Abstract Background and aims: Metabolic syndrome (MetS) is a collection of metabolic risk factors including increased waist circumference (WC), elevated blood pressure (BP), increased triglyceride (TG), decreased high density lipoprotein (HDL-C) and increased fasting blood sugar (FBS). We aimed to examine the relevance between the MetS and its components with reduced lung functions in adult men. Material and method: A total of 3899 adult men underwent screening examination between 2015-2016 in a cross-sectional survey. Results: The mean (± SD) age of our population was 37.25 (± 4.9) years. The overall prevalence of MetS was 7.6%. The total prevalence of reduced lung function in men with MetS was 13.8%. The most common type of reduced lung function was the restrictive pattern (7.1%). The forced expiratory volume of first second (FEV1) and forced vital capacity (FVC) values were significantly lower in men with MetS (both p<0.001). Also these values were significantly lower in diabetic men compared to non-diabetics and those with impaired fasting glucose (IFG). WC and HDL were the most potent predictors of reduced FEV1 and FVC. Conclusions: We obtained a positive independent association between MetS and reduced lung function in adult men which may be related mainly due to increased WC and decreased HDL.

Purpose We examined the cross-sectional association between lung function and metabolic syndrome (MetS), independent of fasting immunoreactive insulin (F-IRI). Methods A total of 3,072 middle-aged, apparently healthy subjects who participated in a general health check-up were included. Lung function, which was expressed as forced vital capacity (%FVC predicted) or forced expiratory volume in 1 second (FEV1% predicted) was examined. Multivariable logistic regression analysis was performed to assess the association between lung function and MetS. Results Men with the lowest quartile of FVC% predicted, or those with the lowest quartile of FEV1% predicted, had a 3.5-fold or 2.6-fold increased risk of MetS, respectively, compared with those with the highest quartile of FVC% predicted or FEV1% predicted. F-IRI had a positive, significant, and independent association with MetS in both sexes. Conclusion Impaired lung function increased the risk of MetS, independent of F-IRI and smoking in men, but not in women.

Metabolic syndrome (MS) is an aggregation of coexisting conditions that can indicate an individual’s high risk of major diseases, including cardiovascular disease, stroke, cancer, and type 2 diabetes. We conducted a cross-sectional survey to evaluate potential risk factor indicators by identifying relationships between MS and anthropometric and spirometric factors along with blood parameters among Korean adults. A total of 13,978 subjects were enrolled from the Korea National Health and Nutrition Examination Survey. Statistical analysis was performed using a complex sampling design to represent the entire Korean population. We conducted binary logistic regression analysis to evaluate and compare potential associations of all included factors. We constructed prediction models based on Naïve Bayes and logistic regression algorithms. The performance evaluation of the prediction model improved the accuracy with area under the curve (AUC) and calibration curve. Among all factors, triglyceride exhibited a strong association with MS in both men (odds ratio (OR) = 2.711, 95% confidence interval (CI) [2.328–3.158]) and women (OR = 3.515 [3.042–4.062]). Regarding anthropometric factors, the waist-to-height ratio demonstrated a strong association in men (OR = 1.511 [1.311–1.742]), whereas waist circumference was the strongest indicator in women (OR = 2.847 [2.447–3.313]). Forced expiratory volume in 6s and forced expiratory flow 25–75% strongly associated with MS in both men (OR = 0.822 [0.749–0.903]) and women (OR = 1.150 [1.060–1.246]). Wrapper-based logistic regression prediction model showed the highest predictive power in both men and women (AUC = 0.868 and 0.932, respectively). Our findings revealed that several factors were associated with MS and suggested the potential of employing machine learning models to support the diagnosis of MS.

ObjectiveResistin increases insulin resistance (IR) in mice. However, the role of resistin in human disease remains controversial. We aimed to assess plasma resistin levels and their associations with inflammatory and fibrinolytic markers, IR and metabolic syndrome (MetS) among Chinese.Design and methodsPlasma resistin was measured in a population-based cross-sectional survey of 3193 Chinese aged from 50 to 70 years in Beijing and Shanghai.ResultsThe median resistin concentration was 8.60 ng/ml (interquartile range, 5.78–14.00) among all participants, and it was higher in women than in men (P=0.008). Resistin was correlated weakly with body mass index, waist circumference, high-density lipoprotein (HDL) cholesterol (negatively), homeostatic model assessment of IR and tumor necrosis factor-α receptor 2 (TNFR2; r=0.04, 0.07, –0.09 and 0.06 respectively, all P<0.05), and more highly with C-reactive protein (CRP), interleukin (IL)6 and plasminogen activator inhibitor (PAI)1 (r=0.12, 0.12 and 0.21 respectively, all P<0.001), but only HDL cholesterol, CRP, IL6, TNFR2, and PAI1 remained significantly associated with resistin in multiple regression analysis (all P<0.05). Furthermore, elevated resistin levels were associated with the higher prevalence of IR and MetS. However, the significant relationships disappeared after adjustment for inflammatory and fibrinolytic markers especially PAI1.ConclusionsThis study suggests that resistin is more strongly associated with inflammatory and fibrinolytic markers than with obesity or IR status. The associations of resistin with IR and MetS could largely be explained by inflammatory and fibrinolytic markers especially PAI1 levels.

