IMPROVING THE SYSTEM OF FINANCING DRUG PROVISION IN THE RUSSIAN FEDERATION

In the current system of financing the drug provision of certain categories of citizens in the outpatient setting in the Russian Federation, there are a number of problems caused by various factors, such as the imperfection of the legislative framework, the implementation of control procedures, etc. The article reveals in detail the problem of lack of funding in connection with the establishment of unified approaches to calculating the standards of financial costs per month per citizen for all regions of Russia, which do not take into account any of the factors of regional development. It has been proposed to change the approaches in calculating the standards, which will ensure their approximation to the real needs of financing preferential categories of citizens.

The article takes a theoretical perspective on the current approaches to ensuring and maintaining sustained development. On the basis of official statistics and regulatory legislation of the Russian Federation, the authors evaluate the social and economic environment in different regions of Russia, identifying the constraints that the current system of regional management imposes upon sustained development progress. The researchers claim the current methods of research to be ineffective for an adequate evaluation of regional sustained development, opting for an innovative customized method.

Changes in the scope and structure of morbidity increase the cost of health care, medical and social services, especially in terms of drug supply. The cost of medicines amounts to the third largest component of costs in the health sector. The Russian Federation possesses the one out provisuinfastest growing pharmaceutical market. However, for the time being the current system of drug supply in the Russian Federation was faced with a number of systemic issues that have a direct effect on the availability of medicines, the quality and timeliness of care. Within frameworks of the executed analysis there was demonstrated the ineffectiveness of the existing drug supply system in an outpatient setting, as well as there was supposed a number of measures to optimize government programs of drug provision in the current economic situation.

The article examines the main strategic documents of the Russian Federation: national projects, state programs aimed at reducing the level of interregional differences in regions. The assessment of the positions of the regions of Russia in the rating by the quality of life is carried out. Conclusions are made about the effectiveness of the implementation of state programs in areas related to balanced regional development and quality of life. The main shortcomings of the strategy of spatial development of the Russian Federation are revealed and the necessity of using additional tools aimed at reducing the lag in the rates of socio–economic development for a number of regions is substantiated.

The current legislative approaches to understanding the essence and implementation of internal control procedures by economic entities have been systematized. The approach to understanding the essence of internal control as an element of the company’s accounting system that prevails in departmental and industry regulations on accounting in the Russian Federation has been critically analyzed. The historical digression of the development of understanding of the term “internal control” has been presented and its connection with the subjects of control has been proved. The transformation of understanding of the essence of internal control, which is contained in the regulatory legal acts of the USSR and the Russian Federation, from a subjective approach to a simple process approach has been traced, in accordance with the needs of business entities. The ways of improving the legislative framework of internal control have been outlined.

Subject. This article analyzes the changes in poverty of the population of the Russian Federation. Objectives. The article aims to identify macroeconomic variables that will have the most effective impact on reducing poverty in Russia. Methods. For the study, I used the methods of logical, comparative, and statistical analyses. Results. The article presents a list of macroeconomic variables that, according to Western scholars, can influence the incomes of the poorest stratum of society and the number of unemployed in the country. The regression analysis based on the selected variables reveals those ones that have a statistically significant impact on the financial situation of the Russian poor. Relevance. The results obtained can be used by the financial market mega-regulator to make anti-poverty decisions. In addition, the models built can be useful to the executive authorities at various levels for short-term forecasting of the number of unemployed and their income in drawing up regional development plans for the areas.

