Public Response to Privatization of Healthcare Services in Saudi Arabia on Twitter (Preprint)

BACKGROUND The Saudi government has been providing free medical care for all Saudis in public healthcare facilities, with the Ministry of Health being the major financing, operating, and supervising body. In July 2017, the Saudi government approved privatization of healthcare services. OBJECTIVE This study aimed to assess public reactions to this reform that have been shared on Twitter. METHODS All tweets that contained the hashtag “خصخصة_الصحة#” [i.e., #Healthcare_Privatization] between 9th and 16th July 2017 were collected. Manual content analysis of the tweets was carried out to identify sentiments, types of sources of the tweets and the general theme of each tweet. Association between the types of sources and sentiments delivered was tested using chi-square test. RESULTS Out of 12,430 tweets collected, 2,119 met the inclusion criteria and were included in the final analysis. Most tweets delivered neutral or undetermined sentiments [37.4%], followed by positive [35.2%] and then negative [27.4%] sentiments. Each tweet has been assigned to one out of nine themes that best fits. The most common theme of the tweets was ‘supportive and explanatory’ [24.6%] then ‘unsupportive and/or expecting failure’ [16.3%]. The majority of contributors were members of the public [85.1%] and the clinical and non-clinical healthcare workers [10.1%]. There was a significant association between the type of the source and the sentiments delivered 2 [6, N = 1740] = 82.1, P-value <0.0001] CONCLUSIONS Our study has shown that Twitter data represents a possible rich source of information to be studied by decision makers to evaluate public responses to major reforms in healthcare systems worldwide. Studying such data provides an insight into common concerns and misunderstandings to be addressed.

Since the Covid-19 pandemic occurred in March 2020 in Indonesia, many aspects of life have been affected, including health care facilities. This condition has changed people's lifestyles, community visits to health care facilities have decreased. Public interest in visiting public health service facilities has also decreased. One of the public health facilities that feel the impact is the Public Health Center. The purpose of this study was to determine the factors - factors associated with patient loyalty at the Public Health Center. This type of research is quantitative analytic with a cross-sectional approach, the types of statistical tests used are the Chi-square test and Logistic Regression test. The location of the research was at the Ciasmara Health Center, the time of the study was from January to July 2021. The number of samples was 120 respondents, the sampling technique was total sampling. The results of the univariate analysis showed that most of the respondents were not loyal to Public Health Center services, namely 51.7%. The results of the chi-square test showed that of the 7 independent variables studied, only 3 variables were significant on patient loyalty (p-value < 0.05), namely: attitude (0.021), service quality (0.034), and employee performance (0.024). While the variables of knowledge, economic level, distance from the house, and infrastructure are not significant because the p-value > 0.05. The results of the logistic regression test showed that the most dominant factor related to patient loyalty was employee performance. The quality of service and employee performance at the Public Health Center needs to be improved as long as it is through improving services and providing rewards and punishments for employees who perform well and poorly. Next, the patient's attitude needs to be improved through education and counseling carried out by the Public Health Center.

Background: Ensuring access to healthcare facilities is a high priority need in developing countries. This research aimed to determine the influence of socio-demographic and economic characteristics of the urban population in Nigeria to access to public healthcare facilities. Methods: We conducted a community-based study in 400 households across the three urban areas of Gombe state, Nigeria. Access to healthcare facilities was quantified in a composite index which considers availability, accessibility and affordability. The head of families was interviewed for information related to access and for the socio-demographic and economic status of the residences. The influence of socio-demographic and economic characteristics was determined using a chi-square test with a significance level of <0.05. Results: Most of the population interviewed within the selected urban areas had good access (84%) to public healthcare facilities. Socio-demographic and economic characteristics of household representatives such as age (p = 0.02), religious status (p = 0.00), level of education (p =0 .00), employment (p = 0.00) and possession of healthcare insurance (p = 0.00) were found to significantly influence access to healthcare facilities in urban areas. Conclusion:  Access to public healthcare facilities within the urban areas was good and the study revealed some modifiable socio-demographic and economic factors that influence access. We recommend the intervention to address the factors to further improve access to public healthcare facilities and to achieve universal healthcare coverage.

