An Isokinetic Study of Submaximal Effort in Elbow Flexion

The purpose of this study was to examine the efficiency of a protocol, used in previous studies in an extended form, for differentiation of submaximal from maximal effort in elbow flexion. 15 healthy men aged 21 to 55 years took part in two experimental conditions. In the maximal effort condition subjects were instructed to exert maximal concentric and eccentric efforts and in the best reproducible effort condition subjects were asked to exert similar but submaximal concentric and eccentric efforts which they could best reproduce. The amount of effort, represented by the average moment, was measured relative to two ranges of motion, 30° and 60°, at two angular speeds, 20°/sec. and 60°/sec, using 4 intermittent concentric-eccentric contractions. Both the eccentric/concentric strength ratios as well as the difference between these ratios at the high and the low speeds were highly effective in distinguishing maximal from submaximal efforts. Moreover, since the protocol was equally effective irrespective of the range of motion, it may be used to differentiate submaximal from maximal elbow flexion effort in noninjured subjects.

BACKGROUND: The difference between isokinetic eccentric to concentric strength ratios at high and low velocities (DEC) is a powerful tool for identifying submaximal effort in other muscle groups but its efficiency in terms of the wrist extensors (WE) and flexors (WF) isokinetic effort has hitherto not been studied. OBJECTIVE: The objective of the present study is to examine the usefulness of the DEC for identifying suboptimal wrist extensor and flexor isokinetic efforts. METHODS: Twenty healthy male volunteers aged 20–40 years (28.5 ± 3.2) were recruited. Participants were instructed to exert maximal and feigned efforts, using a range of motion of 20∘ in concentric (C) and eccentric (E) WE and WF modes at two velocities: 10 and 40∘/s. E/C ratios (E/CR) where then calculated and finally DEC by subtracting low velocity E/CR from high velocity ones. RESULTS: Feigned maximal effort DEC values were significantly higher than their maximal effort counterparts, both for WF and WE. For both actions, a DEC cutoff level to detect submaximal effort could be defined. The sensitivity of the DEC was 71.43% and 62.5% for WE ad WF respectively. The specificity was 100% in both cases. CONCLUSION: The DEC may be a valuable parameter for detecting feigned maximal WF and WE isokinetic effort in healthy adults.

The manner in which membrane-fouling experiments are conducted and how fouling performance data are represented have a strong impact on both how the data are interpreted and on the conclusions that may be drawn. We provide a couple of examples to prove that it is possible to obtain misleading conclusions from commonly used representations of fouling data. Although the illustrative example revolves around dead-end ultrafiltration, the underlying principles are applicable to a wider range of membrane processes. When choosing the experimental conditions and how to represent fouling data, there are three main factors that should be considered: (I) the foulant mass is principally related to the filtered volume; (II) the filtration flux can exacerbate fouling effects (e.g., concentration polarization and cake compression); and (III) the practice of normalization, as in dividing by an initial value, disregards the difference in driving force and divides the fouling effect by different numbers. Thus, a bias may occur that favors the experimental condition with the lower filtration flux and the less-permeable membrane. It is recommended to: (I) avoid relative fouling performance indicators, such as relative flux decline (J/J0); (II) use resistance vs. specific volume; and (III) use flux-controlled experiments for fouling performance evaluation.

Abstract The difference between theoretical and empirical triglyceride content is a powerful tool to detect the presence of any vegetable oil in olive oil. The current drawback of the method is the separation between equivalent carbon number ECN42 compounds, which affects the reliability of the method and, hence, its cutoff limit. The determination of the triglyceride profile by liquid chromatography using propionitrile as the mobile phase has recently been proposed to improve their quantification, together with a mathematical algorithm whose binary response determines the presence or absence of hazelnut oil. Twenty-one laboratories from 9 countries participated in an interlaboratory study to evaluate the performance characteristics of the whole analytical method. Participants analyzed 12 samples in duplicate, split into 3 intercomparison studies. Statistically significant differences due to the experimental conditions were found in some laboratories, which were detected as outliers by use of Cochran's and Grubbs' tests. The relative standard deviations (RSD) for repeatability and reproducibility were determined following the AOAC Guidelines for Collaborative Studies. The analytical properties of the method were determined by means of the sensitivity (0.86), selectivity (0.94), and reliability (72) for a cutoff limit of 8 (probability 94).

