Numerous epidemiological studies suggest that CMD problem is rather present in most countries all over the world, particularly in the Scandinavian and East European countries. There have not still been epidemiological studies on incidence of CMD in our surroundings, either organized or academic treatment programs of these diseases. The results of this study showed that most common CMD signs and symptoms were: impaired mandibular movements, deviation (deflection) in opening and closing the jaws, and TMJ sounds during mandible movements. Pain as a primary symptom was seen only in 5.2% of the analyzed subjects. Such fact indicates that this analyzed population group manifested the initial stage signs and symptoms of CMD. Recording of signs and symptoms mentioned above in young subjects should lead the therapist to perform more detailed analysis and apply adequate therapy modalities.

The craniomandibular dysfunction (CMD) is a pathology that can appear at early ages. In a sample of 36 childrenresiding in Madrid (Spain), of both sexes, with ages between 7 and 13 years, the prevalence of signs and symptomsof CMD was analyzed. For that purpose, we did a dental and muscular examination, together with temporomandibularjoints, functional and occlusal examination, and completed with a specific questionnaire. The results revealedthat 100% showed some sign or sympthom of CMD. 77,8% of the pacients presented 3 or more CMD signs.The most prevalent were painful muscle palpation (94,4%), together with sliding anteriorly (91,7%), painful jointpalpation (69,4%), wear facets in permanent teeth (41,7%) and altered opening and closing trayectory (38,9%). Onthe other hand, only 38,9% showed any CMD symptom. The most prevalent symptoms were night teeth grinding(27,8%), followed by tooth sensitivity (19,4%) and fullness in the ears (16,7%). We conclude that in our sample,25% presented mild CMD (less that 3 signs or symptoms), 58,3% presented moderate CMD (from 3 to 6 signs orsymptoms) and 16,7% showed severe CMD (more than 6 symptoms). Nevertheless, it’s important to remark thatnone of these pacients came seeking treatment for his CMD and these symptoms were refered only when beingasked. Hence the importance of a comprehensive clinic history to precociously diagnose this pathology and havethe ability to prevent its progression.

Introduction. The aim of this study was to determine the prevalence of cra- niomandibular dysfunction (CMD) in the military personel of the Republic of Serbia, as well as the distribution of sings and symptoms of CMD in this population group. Material and methods. The epidemiological investigation included 284 male persons between 18-25 years of age. The sample consisted of Military Academy students, and soldiers from different regions of the Republic of Serbia. A questionnaire was designed using Helkimo's clinical dysfunction index and sings and symptoms of CMD were evaluated, namely the anamnestic index (Ai) and the dysfunction index (Di). Results. According to the anamnestic dysfunction index (Ai), 85% of persons among the examined group presented without any symptoms of CMD (Ai=0), 14% had mild symptoms (Ai =1) and 1% had severe symptoms of CMD (Ai=5). Functional analysis of the orofacial system in the examined group (evaluation of dysfunction index - Di) revealed, however, that 70% of solders had at least one sign of CMD, precisely a dysfunction index Di>0. The analysis of signs and symptoms of CMD revealed that disturbances in mandibular kinematics were the most prevalent sign of CMD in this population group, which was confirmed in 56% of examined persons. Disturbed functions of temporomandibular (TM) joints (click, and reciprocal click, deviation and deflection during mandibular opening) were established in 31% of examined persons, and sensitivity of TM joints and masticatory muscles during palpation in 4% of examined persons. Pain during mandibular movements was established only in 1% of examined persons. Conclusion. The results of this investigation point to significant prevalence of Craniomandibular disorders in the examined population group. The incidence of TMJ and masticaory muscle pain in 4% of examined persons and of pain during mandibular movements only in 1% of examined persons, points to presence of mild or initial types of CMD in this population group. .