Purpose of study: The purpose of this study was to assess patient satisfaction levels within South African public healthcare facilities. The influence of gender and ethnic grouping (race) perceptions of satisfaction of healthcare services was investigated. Methodology: The study followed a cross-sectional research design and a quantitative research method. The data was collected as part of the General Household Survey in 2018 by Statistics South Africa (the national statistics service of South Africa). Descriptive statistics and cross-tabulation were performed to address the research objectives of the study. Main findings: The results show that the majority of the patients who participated in the survey are satisfied with the public healthcare service they received. The leading provinces that achieved very satisfied patients are Limpopo, the Eastern Cape, Mpumalanga, KwaZulu-Natal, and Gauteng. Applications of the study: The study is important in many ways as it highlights the discrepancies of healthcare provision to the public health decision-makers. For example, the results show that generally, the male patients were slightly more satisfied with the healthcare services than their female counterparts. In terms of ethnic grouping, it appears that white patients are generally more satisfied with the public healthcare services they receive than other race groups. Novelty/originality of study: A study of this nature has not been conducted in South Africa apart from the anecdotal reports of the department of health and Statistics South Africa. The study delved to analyze the public healthcare service in all provinces of the republic and also provided insight into gender and racial perception of healthcare services in the country.

Hipertensi di Puskesmas Kedungmundu meningkat dari tahun ke tahun. Tujuan penelitian ini adalah untuk mengetahui faktor risiko yang berhubungan dengan kejadian hipertensi usia produktif (25-54 tahun) di wilayah kerja Puskesmas Kedungmundu tahun 2013. Jenis penelitian ini adalah survey analitik dengan pendekatan case control. Sampel berjumlah 30 orang pada masing-masing kelompok kasus dan kontrol yang diambil dengan teknik accidental sampling. Analisis data menggunakan uji chi square dengan derajat kemaknaan (α) = 0,05. Hasil penelitian menunjukkan bahwa faktor risiko yang berhubungan dengan hipertensi usia produktif (25-54 tahun)  adalah faktor genetik (p value=0,019, OR=4,125), obesitas (p value=0038, OR=3,5), kebiasaan merokok (p value=0,017, OR=6,0), konsumsi garam (p value=0,004, OR=5,675), penggunaan minyak jelantah (p value=0,009, OR=4,929) dan stress psikis (p value=0,002, OR=6,417). Variabel yang tidak berhubungan adalah aktifitas fisik (p value=0,065), konsumsi alkohol (p value=0,148), jenis pekerjaan (p value=0,333), pendapatan keluarga (p value=0,531) lama kerja (p value=0,588). Saran bagi penderita hipertensi usia produktif (25-54 tahun) di Kedungmundu supaya lebih meningkatkan status kesehatan dengan lebih teratur memeriksakan kesehatannya, khususnya tekanan darah. Bagi Puskesmas untuk meningkatkan pengetahuan masyarakat tentang hipertensi. Bagi peneliti lain untuk menambah faktor risiko lain yang berhubungan dengan hipertensi usia produktif. Hypertension  at Kedungmundu Puskesmas was ascending from year to year. The purpose of this study is to find risk factors which related to the Hypertension productive age (25-54 years) at working area health centers Kedungmundu Year 2013. This is a research of analytical survey with casecontrol approach. Sample of 30 people in each case group and controlgroup who were taken using accidental sampling technique. Data analysis using chi square test with degrees of significance (α) = 0,05. The study results show that the risk factor which related to the Hypertension productive age (25-54 years) genetic factor  (p value=0,019, OR=4,125), obesity (p value=0,038, OR=3,5), smooking habit (p value=0,017, OR=6,0), salt consumption (p value=0,004, OR=5,675), use jelantah oil (p value=0,009, OR=4,929) and psychological stress (p value=0,002, OR=6,417). The risk factor which are not related are physical activities (p value=0,065), alcohol consumption (p value=0,148), type of work (p value=0,333), family’s income (p value=0,531) and duration of employment (p value=0,588). The advice to patient hypertension productive age (25-54 tahun) at Kedungmundu to improve their health status by having themselves checked on more regular basis, particularly for their blood pressure. For the public health center to improve knowledge of the hypertension to the public. For the other researcher could find out another risk factors relate to the hypertension.

