Navigating the online world of lifestyle health information: an adolescent perspective (Preprint)

BACKGROUND Adolescence is a critical life stage characterised by an interplay of biological, social and environmental factors. Such factors influence lifestyle health related trajectories, including dietary behaviours, physical activity levels, body weight and sleep hygiene. Generation Z (born 1995-2015) are the most internet-dependent and technologically savvy generation in history with increasing rates of smartphone ownership in both developed and developing nations. Gaps exist in our understanding of what online platforms adolescents are using and the barriers and facilitators of these platforms to seek lifestyle health information. OBJECTIVE We evaluated adolescents’ perceptions on the use of contemporary digital platforms (websites, social media platforms and smartphone apps) to seek information or advice related to lifestyle heath. METHODS Virtual focus groups were held via Zoom teleconference between July to August 2021. Eligible participants were 13-18 years, living in Australia and had searched for online lifestyle health information in the previous three months. For this study, lifestyle health information referred to key behaviours and risk factors for chronic disease, namely, diet, physical activity, weight management and sleep. Participants were recruited through an existing database of research participants and networks of the research team. Focus groups were analysed using the Framework approach, where data is systematically searched to recognize patterns in the data and manage, analyse and identify themes. Focus group audio files were transcribed verbatim and independently coded by two researchers (RR, SJ). Through an iterative and reflexive process, a final coding matrix was agreed on by all researchers and used to thematically analyse the data. RESULTS Five focus groups were held (n=32, mean age 16.3(1.4), 18/32, 56% female, 13/32, 41% spoke language other than English at home). Thematic analysis revealed participants searched for information both actively (e.g., on Google or YouTube) and passively (e.g., scrolling social media and using existing apps preloaded to their smartphone such as Apple ‘Health’, Samsung ‘Health’ or ‘Google Fit’ apps). Participants identified that the most helpful information was well-organised in terms of aesthetic appeal and layout, came from a credible and reliable source (e.g. any sponsorships disclosed) and expressed the need for the information to be relatable. Mixed views were reported for the application of lifestyle health information found online. Some participants reported behaviour change, while others noted that certain advice was hard to maintain and incorporate into their lifestyle. CONCLUSIONS This study highlights the abundance and complexity of lifestyle health information online for adolescents. Adolescents in the digital age seek access to information that is appealing, credible, relevant and actionable for lifestyle health behaviours. To appeal to the needs of adolescents, future interventions for adolescents relating to lifestyle health must consider co-design methodological approaches. Furthermore, the regulation of lifestyle health information available online warrants further investigation.

BACKGROUND Adolescent girls report the lowest levels of physical activity (PA) of any demographic group, with rates especially low for racial/ethnic minority girls. Only 3% of Latina teens meet national PA guidelines, and these habits appear to persist into adulthood. Developing effective interventions to increase PA in Latina teens is necessary for preventing disease and reducing disparities. OBJECTIVE The study objective was to develop a mHealth PA intervention for Latina adolescents using a multi-stage iterative process based on an adaptation of the IDEAS (Integrate, DEsign, Assess, Share) framework. METHODS Development of the intervention was accomplished through three iterations: 1) feedback from a previous web-based pilot study, Niñas Saludables; 2) conducting focus groups with the target population; and, 3) vetting the final materials with a youth advisory board (YAB) comprising Latina adolescents. Based on feedback from participants in the Niñas Saludables study, plans to improve the intervention included incorporating visual social media posts and text messaging, adding a commercial wearable tracker, and revising the existing website to be primarily visual; proposed procedures and material were then refined through focus groups. Participants for focus groups (N=50) were girls age 13-18 who could speak and read in English who were recruited from local high schools and after school programs serving a high proportion of Latinos. Facilitated discussions focused on experience with PA and social media apps, and specific feedback on intervention material prototypes and possible names and logos. Materials were refined based on their feedback, then were beta tested by the YAB. YAB members (N=4) were Latinas age 13-18 who were not regularly active and were recruited via word of mouth and selected through an application process. RESULTS The focus group discussions yielded the following findings: PA preferences included walking, running and group fitness classes, while the least popular activities were running, swimming, and biking. Most participants used some form of social media, with Instagram being the most favored. Participants preferred text messages be sent no more than once a day, be personalized, and be positively worded. Focus group participants preferred an intervention directly targeting Latinas, and social media posts that were brightly colored, included girls of all body types, and provided specific tips and information. Modified intervention materials were generally perceived favorably by the YAB members, who provided suggestions for further refinements including incorporation of some Spanish phrases. CONCLUSIONS Latina teens were generally enthusiastic about an mHealth PA intervention, provided the materials were targeted specifically to them and their preferences. Through multiple iterations of development and feedback with the target population, we gained insight into the needs of Latina teens and joined with industry partners to build a viable final product.

