Background: People with Dementia (PwD) are frequently admitted to hospital settings. The lack of proper dementia knowledge, poor communication skills, negative attitudes toward dementia, and lack of confidence affects the quality of care, thus development of dementia trainings has increased. Nevertheless, literature regarding the effectiveness of training implementation is limited. Objective: The aim of this narrative synthesis is to 1) identify the characteristics of training programs and 2) explore the effectiveness of these training programs in everyday clinical practice. Methods: A systematic search in PubMed, PsycINFO, CINAHL, and Cochrane was conducted, including qualitative and quantitative peer-reviewed studies. Holton’s evaluation model with its three outcome levels (learning, individual performance, and organizational results) was adopted. 14 studies were included. Results: The synthesis of the results was divided into two parts: 1) to describe the characteristics and content of trainings 2) to evaluate the effectiveness of training programs according to the three outcome levels of Holton’s model, taking into consideration its construct domains: ability, motivation, and environment. Learning outcomes were assessed in all selected studies: 13 studies observed changes in individual performance, four studies reported changes within the organizational level, and only five showed sustainable changes over time. Conclusion: Person-centered care (PCC) approaches, interactive and varied teaching methods, supporting conditions like champions, action plans, and setting care policies, are all characteristics of effective trainings. Successful programs should be sustainable over time, demonstrating positive outcomes across the organization. Based on current findings, there is a lack of adequate evaluation with regard to training programs on the organizational level.

People with Dementia (PwD) are frequently admitted in general hospitals. However, health care professionals have lack of dementia knowledge, negative attitudes toward dementia, and lack of confidence in caring those patients. The aim of this study is to develop, implement and evaluate a dementia staff training program in Greek general hospitals. It was a repeated-measures research design. Fourteen (14) two-day workshops were conducted, consisting of six targeted and interactive modules. Staff members (N = 242) attended the program and were assessed according to (1) individual performance: questionnaires about attitudes towards dementia, confidence in care, knowledge about dementia and anxiety before, immediately after the training and three months later, (2) an overall training evaluation immediately after the training and (3) an evaluation of training implementation three months later. Positive attitudes towards dementia, improvement of confidence in care and decrease of feeling of anxiety as a trait, were sustained over time. Knowledge about dementia also increased after the training and sustained, with, however, a slight decrease over time. A well applied training program seems to provide the basis of a better care in PwD during hospitalization. However, changes in the organizational level and a transformation of care culture are necessary for training sustainability over time.

Curriculum evaluation is a process carried out to decide the effectiveness of training programs. In this process, the strengths and aspects of the training programs that are open to improvement are emphasized. Therefore, curriculum evaluation studies are an important part of curriculum development. In this respect, it is important to consider the research studies on curriculum evaluation studies together in order to develop curriculum. This study aims to review the postgraduate theses completed on the evaluation of curricula in Turkey between 2007 and 2019 according to several variables and to determine the research trend in this field. The method of the research was formed by systematic review. In this context, 586 postgraduate theses written on curriculum evaluation were reviewed. It was determined that the theses showed a balanced distribution according to years and were generally at master’s level. The postgraduate theses were mainly based on quantitative research methods, and in recent years, there has been a tendency towards qualitative and mixed-method studies. Questionnaires and interviews were used in the theses mostly prepared in accordance with the survey model and the data were collected from teachers and students. In the one-fifth of the theses on the evaluation of curricula which were examined within the scope of this research (105 theses), it was determined that curriculum evaluation models were taken into consideration and that the most preferred model was Stufflebeam’s CIPP model. Following this model, Erden’s "Element-Oriented Curriculum Evaluation Model" and Tyler’s "Objective-Centered Evaluation Model" were also preferred frequently.

In this work, the effects of using immersive media such as virtual reality on the performance of training programs to avoid ergonomics risks are analyzed. The advance of technology has made it possible to use low-cost portable devices able to generate highly immersive experiences in training programs. The effects of using this kind of device in training programs have been studied in several fields such as industrial security, medicine and surgery, rehabilitation, or construction. However, there is very little research on the effects of using immersive media in training workers to avoid ergonomics risk factors. In this study, we compare the effects of using traditional and immersive media in a training program to avoid three common ergonomics risk factors in industrial environments. Our results showed that using immersive media increases the participant’s engagement during the training. In the same way, the learning contents are perceived as more interesting and useful and are better remembered over time, leading to an increased perception of the ergonomics risks among workers. However, we found that little training was finally transferred to the workplace three months after the training session.

