The discrepancy between the data of different authors on the prevalence of comorbidity pathology in patients with bronchial asthma, in particular the digestive organs, and the impact of comorbidities during asthma has led to the feasibility of this study.

Objectives to reveal cause-and-effect relationships in the system "air pollution morbidity of the population" in a large industrial city. Material and methods.The dynamics of the pollutants concentration, such as formaldehyde and benzopyrene, through the period of 20152017 was analyzed in relation to the indicators of the morbidity of the respiratory system in the population of Samara. In particular, the study focused on the ratio of allergic rhinitis and asthma cases and the concentration of the formaldehyde and benzopyrene. Results.The incidence of respiratory diseases and the general morbidity increased both among children (by 46.7%) and adults (by 11.4%). There was the causal correlation between the effect of benzopyrene on the development of allergic rhinitis and bronchial asthma in children(r = 0.57 and r = 0.76) and in adults (r = 0.45 and r = 0.51) in Samara. The significant differences between the level of the pollutants and the frequency of the respiratory system diseases were registered in children and adults in different areas of Samara. Discussion.As a result of the increase of the maximum allowable concentration (MAC) of formaldehyde in atmospheric air, this pollutant has now ceased to be a priority. The revealed negative correlations point to the absence of an adverse effect of formaldehyde on the development of allergic rhinitis and bronchial asthma in both children and adults in Samara. Conclusion.According to the results of the study, the preventive measures aimed at reducing the impact of main air pollutants on the health of children and adults in Samara were suggested. Their implementation is possible by the immediate informing of the executive authorities to ensure they can make timely management decisions to improve the quality of the environment.

The small respiratory passages dysfunction (SRPD) is found in the vast majority of patients with bronchial asthma (BA). The SRPD is currently recognized as the important pathogenetic feature of BA. The purpose of this review is to analyze the current scientific knowledge about the poorly studied aspects of the small respiratory passages (SRR) participation in the development of pathological process in BA, as well as the impact of small bronchial dysfunction on the clinical course, the exacerbation frequency and the disease control. The importance of SRPD diagnostics in BA patients for optimal and timely treatment is discussed. The modern methods of RPD pathology diagnostics are described; their informative use in the comparative study aspect is considered.

Topicality. Prevalence of bronchial asthma (BA) and COPD achieves in different countries 18 and 20% respectively. The prevalence of «OVERLAP» syndrome - ASTHMA/COPD in Russia is unknown. Objective. To determine the prevalence ofBA, COPD and «OVERLAP» syndrome - ASTHMA/COPD. Materials and methods. The study included a survey using a specially developed questionnaire to identify asthma-like symptoms (ALS), risk factors for BA, COPD, and the definition of FEV^ FEV^FVC in adults in Ekaterinburg. Results. The study has revealed that the risk factors for developing ALS are exposure to tobacco smoke and age over 40 years. The impact of allergens and family history of allergy are less significant. Decrease of spirometry indices is associated with smoking, age over 40 years and the presence of ALS. The diagnostic criteria for the «OVERLAP» syndrome - ASTHMA/COPD were developed based on the answers to the questionnaire on ALS, risk factors and the results of the screening spirometry. Conclusions. Preliminary prevalence ofBA, COPD, «OVERLAP» syndrome - ASTHMA/COPD and actual risk factors have been established in Ekaterinburg in 2018. Unfavorable situation with prevalence of smoking in Ekaterinburg has been showing.

<p class="Default"><strong>The aim </strong>is to analyze the effectiveness of non-drug methods of treatment and rehabilitation of children and adults with bronchial asthma.<strong></strong></p><p class="Default"><strong>Methods. </strong>To realize the purpose of the study, a system-functional analysis of special literature and materials of the Internet was used.<strong></strong></p><p class="Default"><strong>Results. </strong>The standard approach to antiasthmatic therapy does not always lead to a controlled course. Reducing the impact of allergens both in the home and in the workplace is an effective measure to control the disease. Phytotherapy and reflexology are widely and effectively used. Diet therapy is ineffective, the use of vitamin D in the treatment and rehabilitation of patients with bronchial asthma is encouraging. The vaccine does not increase the risk of the disease, but it is not effective for prevention. Increasing physical activity is the most effective method of rehabilitating patients. Evidence from recent studies suggests that interventions aimed at eliminating a single allergen may be effective in reducing allergen levels and asthma incidence, and this approach deserves further study. Currently, a comprehensive multifaceted study of acupuncture, which will be able to provide reliable clinical evidence of the effectiveness of acupuncture for the treatment and prevention of asthma. At the same time, there is a need for comprehensive research in the field of phytotherapy and training of respiratory muscles.</p><p class="Default"><strong>Conclusion. </strong>Non-drug methods of prevention and rehabilitation are widely used (as additional) in bronchial asthma. Despite many years of use of phytotherapy, acupuncture, breathing exercises in the treatment and rehabilitation of asthma, so far there are no clear recommendations for their use: indications, dosage, duration, frequency and the lack of standardization makes it difficult to use them.</p>