ObjectivesWe examined the use of data from social media for surveillance of physical activity prevalence in the USA.MethodsWe obtained data from the social media site Twitter from April 2015 to March 2016. The data consisted of 1 382 284 geotagged physical activity tweets from 481 146 users (55.7% men and 44.3% women) in more than 2900 counties. We applied machine learning and statistical modelling to demonstrate sex and regional variations in preferred exercises, and assessed the association between reports of physical activity on Twitter and population-level inactivity prevalence from the US Centers for Disease Control and Prevention.ResultsThe association between physical inactivity tweet patterns and physical activity prevalence varied by sex and region. Walking was the most popular physical activity for both men and women across all regions (15.94% (95% CI 15.85% to 16.02%) and 18.74% (95% CI 18.64% to 18.88%) of tweets, respectively). Men and women mentioned performing gym-based activities at approximately the same rates (4.68% (95% CI 4.63% to 4.72%) and 4.13% (95% CI 4.08% to 4.18%) of tweets, respectively). CrossFit was most popular among men (14.91% (95% CI 14.52% to 15.31%)) among gym-based tweets, whereas yoga was most popular among women (26.66% (95% CI 26.03% to 27.19%)). Men mentioned engaging in higher intensity activities than women. Overall, counties with higher physical activity tweets also had lower leisure-time physical inactivity prevalence for both sexes.ConclusionsThe regional-specific and sex-specific activity patterns captured on Twitter may allow public health officials to identify changes in health behaviours at small geographical scales and to design interventions best suited for specific populations.

Background:The current study aimed at describing influencing factors for physical activity among young children to determine the best approaches for developing the IDEFICS community based intervention.Methods:In 8 European sites a trained moderator conducted a minimum of 4 focus groups using standardized questioning guides. A total of 56 focus groups were conducted including 36 focus groups with parents and 20 focus groups with children, of which 74 were boys and 81 girls. Key findings were identified through independent reviews of focus group summary reports using content analysis methods.Findings:Findings were generally consistent across countries. The greatest emphasis was on environmental physical (eg, seasonal influences, availability of facilities and safety), institutional (eg, length of breaks at school), and social factors (eg, role modeling of parents). Most cited personal factors by parents were age, social economical status, and perceived barriers. Both children and parents mentioned the importance of children’s preferences.Conclusions:To increase physical activity levels of young children the intervention should aim at creating an environment (physical, institutional, social) supportive of physical activity. On the other hand strategies should take into account personal factors like age and social economical status and should consider personal barriers too.

ObjectivePrimary schools are crucial settings for early weight management interventions but effects on children’s weight are small and evidence shows that deficiencies in intervention implementation may be responsible. Very little is known about the roles of multiple stakeholders in the process of implementation. We used a multiple-stakeholder qualitative research approach to explore the implementation of an intervention developed to improve the diet and increase the levels of physical activity for children living in some of the most deprived areas of England.DesignFor this qualitative study, interviews and focus groups were carried out using semi-structured topic guides. Data were analysed thematically.SettingSeven primary schools (pupils aged 4 to 11) in Manchester, England.ParticipantsWe conducted 14 focus groups with children aged 5 to 10 years and interviews with 19 staff members and 17 parents.InterventionManchester Healthy Schools (MHS) is a multicomponent intervention, developed to improve diet and physical activity in schools with the aim of reducing and preventing childhood obesity.ResultsThree themes were developed from the data: common understandings of health and health behaviours; congruence and consistency of messages; negotiations of responsibility.ConclusionAll participant groups had a common conceptualisation of health as having physical and psychological components and that action could be taken in childhood to change behaviours that protect long-term health. When parents and staff felt a shared sense of responsibility for children’s health and levels of congruence between home and school norms around diet and physical activity were high, parents and children were more likely to accept the policies implemented as part of MHS. Effective two-way communication between home and school is therefore vital for successful implementation of this intervention.