This research aimed to evaluate mandibular movements in children with and without signs and symptoms of temporomandibular dysfunction. The sample taken consisted of 99 children aged 3 to 5 years distributed in two groups: I - Absence of signs and/or symptoms of TMD (25 girls/40 boys); II - Presence of signs and symptoms of TMD (16 girls/18 boys). The symptoms were evaluated through an anamnesis questionnaire answered by the child's parents/caretakers. The clinical signs were evaluated through intra- and extraoral examination. Maximum mouth opening and left/right lateral movements were measured using a digital caliper. The maximum protrusive movement was measured using a millimeter ruler. The means and standard deviations for maximum mouth opening in Group I and Group II were 40.82mm±4.18 and 40.46mm±6.66, respectively. The values found for the left lateral movement were 6.96mm±1.66 for Group I and 6.74mm±1.55 for Group II, while for the right lateral movement they were 6.46mm±1.53 and 6.74mm±1.77. The maximum protrusion movements were 5.67mm±1.76 and 6.12mm±1.92, in Groups I and II, respectively. The mandibular movement ranges neither differed statistically between groups nor between genders. The present paper aimed at evaluating the validity of the Portuguese version of the Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD) Axis II Questionnaire. The sample was comprised of 155 patients with signs and symptoms of Temporomandibular Disorders (TMD), evaluated at the Orofacial Pain Control Center, School of Dentistry, University of Pernambuco, Brazil, between July 2003 and February 2004. Data collection was performed with the following tools: the RDC/TMD Axis I (clinical evaluation and TMD classification), and Axis II (psychosocial evaluation), as well as specific questionnaires for evaluation of Oral Health Related Quality of Life, namely, Oral Impacts on Daily Performances and the Oral Health Impact Profile-14, considered to be gold standard criteria. Validity evaluation consisted of internal consistency evaluation by the Cronbach alfa reliability test, reliability and reproducibility estimated by the Kappa test and the Spearman's correlation, and concurring validation through Spearman's correlation. The Portuguese version of the RDC/TMD Axis II questionnaire was considered consistent (Cronbach alfa = 0.72), reproducible (Kappa values from 0.73 to 0.91, p < 0.01), and valid (p < 0.01). It was concluded that this version showed valid and reproducible results for the Brazilian population, thus paving the way for including Brazil in transcultural epidemiological studies on TMD.

The aim of this study was to analyze the general, geometric, and kinematic characteristics of the masticatory cycle’s movements in a tridimensional way, using a method developed by our study group to provide a new insight into the analysis of mandibular movements due to advancement in the potential of computational analysis. Ten individuals (20.1 ± 2.69 years), molar class I, without mandibular movement problems participated in this study. The movements of the masticatory cycles, frontal and sagittal mandibular border movements, were recorded using 3D electromagnetic articulography and processed with computational scripts developed by our research group. The number of chewing cycles, frequency (cycles/s), chewing cycle areas/mandibular border movements areas ratios, and the mouth opening and closing speeds on the 3D trajectory of the chewing cycle were compared. The cycles were divided and analyzed in thirds. The masticatory cycles showed high variation among the individuals (21.6 ± 9.4 cycles); the frequency (1.46 ± 0.21 cycles/s) revealed a moderate positive correlation (R = 0.52) with the number of cycles. The frontal area ratios between the cycle area and the mandibular border movement presented higher values in the first third (6.65%) of the masticatory cycles, and the ratios of sagittal areas were higher and more variable (first, 7.67%; second, 8.06%; and third, 10.04%) than the frontal view. The opening and closing mouth speeds were greater in the second third of the masticatory cycles (OS, 57.82 mm/s; CS, 58.34 mm/s) without a significant difference between the opening and closing movements when the same thirds were evaluated. Further studies are necessary to improve the understanding of the masticatory cycles regarding the standardization of parameters and their values.

Abstract People who used illicit drugs (PWUDs) represent an important population group for acquisition and viral dispersion. In Brazil, high rates of the human T lymphotropic virus 1 (HTLV-1) and 2 (HTLV-2) have been reported in epidemiological studies. However, the epidemiological scenario of HTLV-1/2 infections in PWUDs is still poorly understood. Thus, this cross-sectional study determined the prevalence, frequency of subtypes and factors associated with HTLV-1/2 infections among PWUDs in the Brazilian state of Pará, an area considered endemic for this virus and with poor health services. Among 826 PWUDs, 53 (6.4%) presented anti-HTLV-1/2 antibodies by EIA and 44 (5.3%) presented proviral DNA by PCR. HTLV-1 and HTLV-2 were detected in 25 (3.0%) and 19 (2.3%) PWUDs, respectively. Subtypes 1a (25/44), transcontinental (23/44) and Japanese subgroups (2/44), 2b (6/44) and 2c (13/44) were identified. Involvement in illicit/criminal activity, daily use of illicit drugs, illicit drug use over 12 years, unprotected sex with other PWUDs, changes in genitalia (including ulcers and wounds), and more than 12 sexual partners were associated with HTLV-1/2 in PWUDs. This high prevalence and intense circulation of subtypes and subgroups of HTLV-1/2 is very worrying, and indicate the need for urgent actions for its control, prevention and treatment.