Purpose of the study. Healthcare budget impact analysis in the Russian Federation of providing adult patients with chronic obstructive pulmonary disease (COPD) with basic therapy for outpatient treatment. Material and methods. The hypothesis of reducing costs for the treatment of exacerbations of COPD was tested while providing all patients with basic therapy on an outpatient basis. The model calculates the direct medical costs of drug provision and treatment of exacerbations in patients with COPD in the current situation and when all patients with COPD are covered with basic therapy as part of a program of preferential drug provision. The simulation period is 1 year. The analysis does not take into account social losses associated with disability, payment of disability certificates, reduced labor productivity and mortality. Results. The cost of providing all patients with COPD with the recommended basic drug therapy on an outpatient basis is substantially offset by a decrease in the cost of treating exacerbations: drug costs should increase by 14.1 billion rubles compared with the current situation, while the cost of providing medical care to patients with exacerbations is reduced by 9.7 billion rubles. As a result, the difference in direct medical costs, or the necessary additional investments, is 4.4 billion rubles. The price of drugs used for basic therapy has the greatest impact on the size of the required additional investments. Conclusion. Providing all patients with COPD with the recommended basic drug therapy on an outpatient basis, we can expect a decrease in the number of moderate exacerbations per year by 14.7% and severe exacerbations by 31.2%. Taking into account not only medical, but also socio-economic costs can not only offset additional investments, but also demonstrate significant savings. When implementing a pilot project of preferential drug provision for patients with COPD in a specific region of the Russian Federation, it is possible to reduce the cost of purchasing medicines when forming a municipal order.

Russian President Vladimir Putin defined increase in life expectancy from 72.7 to 78 years by 2024 as a national aim in the Decree № 204 of May 7, 2018. Achievement of this aim depends on drug provision system among other factors. Strategy of drug provision for the population of the Russian Federation for the period until 2025 sets the goal of increasing availability of high quality, effective and safe medicines to meet needs of the population and the health system based on the formation of a rational and balanced system of drug provision for the population of the country with available resources. The health care system should expand the possibilities of using modern and effective mechanisms to ensure the financing of drug provision for the population.

The optimal list of laboratory tests used in the practice of a general practitioner and organizational options for performing laboratory tests, the features of the process that affect the speed of obtaining a result by a doctor in an outpatient setting and in hospital, are presented.

The topic of quantitative research on informal employment has a consistently high relevance both in the Russian Federation and in other countries due to its high dependence on cyclicality and crisis stages in economic dynamics of countries with any level of economic development. Developing effective government policy measures to overcome the negative impact of informal employment requires special attention in theoretical and applied research to assessing the factors and conditions of informal employment in the Russian Federation including at the regional level. Such effects of informal employment as a shortfall in taxes, potential losses in production efficiency, and negative social consequences are a concern for the authorities of the federal and regional levels. Development of quantitative indicators to determine the level of informal employment in the regions, taking into account their specifics in the general spatial and economic system of Russia are necessary to overcome these negative effects. The article proposes and tests methods for solving the problem of assessing the impact of hierarchical relationships on macroeconomic factors at the regional level of informal employment in constituent entities of the Russian Federation. Majority of the works on the study of informal employment are based on basic statistical methods of spatial-dynamic analysis, as well as on the now «traditional» methods of cluster and correlation-regression analysis. Without diminishing the merits of these methods, it should be noted that they are somewhat limited in identifying hidden structural connections and interdependencies in such a complex multidimensional phenomenon as informal employment. In order to substantiate the possibility of overcoming these limitations, the article proposes indicators of regional statistics that directly and indirectly characterize informal employment and also presents the possibilities of using the «random forest» method to identify groups of constituent entities of the Russian Federation that have similar macroeconomic factors of informal employment. The novelty of this method in terms of research objectives is that it allows one to assess the impact of macroeconomic indicators of regional development on the level of informal employment, taking into account the implicit, not predetermined by the initial hypotheses, hierarchical relationships of factor indicators. Based on the generalization of the studies presented in the literature, as well as the authors’ statistical calculations using Rosstat data, the authors came to the conclusion about the high importance of macroeconomic parameters of regional development and systemic relationships of macroeconomic indicators in substantiating the differentiation of the informal level across the constituent entities of the Russian Federation.

The author touched upon topical issues of inter-budget alignment, budget provision of regions with financial resources, focusing on factors that directly affect the stability of the budget system, as well as highlighting problems in the budget sphere that require urgent attention from the authorities. It is concluded that the level of interregional differentiation continues to increase, and the modern mechanism of budget equalization requires urgent amendments and adjustments. It is emphasized that the main goals of socio-economic development of the Russian Federation related to the transition to an innovative type of development, changes in the structure of the national economy, set out in strategic documents and messages of the President of the Russian Federation and declared since 2009, are not being implemented, and the effectiveness of state economic policy and Federal budget expenditures for its implementation remains low. This indicates the need to review the current system.