In today's technology age, our communication style has changed and social media channels that provide remote and mutual interaction on digital platforms have become an important communication tool. In this digital communication age, when the need for remote communication has increased and is needed more with the pandemic period, it is seen that health information is shared more and more on social media platforms. In this process, it is seen that the health ministries of all countries, especially the World Health Organization (WHO), actively use social media channels as well as media channels for sharing health information. In this chapter, basic information about social media, social media channels, health communication, health communication in social media, and the effect of this communication on health literacy will be explained. For this purpose, the relevant social media platforms and studies have been examined and conveyed.

Purpose – The workplace offers an ideal setting for facilitating physical activity and reducing sedentary behaviours. Understanding employees’ current health behaviours is required to inform appropriate, tailored, health promotion interventions. The purpose of this paper is to compare the physical activity and sedentary behaviours over 12 months of employees within and across five UK organisations. The paper also explores the association of these health behaviours with objective and self-reported health outcomes; and investigates the association between physical activity and sedentary behaviours. Design/methodology/approach – Self-reported physical activity and sedentary behaviours were recorded at four time points (baseline, three, six, 12 months). BMI, per cent body fat, waist circumference, blood pressure and resting heart rate were collected in health checks (baseline, 12 months). Well-being and health were collected via questionnaire. Findings – Low physical activity and high sedentariness were evident. Sitting levels varied by occupational role and organisation. More activity was associated with improved health outcomes; no association was evident for sedentary behaviour. No direct effects of occupational role or organisation on health outcomes emerged after accounting for physical activity/sedentary behaviours. Physical activity and sedentary levels were weakly associated. Practical implications – The low activity levels are of particular concern as linked to health outcomes for this sample. The weak association between behaviours suggests worksite interventions should target both behaviours. Originality/value – This study provides insight into both the physical activity and sedentary behaviours of employees of large UK employers across different occupational sectors over 12 months; importantly it is informed by the most recent guidance for these health behaviours.

Summary Study aim: the purpose of the present study was to survey the impact of a social media platform on physical fitness, physical activity levels and daily sitting time. Material and methods: a total of 2039 users (1445 women and 594 men) of the social media service (HeiaHeia, Helsinki, Finland) voluntarily participated in the study by answering an online questionnaire provided by a survey. Results: about 63.8% of the participants reported that the service has advanced their perceived level of physical fitness, while 36.2% reported no impact on their fitness. Most participants (71.3%) with BMI over 25 reported that the service had helped them to improve their physical fitness. Participants with BMI over 35 reported a more positive impact than in any other weight range groups. One-third of the participants (32.3%) sat for more than seven hours a day; 23.5% sat less than five hours a day. More than half of the participants (56.8%) were encouraged to be physical active during the day and aimed to reduce sitting time at their jobs during the workday. Conclusions: there seems to be a positive impact of web services that promote physical activity on the physical fitness among social media users. Although the present service is not merely well suited for physically active and physical fit users, it motivates users of all fitness levels to exercise. However, more studies are needed to clarify effects of social media on physical activity, fitness and health.

Abstract Background This mixed methods study explored how social media use informed physical activity and diet-related behaviours, and self-perceived Quality of Life (QoL) during COVID-19, and assessed the contextual factors that drive social media use for health-related behaviour change in diverse groups. During the COVID-19 lockdown periods there were reported changes to social media use and health behaviours, and this gave an opportunity to investigate potential relationships. Methods An explanatory sequential research design of two parts was used: (1) An online survey that assessed social media use in relation to physical activity levels, diet quality and QoL (n = 786; Mage 45.1 ± 19.1 (range 16–88) years; Female =69%); (2) 20 purposive focus groups (n = 69; Mage = 52.88 ± 18.45 years, Female n = 68%) to understand the contextual factors that drive social media use for health-related behaviour change. Descriptive and thematic analysis were conducted. Results Participants in this study reported that social media facilitated the self-management of behaviours related to physical activity, diet and QoL, through access to information to inform workouts and dietary quality, and the opportunities for interaction with peers, family members and within social groups. Contextual factors including work, home and lifestyle arrangements, pre-existing health-related knowledge and behaviours, and the perceived value of social media for health influenced the relationship between social media use and self-reported outcomes. Social media influencers, peers/family members, and official organisations influenced the application of health-related information accessed via social media. Conclusions The evidence shows that participants were critical users of social media and were able to use social media to derive benefit for their health and wellbeing. Detailed guidance for those who use social media, as well as those who recommend and endorse social media content is required to maximise the potential of social media to support health behaviours. Future public health strategies and social media interventions should acknowledge diversity in contextual factors driving social media use for health behaviour change.