In banking industries, employees are entrusted with different roles and responsibilities, and training enables them to carry out these roles and responsibilities efficiently by let them to learn new things. Moreover, it will prepare them to take up higher responsibilities in the future. Therefore, this study focuses to analyze the employee perceptions on effectiveness of Training Programs in Myanmar Citizens Bank (MCB).  By using the descriptive research method, primary data are collected from the responsible persons and employees of MCB in head office, branches. Secondary data are gathered and scrutinized from relevant text books, records and annual reports from MCB. The research revealed that there are four kinds of training programs in MCB. Moreover, this paper revealed that MCB successfully delivered its training programs in year 2015 to 2018 and the trainees have positive perceptions on effectiveness of training programs in MCB. Based on these results, this paper pointed out the important facts that can give improvement actions for effective and efficient training programs in Myanmar Citizen Banks.

Background: Guidelines regarding asymptomatic bacteriuria (ASB) have consistently recommended against screening and treatment in most circumstances. However, screening of patients with spinal cord injury (SCI) is common practice and in some cases is a formal protocol at the organizational level. A previous study found that more than one-third of patients with ASB detected on routine screening cultures performed at annual visits in 2012 received antibiotics. However, the role of antibiotic stewardship has become more prominent over the last decade. We hypothesized that diagnostic and therapeutic stewardship efforts may be impacting the practice of annual urine-culture screening for SCI patients. We evaluated urine culture screening and treatment rates over a 10-year period. Methods: Patients with SCI seen in the VA Boston HCS for an annual exam in 2018 were eligible for inclusion and formed the baseline cohort for this study. Annual visits for the cohort over a 10-year period (January 1, 2009–December 31, 2018) were included in the analysis. Electronic data collection and manual chart review were utilized to capture outcomes of interest including urine culture, antibiotic prescriptions and indication within 15 days, and documentation of urinary or infectious symptoms. The main outcomes were (1) rate of urine cultures performed ±3 days of the visit, (2) rate of antibiotic treatment in asymptomatic patients, and (3) trend over time of urine culturing and treating. The χ2 test for trend was used to compare rates over time. Results: In total, 1,962 annual visits were made by the 344 unique patients over the 10-year period and were available for analysis. Among these, 639 (32.6%) visits had a urine culture performed within 3 days. The proportion of visits with a collected culture decreased from (109 of 127) 85.8% of visits in 2009 to (65 of 338) 19.2% of visits in 2018, P ≤ .001 (Fig. 1). In the treatment analysis, 39 visits were excluded for active symptoms, concern for uncontrolled infection, or prophylaxis as antibiotic indication. Among 600 remaining screening cultures, 328 had a bacterial pathogen or >100,000 mixed colonies consistent with ASB. Overall, 51 patients (17%) received antimicrobials. The rate of antibiotic treatment for ASB did not significantly decrease over time pP = 0.79 (Fig. 2). Conclusions: Over a 10-year period of annual SCI visits, the proportion of visits with a urine culture performed as routine screening significantly and consistently decreased. However, the rate of treatment for positive urine cultures remained consistent. These data support targeted diagnostic stewardship in this population to reduce unnecessary antibiotic use.Funding: NoneDisclosures: None