SummaryObjectives: The aim of the present study is to investigate the variations of the electroencephalographic (EEG) alpha rhythm in order to measure the appreciation of bilateral and unilateral young cochlear implant users during the observation of a musical cartoon. The cartoon has been modified for the generation of three experimental conditions: one with the original audio, another one with a distorted sound and, finally, a mute version.Methods: The EEG data have been recorded during the observation of the cartoons in the three experimental conditions. The frontal alpha EEG imbalance has been calculated as a measure of motivation and pleasantness to be compared across experimental populations and conditions.Results: The EEG frontal imbalance of the alpha rhythm showed significant variations during the perception of the different cartoons. In particular, the pattern of activation of normal-hearing children is very similar to the one elicited by the bilateral implanted patients. On the other hand, results related to the unilateral subjects do not present significant variations of the imbalance index across the three cartoons.Conclusion: The presented results suggest that the unilateral patients could not appreciate the difference in the audio format as well as bilaterally implanted and normal hearing subjects. The frontal alpha EEG imbalance is a useful tool to detect the differences in the appreciation of audiovisual stimuli in cochlear implant patients.

Abstract To explore the diffusion behavior of 75Se(IV) in Beishan granite (BsG), the influences of temperature, oxygen condition and ionic strength were investigated using the through-diffusion experimental method. The effective diffusion coefficient De of 75Se(IV) in BsG varied from 4.21×10−14 m2/s to 3.19×10−13 m2/s in our experimental conditions, increased with increasing temperature. The formation factor Ff of BsG was calculated to be nearly constant in the range of temperatures investigated, suggesting that the inner structure of BsG had no significant change in the temperature range of 20–55°C. Meanwhile, the De values of 75Se(IV) in BsG under anaerobic condition was significantly larger than that under aerobic condition, which may be attributed to the difference in the sorption characteristics and species distribution of Se and pH values. Moreover, the diffusion of 75Se(IV) was promoted with ionic strength increased from 0.01 M to 0.1 M, and then decreased at 0.5 M, mainly due to the combined effects of reduced double layers with increased ionic strength and increase of the solution viscosity at higher ionic strength.

1. The iodination of insulin was studied under various experimental conditions in aqueous media and in some organic solvents, by measuring separately the uptake of iodine by the four tyrosyl groups and the relative amounts of monoiodotyrosine and di-iodotyrosine that are formed. In aqueous media from pH1 to pH9 the iodination occurs predominantly on the tyrosyl groups of the A chain. Some organic solvents increase the iodine uptake of the B-chain tyrosyl groups. Their efficacy in promoting iodination of Tyr-B-16 and Tyr-B-26 is in the order: ethylene glycol and propylene glycol≃methanol and ethanol>dioxan>8m-urea. 2. It is suggested that each of the four tyrosyl groups in insulin has a different environment: Tyr-A-14 is fully exposed to the solvent; Tyr-A-19 is sterically influenced by the environmental structure, possibly by the vicinity of a disulphide interchain bond; Tyr-B-16 is embedded into a non-polar area whose stability is virtually independent of the molecular conformation; Tyr-B-26 is probably in a situation similar to Tyr-B-16 with the difference that its non-polar environment depends on the preservation of the native structure.

