disease modifying anti-rheumatic drug therapy. Methods: The cross sectional comparative study is conducted at Department of Biochemistry and Molecular Biology and Center for Research in Experimental and Applied Medicine (CREAM-1Lab), Army Medical College, Rawalpindi, in collaboration with Rheumatology Department, Military Hospital, Rawalpindi. Study was conducted on two groups consisting of Group-I of rheumatoid arthritis patients on diseases modifying anti-rheumatic drugs and control Group-II comprising of normal healthy individuals. Non-probability purposive sampling was done from patients and controls. The duration of study was one year i-e from November 2015 to November 2016. Relative quantification of gene expression of interleukin-18 was done by Real time PCR using ∆∆CT method. Results: Expression analysis for interleukin-18 showed down regulation of gene in rheumatoid arthritis patients as compared to controls. Conclusion: Interleukin-18 gene shows down regulation in rheumatoid arthritis patients on disease modifying anti-rheumatic drugs therapy. doi: https://doi.org/10.12669/pjms.35.3.1070 How to cite this:Khalid S, Yousaf MJ, Rashid A, Khan SA.