This year’s World Asthma Day has the theme “Uncovering Asthma Misconceptions”. This position calls for action and action to clarify common myths and misconceptions about asthma that prevent asthma sufferers from receiving optimal benefits from major success in treating the condition. According to GINA experts, the most common mistakes in the world about asthma are as follows: 1. Myth: asthma is a childhood disease; people “outgrow” it as they age. True: asthma can occur at any age. 2. Myth: Asthma is an infectious disease. Truth: asthma is not contagious disease. 3. Myth: People with asthma should not exercise. Truth: When asthma is well controlled, individuals with asthma can exercise and achieve high performance in sports. 4. Myth: asthma can only be controlled with high doses of steroids. Truth: Most often, asthma is controlled with low doses of inhaled steroids. To a large extent, these theses refer to one of the most important challenges of our time — the management of patients with bronchial asthma in the context of the COVID-19 pandemic. Patients with asthma have a lower susceptibility to COVID-19, a less severe course, and a lower risk of hospitalizations due to COVID-19. Allergic asthma or its eosinophilic phenotype, intake of inhaled corticosteroids (ICS) have a positive effect on the course of the COVID-19 disease, since in such patients the lower expression level of ACE 2 receptors in the upper and lower respiratory tract, which are input receptors for SARS-CoV-2 virus. ICS such as budesonide or ciclesonide are capable of inhibiting the replication of genomic SARS-CoV-2 RNA due to the influence of viral endonuclease NSP15 and TMPRSS2 (transmembrane serine protease 2), a protease involved in viral entry into the cell. Some ICS (including budesonide) reduce or block SARS-CoV-2 replication in vitro. Experts from international asthma groups note that in the face of the COVID-19 pandemic, asthma patients should continue to take basic therapy, including corticosteroids. And in current studies, ICS budesonide in COVID-19 patients reduces the risk of hospitalization or emergency care by 91 % and significantly improves clinical recovery. The review presents data on the role of respiratory agents in the pathogenesis of exacerbations of bronchial asthma and the impact of vaccination on the clinical course of the disease. The features of the formation of protective immunity in various immunization schemes are analyzed. In patients with bronchial asthma, the substantiation of the vaccination scheme with the advantage of priority administration of conjugated polysaccharide vaccine followed by the introduction of polysaccharide pneumococcal vaccine after 8 weeks is given. It was shown that vaccination against pneumococcal infection resulted in elimination from sputum S. pneumoniae. Influenza vaccination in patients with bronchial asthma reduced the frequency and duration of exacerbations. Combined vaccination against influenza and pneumococcal infections did not reduce the clinical and immunological effect.

The article presents the results of the application of photochemotherapy for the combined rehabilitation treatment of the patients suffering from bronchial asthma. The assessment of the effectiveness and the impact of this treatment was made on the basis of changes of the indicators of blood rheology as one of the components of pathogenesis of this disease with due regard for the signs and symptoms characterizing the clinical course of this condition. It has been demonstrated that 74 patients presenting with bronchial asthma experienced the statistically significant restoration of the altered rheological parameters of blood and the general improvement of the clinical course of the disease under the influence of photochemotherapy in comparison with the results of the baseline treatment obtained in the remaining 42 patients. The effect of optical radiation of blue and red light on the clinical course of bronchial asthma was more pronounced than that of the conventional treatment. The two therapeutic modalities were virtually identical when evaluated based on the effectiveness criteria, but differed in terms of the mechanism of action on the body.

Bronchial asthma is the most common chronic disease in children affecting 30% of the child population. The purpose of this review was to analyze and summarize the works of foreign and Russian authors who evaluated the impact of mental disorders on the course of asthma in children. The article presents the definition of asthma as a psychosomatic disease and relevance of the study against this background. The authors adduce the results of research studies which allow us to estimate the interaction between the degree of anxiety, accentuation of the nature, severity of asthma, and the duration of the disease. The existing publications demonstrate that the researchers' opinions are polarized regarding the cause-and-effect relationship between psychological disorders and controlled asthma. The authors came to a conclusion that early detection of psychopathological symptoms and appropriate psychotherapeutic administration would improve the efficacy of treatment in children with bronchial asthma and the social adaptation, adjust the system of family relations, and have a positive effect on the course of the disease.