At the age of less than 28 days newborns are very span to death, so at this time need to be aware of some signs and dangers in the newborn. One of the efforts undertaken to reduce the infant mortality rate is neonatal visits conducted in health services. This study aims to determine several factors related to Neonatal Visits (KN) in the work area of Puskesmas Kampung Baqa in 2018. This study uses Analytical Survey, with cross sectional approach. Respondents of this study were all mothers who had babies aged 0-28 days as many as 87 respondents with sampling techniques using puposive sampling. Data analysis performed was univariate and bivariate with Chi-Square test. The results of this study found that all the variables studied were associated with neonatal visits in the work area of Puskesmas Kampung Baqa in 2018, namely knowledge (p value 0,000), attitude (p value 0,000) and family support (p value 0,006). The conclusion in this research is that knowledge, attitude, and support of mother's family can influence visit of neonatal mother to conduct Neonatal Visit (KN). Therefore, it is necessary to be counseled about the importance of conducting neonatal visits to the public by health workers intensively so that the knowledge of the community especially the mothers who have babies 0-28 days to increase. Keywords : Visit Neonatal, Knowledge, Attitude, Family SupportBibliograpy : 25 (1991-2017).

Pneumonia is one of the biggest health problems and the leading cause of death of children under five years old (infants). The public health center Kebun Handil is the public health center with the highest number of pneumonia infants in Jambi city as many as 222 toddlers in 2016.           This study aimed at determining the correlation of mother’s knowledge and the characteristics of the house with the incidence of pneumonia in infants at the public health center Kebun Handil Jambi city. This is a quantitative research, using Cross Sectional design, and the population of this research is all of infants at the public health center Kebun Handil year 2016 as many as 3.731. The sample was using Accidental Sampling technique, as many as 94 people. This research used Chi Square test. The research was conducted at the public health center Kebun Handil.    The findings indicated that almost respondents (56.4%) have low knowledge, and (53.2%) infants respondents with pneumonia. The result of analysis bivariateChi-Square indicated that there is the correlation between mother’s knowledge with pneumonia in infants (p-value = 0.003 < alpha 0.05). It is necessary to increase the extension to the public about pneumonia toddlers in order to improve the knowledge of the respondents. The counseling results are expected to increase public knowledge of pneumonia and its prevention.

Objectives: To determine frequency of thrombocytopenia and its association with mortality among neonates having probable or culture proven sepsis. Study Design: Descriptive Analytical study. Setting: Multicentre study done at Neonatology Unit of Teaching Hospital Dera Ghazi Khan and Nishtar Medical University, Multan. Period: February 2019 to July 2019. Material & Methods: Three hundred neonates with culture proven sepsis were enrolled. Platelet counts along with frequency of thrombocytopenia (<150000/mm3) and mortality among all the neonates were recorded. Mortality among different groups according to platelet counts was compared adopting chi-square test. Results: Out of 300 neonates considered in the final analysis, 164 (54.7%) were male. Overall, median platelet count was 213.0mm3. TCP was present in 78 (26.0%) cases. Among 146 culture proven sepsis cases, 88 (61.8%) had gram positive while 58 (38.2%) had gram negative pathogens. Cases having thrombocytopenia had significantly higher mortality (n=26/78, 33.3%) as compared with those who had normal or increased platelet count (n=25/222, 11.3%) representing a significant p value (<0.00001). Conclusion: Thrombocytopenia is a common complication of culture proven and probable neonatal sepsis. Mortality is significantly high in neonatal sepsis along with thrombocytopenia.

KEK (Chronic Energy Deficiency) is a condition caused by an imbalance in nutritional intake, which lasts (chronic) while the anemia in pregnant women is closely related to the nutritional status of pregnant women because anemia is one sign that the mother suffers from malnutrition. This study aims to determine the relationship between chronic energy deficiency and anemia in pregnant women at the Public healthcanter Mangasa, Makassar City. The type of research used is an analytic survey with a cross-sectional design. The sampling technique was purposive sampling with 78 respondents.  The results of the statistical test with the chi-square test are known as the significance value of p (0.005) <α (0.05), meaning that the p value is smaller than the value of α at 95% confidence level with 1 degree of freedom, it can be concluded that the null hypothesis is rejected and this accepted hypothesis means that there is a chronic lack of energy relationship with the incidence of anemia in pregnant women at the Public healthcenter Mangasa, Makassar City. Therefore, health workers are expected to be able to carry out continuous strengthening of KIE (communication, information, and education) in pregnant women with KEK risk and anemia to pay more attention to their health status from all factors in readiness to face pregnancy with a cross-sectoral approach.