IntroductionThe quality of long-term care provided to people with dementia (PwD) in care homes can be improved by implementing person-centered care. Family carers of PwD living in care homes are an important part of person-centered care. However, they often experience high levels of burden, even when a family member with dementia lives in a care home. This study examines trends in the involvement of family carers and their perceived role, role overload and self-efficacy.MethodsData from the Living Arrangements for People with Dementia study, a cross-sectional monitoring study that evaluates developments in care for PwD in care homes in the Netherlands, with four measurement cycles between 2008-2017 was used. Data from the three most current measurement cycles were used, with respectively 144, 47 and 49 participating care homes and 888, 392 and 401 participating family carers. Family involvement was measured by the number of hours per week that family carers visited the PwD and the type of activities they undertook. Perceived caregiving role was measured with the Family Perceptions of Caregiving Role instrument. We used the Self-Perceived Pressure by Informal Care to measure role overload and a 37-item questionnaire was used to measure self-efficacy. We investigated the changes over time (e.g., measurement cycles) of these measures.ResultsThe number of hours that family carers visited the PwD did not change over time. However, there was a trend towards an increase in the variety of activities they undertook. Their perceived caregiving role did not change over time. Self-efficacy of family carers increased over time and they reported more feelings of role overload.ConclusionsDuring the past decade, there has been an increasing trend towards family carers doing more diverse activities with PwD in carer homes. Family carers did feel more competent in their caring role, although their feelings of role overload also increased. Continuing attention is therefore needed for the psychological well-being of family carers after a person with dementia moves to a care home.

Government increasingly relies on complex arrangements of providers to deliver public services. There is burgeoning public administration literature on contract management and performance. This literature emphasizes contract management strategies such as contract design and ex post monitoring and relationship building to promote contractor performance. The literature does not examine effects of structural variables on contract performance in ex post contract markets, though work on interorganizational networks has long established that structural factors influence individual performance. This study examines the influence of structural variables on publicly funded contract performance in networked structures of exchange using 5 years of state-level contract data. Network concepts are used to develop contracts as networked exchange structures and develop measures of structural embeddedness for individual programs. Findings include that the structural embeddedness of individual programs influences individual contract performance on quality and cost dimensions over time.

The way news is produced and consumed has changed dramatically during the first two decades of the 21st century due to digitalization and economic pressures. In a globalized world, current events are reported in almost real time in various countries and are diffused rapidly via social media. Thus much scholarly attention is devoted to determining whether these developments have changed news content. Comparative research in the area of journalism focuses on whether news content across countries converges over time and to what degree national differences persist across countries. When studying the research on long-term trends in news content, three main observations can be made. First, theoretical assumptions are often rooted in different models of democracies, but they are rarely explicitly discussed. Second, many studies focus on the organizational level using theoretical concepts related to increased market orientation of news outlets, such as personalization, emotionalization, or scandalization. Furthermore, commercialization is associated with the effects of digitalization and globalization, namely, decreased advertising revenues and increased competition. A commonly expressed fear is that these changes have consequences for democracy and informed citizenship. Third, in recent years, there has been a steady increase of studies employing international comparisons as well as a growing standardization for measurements. These developments lead to more multicountry studies based on large samples but come at the expense of more fine-grained analysis of the way news content changes over time. Finally, the vast majority of cross-national and single-country studies focus on Western democracies. Thus our knowledge about recent changes in news content is limited to a small set of countries. Overall, many studies provide evidence for constant changes of news content driven by social, political, and economic developments. However, different media systems exhibit a sustained resilience toward transnational pressures reflected in a persistence of national differences in news content over time.

Training programs are a popular method, in industry globally, to increase awareness of desired concepts to employees and employers and play a critical part in changing or supporting performance improvements. The predominant method to assess the effectiveness of training programs is to have the participants answer Multiple Choice Question (MCQ) and True/False (T/F) questions after the training; however, the metrics typically used to report the outcome of such assessments have drawbacks that make it difficult for the trainer and organization to easily identify the concepts that need more focus and those that do not. This study introduces measures of the Assessment of Training Effectiveness Adjusted for Learning (ATEAL) method, which compensate the assessment scores for prior knowledge and negative training impact in quantifying the effectiveness of each concept taught. The results of this method are compared to the results of the most popular methods currently used. A simulation of various scenarios and the training effectiveness metrics that result from them is used to illustrate the sensitivity and limitation of each method. Results show that the proposed coefficients are more sensitive in detecting prior knowledge and negative training impact. Additionally, the proposed ATEAL method provides a quick and easy way to assess the effectiveness of the training concept based on the assessment results and provides a directional guide on the changes that need to be made to improve the training program for the participants. A companion paper expands the concepts using results from actual training sessions in multiple industries.