The prostheses used by the majority of persons with hand/arm amputations today have a very limited range of motion. Transradial (below the elbow) amputees lose the three degrees of freedom provided by the wrist and forearm. Some myoeletric prostheses currently allow for forearm pronation and supination (rotation about an axis parallel to the forearm) and the operation of a powered prosthetic hand. Older body-powered prostheses, incorporating hooks and other cable driven terminal devices, have even fewer degrees of freedom. In order to perform activities of daily living (ADL), a person with amputation(s) must use a greater than normal range of movement from other body joints to compensate for the loss of movement caused by the amputation. By studying the compensatory motion of prosthetic users we can understand the mechanics of how they adapt to the loss of range of motion in a given limb for select tasks. The purpose of this study is to create a biomechanical model that can predict the compensatory motion using given subject data. The simulation can then be used to select the best prosthesis for a given user, or to design prostheses that are more effective at selected tasks, once enough data has been analyzed. Joint locations necessary to accomplish the task with a given configuration are calculated by the simulation for a set of prostheses and tasks. The simulation contains a set of prosthetic configurations that are represented by parameters that consist of the degrees of freedom provided by the selected prosthesis. The simulation also contains a set of task information that includes joint constraints, and trajectories which the hand or prosthesis follows to perform the task. The simulation allows for movement in the wrist and forearm, which is dependent on the prosthetic configuration, elbow flexion, three degrees of rotation at the shoulder joint, movement of the shoulder joint about the sternoclavicular joint, and translation and rotation of the torso. All joints have definable restrictions determined by the prosthesis, and task.

Measurement of lumbar range of motion (LROM) is a routine method in the examination of patients with low back pain. There is no standard technique which may be used to accurately describe the range of motion in the different plane. So in present study, an attempt has been made to compare goniometric measurement with tape measurement for its sensitivity, specificity and accuracy for measuring spine mobility in normal adults. In this cross sectional study, 137 healthy adults between the ages of 18-26 years of age were included consecutively and assessed using Tape method and goniometry for trunk mobility in all planes (sagittal, frontal and transverse). The association of goniometry with tape method was assessed using chi square test. The study showed that the difference between goniometry and tape method was statistically significant (p value <0.05) for all movements except flexion (p value 0.215). Also the sensitivity, specificity and accuracy of goniometry is not as good as tape method except for flexion compared to tape method. So, from this study we can conclude that Goniometry was not as good as tape method for all movements except forward flexion where both can be equally used.

The purpose of this study is to know the effectiveness of intradialysis exercise using barbells and Range of Motion (ROM) on the adequacy of hemodialysis in patients with chronic kidney disease in the hemodialysis room of RSIJ Sukapura in 2018. The design of this study uses a design with non-probability pre and post test two groups without control design . The result of the difference in the effectiveness of the adequacy values between the two intervention groups after the intervention was given was the barbell intervention obtained 1,33 with a standard deviation of 0.485, an error standard of 0.114. Whereas in the ROM intervention group 1.67 the standard deviation was 0.485, the standard error was 0.114 and the p-value was 0.047 (> 0.05). Conclusion, there was no significant difference in the value of hemodialysis adequacy between the barbellROM intervention groups after the intervention.   Keywords: Hemodialysis Adequacy, Barbell, Exercise Effectiveness, Intradialysis, Range Of Motion (ROM)

Abstract Context Interleukin-2 (IL2), a proinflammatory cytokine, has been used to treat malignancies. Increased cortisol and ACTH were noted, but GH secretion was not investigated in detail. Objective We quantified GH secretion after a single sc injection of IL2 in 17 young and 18 older healthy men in relation to dose, age and body composition. Design This was a placebo-controlled, blinded, prospectively randomized cross-over study. At 20:00 h IL2 (3 or 6 million units per m2 ) or saline was injected sc. Lights were off between 23:00 and 07:00 h. Blood was sampled at 10-min intervals for 24 h. Outcome measures Deconvolution analysis of GH secretion. Results GH profiles were pulsatile under both experimental conditions and lower in older than young volunteers. Since the effect of IL2 might be time-limited, GH analyses were performed on the complete 24-h series and the 6 h after IL2 administration. Total and pulsatile 24-h GH secretion decreased nonsignificantly. Pulsatile secretion fell over the first 6 h after IL2 (P=0.034), with visceral fat as covariate (P=0.003), but not age(P=0.10). Plots of cumulative 2-h bins of GH pulse mass showed a distinction by treatment and age groups: a temporary GH decrease of 32% and 28% occurred in the first 2-h bins after midnight (P=0.019 and 0.038) in young subjects, while in older subjects no differences were present at any time point. Conclusion This study demonstrates that IL2 temporarily diminishes GH secretion in young, but not elderly, men.