Abstract This study aimed to investigate whether obesity and metabolic syndrome (MetS) are associated with pulmonary function in Korean children and adolescents. Data from the 2009–2011 Korea National Health and Nutrition Examination Survey which is cross-sectional, nationwide, and representative survey were used. Adjusted regression analysis was performed to evaluate the association of obesity and MetS with lung function in children and adolescents. A total of 763 children and adolescents aged 10–18 years were evaluated. We found no significant difference in FEV1% predicted, FVC% predicted, and FEV1/FVC ratio among the obesity groups. Subjects with MetS showed a significantly lower FEV1 predicted (91.54 ± 0.74% vs 94.64 ± 0.73%, P = 0.004), lower FVC% predicted (91.86 ± 0.63% vs 95.20 ± 0.63%, P < 0.001), and lower FEV1/FVC ratio (76.76 ± 0.43% vs 80.13 ± 0.43%, P < 0.001) than those without MetS. Elevated waist circumference (WC), systolic blood pressure, fasting glucose, and lower high-density lipoprotein cholesterol (HDL-C) were independently associated with lower FEV1/FVC ratio (all P < 0.05, respectively). Among MetS components, increased WC was the most important factor influencing lower FEV1/FVC ratio. In conclusion, lung function in MetS patients was significantly lower, and the MetS component was independently associated.

Background: Metabolic syndrome is defined as at least three of the five following medical conditions: central obesity(mandatory), high blood pressure, high blood sugar, high serum triglycerides, and low serum high-density lipoprotein (HDL). It has been shown that obesity causes physiological impairment in respiratory system.Methods: In this observational cross sectional study, 60 patients who fulfilled the IDF criteria for metabolic syndrome were included after applying the exclusion criteria and were subjected to spirometry.Results: Most of the patients had a restrictive pattern (43.3%) (n=26). 33.3% (n=20) of the patients had a mixed pattern while 16.7% (n=10) of the patients had a normal picture. Only 6.7% (n=4) of the patients had an obstructive pattern (p<0.001). Mean value of FEV1 had no correlation with increasing number of components of metabolic syndrome whereas mean value of FVC showed a decreasing trend with increasing number of components of metabolic syndrome. Mean FEV1 of the patients was significantly higher in males (1.82±0.71 L) than females (1.24±0.42 L) (p 0.007). Similarly, the mean FVC was significantly higher in males as compared to females. Mean FVC was 2.45 (±0.86) L in males while females had a mean FVC of 1.65 (±0.49) L (p 0.002).Conclusions: In conclusion, Patients with metabolic syndrome have significant impairment of the pulmonary function with restrictive pattern being the most common one.

ObjectiveTo assess the association of lung function and respiratory symptoms with farming, particularly pesticide use, in an agricultural province in Thailand.MethodsWe undertook a cross-sectional survey of adults aged 40–65 in Nan province, Thailand, between May and August 2019. We randomly recruited 345 villagers and enriched the sample with 82 government employees. All participants performed post-bronchodilator spirometry and completed a questionnaire covering information on respiratory symptoms, farming activities, pesticide use and known risk factors for respiratory disease. Associations of respiratory outcomes with farming and pesticide exposures were examined by multivariable regression analysis.ResultsThe response rate was 94%. The prevalence of chronic airflow obstruction among villagers was 5.5%. Villagers had, on average, a lower percent predicted post-bronchodilator forced expiratory volume in one second/forced vital capacity (FEV1/FVC) than government employees (98.3% vs 100.3%; p=0.04). There was no evidence of association of lung function with farming activities, the use of specific herbicides (glyphosate and paraquat), insecticides (organophosphates and pyrethroids) or fungicides. The exceptions were poultry farming, associated with chronic cough and an increase of FEV1/FVC, and atrazine, for which duration (p-trend <0.01), intensity (p-trend <0.01) and cumulative hours (p-trend=0.01) of use were all associated with higher FEV1/FVC in an exposure–response manner. Cumulative hours (−280 mL/hour), low duration (−270 mL/year) and intensity (−270 mL/hour/year) of atrazine use were associated with lower FVC.ConclusionsChronic airflow obstruction is uncommon among villagers of an agricultural province in Nan, Thailand. Farming and pesticide use are unlikely to be major causes of respiratory problems there.