Abstract Background The objectives of this systematic review were to update the evidence base on social media interventions for physical activity and diet since 2014, analyse the characteristics of interventions that resulted in changes to physical activity and diet-related behaviours, and assess differences in outcomes across different population groups. Methods A systematic search of the literature was conducted across 5 databases (Medline, Embase, EBSCO Education, Wiley and Scopus) using key words related to social media, physical activity, diet, and age. The inclusion criteria were: participants age 13+ years in the general population; an intervention that used commercial social media platform(s); outcomes related to changes to diet/eating or physical activity behaviours; and quantitative, qualitative and mixed methods studies. Quality appraisal tools that aligned with the study designs were used. A mixed methods approach was used to analyse and synthesise all evidence. Results Eighteen studies were included: randomised control trials (n = 4), non-controlled trials (n = 3), mixed methods studies (n = 3), non-randomised controlled trials (n = 5) and cross-sectional studies (n = 3). The target population of most studies was young female adults (aged 18–35) attending college/university. The interventions reported on positive changes to physical activity and diet-related behaviours through increases in physical activity levels and modifications to food intake, body composition and/or body weight. The use of Facebook, Facebook groups and the accessibility of information and interaction were the main characteristics of social media interventions. Studies also reported on Instagram, Reddit, WeChat and Twitter and the use of photo sharing and editing, groups and sub-groups and gamification. Conclusions Social media interventions can positively change physical activity and diet-related behaviours, via increases in physical activity levels, healthy modifications to food intake, and beneficial changes to body composition or body weight. New evidence is provided on the contemporary uses of social media (e.g. gamification, multi-model application, image sharing/editing, group chats) that can be used by policy makers, professionals, organisations and/or researchers to inform the design of future social media interventions. This study had some limitations that mainly relate to variation in study design, over-reliance of self-reported measures and sample characteristics, that prevented comparative analysis. BACKGROUND Social media are potentially powerful channels for communicating relevant health information in culturally sensitive and influential ways to key audiences. Moreover, these channels hold promise for promoting awareness and knowledge of health risks, prevention, and treatment by utilizing opinion leaders for message dissemination. Despite limited empirical evidence to-date, early promising results suggest that blogs are a form of social media that should be examined as worthy channels for health communication. OBJECTIVES This formative study explored mommy bloggers’ perceptions about sharing health-related information on their blogs with their readers. It also sought to analyze which topics would be of most interest to mommy bloggers, what motivates them to write about health issues, and how they perceive interest in these topics among their readers. METHODS This study employed survey methodology, including the use of open-ended questions, the responses to which were coded for analysis. Specifically, a 14-item survey was fielded with mommy bloggers between October 1 and October 28, 2016. Bloggers were recruited through The Motherhood network. A total of 461 mommy bloggers responded to the survey; 163 were removed for low quality responses and incomplete data. As a result, 298 eligible participants completed the survey. For open-ended questions in the survey, a sample of responses were coded and analyzed. RESULTS The majority of the respondents (87.2%, 260/298) reported that they have written about health issues in the past; 97.3% (290/298) of the respondents reported that they would consider writing about health issues sometime in the future, and 96.3% (287/298) of the respondents reported that their readers like to read about health issues on their blogs. In terms of content priorities for this sample of bloggers, Nutrition and Physical Activity dominate the current conversation and similarly, Physical Activity and Nutrition remain top content priorities for these bloggers for the future. Moreover, 21.3% of the respondents reported that their readers would be interested in these topics. Finally, having a personal connection with a health issue was found to be positively associated with likeliness to write about health issues on their blog (P<.001). CONCLUSIONS This study illustrates that there are potentially rich opportunities for working with mommy bloggers to communicate with key health decision makers (moms) on important health issues. There is a great support among mommy bloggers for health information dissemination as well as interest for accessing relevant health information from their readers. This presents an opportunity for public health research and communication campaigns to more broadly promote their messages, thereby contributing to their behavior change objectives. Limitations included overrepresentation of white, higher-educated, and younger women. It suggests a need for more targeted engagement of a diverse sample for future work.
