Bacteria found in blood circulation either consistently intermittently are commination to every organs of body. These infections can affect life and death. In India also blood stream infections are major causes of health problem that caused demise of patients in hospital. Timely diagnoses of infection with antimicrobial susceptibility assessment are important for optimization of treatment and best way to reduce hospital stay and improve patient health. In spite of recent advances in clinical diagnostics, blood culture remains the gold standard for the detection of blood stream infections. Studies in different places and regions have indicated the varying microbiological pattern of bloodstream infections which support the need for a continuous examination of the causative organisms. For the diagnosis of septicemia, Blood cultures are the “gold standard” are based on the detection of viable microorganisms in the blood. The main aim of this is to identify the bacteria causing bloodstream infections and to determine and analysis their antibiotic sensitivity pattern in a tertiary center. In this study blood for culture was collected from 940 clinically suspected cases of blood stream infection from the hospital. Collected blood samples were processed in the bacteriology section at microbiology laboratory and standard laboratory methods were used to identified isolates and then antibiotic susceptibility test was performed using CLSI guidelines. Total 940 blood samples were cultured in which 139(14.8%) were found positive. Among isolates, the most predominant organism was Staphylococcus aureus (51.8%) followed by Escherichia coli (24.5%) and the least was Salmonella species (1.4%), Proteus species (1.4%) and Acinetobacter species (1.4%). Among Gram positive isolates, Penicillin and Erythromycin showed high degree of resistance. Imipenem was particularly susceptible among the isolated. Gentamicin and Amikacin showed high in vitro susceptibility for both Enterobacteriaceae and Nonfermenters. This study provides information on bacteriological profile of septicemic isolates. Therefore continuous monitoring of the susceptibility of organisms towards antibiotics is necessary to prevent and spread of drug resistance.

Abstract A comparative retrospective study to quantify the impact of Coronavirus Disease 2019 (COVID-19) on patient safety. We found a statistically significant increase in central line-associated blood stream infections and blood culture contamination rates during the pandemic. Increased length of stay and mortality was also observed during COVID-19.

ABSTRACT Purpose: Blood stream infection (BSI)/bacteremia is a potentially life threatening infection and are associated with a high crude mortality. Coagulase negative Staphylococcus (CONS) and Staphylococcus aureus are the most commonly isolated gram positive bacteria from blood culture samples. While S. aureus is a known pathogen causing BSIs, CONS are considered to be common contaminants of blood culture. Of late many studies have challenged this traditional viewpoint. The aim of this study was to determine the epidemiology and significance of Staphylococcus aureus and CONS bacteremia, their resistance patterns and associated mortality in critically ill trauma patients admitted to a level 1 trauma center. Materials and Methods: The study was conducted from January 2009 to June 2011. All patients from whose blood samples yielded a S. aureus or CONS on culture were included in this study. A detailed history was obtained and follow-up of the patients was done. The isolates of Staphylococci were identified to species level. Antibiotic susceptibility was performed by the disc diffusion method and VITEK-2 system. Results: During this 30 month period, total of 10,509 blood samples were received from 2,938 patients. A total of 1,961 samples taken from 905 patients were positive for one or more pathogens. S. aureus/CONS were isolated from 469 samples from 374 patients. Crude mortality amongst the patients having Staphylococcal BSI was 25% (94/374). Conclusion: Staphylococcal blood stream infections are a leading cause of morbidity and mortality.

Aim: Bloodstream infections are a major cause of morbidity and mortality worldwide. The prevalence of causative microorganisms varies from one geographical region to another. This study was aimed at determining the etiological agents prevalent in our environment and their susceptibility profile. Study design: This is a retrospective study carried out at the University of Calabar Teaching Hospital, Calabar, Nigeria. Methodology: Blood culture results of patients documented over a two-year period were retrieved and analyzed. Blood culture positive isolates were detected using conventional method and Oxoid signal blood culture systems. Antimicrobial sensitivity tests were carried out by Kirby-Bauer disc diffusion method. Methicillin resistance in Staphylococcus aureus and coagulase negative Staphylococcus species (CoNS) was detected by disk diffusion method using 30 µg cefoxitin disk. ESBL production was detected by phenotypic confirmatory disc diffusion test (PCDDT) and the double disc synergy test (DDST). Results: A total of 413 blood culture antimicrobial susceptibility test results were analyzed, of which 116 (28.09%) were identified as culture positive. Sixty-nine (59%) of the positive isolates were from female patients. Out of 116 positive cultures, 58.62% (68/116) were Gram positive organisms, 40.52% (47/116) were Gram negative organisms, non albicans Candida accounted for 0.86% (1/116).  Staphylococcus aureus (n=41, 35.3%) was the predominant isolate and showed high sensitivity to levofloxacin (100%), Linezolid (100%) and Amikacin (100%). Twelve isolates of S. aureus were methicillin resistant, while 1 isolate was inducible clindamycin resistant. Of the 116 isolates identified in this study, forty-three (43) were multidrug resistant with highest number of multidrug resistant isolates from Staphylococcus aureus (n=20). 21.28% (n=10) of the Gram-negative isolates were positive for extended spectrum beta lactamases. Conclusion: A high rate of antimicrobial resistance is observed among microorganisms causing blood stream infections. This emphasizes the need for antimicrobial sensitivity testing in the management of blood stream infections.