Background and aimsIn sub-Saharan Africa, infectious diseases are still the leading causes of mortality; however, this may soon be surpassed by non-communicable illnesses, namely hypertension, diabetes and cardiovascular disease. This study determined the prevalence and patterns of metabolic syndrome and cardio-risk factors in men and women in rural Uganda.MethodsA household-based, cross-sectional survey was carried out following the WHO STEP-wise approach to surveillance. It included demographic and lifestyle questionnaires, anthropometric measurements and biochemical analyses. Of the 200 randomly recruited participants, 183 successfully completed two steps of the study and 161 provided blood samples.ResultsData were collected from 183 adults, aged 18–69 years; 62% were female. Based on the National Cholesterol Education Program-Adult Treatment Panel-III criteria, the prevalence of metabolic syndrome was 19.1% (95% CI 14.0 to 22.5). Elevated fasting plasma glucose was observed in 14.2% (95% CI 9.1 to 19.3) of participants, hypertriglyceridaemia in 16.9% (95% CI 12.1 to 23.1); hypertension in 36.1% (95% CI 29.0 to 43.0) and 52.5% (95% CI 45.2 to 59.6) had low HDL (high-density lipoprotein) cholesterol. Abdominal obesity was found in 24.6% (95% CI 18.8 to 31.4) of participants. Sex disparities were significant for several risk factors. Females had significantly higher prevalence of abdominal obesity (38.6% vs 1.5% in males, p=0.001) and twice the rates of low HDL (65.8% vs 30.4%, p=0.001). Men tended to have higher but not significant rates of hypertension (42.0% vs 32.5%) and smoked significantly more than women (49.3% vs 21.1%, p<0.001). Alcohol consumption was also higher in men (55.1% vs 18.4%, p<0.001) and quantities consumed were approximately three times greater than in females (p<0.001).ConclusionMetabolic syndrome exists at worrying rates in the rural Ugandan population. Sex disparities are evident in risk factor prevalence, reflecting physiological variables and deeply entrenched cultural and lifestyle norms.

Background: The increasing incidence of metabolic syndrome has been on the rise especially in urban population and leading to increased risk of cardiovascular disease (CVD) and diabetes mellitus. It has been associated with impairment of pulmonary functions. However, there is limited data regarding the association with individual components of metabolic syndrome and overall effect on components of pulmonary functions.Methods: This is a cross sectional study consisting of 50 subjects with metabolic syndrome. All the subjects underwent pulmonary function tests and the association between different components of metabolic syndrome and pulmonary function were examined using unpaired t-test and Pearson’s partial correlation coefficient. This data was analysed by using statistical package for the social sciences (SPSS) version 12.0.Results: In females, moderate negative significant correlation was seen between forced vital capacity (FVC) and systolic blood pressure (SBP), diastolic blood pressure (DBP), fasting blood sugar (FBS), triglyceride (TG), waist circumference (WC) and body mass index (BMI) whereas positive weak non-significant correlation was seen between FVC and high density lipoprotein cholesterol (HDLC), while no such relation was found with forced expiratory volume in one second (FEV1). In males, negative, moderate significant correlation was seen between FVC and FBS whereas strong, positive, significant correlation was seen between FVC and WC; between FVC and BMI. Negative, moderate, significant correlation was seen between FEV1 and WC; between FEV1 and BMI.Conclusions: Our study concluded that there was a significant impact of FBS and WC on decreasing FVC on both genders with minimally significant impact of other components of metabolic syndrome on FVC with no effect on FEV1 hence indicating a restrictive pattern of pulmonary function derangement. Hence, further studies with larger sample size is needed to confirm whether there are direct or indirect mechanisms through which insulin resistance could affect pulmonary function.

IntroductionThere is growing interest in the impact of greenness exposure on airway diseases, but the impact of greenness on lung function in children is limited. We aimed to investigate the associations between greenness surrounding schools and lung function in children and whether these associations are modified by air pollution exposure.MethodsBetween 2012 and 2013, a cross-sectional survey and spirometry were performed among 6740 school children. Lung function patterns were determined as obstructive forced expiratory volume 1 s/forced vital capacity (FEV1/FVC <0.8) or restrictive (FEV1/FVC ≥0.8 but FVC <80% of predicted). School greenness was defined by Normalized difference vegetation index (NDVI) and soil-adjusted vegetation index. Nitrogen dioxide, sulphur dioxide and particular matter concentrations were assessed using a spatiotemporal model and national monitoring data. Two-level generalised linear models were used to investigate associations and interactions.ResultsOverall, an IQR in NDVI within 500 m was associated with higher FEV1 (+57 mL 95% CI 44 to 70) and FVC (+58 mL 95% CI 43 to 73). NDVI was similarly associated with 25% reduced odds of spirometric restriction (OR: 0.75, 95% CI 0.65 to 0.86). However, among children exposed to the highest compared with the lowest quartile of particulate matter, increasing NDVI was paradoxically associated with lower −40 mL FVC (95% CI −47 to –33, p interaction <0.05).DiscussionOur findings suggest that, in this study population, greening urban areas may promote lung health in low–moderate pollution areas but not in high air pollution areas. If the findings are replicated in other moderate-to-high pollution settings, this highlights a need to have a flexible green policy.
