With his report, Mons. Francesco Giovanni Brugnaro, Archbishop of Camerino - San Severino Marche, offers a personal testimony of his experience as a priest, friend and colleague of the Rectoral Prof. Paolo Mantegazza, who he met in the years ‘89-’95 during his chaplain service at the Rectory of Santa Maria Annunciata of the University of Milan. In addition to the affectionate and grateful remembrance towards the exemplary role of rector, father and teacher, he adds some characteristics regarding the relationship that Prof. Mantegazza knew to weave with the university students, careful to consider the person as a whole rather than as a subject impersonal and anonymous. This special attention stemmed from the profound conviction that education is much more than simple teaching. He introduced in his long experience as a teacher the one that transformed his courses into real schools of life: the human and ethical contribution to face the most difficult future challenges in the professional field. For this reason he was also a great mediator for the very talented teachers to keep the relationship between scientific and didactic research high, between the quality of the preparation for the doctorate-specialization and the moral and methodological seriousness of the future professional. In the memory of Mons. Brugnaro, the pain that struck the Mantegazza family for the tragic and premature disappearance of the two beloved sons is preserved. Inspired also by Don Giussani’s charism, together with his wife Andreina, he was able to transform that lacerating pain into an exemplary witness of Christian life founded on the Resurrection of Christ. The common attendance of the Archbishop of Milan from 1979 to 2002, allowed both to live a daily faith, drawn from the prophetic vision of the biblical pastoral of Card. Martini also within the University, stimulating the chair of non-believers.

The objective of this study was to find out the barriers of sentinel events reporting at tertiary hospital and then formulate recommendations to ameliorate based the findings of this study. The study was carried out in 2015 and conducted as two parts; the first part is a retrospective data, review of Occurrence Variance Reports forms according to the reporter. The second part was through self-administered questionnaires, which was randomly distributed to the hospital staff. A total of 120 completed questionnaires out of 135 were received.  In this study, we found that the commonest reasons that prevent the hospital staff from reporting the sentinel events are poor communication of policy and procedure of sentinel events reporting, lack of motivation among the staff to report sentinel events in 28 % and 26 % respectively. Staff fear of punitive actions from the hospital administration was rated as the first and second priority reason in preventing employee reporting sentinel events in about half of the cases. No feedback from reporting sentinel events was the reason for underreporting in about one fifth. Sentinel events definition was not clear and sentinel cases reporting form was not available in 14 % and 15 % respectively. This study highlights the common factors that may contribute to under-reporting of sentinel events in tertiary hospital. The findings may be useful in formulating beneficial strategies to improve reporting which will have great value on quality of care and patient safety.

In the context of growing competition between rail transport and other transport types, there is a need to improve the transportation process. In modern conditions, railway transport is the main link in the logistics chain of moving goods from the shipper to the receiver. The efficiency of the railway transport complex can be improved by using logistics principles in the organization of local work on the network sections. In this study, we suggest ways to solve the problem of improving the quality of service for freight owners and the efficiency of organizing local work, which is relevant for railways. To make decisions aimed at improving the efficiency of the organization of local work, a procedure for determining the costs of organizing and promoting local trains was developed, taking into account the costs on maintaining the railway infrastructure. Not only the operational activity of the railways, but also the quality of transport customers service, compliance with the regulatory deadlines for cargo delivery, depends on the choice of the optimal option for organizing local car traffic into trains and their promotion along the section. The option is selected by comparing the costs for each of the possible options for organizing local car traffic into trains and moving them along the section.

The article analyzes the EU rail transport directives to be implemented in Ukraine after signing the Association Agreement between the EU and Ukraine, as well as the systematic positive and negative consequences and reservations of the implementation of the directives in the Ukrainian rail transport legislation. However, bringing of rail transport to the state declared in these directives requires significant investments including foreign ones. At the same time, the investment attractiveness of the railway industry is very low. The reason for this is the unsatisfactory state of the non-current assets, low quality of transportation, unprofitable passenger transportation which requires cross-subsidization, etc. Upgrade of the transport infrastructure should be carried out with the use of the latest energy efficient technologies. There is a need to change the management principles of the railway industry as a whole. Fulfilment of implementation conditions will trigger the attraction of investments and increase of the revenue part of the budget of Ukrzaliznytsia, which in turn will increase the competitiveness of the domestic railway.

While transporting passengers and cargo by rail transport, there is always a risk of an unusual situation arising as a result of exposure to various factors or actions of people (operators) namely a railway transport accident. In case of such a situation official (special) investigations are performed; the reasons for its occurrence are detected. Materials, testimonies and facts determinated while performing this type of investigation are important enough and used while carrying out the research in the framework of railway transport forensic examination. All efforts to improve the methods of investigating the causes and factors of influence on the occurrence of railway transport accidents are reduced to one goal: improving the quality of their investigation and preventing crashes and catastrophes in rail transport in the future. There are certain methodological approaches while conducting an official investigation of railway transport accidents abroad (in the Baltic States, Kazakhstan, Russia) and in Ukraine. The process of performing an official investigation in Ukraine is examined in detail according to current Methodological Guidelines. Work towards improving and perfection these approaches has been ongoing and this has undoubtedly been positively reflected on the research process within the frameworks of implementation of forensic railway transport examinations. It should be also noted that the competent, complete and high-quality execution of materials submitted to forensic experts for performing examination will allow to fulfill the assigned forensic railway transport examinations and provide substantiated conclusions in optimal time frame.