Multiple-injection strategy that has been applied widely in diesel engines usually features a short duration for each injection pulse. As a result, the shortened injection makes the needle opening and closing transients increasingly important for spray in an injection event. Owing to the needle movement, the spray development during the transient processes is complex and quite different from the spray at the quasi-steady state. However, so far modeling of the spray development during the transient processes is far from adequate. Particularly, a theoretical zero-dimensional (0-D) spray tip penetration model considering the needle opening and sac pressurization processes as well as ambient and injection conditions during the start-of-injection (SOI) transients is still absent. In this paper, considering the sac pressurization processes, the 0-D model of spray tip penetration during the SOI transients is derived. Then, the model is validated against the experimental spray data using a long-distance microscope together with an ultrahigh speed CMOS camera. The model and experimental results show that the spray tip penetration shows a t3/2 dependence at the initial stage of injection rather than the t dependence suggested by Hiroyasu’s model. Later, the spray tip penetration shows a t3/4 dependence owing to the spray breakup, and a t1/2 dependence with the completion of sac pressurization. The models and analysis are believed to provide new insights into the transient spray behaviors and valuable reference for engineers and researchers who are considering the model-based development of next-generation diesel engines.

Few studies were performed to assess workers’ exposure to pesticides in orchards and even less during re-entry activities. However, nearly 40 pesticide treatments per year are carried out in apple-growing especially against fungal diseases, mainly scab. In order to characterize pesticide exposures of farmers in epidemiological studies, we performed a non-controlled field study in 3 regions of France (Normandy, South-West and Rhône-Alpes) in apple orchards during the 2016 and 2017 seasons. We observed the activity of the workers trying to disrupt it as little as possible. Workers’ external contamination and their determinants were assessed during 158 working days, corresponding to 31 treatment days (including mixing, spraying and equipment cleaning), 69 re-entry days (including apple hand-thinning, anti-hail net opening and closing tasks) and 58 harvesting days. We performed both detailed observations of work characteristics on the whole day (including an ergonomic approach) and pesticide measurements for dermal contamination during each task (following the OCDE guidelines). Potential dermal exposure was measured with cotton pads placed onto the skin (11 body areas), and cotton gloves or hand rinsing for each task. Captan and dithianon, fungicides representative of pesticide use in apple-growing, were used as markers for exposure. Relative contribution of the different body areas to total exposure was assessed. In addition, to investigate potential sources of contamination, we measured dislodgeable residues in the environment of the workers by analyzing leaf and fruit samples and surfaces of work equipment. Results indicated that workers’ dermal exposure was higher during re-entry tasks than treatments and harvests. The median dermal contamination during treatment was 4.03 mg of active ingredients per day, levels ranging from 0.95 mg to 64.19 mg. Most of the contamination was observed on the hands, especially during mixing. We will focus our presentation on levels and main determinants of pesticide exposure during treatment tasks.

Dupuytren's Disease (DD) is a common, fibroproliferative disorder affecting the palmar surface of the hands which is often irreversible and progressive. Understanding the epidemiology of DD is important in order to provide clues to its etiopathogenesis. This review aims to evaluate the epidemiological studies carried out in DD since 1951. Studies evaluating the epidemiology of DD were searched using Medline, Pubmed, and Scopus which dated back from 1951 to current date. Inclusion criteria were any studies investigating the prevalence or incidence of DD in any population group. A total of 620 articles were cited. Forty-nine studies were subsequently identified as relevant to evaluating the epidemiology of DD. The prevalence of DD in all studies increased with age with a male to female ratio of approximately 5.9:1. Prevalence rates ranged from 0.2% to 56% in varying age, population groups, and methods of data collection. The highest prevalence rate was reported in a study group of epileptic patients. Although, only one study calculated the incidence (as opposed to prevalence) of DD to be equal to 34.3 per 100,000 men (0.03%). In conclusion, the prevalence of DD in different geographical locations is extremely variable, and it is not clear whether this is genetic, environmental, or a combination of both. The majority of the prevalence studies have been conducted in Scandinavia or the UK, and the vast changes in population structure, the changes in prevalence of associated diseases, and the change in diagnostic criteria of DD makes understanding the epidemiology of this condition difficult.

Inflammatory processes are involved in the pathogenesis of the most common chronic non-communicable diseases and may also play an important initiating role in their development. Only recently have inflammatory markers been included in epidemiological studies focusing on nutritional status, body composition and physical activity. We are just starting to understand how different lifestyles can determine basal levels of inflammatory biomarkers in early ages. This review aims to summarise what is known about the relationships between lifestyle-related determinants (focusing on overweight, physical activity and dietary habits) and inflammatory markers in apparently healthy young populations. Obesity is the most widely studied determinant. Several large-scale studies have now demonstrated that healthy young subjects with more body fat or higher BMI have moderately higher concentrations of inflammatory markers than their leaner peers, supporting the idea that obesity should be considered as a state of chronic low-grade inflammation. Less data is available to allow us to elucidate how physical activity/fitness or dietary patterns may have a direct effect on inflammation in apparently healthy, disease-free young populations.