Hypertension as Non-Communicable Diseases (NCD) is the largest contributor to the causes of death in the world. Complications of hypertension account for the majority of deaths globally. Uncontrolled health behavior in patients with hypertension is closely related to an increase in morbidity rates due to complications from hypertension each year. Medication adherence is one of an effort to form positive health behaviors to prevent hypertension complications. This study aims to identify the relationship between health behavior with medication adherence in hypertensive clients in Cipayung District, East Jakarta. This study using cross sectional design, approach with 109 participants, who were selected by purposive sampling, are choosen primary hypertensive clients of middle adulthood. The results of the chi square test showed that there was a relationship between health behavior and medication adherence (p value: 0.024) 90% CI. The results of the study it can be concluded that the more positive the health behavior, the higher the adherence to taking medication in hypertensive clients. Community nurses have an important role in modifying the behavior of hypertensive patients with a variety of approaches both as individuals, families, and the public can maintain their health well-being. Abstract: Individual Factors Related To Implementation Of Integrated Management Of Childhood Illness (IMCI) In Sambas. Integrated Management of Childhood Illness (IMCI) is one of the government programs aimed at improving officer skills, strengthening health systems and improving care capabilities by families and communities. A public health centre is said to have implemented IMCI if it meets the criteria of carrying out IMCI at least 60% of the number of visits. Achievement of Implementation of IMCI at the public health centre in Sambas district in 2014 is still below 60%. Conducted research to determine the correlation between knowledge factor, performance and motivation toward the implementation of IMCI in public health centre Sambas District. This research was analytical descriptive with the cross-sectional design. The subjects of the study were the staff of the public health centre in Sambas district were 40 respondents. Data collection was done by using primary data through a checklist. Data were analyzed using univariate analysis with frequency distribution table and bivariate analysis using chi-square test. The result of bivariate analysis with chi-square test and significance of alpha 0,05 found no correlation between age (p-value = 0,905), knowledge (p-value = 0,064) and performance (p-value = 0,057) with IMCI implementation. Motivation factor (p-value = 0,013) had significant relation with IMCI implementation.Absrak: Faktor-Faktor Individu Yang Berhubungan Dengan Pelaksanaan Manajemen Terpadu Balita Sakit Di Sambas.Manajemen Terpadu Balita Sakit (MTBS) merupakan salah satu program pemerintah yang bertujuan untuk meningkatkan keterampilan petugas, memperkuat sistem kesehatan serta meningkatkan kemampuan perawatan oleh keluarga dan masyarakat. Puskesmas dikatakan sudah menerapkan MTBS apabila memenuhi kriteria melaksanakan MTBS minimal 60% dari jumlah kunjungan. Pencapaian Pelaksanaan MTBS pada puskesmas di wilayah Kecamatan Sambas tahun 2014 masih di bawah 60 %. Dilakukan penelitian untuk mengetahui hubungan faktor-faktor individu berupa pengetahuan, kinerja dan motivasi terhadap pelaksanaan MTBS di Puskesmas Wilayah Kecamatan Sambas. Penelitian ini bersifat deskriptif analitik dengan desain cross-sectional. Subjek penelitian adalah petugas Puskesmas wilayah kerja Puskesmas Sambas berjumlah 40 responden. Pengumpulan data dilakukan menggunakan data primer melalui cheklist. Data dianalisis menggunakan analisis univariat dengan tabel distribusi frekuensi dan analisis bivariat menggunakan uji chi-square. Hasil analisis bivariat dengan uji chi-square dan kemaknaan alpha 0,05 didapatkan tidak ada hubungan antara umur (p value = 0,905), pengetahuan (p value=0,064) dan kinerja (p value=0,057) dengan pelaksanaan MTBS. Faktor motivasi (p value= 0,013) mempunyai hubungan signifikan dengan pelaksanaan MTBS.