The relatively long turnaround time and low sensitivity of traditional blood culture may delay the effective antibiotic therapy in patients with bloodstream infection (BSI). To reduce the morbidity and mortality of BSI, a rapid and sensitive pathogen detection method is urgently required. Acinetobacter baumannii and Klebsiella pneumonia are two major microorganisms responsible for BSI. Here we reported a novel droplet digital PCR (ddPCR) method that can detect A. baumannii and K. pneumonia in whole blood samples within 4 h, with a specificity of 100% for each strain and limit of detection at 0.93 copies/microliter for A. baumannii and 0.27 copies/microliter for K. pneumonia. Clinical validation in 170 patients with suspected BSIs showed that, compared with blood culture that reported 4 (2.4%) A. baumannii cases and 7 (4.1%) K. pneumonia cases, ddPCR detected 23 (13.5%) A. baumannii cases, 26 (15.3%) K. pneumonia cases, and 4 (2.4%) dual infection cases, including the 11 positive patients reported by blood culture. In addition, the positive patients reported by ddPCR alone (n = 42) had significantly lower serum concentrations of procalcitonin and lactate, SOFA and APACHE II scores, and 28-day mortality than those reported by both blood culture and ddPCR (n = 11), suggesting that patients with less severe manifestations can potentially benefit from the guidance of ddPCR results. In conclusion, our study suggests that ddPCR represents a sensitive and rapid method to identify causal pathogens in blood samples and to guide the treatment decisions in the early stage of BSI.

Data regarding the incidence, etiology and outcome of Bacteraemia taken from AL-Kindi hospital were collected, and compared the situation between the three years: 2007, 2008, 2009. A total number of blood samples was (505), from this number only (70) (13.8%) is positive. The rate of positivity was significant greater in 2009 (17.7%) than 2008 (11.8%) and 2007 (9.1%). The calculated incidence of significant episodes of bacteraemia was 12.6% .The five most commonly isolated microorganisms were: Salmonella typhi30(46.8%), Staphylococcus aureus 12 (18.7%), Escherichia coli 10 (15.6%),Klebsiella 4(6.2%) and Enterobacter sp.3(4.6%) varied throughout this period, significant increase was noted in 2009. The acquisition of blood stream infections was nosocomial in 8.5% of cases.60% of Salmonella typhi were Amoxicillin resistant. Also Staph.aureus isolates, 58%were resistant to Amoxicillin and Erythromycin. Cephalexin-resistant E.coli in70% of the isolates and Gentamycin-resistant Klebsiella occurred in 75% of the isolates. The incidence of BSI did not differ significantly from other reported studies.

Background Blood stream infections (BSI) are significant cause of morbidity and mortality in the world. This study was conducted to determine the common bacterial agents associated with BSI with their antimicrobial susceptibility patterns in a tertiary care centre in the Western region of Nepal. Method This cross-sectional study was conducted for a period of two years from May 2010 to May 2012. All patients with fever (temperature ≥ 38°C) were included in the study. Results A total of 4,145 patients with febrile illness were included in this study, bacterial agents were isolated in 251 (6%) cases. Common bacterial isolates were Salmonella spp., Klebsiella pneumoniae, Escherichia coli, Pseudomonas species, Acinetobacter species, Staphylococcus aureus and Coagulase negative Staphylococci. Paratyphoid fever (Caused by S. Paratyphi A) is more common than typhoid fever. The members of Enterobacteriaceae were found to be resistant to ampicillin and cefazolin. Majority of the nonfermenters were found to be sensitive to most antibiotics. Gentamicin and Ciprofloxacin were sensitive to majority of gram positive bacteria. Conclusion Gram-negative bacteria were the predominant causes of BSIs. The occurrence of drug resistance among the isolated bacteria is of great concern. Imipenem showed 100% sensitivity against Pseudomonas aeruginosa indicating lack or low level of MBL activity. Background: Non-fermenting Gram-negative bacilli (NFGNB) are emerging as important causes of blood stream infections (BSI) and they are a major cause of morbidity and mortality worldwide. High intrinsic resistance of NFGNB to antimicrobial compounds makes the treatment of BSIs caused by them difficult and expensive. The aim of this study was to assess frequency and antibiotic susceptibility pattern of non-fermenting gram-negative rods isolated from blood culture of patients.Methods: A total of 3016 blood samples were received in the Department of Microbiology during the study period. All samples were processed according to standard microbiological procedures. Blood culture was done by automated blood culture system, (BacT/Alert) and identification and antibiotic susceptibility of non-fermenting gram negative bacilli was done by VITEK2 Compact System.Results: A total of 120 NFGNB were identified out of which the most common non-fermenters isolated were Acinetobacter sp. (95) followed by Pseudomonas aeruginosa (11), Burkholderia cepacia (09) Sternotrophomonas maltophilia (03) and Sphingomonas sp. (02). Most of the non -fermenters were multi drug resistant showing a high level of antibiotic resistance to most of the first- and second-line drugs. The most effective drugs were colistin and tigecycline.Conclusions: This study underlines the need to identify NFGNB in tertiary care hospitals and to monitor their susceptibility pattern to guide the clinician for better care and management of patients. Improved antibiotic stewardship and strict infection control measures especially hand washing need to be implemented to prevent emergence and spread of multidrug resistant NFGNB in health care settings.

Staphylococcus pettenkoferi was first isolated from blood culture a 25-years-old patient with fever of unknown etiology. Based on the literature analysis, it was found that it is an opportunistic microorganism. It causes blood stream infections and was an causative agent of osteomyelitis. Staphylococcus pettenkoferi is a component of the human microbiome, was found in a hospital environment and was also isolated from animals and their surroundings.
